Established in 1878, the Louisiana Dental Association (LDA) is the largest professional health organization for dentists in Louisiana. It is a statewide grassroots organization whose purpose is to promote, advocate, and protect the dental professional. The LDA has over 1,900 members.

WLet it Sneaux?!

hat just happened? Some of my friends and family in South Louisiana said it snowed there back in January. No, really. Snow in South Louisiana. “Flurries?” I asked. “No!” they exclaimed, then sent me videos of blizzard-like snow that were obviously filmed in some other place like Colorado. Then I started receiving pictures and supposed snowfall amounts from places like New Orleans (10 inches), Baton Rouge (9 inches), Alexandria (3 inches), Lafayette (9 inches), Lake Charles (6 inches), and New Iberia (8 inches). What a joke! Who would believe any of that? If it snows anywhere in Louisiana, we all know it snows in North Louisiana where I live. So, I checked outside to see if we were experiencing our own blizzard. Nope, sunny and near 40 degrees. I checked with my son in Ruston: same thing. Monroe: the same.

Believing my friends and colleagues in Southern Louisiana must be involved in staging some sort of coordinated hoax, I turned to a trusted source of truth for the information I desperately needed, the Internet. And what did I find, you ask? I saw a couple skiing down Bourbon Street. Others with ice skates on, playing hockey on Canal Street. What sort of alternate universe was this?

As dentists, we should all appreciate the old expression: “Don’t look a gift horse in the mouth.” What a wonderful, probably once in a lifetime experience, that so many got to enjoy for several days this Winter. Something that made people forget for a bit the bigger things in life that tend to drag us down and demand our full attention. Setting those things aside for a minute and feeling like a kid again is good for the soul. And something that puts a smile on people’s faces is always going to get a thumbs up from this dentist!

Things are Changing So Quickly!

As I went back and read my previous entries in preparation for writing this next installment, I almost got whiplash reading about how fast school is changing!

Since the last article, we’ve continued seeing operative patients each Friday, slowly becoming more and more comfortable both with injections and with the drilling and filling itself. One of the most valuable things I’ve gained from weekly operative clinic so far is the confidence to deal with various situations that are less than ideal, contrary to what it’s normally like drilling on extracted or plastic teeth. For instance, a few weeks ago, I decided to complete one of my amalgam filling requirements on #16 that turned into two separate restorations: an O with a slight buccal component, and an OL. In the moment, I couldn’t help but wonder what I’d gotten myself into as I watched carrier after carrier of amalgam fall out of the prep I had cut as I tried to fill it. In hindsight, I now know how valuable having patience and an assistant can be!

Since returning from Christmas, we have also begun periodontics clinic, where we’ll perform an initial comprehensive exam, 4 quadrants of SRP, a re-evaluation, and maintenance appointments on patients deemed to be eligible candidates following their oral diagnosis appointment. After finishing our complete dentures course a few weeks ago, we have started learning about removable partial dentures. Good thing I’ve always enjoyed physics! While we have yet to set teeth on an RPD framework, we’ve instead spent the first few sessions designing appliances on plaster models. Additionally, we recently began our course on endodontics, where we’ve learned about creating access to the pulp chamber, measuring working length, and obturation. Besides those courses that have a pretty major lab component, we also are

currently taking pediatric dentistry and nitrous oxide sedation, which have been primarily didactic.

Outside of school, a whole lot has occurred since my last article including Thanksgiving, Christmas, a historic snow event in New Orleans, the Super Bowl, and we celebrated both my and my fiancee’s birthdays. Over the holidays, we spent time with our families and worked on some final details for our wedding in March. We started back to school for a couple of weeks only for New Orleans to get hit with up to 10 inches of snow in some places! Since we were snowed in for a few days, we had lots of time to stroll around Old Metairie and we were even able to build a snowman we named “Gumbo!” Most recently, the Super Bowl came to town and along with it came lots of traffic and a light show projected onto St. Louis Cathedral that was really cool. While the game may have been boring, it was exciting seeing all the new decorations set up all around the city to celebrate the big game! The next time I write for the Journal, I will have just finished my second year of dental school and I’ll be a newly married man. Until then…

Getting to Know Our President-Elect

As all our members might not know you, can you tell us a little about your background, education, family, and hobbies?

Absolutely! I was born in Baton Rouge but raised in Saint Gabriel, where I graduated from The Dunham School in 2002. I then graduated from LSU in 2006 with a degree in Kinesiology, then the LSU School of Dentistry in 2011. Shortly after, I completed an AEGD Residency at the UAB-VA Hospital in Birmingham, AL, before moving back home. I practiced with Louisiana Dental Center exclusively until 2023. From there, I transitioned into practice with my good friends Dustin and Liz Kidder in Baton Rouge. A significant clinical focus of mine is the surgical and prosthetic reconstruction of the failing dentition. I am passionate about prosthetically-driven implant surgery and treatment modalities that afford my patients the most predictable long-term result.

I have been happily married to my wife and high school sweetheart Rebekah for over 16 years now. We have three kids: David, 15; Jacob, 12; and CeCe, 10. We are in “that stage” of life: practices, games, competitions, etc. Currently, our kids compete in powerlifting, track, cross country, basketball, and soccer. It’s busy, but a lot of fun!

Outside of clinical practice and family life, I am the Dunham Assistant Cross Country and Track Coach. One of my dreams when I was younger was to continue coaching at the school. That dream is being fulfilled now! I have also begun to work closely with a couple of my colleagues on performance coaching for young men. This endeavor is a new arena for me but, as a self-help junkie, it is something I am both curious and passionate about.

My hobbies include all things physical fitness, like competing in triathlons,

cycling, running, and playing tennis with Rebekah. I also love travelling with my family, reading, and cooking.

My family and I are active members at Healing Place Church in Baton Rouge and love being a part of such a thriving and strong church community!

What inspired you to become an active LDA leader?

Being volun-told. Ha! Seriously, though, I got into local leadership in the GBRDA because, frankly, it felt like it was the right thing to do. It is so easy to find a million reasons to not get involved like the time commitment or family event

schedules. However, there are so many leaders that came before that truly elevated the LDA to the organization that it is now, and I felt obligated to help continue the track record of excellence that we have in protecting the profession and supporting our members.

What are your goals for your term as president?

As LDA president, my focus will be on the LDA continuing to set a standard of nationally-recognized and aspirational excellence. Our membership numbers are well above average, our advocacy efforts are incredible, and our member benefits far outweigh the cost of being a member. While this is all exciting and encouraging, in no way does it suggest that we should be content; rather, I look forward to our organization’s work to improve grassroots work with the dental school, to increase membership numbers (particularly those within DSO models), and to build upon last year’s legislative wins.

What are the most pressing issues facing dentists right now, and how would you like the LDA to respond to these issues?

Right now, the most pressing issue facing dentists is staffing…or lack thereof! Interestingly, through discussion with our colleagues in other states, while our issues with the lack of available hygienists and assistants are challenging, we are in a better position than most. The LDA has done a great job at hearing our members’ concerns regarding this issue. We have worked diligently with dental hygiene programs across the state to increase enrollment and thus create more hygienists to fill the void. Expansion and improvement in the EDDA program has been a huge success ever since the LDA had the opportunity to oversee and manage it. In taking on this course, we are excited to

help educate and certify more assistants each year than has been to date in our great state.

What issues are you eager to dive into during your term as president?

During my term as president, I am eager to see how our advocacy efforts will continue to create change for the betterment of our members and profession. With our successes in 2024’s legislative session, we are blessed with the opportunity to create further clarity about how dental insurance companies are supporting their patients and interacting with their contracted providers. Continued exploration of transparency with Dental Loss Ratio and reform to improve numbers will serve to not only better patient coverage but also improve life for practitioners through decreased denials and improved clarity with patient coverage. Further, the dental insurance companies that are doing a great job at covering their patients will be afforded an opportunity to gain better market share through positive publicity and both patient and doctor satisfaction. What are your predictions for organized dentistry and the overall profession of dentistry? How do you think dentistry will evolve?

It is interesting looking at the landscape of our profession right now. While DSOs and large group practices become

more common, I foresee a continued presence of thriving, traditional, small practices. There will always be patients seeking care in both models. The luxury we have in dentistry is the opportunity to practice in a wide range of modalities. At the end of the day, as long as we do our best to care for our patients in the manner that they deserve, the doors will stay open.

still being the voice of the profession in general; there is no need to adopt an ‘us v. them’ mindset. We all have the same desires and goal: to improve the lives and wellbeing of or patients. With the ADA making changes to its membership model, the goal is to improve membership within a population that has traditionally had a lower membership percentage. I applaud our leaders for taking note and attempting to address it. It will take time, effort, and adaptability to truly make this happen.

You Know that the LDA’s Lobbying Initiatives are

Reason Louisiana Has Sales Tax Exemptions on Some Dental Materials?

t may grab your attention if you hear that every dentist in Louisiana could lose a $10,000 PER YEAR tax exemption, right? Most dentists don’t know about or don’t remember the LDA’s initiatives to achieve sales tax exemptions for some dental supplies. Because the LDA lobbied for an expanded sales tax exemption that passed in 2002, some dental supplies are exempt from sales tax, thus saving dentists and practices thousands of dollars EVERY YEAR.

The LDA also pursued and received a revenue ruling from the LA Department of Revenue in November 2004 regarding sales tax exemption. Louisiana grants a sales and use tax exemption on dental devices that are used exclusively by a patient or administered exclusively to a patient during health care treatment. On November 15, 2004, the Louisiana Department of Revenue and Taxation issued Revenue Ruling No. 04-008, which clarifies the department’s interpretation of an expanded sales tax exemption on dental devices. As a result, most Louisiana dentists are paying significantly less in sales taxes than they did before that date.

After Louisiana budgetary concerns caused all exemptions to be placed on hold during the Jindal administration, the LDA fought to have the dental sales tax exemption reinstated on July 1, 2018. So, again, basically what is exempt are materials that stay in the patient’s mouth when they leave the office, and most items that are used a single time in the course of treatment and either used up (e.g., prophy paste) or disposed of (e.g., gauze). An important distinction is that materials used primarily for diagnostic purposes are generally not considered as being used for treatment and therefore not covered by the exemption.

On October 8, 2019, the Louisiana Department of Revenue issued an updated

publication that shows the applicable sales tax rates on all manner of items that either are, or have been recently, exempt or excluded from state sales tax. The document shows the applicable rates as they have changed due to various legislative efforts to address the state’s budget shortfalls beginning 7/1/2013 and extending to 6/30/25, when many of the current exemptions were set to expire.

Then, in November 2024, the governor called for a special tax session focused on improving Louisiana’s tax structure and returning more money to its citizens. The LDA lobbying team went to bat again for the dentists and met with the Department of Revenue to ensure that the dental sales tax exemptions were still included. We partnered with the LA

State Medical Society to ensure that health care kept these very important exemptions.

HB 10, now Act No. 11, effective 12/4/24, includes: Orthotic devices, prosthetic devices, prostheses, and restorative materials utilized by or prescribed by dentists in connection with health care treatment or for personal consumption or use and any and all dental devices used exclusively by the patient or administered exclusively to the patient by a dentist or dental hygienist in connection with dental or health care treatment.

This issue will always be on our radar and our lobbying team will certainly lobby for this exemption.

Big Bite Fishing Rodeo: A New Chapter for our LDA Foundation Grand Isle Event

The annual fishing rodeo hosted by the LDA Foundation is getting a fresh rebrand this year. Everyone, get ready for the Big Bite Fishing Rodeo in Grand Isle this July 18-19! Whether you’re a seasoned fisherman or a first-timer, this year’s event promises something for everyone.

The Big Bite Fishing Rodeo is all about fostering a sense of community, encouraging friendly competition, and most importantly, raising money for our community dental outreach programs. Grand Isle has been a wonderful location for our rodeo, and we will be back at the Grand Isle Marina this year. The event has a range of categories, from adults to juniors, large to small fish, ensuring that anglers of all ages and experience levels can get in on the action!

IBoard Complaints and Continuing Education

t is with great honor and privilege that I introduce myself as the newly-appointed Louisiana State Board of Dentistry president. This is my seventh year on the state board, as well as my second term. There are 16 board members (including myself) who represent the Louisiana State Board of Dentistry. We are a diverse board that includes two oral surgeons, an endodontist, a hygienist, multiple general dentists, and a consumer member. Act No. 296 of the 2024 legislative session amended the composition of the Louisiana State Board of Dentistry to permanently include a board seat reserved for a dentist who is chosen from a roster of Louisiana licensed dentists who are board-eligible or board-certified pediatric dentists. We are currently awaiting an appointment from Governor Jeff Landry for this seat.

Each and every board complaint is reviewed by the state board. The board receives approximately 130 complaints annually. In 2024, the state board received 114 complaints. Once we receive a complaint, we send a letter via United States Postal Service mail and/or email to the dentist or hygienist requesting a response and records. Once the Disciplinary Oversight Committee (DOC) has reviewed the complaint, a letter and/or email is sent back out with a decision.

Some examples seen in board complaints include the following:

1. Poor record keeping

2. Poor communication with patients and office staff

3. Poor X-rays

4. Overtreatment

5. Substandard care

Please do not hesitate to contact the state board office with any inquiries. The staff is exceptionally knowledgeable and supportive.

Continuing education is mandatory for all dentists and hygienists in Louisiana. Each dentist must complete a minimum of 30 hours of continuing education during each renewal period (every 2 years) for the renewal of their license to practice dentistry. Of these 30 hours, 20 hours must be personally-attended clinical courses that directly involve the delivery of dental services to patients. The remaining 10 hours must be clinical courses pertaining to the actual delivery of dental services to patients that may be done in person, online or via correspondence. If completed online or via correspondence, the courses must require the successful completion of a written examina tion at the conclusion of the course.

Each dental hygienist must complete a minimum of 20 hours of continuing education during each renewal period (every 2 years) for the renewal of their license to practice dental hygiene. Of these 20 hours, 12 hours must be personally-attended clinical courses that directly involve the delivery of dental or dental hygiene services to patients. TSpringing into 2025 Strong

he first few months of the year are always a busy time at the LSUHSC School of Dentistry. Seniors are preparing for graduation, the Office of Admissions is finalizing next year’s entering class, and several significant events are taking place.

I am pleased to share a few recent milestones from these past few months that continue to move us toward more success.

New Department Chair of Endodontics

After an extensive search to fill the position of Department Chair of Endodontics, I am excited to announce that Dr. Albert F. McMullen III has accepted the position effective February 1.

Dr. McMullen is a highly-respected endodontist with over 30 years of expertise in both clinical practice and dental education. He received his Doctor of Dental Surgery (D.D.S.) from Louisiana State University School of Dentistry in 1987 where he also completed his presidency in Endodontics in 1989.

Dr. McMullen is a board-certified endodontist, having earned Diplomate status with the American Board of Endodontics in 1993.

Dr. McMullen built his career in private practice in Monroe, Louisiana, from 1989 until 2013, after which he transitioned to academia. Before his appointment as Department Chair of Endodontics, he served as Interim Department Chair for Endodontics at the LSUHSC School of Dentistry since September of 2024. He also serves as the Director of the Post-Graduate Endodontics Program. In addition to his leadership roles, Dr. McMullen is a full-time Assistant Professor of Clinical Endodontics and serves as the course director for multiple endodontics courses.

In his academic career, Dr. McMullen has been instrumental in shaping the curriculum and mentoring future endodontists. He has been on the front lines in the restructuring of LSUHSC’s Endodontic residency program, emphasizing a comprehensive, hands-on approach to training.

Throughout his experiences in endodontics, Dr. McMullen has earned recognition as an excellent educator, clinician, and leader in advancing the practice of endodontics. We are excited to have Dr. McMullen as the Department Chair of Endodontics and are confident in his ability to positively shape and guide the program.

Give Kids a Smile (GKAS) is a national initiative that the LSUHSC School of Dentistry is proud to take part in. The LSUHSC School Dentistry has participated in Give Kids a Smile Day since its inception in 2003. This year, on February 7th, for the 22nd time, our faculty, students, and residents teamed up with local community dentists to provide free dental care to underserved children. From screenings to sealants, we were able to address many critical health issues among these children as well as provide education and resources for follow-up. Give Kids a Smile was more than just a day of dental care—it was an opportu-

nity to educate these children and their families about the importance of oral health.

The day was also a great learning experience for our students, who were able to engage directly with the community, gaining valuable hands-on experience while helping children who may not otherwise have access to dental care. Children left our campus with brighter smiles and an understanding of how to take care of their teeth. For many, this was their first positive encounter with dentistry. As always, it is a privilege to be part of an event that not only helps children today but helps shape the future of dental care in our community.

Clinic Renovations Update

Renovations to our clinics will continue this Summer. Last Summer, the third-floor clinic was completely renovated, which included not only the replacement of the dental chairs but also a new floor, countertops, cabinetry, and the addition of centralized nitrous and natural gas. New dental chairs have been ordered for the remaining areas that are still operating with the old chairs, putting us on track for complete chair replacement by this summer. Complete renovation of the second and fourth floors as was done on the third floor is in the planning stages.

Our clinics are at the heart of providing our students with excellent clinical training. The recent, ongoing, and future upgrades to our clinics will ensure that our students receive the best possible experience and that our patients receive the best possible treatment.

Miles for Smiles

For the first time this upcoming April, the American Student Dental Association (ASDA) chapter of the LSUHSC School of Dentistry is hosting a Miles for Smiles 5k Fun Run that will benefit the student dental clinic at Ozanam Inn.

The Ozanam Inn is a non-profit, direct-service agency that serves the homeless and needy. Totally student-run and voluntary, a dedicated group of LSUSD students and faculty members volunteer their time and expertise to deliver free dental exams and extractions to the homeless and underserved individuals in New Orleans.

LSUSD ASDA aims to support this mission by allocating 100% of the race proceeds to the clinic. The event also serves as a platform to promote oral health awareness, encourage community engagement, and foster wellness within both the dental and broader New Orleans communities.

The event will be held on Sunday, April 13th, 2025, at the Bucktown Marina at 8:30 am. We invite all to participate and support our student organizations in serving our community.

ello, my LDA family. It appears that the Winter festivities are done and it’s time for Spring. And wow, what a Winter it was. If I wanted to live in Alaska, I would just move there! I even had to use Mardi Gras beads on my tires as chains to get through the snow. Just a little heads up: it doesn’t work.

With Spring here, there’s one important thing to do: register for the New Orleans Dental Conference/LDA Annual Session 2025. This conference will be the highlight of your year. As I’ve mentioned before, this year’s conference will take place on the same weekend as the French Quarter Fest, an event with great music, food, entertainment, and shopping opportunities. The combina tion of these events will get you on your feet dancing and yelling “Laissez les bons temps rouler!”

Seriously, my conference board and I are extremely proud of the conference we have put together for our constituents on the heels of Smilecon®. We can’t compete with the size of that event, but we sure make up for it in hospitality, knowledgeable speakers, and good old-fashioned NOLA fun. Moderate Pediatric Sedation Review Course 2025

Instructor: Dr. Priyanshi Ritwik

This course serves as a review and update of moderate sedation for pediatric patients. It is intended to provide dentists with the basic knowledge and core competencies that promote safe and effective dental sedation in children. The course focuses on patient selection and risk assessment (safety), the general approach to procedural sedation in pediatric dentistry, sedation monitoring, sedative drug pharmacology, and the recognition and management of the more common sedation-related adverse events.

The Louisiana State Board of Dentistry requires that dentists with sedation permits for pediatric patients attend a live, 6-hour course every two years. This course meets the requirements for renewal of a moderate sedation permit for pediatric patients in Louisiana.

I got involved with organized dentistry when I graduated and have always been involved at some level. When I joined originally, it just seemed like the right thing to do! Over the years, I’ve grown to know and love the dental community through local, statewide, and national meetings, and it’s opened some amazing doors for both networking and collaboration. As a community, I think it’s important that all dentists join and take an active role in preserving our beautiful profession. Organized dentistry is all about advocacy and support. This function is so important when it comes to government regulation, as well as how our profession handles overall changes in the market. I sought out a leadership role so that I can engage with and advocate for my colleagues.

My goals include:

1. Figure out a solution (or various solutions) for the hygienist shortage, and

2. Engage our lawmakers to enact dental loss ratio (DLR) for Louisiana.

I got involved shortly after completing my orthodontic residency and after going to a few of my district’s meetings. I felt that we had a very strong and involved membership, so I wanted to help out where I could and make our district the best it could be! Organized dentistry, to me, means being part of a family of colleagues and friends that share a common goal of improving our profession and patient care. I sought out a leadership role to have the ability to make lasting changes and improvements for my district. A few of my goals as president are to provide the best CE at our meetings to help improve patient care in our area, increase membership numbers for our district, and to continue to make the Bayou Dental District one of the strongest districts in the state.

I followed an upperclassman from dental school, Dr. Matt Ganey, into the LDA. He invited me to meetings right out of dental school in 2011 and it just stuck.

Organized dentistry has given me a sense of purpose and friendships that mimic the

bonds formed in dental school. I like being able to talk to people who are dealing with the same stresses, and it helps me realize I am not an island. I see organized dentistry as a great tool for me to stay grounded because I now see the selfless volunteer effort from others that have helped me before I ever entered dental school. The ability to mold and protect your profession and livelihood speaks to me as well. As I have spent more and more time in leadership roles locally and on the state level, I can see farther outside my own practice. Organized dentistry has helped me stay up to date on current issues that affect my ability to support my family. It is easier to face these issues together because many hands make light work. I have never liked to ride the coattails of others, though I have benefitted from other great dentists’ work before me, and it feels right to pay it forward.

My goals are to help get more people involved with the state and local organizations and to assist with any issues that impact my colleagues.

I got involved in organized dentistry through ASDA in dental school. Our profession has benefitted from the efforts of past colleagues and their willingness to safeguard issues which affect our patients and our practices. Since then, my membership in both the ADA and NODA has been invaluable, providing me with essential resources, professional networking, and strong advocacy for our profession. Over the years, I have had the privilege of learning from exceptional leaders and mentors within NODA—individuals who have inspired me to become more involved and to give back to the dental community. Their guidance and dedication have shaped my perspective

on leadership and reinforced the importance of a strong local and statewide organization in supporting our profession.

As I begin my term, my goal is to build upon the successes of my predecessors. Alongside the talented members of our governing board, I am committed to delivering meaningful continuing education programs that enhance our skills and keep us at the forefront of dentistry. During my time in dental school, I was involved in both the American Student Dental Association (ASDA) and the Academy of General Dentistry (AGD), and I remain committed to continuing my engagement with both organizations as I advance in my career. My journey into leadership began similarly to many others in my position with encouragement from colleagues within the chapter and a recognition of an opportunity to contribute. Once I became involved, I realized the significant impact that active participation can have on our profession and the importance of maintaining that engagement.

As president of the Northeast Louisiana Dental Association (NELDA), one of my primary goals is to strengthen our chapter by increasing membership while maintaining one of the highest recurring memberships in the state. Additionally, I am committed to providing high-quality continuing education opportunities that benefit our local dental community. By fostering a collaborative and supportive environment, I aim to promote professional growth and ensure the long-term success of our association.

I got involved in organized dentistry after graduation from residency in 2021. Our local component leaders were some of my friends and colleagues who reached out asking if I would be interested. Other mentors suggested it would be a great service to our community and an easy way to get connected with those around me! As I became more involved in leadership roles in the NWLDA I realized what a huge impact organized dentistry has day to day, behind the scenes. Those who are less involved may not even realize what's in the works. The big accomplishments over the last few years for our dentists are what keep me coming back! As NWLDA President this year, my goal is to enhance the member experience with our component meetings and CE. I am also working to spread the word to our younger dentists about what all organized dentistry is doing for us, and to get them excited!

I have been involved in organized dentistry in some capacity since dental school. Whether it was through student organizations, ACGME, or national meeting attendance, networking and collaboration have been a continued focus throughout my professional career. I joined our local group as a way to get to know my colleagues and it has been a natural progression over the past five years. Introduction

Oro-antral communications (OACs) are a common complication following the extraction of maxillary posterior teeth, particularly in cases where the sinus floor is thin or pneumatized. Proper identification and early intervention are crucial to preventing the development of an oro-antral fistula (OAF), which may require more aggressive surgical closure. This article outlines the steps general dentists should take to recognize, manage, and refer cases of OACs to optimize patient outcomes and reduce the possibility for chronic sinus issues.

Identifying

Oro-antral Communication vs. Oro-Antral Fistula

A key distinction in managing these cases is understanding the difference between an oro-antral communication and a fistula. An OAC is a direct opening between the oral cavity and the maxillary sinus, usually occurring immediately after extraction. At this stage, the communication is not yet epithelialized. Without epithelization of the sinus tract, an OAC is essentially a wound which still holds potential to spontaneously close. In contrast, an OAF is a persistent epithelial-lined tract between the oral cavity and the maxillary sinus. This OAF is chronic and usually will not spontaneously close without removal of the tract and subsequent closure with vascularized tissue.

Clinically, patients with an OAC may report a sensation of air passage between the mouth and nose when speaking or breathing, liquid transit into the nose when drinking, or acute sinusitis symptoms such as pressure and postnasal drainage. The Valsalva test, in which the patient gently blows through their nose while pinching it closed, may demon-

strate air leakage through the extraction site; excessive force should be avoided to prevent further disruption. If the communication is small, typically 2 mm or less, it may be asymptomatic and often close spontaneously. Larger defects, however, require active management to prevent further complications.

Risk Factors for Fistula Formation

Several factors increase the likelihood that an OAC will progress to an OAF rather than heal spontaneously. Anatomically, a thin or absent sinus floor, large sinus pneumatization, and proximity of the root apex to the sinus all contribute to an increased risk. Surgically, excessive force during extraction, sectioning of roots, or the removal of periapical pathology extending into the sinus can lead to OAC formation. Infection and inflammation, such as pre-existing sinusitis, periapical infection, or chronic periodontal disease, also increase the risk. Postoperatively, poor clot formation, excessive negative pressure from sneezing or nose blowing, and systemic conditions such as smoking, diabetes, or immunosuppression can further hinder proper healing.

Conservative Management of OAC to Prevent Fistula Formation

For small OACs measuring 2-4 mm or less, conservative management is often sufficient to promote natural healing. Immediate primary closure or placement of a figure-of-eight suture over a digitally compressed extraction site can aid in closure, though care must be taken to avoid excessive tension. Patients should be instructed on sinus precautions, including avoiding forceful sneezing, nose blowing, or using straws, which could create negative pressure and disrupt healing. In

our practice the use of nasal decongestants such as oxymetazoline and Sudafed may be prescribed for short-term use, along with saline sprays to keep the nasal passages clear. In cases where there is preexisting sinus or dental pathology a ten-day course of antibiotics with good sinonasal coverage such as amoxicillin/clavulanate or clindamycin may be considered. Patients should be monitored closely, with follow-up evaluations at one and two weeks to assess healing. We also encourage patients to wear any fabricated prosthesis such as a denture or partial denture as long as it has been well adjusted. The prosthesis may often act as a healing splint over the area allowing the oral mucosa to be undisturbed during healing.

Surgical Management to Prevent Fistula Formation

For moderate communications, typically 5 mm or greater, or cases at elevated risk of persistent patency, surgical inter-

vention is necessary. The buccal advancement flap, or Rehrmann flap, is the most commonly used technique, involving a mucoperiosteal flap that is advanced to cover the defect. We often recommend a modification of the Rehrmann is best accomplished with sulcular extension around any distal dentition and a generous distobuccal releasing incision posterior to the tuberosity with a back cut and periosteal scoring to reduce tension. This modification allows for a wider base of vascularity and greater rotation of the flap with incorporation of a back cut; however, it may require gingivectomy around the distal dentition at a later date. Watertight closure should be obtained with the suture line palatal to the fistula to prevent dehiscence. Any sharp or over contoured bone at the site of the extraction should be smoothed and reduced before advancement. The addition of Collagen or Platelet Rich Fibrin (PRF) membranes may act as adjuncts but should not be relied upon to close the communication in cases where the buccal tissue advancement is tenuous or under tension. It is our preference to reserve a palatal rotational flap or the buccal fat pad (BFP) graft as salvage options, particularly for larger defects which have failed buccal advancement flap.

When to Refer to an Oral and Maxillofacial Surgeon

Referral to a specialist is recommended in cases where an OAC persists beyond two to three weeks despite conservative management. Large communications measuring 7 mm or more, or those presenting significant anatomic challenges (buccal exostosis, inadequate soft tissue for closure, or closely adjacent to Stensons duct) should also be referred. If the site is infected or inflamed, requiring extensive debridement, or if there is suspected or confirmed sinusitis complicating healing, an oral and maxillofacial surgeon should evaluate the patient. Additionally, cases involving recurrent or chronic fistulas require specialized surgical techniques and management beyond the scope of general dentistry.

Management of Established Oro-Antral Fistulas

If an OAC progresses to a chronic OAF, surgical closure is required to eliminate the epithelialized tract. Treatment involves excision of the fistulous tract to prevent

recurrence, followed by flap-based closure, with buccal advancement flaps, palatal rotational flaps, and/or buccal fat pad grafts depending on the size and location of the defect. Underlying sinus disease must also be addressed, and in some cases, Functional Endoscopic Sinus Surgery (FESS) or a Caldwell-Luc sinusotomy may be necessary to clear sinus disease at time of fistula closure, particularly if chronic sinusitis is present. Access to the maxillary sinus also allows for packing of the maxillary sinus with either medicated gauze packing or dissolvable foam dressings allowing us to eliminate the pressure differential across the closed fistula. If significant bone loss has occurred, bone grafting may be required for later dental rehabilitation, but we find this is best delayed after the site has healed for 3-4 months.

Conclusion

Early recognition and appropriate management of OACs can prevent complications and improve patient outcomes.

General dentists play a crucial role in the initial treatment and determining when referral to an oral and maxillofacial surgeon is necessary. By following structured protocols for conservative and surgical management, clinicians can ensure optimal healing, reduce the likelihood of persistent oro-antral fistulas, and provide the best possible care for their patients.
