Winter is Coming?

ow many ways do we reflect at the end of a year, and why do it? We find ourselves making mental lists about what went well and what we could improve. We make resolutions for the new year, but usually realize that change is difficult. For me, change can be downright uncomfortable. If things are going well, why change? The thing is, life changes all the time. Every day, kids grow up a little more, people learn more about themselves, values change, circumstances change, and not so suddenly, things are different. We are much more likely to notice sudden changes: a death, a health scare, a graduation, or a career change. Subtle and shocking changes happen all the time, and noticing that change is crucial for our growth.

Since its founding in 1859, the ADA has been the voice of dentists all over the country and its stated mission is “to help dentists succeed and support the advancement of the health of the public.” It functions as a tripartite system with the local, state, and national components working together to try to achieve the mission. Without any one of the three, the framework fails. Some things never change, but the ADA is changing in terms of membership, and it requires our full attention.

Nationally, the percentage of licensed dentists who are ADA members has been declining since 2006 when the number was 72%.

In 2022, that number was 57%, and it’s projected to drop below 50% in 2 LDA Journal • Winter 2023

2027 if things don’t change. A below 50% membership rate means that less than half the dentists in the country will be members, which compromises the ADA’s national reputation as an authority in organized dentistry. Fortunately, in Louisiana, due to a long tradition of strong leadership, our membership rate for 2022 was 73% and will end up being nearly the same for 2023. As a result, when the LDA is supporting (or opposing) a bill in the legislature or taking up an issue with the LA Department of Health or the insurance commissioner, our numbers carry more weight to those in power.

Dr. Ray Cohlmia took over as executive director of the ADA in November 2021 and has been setting a new course for the organization with the Board of Trustees and the House of Delegates. An entirely new strategy and budget is in place, and members should see the effects of these changes by early 2025.

You may be wondering, “Do these changes really affect me?”

Know with certainty that they do.

The state of the ADA’s membership has everything to do with you, me, and every other dentist out there, no matter how or where they practice. The entire dental community

benefits from a strong organization that advocates for us and supports us in legislation, in our careers, and in our communities. The LDA will continue to highlight how organized dentistry provides benefits to its members in subsequent issues, and I encourage you to consider what membership means to you.

For me, working with the ADA as a member is how I make my voice heard, whether it’s on a local, state, or national issue.

I’m sure you’ve received a dues statement for 2024 recently (ADA dues went down by $30, and LDA dues have stayed the same!) so I would encourage you to renew. If you have a dentist friend who is not a member, encourage them to join. If they want to know what organized dentistry has done for them lately, someone from the LDA office would be glad to call them and discuss what the ADA and LDA have accomplished for members. As you’re pondering what changes you want to make for 2024, I would also encourage you to consider participating in organized dentistry if you haven’t before or if it’s been a long time. Start by just attending your local component meetings and see where that leads. The dental community needs you!

And so it begins…

S

o much has happened since I wrote about my experience applying for and being accepted into dental school! First year dental students (D1s) started their journey at LSUSD on July 10 with an orientation. I was a little anxious about my first day at the school I have worked so hard to get into, but it helped having one of my roommates going through the same thing as me.

Also, there were several social events in the first couple of weeks that LSUSD or ASDA (American Student Dental Association) sponsored. Those events helped to me get to know my new classmates better and get to meet my big buddy (a D2 student). Coincidentally, his name is Caleb, just like my real big brother.

One of the interesting things we did at orientation was to inventory the big box of various dental instruments and supplies I had just purchased, though I had no idea what most of it was used for. I’m sure I’ll know soon enough!

Another thing we did at orientation was get our introduction to the unique matrix that is our class schedule for the year. It’s color-coded and lists out every day of class until we finish the academic year on June 3, 2024. My dad said that my schedule definitely brought back memories and looked the same as it did 27 years ago when he started dental school. The complicated schedule and the concept of being at school from 8 or 9 to 4 or 5 o’clock every day will be a big change from the mostly online MBA program I was in for the past year!

Since then, we’ve started and subsequently finished morphology, waxed more teeth than I can count, and completed the first few gross anatomy tests and associated cadaver lab.

While it was a pretty daunting thought at first to dissect a cadaver, it was such

a big help. Some other classes we’ve had in the first semester include: General Histology, Oral Histology, Oral Diagnosis, Professional Development I, Evidence Based Dentistry, Infection Control, Interprofessional Education, and Operative Dentistry (more on that one next time).

Thankfully, I’ve also had lots of extracurricular opportunities. We try to have fun social events on the weekend like going out to watch the LSU game, playing golf with classmates, or

an invaluable (and smelly) experience seeing all kinds of structures I’ve never seen or held in my hands. Having the lab alongside the lecture really helped me to learn structures and their locations as we would study a body section and go dissect that section immediately after. A week or two after starting anatomy, we started morphology, where our instructors taught us how to use the old-school alcohol lamp and move the wax on a little plaster pad. My first waxes were ROUGH to say the least, but by the end (after a lot of help from the lab technicians) I was waxing up teeth I was really proud of for our practicals.

Another component of the course was timed ID tests, where the instructors set up extracted teeth, both primary and permanent, for us to identify.

Although stressful, it was really helpful in learning to differentiate the teeth from each other and learn tooth anatomy. Working at a dental lab before I started school gave me a bit of head start in morphology and was definitely

having TGIFs at the school. I’ve also really enjoyed trying all kinds of new restaurants on the weekend with my girlfriend. There are so, so many options around New Orleans and Metairie that I think I could try a new place every single week! As I began this dental school journey I was pretty nervous about what was in store for me, but I’m happy to report I’ve settled in to both the community and the school. So far so good!

A Day in the Life at the LDA

Here we are, quickly approaching the end of another year. Though Thanksgiving has passed, let’s give a belated thanks for the uneventfulness of the past year, if only in the sense that we had no pandemics or hurricanes. I also wanted to bring to light that our new LDA office in Baton Rouge had its fourth birthday this past October 4th. How time flies!

Since that time, your LDA staff has changed. I would like to introduce the staff, or “the team,” as many members call them, which consists of six individuals. Let’s learn about a day in the life at the LDA!

In October, your LDA delegates and alternate delegates attended the 164th ADA Annual Session and House of Delegates (HOD) in Orlando. The ADA HOD consists of 17 districts across the country. Louisiana is in the 12th District, along with Kansas, Arkansas, and Oklahoma. The HOD is the legislative and governing body of the ADA, representing more than 161,000 members of the dental profession. Dr. Linda Edgar, of Federal Way, Washington, was installed as the 160th President of the ADA, with Dr. Brett Kessler, of Denver, Colorado, voted President-Elect. Dr. Terry Fiddler, the 12th trustee, handed over the reins to Dr. Allen Reavis of Kansas. He will serve as such for the next 4 years. Louisiana will be in line after that. The next meeting of the HOD will be October 17-19, 2024, during the ADA Annual Session (SmileCon®) in New Orleans.

Finishing the Year Strong and Preparing for 2024

T

he ADA held its House of Delegates in Orlando this October alongside the ADA’s signature convention, SmileCon®. Once again, the LDA brought a group of dedicated Louisiana dentists to represent our state. LDA member and past ADA President Dr. Gary Roberts even dropped in on our caucus meetings. It is exciting that for the first time I can remember in my 20 years at the LDA, the ADA has reduced their dues by $30.

This change is indicative of the ADA’s plan to prioritize member benefits.

There has been much discussion about the ADA’s plan to serve our members on the federal level. Many times, our members understand state-level activities but are less connected with happenings at the national level. As mentioned in Dr. Brian Basinger’s editorial, the efforts from all three parts of the Tripartite are the reason that dentistry remains a vibrant profession that empowers dentists.

In the coming issues and through our e-newsletter, LDA E-Bytes, we will continue to provide members with resources and important benefits through the ADA. For instance, just in the last few days, the ADA publicized an informative document that many of you should review. The ADA Council on Dental Benefit Programs comprised of members just like you has created a new online document, Terminating a Network Agreement, which seeks to answer frequently asked questions about the subject. The guide also reminds dentists to consider the use of the ADA’s Contract Analysis Service before signing the initial agreement. You can find this information on the ADA website. We also have a link to it in the news section of our LDA website home page.

However, your LDA has remained steadfast and will continue to work diligently to meet the needs of our member dentists. If you have not already heard, there will be no 2024 New Orleans Dental Conference/LDA Annual Session because SmileCon® is coming to New Orleans, October 17–19, 2024. In 2024,

you will need in-person C.E., online C.E., accredited C.E. that meets the new DEA requirements, or maybe just C.E. that takes place at your favorite vacation spot.

Whatever the case, we have a course for you! Another DEA MATE Act C.E. Course (8 clinical hours) “Dental Implications of Substance Use Disorders” is scheduled for January 12, 2024, with Professor Karen Baker. The course will be held via Zoom so you can take the course from your home or office, but it is LIVE and will count towards your in-person hours if you want to use the course for your LSBD requirements.

Additionally, the LDA now offers Expanded Dental Duty Assistant courses. Beginning in 2024, we will

be hosting as many courses as are needed for the dentists of Louisiana to train their assistants. If you would like an update on new course dates, visit the EDDA page on our website at www.ladental.org/EDDA. Since the course is now hosted by the LDA, a new member perk will be a lower cost for dental assistants who work for LDA members (savings of $200 per registration).

Strengthening Bonds: LSU Health School of Dentistry Expands Dental Hygiene Program to Alexandria

ctober was National Dental Hygiene Month. To celebrate, the LSU Health School of Dentistry (LSUSD) and LSU Alexandria (LSUA), celebrated with a ribbon-cutting ceremony to unveil an off-site LSUSD dental hygiene program on the LSUA campus.

The collaboration is one of the ways LSUSD is helping to alleviate the shortage of dental hygienists in rural parts of Louisiana. Alexandria is recognized as a Health Professional Shortage Area, and the dental school, along with the LSU Health Sciences Center, has been improving access to care in these rural areas.

The existing LSUA Allied Health building in downtown Alexandria now houses a new, state of the art dental clinic and six dental hygiene students who began their journey to becoming dental hygienists this fall. They learn in conjunction with the 69 dental hygiene students at the New Orleans campus via Zoom for classes and with local clinical faculty members. The challenging curriculum empowers all students, including those in Alexandria, to become oral health professionals who will serve their communities.

LSUSD alumnus Trey Carlton, DDS ’98, began this project when he reached out to LSUA Chancellor Paul Coreil and expressed his concerns about the lack of dental hygienists in central Louisiana.

Dr. Carlton, past Central and Louisiana Dental Association (LDA) president, through his connection to the LDA joined forces with Executive Director Annette Droddy, 10 LDA Journal • Winter 2023

LSU Health New Orleans Interim Chancellor Dr. Steve Nelson, and LSUSD leadership. It was through this partnership that the LSUA dental hygiene program was born.

Jane Walsh, RDH, JD, director of the dental hygiene program, also deserves thanks for her tireless work in making this partnership a reality.

Walsh travels frequently to the LSUA campus and works closely with the dental hygiene faculty and students.

By welcoming these six dental hygiene students, LSUA and LSUSD are working to create dental health providers who stay in the community they learn in, which in turn will serve central Louisiana for generations to come.

This Year in Review

T

his year has gone by quickly, but working with my fellow board members has been very gratifying, and I’m honored to have served as the board’s 2023 president.

Over the past year, the board made a few rule changes to better serve the public and make our regulations a little clearer for our licensees. The LDA and the board have continued working together to ensure dentistry in Louisiana remains a high-quality institution.

I have learned quite a bit about how the board operates over the past year. I’ve learned that we get fewer complaints than some people might think. We receive approximately 130 complaints a year, a number that has remained steady for several years, but even that number could be reduced with just a bit more patience and communication. If we can all take our time and ensure that our patients fully understand their treatment, options, and achievable results, fewer patients would feel the need to contact the board.

However, even with everyone’s best intentions, complaints will always be a part of board business.

And if you get a report that there has been a complaint about you, please do not panic. The board is committed to reviewing all complaints fairly and hearing the licensee’s side. If you have any questions regarding this process, you may always speak to Dr. Arthur Hickham at the board office or with your local board member.

We are currently finishing up a round of office inspections that was

delayed by the pandemic.

If your office has not yet been inspected for this round, our investigator, Alexx Smith, will be contacting you before the end of the year to schedule that inspection.

Most of the remaining offices are smaller satellite-type practices with fewer open hours. If Ms. Smith contacts you about that inspection, I ask that you work with her to schedule it so that she can complete these inspections before December 31.

As a reminder, many of us have licenses that are up for renewal by December 31. You’ll need to have completed and submitted all your 2022-2023 C.E. to C.E. Broker prior to renewing your license. If you still need certain types of C.E., including anesthesia and sedation, please contact Erin Conner at the board office as soon as possible so she can review options with you.

Most of you were notified earlier this year about the required opioid C.E. from the DEA. If you completed that course, you may be able to use it toward your Louisiana dental license renewal requirements if you earned it from an approved provider.

You can upload your certificate in the regular clinical C.E. section of C.E. Broker.

If you have any questions about C.E. requirements, please call the board office. Any licensee who renews after December 31 is automatically charged a late fee. If you wait until December 31 to submit

your C.E. and renew your license and run into problems, the board staff will not be available because of the holiday.

The staff at the board office have helped make 2023 a good year, and I’m grateful for each of them and their work. The team consists of six dedicated employees who are available to serve the public as well as the dental professional. Please email or call them with any questions you may have. They are always happy to assist.

Finally, I wish to thank each of my fellow board members for their dedication and hard work over the year. I am proud to be the 2023 LSBD Board President, and I could not have served with a better group of people. I’m certain we can continue to keep dentistry the wonderful profession it has always been.

Estate Planning: A Helpful Checklist

state planning is a key element of financial planning, and it can be very daunting. There are many considerations to make, and no two plans are the same. However, the reason why we need estate plans provides the necessary motivation to overcome procrastinating and get the plan done. As with any daunting task, a checklist is a proactive tool in breaking down the steps. However, the checklist alone is not enough. Followthrough is key to success.

Your estate plan needs to be addressed promptly, properly, and periodically.

Without a plan, your wishes may not be honored and your loved ones may be forced to deal with problems that could have been avoided.1 When a person dies without a will, which is called intestate, it allows the state to determine who the heirs will be and how and when they receive their inheritance.2 Louisiana laws are unique as they relate to some estate planning issues. These topics are complicated, which is why you should consult with proper planning professionals.

Do you need an estate plan?

Research shows that only one in three (34%) of American adults have an estate plan, and of those who do have an estate plan, 20% have not updated their plan in the last five years.3 In addition, 72% of women are without an estate plan compared to 59% of men.4 It is important to note that estate planning is not only put in place as the result of a death, but can also include documents that help your loved ones care for you and your finances if you become incapable of doing so on your own. In short, if you own anything, you should have an estate plan.

Estate Planning Checklist5 1. Take stock of what you own, including real estate, vehicles, personal items, financial accounts, investments, insurances, and business interests.

2. Compare estate planning options like wills and trusts.

3. Identify your beneficiaries.

4. Complete the necessary paperwork in the proper form for your decision makers. Typically, an estate plan includes: a last will and testament, a living will, health care power of attorney, financial power of attorney and a living trust.

5. Outline your funeral arrangements and wishes: There are also practical estate planning exercises, such as having a discussion with your family about your plan and wishes, including ways to access information.

A comprehensive estate plan attempts to take as much burden from the grieving family as possible.

6. Hire an estate planning attorney, which can help you address complex and current law.

7. Revisit your estate plan often. Ideally, a check in would occur every three to five years and after a triggering event, such as marriage, divorce, special care needs for beneficiaries, relocation, legislation changes, and more. (For example, your estate plan should be assessed if it will be impacted by the reduction in the estate tax exemption amount. There are trust instruments to consider, including Irrevocable Life Insurance Trusts (ILITs) and Spousal Lifetime Access Trusts (SLATs).)

Proper and proactive estate planning is an integral part of comprehensive financial planning because it extends legacy and can help build generational

wealth, including addressing potential tax implications.

bstructive Sleep Apnea (OSA) is a common sleep disorder that affects millions of individuals with potentially severe health consequences.

Dentists play a vital role in recognizing the signs and symptoms of OSA, facilitating diagnosis, and treating or referring patients for appropriate treatment. This article outlines the key points that dentists need to know to help patients with OSA. OSA is characterized by repeated episodes of breathing cessation during sleep due to upper airway resistance or obstruction. Patients with OSA often experience symptoms such as loud snoring, excessive daytime sleepiness, and witnessed apneas or choking during sleep. OSA can have serious health implications, including cardiovascular issues, cognitive impairment, and mood disturbances.

Clinical Assessment During routine examinations, dentists should be attentive to common OSA risk factors and symptoms, including snoring, excessive daytime sleepiness, dry mouth, and bruxism; inquiring about these symptoms can help identify those at risk.

Physical Examination Dentists can conduct oral examinations to assess anatomical factors that may contribute to OSA, such as a narrowed or V-shaped palate, an oversized tongue, or retrognathia.

Imaging Exams Dentists can identify potential candidates for OSA treatment by examination of cephalometric radiographs or cone beam computerized tomography (CBCT). In our practice, three critical landmarks are measured: soft palate length, posterior airway space (PAS), and hyoid to mandibular plane distance. In addition, airway volumetrics can be assessed, but this should be used merely as an adjunct diagnostic and patient education tool since the radiograph is usually taken awake and in a seated position.

Diagnostic Methods Dentists can help facilitate OSA diagnosis by referring patients for sleep studies (polysomnography or home sleep apnea testing) when OSA is suspected.

These diagnostic tests are the gold standard for obtaining an accurate assessment of the severity of OSA. A recent sleep study (within 1 year) and current titration of CPAP can help determine the next steps in treatment

and are often required by insurance to approve any surgical treatment. Finally, an adjunct to determine the level of worst obstruction is drug induced sleep endoscopy (DISE), where patient is sedated in a reclined position and the airway examined by nasopharyngoscopy to assess for the level of airway collapse. Though easy to perform, this technique still carries doubt over comparing obstruction of true sleep to that of drug-induced sleep.

Referring for Treatment Once OSA is diagnosed, dentists also play a vital role in treating or referring patients for appropriate treatment:

Hypopharyngeal Hyoid suspension can be performed via several techniques to suspend the hyoid bone to the inferior border of the mandible preventing airway collapse through a small midline submental incision.

Multisite obstruction Maxillomandibular advancement (MMA) through combined LeFort I osteotomy and bilateral sagittal split osteotomies allow advancement and counter-clockwise rotation of the lower and midface, opening the airway across all levels of obstruction.

1. Continuous Positive Airway Pressure (CPAP) Therapy CPAP is a highly effective treatment for moderate to severe OSA. Prescription is titrated to effectiveness by a sleep medicine doctor and is the gold standard when patients tolerate therapy.

2. Oral Appliance Therapy Dentists can work with patients to provide customfit oral appliances that reposition the jaw and tongue to maintain an open airway. These devices have been shown to have improvement in symptoms in patients with mild sleep apnea.

3. Surgical Evaluation Surgical evaluation should be considered in patients not tolerating CPAP, are not compliant with CPAP, or with severe OSA not improved with CPAP alone. Surgical evaluation will determine the level of obstruction and options for surgical correction.

Surgical Correction In treating OSA surgically, the operative approach is based on individual factors, as well as the level of obstruction (or combination of levels): Nasal Obstruction Turbinectomy, septoplasty, or functional rhinoplasty can all open the nasal airway allowing for improved compliance with CPAP. Nasopharynx Uvulopalatopharyngoplasty (UPPP), which can be performed with or without transoral robotic surgery (TORS), can greatly improve snoring and nighttime arousals by reducing the soft palate length.

Conclusion Recognizing and managing OSA in a dental practice is essential for improving our patients’ quality of life and overall health. Dentists can help identify at-risk patients, facilitate diagnosis, treat or refer individuals for appropriate treatment, whether it be CPAP therapy, oral appliances, or surgical evaluation. Collaborating with sleep specialists, both dental and medical, and educating patients about OSA are vital steps in providing comprehensive care for those with this common sleep disorder. By being proactive in identifying and managing OSA, dentists can make a significant difference in their patients’ well-being.

How to Handle HIPAA with Job Shadowing

I

n many cases, when applying to dental schools, students must first complete job shadowing at a practice. As these students shadow the dentist, who interacts with patients and performs procedures, HIPAA concerns must be factored in. What should you do if a prospective dental student would like to shadow you?

Rules for Job Shadowing in Healthcare The HIPAA Privacy Rule regulates how patient protected health information (PHI) can be used and disclosed. Under this rule, dentists can use and disclose PHI for treatment, payment, or healthcare operations. Healthcare operations is defined as, “conducting training programs in which students, trainees, or practitioners in areas of healthcare learn under supervision to practice or improve their skills as health care providers, training of non-health care professionals, accreditation, certification, licensing, or credentialing activities.”

Members of a practice’s workforce include, “employees, volunteers, trainees, and other persons whose conduct, in the performance of work for a covered entity, is under the direct control of such entity, whether or not they are paid by the covered entity.”

Under these definitions, a job shadow wouldn’t be considered a member of the workforce, and their work cannot be described as “healthcare operations.” So, how can you have a student shadowing in your 18 LDA Journal • Winter 2023

practice without violating HIPAA? Students must receive HIPAA training before they can shadow, and patients must provide written authorization for a shadower to observe a specific procedure.

HIPAA Training Prospective dental students in a pre-med program should receive HIPAA training through their school. However, your practice needs to check with the pre-med school to see what agreement is in place and who is responsible for training.

If a student wants to shadow at a private practice and the pre-med program doesn’t provide HIPAA training, the private practice can make the shadower a member of the workforce. In this case, the practice would be responsible for providing the student with HIPAA training prior to any shadowing activities.

Patient Authorization Since job shadowing does not constitute as “healthcare operations,” signed patient authorization is required before a shadower can observe a specific procedure. The patient must sign an authorization form that specifically authorizes the student to observe procedures. If a patient does not want to sign an authorization form, the student is not permitted in the exam room.

Reviewing Pre-Medical School Agreement Typically, regarding shadowing, there is an agreement between the pre-medical school that the student

is attending and the practice, in which the school agrees to provide HIPAA training. This agreement is sometimes called an affiliation agreement.

If a shadowing/affiliation agreement is already in place between a provider and the school, there are often confidentiality agreements that students must sign, in addition to the requirement that the school provide HIPAA training. 4 Tips for Navigating the New Interest Rate Environment

T

he last year has created an interest rate environment young practice owners have never experienced, and veteran practice owners haven’t encountered in 15 years. As with any macroeconomic changes, there are both threats and opportunities from recent developments. Practice owners should continually assess the wider landscape and be aware of the impacts for themselves and their businesses.

1. Assess working capital within your practice Working capital is the cash required to operate a business.

Often referred to as operating cash, this is the amount of cash required for your business to function through typical day-to-day changes.

There are periods within a month when collections may be very strong and others when many bills come due. Most practice owners are hyper aware of the dates in a month when staff payroll is withdrawn from their accounts. These payments are typically some of the largest outflows within a dental practice, but other large expenses could also include supply orders, rent payments, loan payments, or insurance. Practices are typically collecting revenue each day and waiting for payments from insurance companies to be paid within 2-6 weeks. This financial ecosystem provides a constant inflow of revenue while many expenses appear periodically and in larger amounts. Practices should be vigilant about the true cash needs for operations to handle these 24 LDA Journal • Winter 2023

unequal cash flows. In an interest paying environment, excess cash in your operating account is a lost opportunity for income on those deposits. At a 5% interest rate, $50,000 of excess cash misses the opportunity to earn $2,500 annually or over $200/month. It is advisable to determine your average monthly overhead and keep enough cash on hand to cover 1-3 months. Funds above this amount could be earning interest income and can remain in accounts where the money is still available for emergencies. The pandemic taught a hard lesson to many about the benefits of liquidity and how it is wise to keep these funds accessible. However, returns from interest on these dollars were virtually impossible to find in 2020 while interest on deposits in 2023 is available.

2. Fixed rate debt Much has been published regarding the mortgage rate increase for a single-family home over the last 12 months. The same concept is true within a dental practice! The fortunate recipients of a fixed rate loan under 5% over the last 10 years should analyze the new environment when considering accelerating principal payments. For example, will the loss of liquidity from a principal paydown cause the practice to need a loan for a near-term equipment purchase vs preserving the funds and buying with cash? In the current rate environment, a newly originated loan is likely to be between 6-10%.

It is also important to consider the

“after-tax” rate on business debt that results from the tax deductibility of the interest payments. At a 35% tax rate, a 5% business loan is a 3.25% loan after tax considerations. With cash accounts paying around 5% interest to depositors, many will prefer to keep the cash on hand and pay their loans off over the entire term vs making large principal paydowns.

3. Income Tax Considerations Remember that interest income is taxable as ordinary income, subject to your individual federal and state tax rates. Many taxpayers will see an increase in this income item on their 2023 tax returns even with the same amount of cash in emergency savings. This could lead to the dreaded surprise tax bill come 2023 filing season. Be sure to maintain a reserve for potential tax liability and inform your CPA of this expected income increase when projecting 2023’s tax liability.

4. Estimated Payments The IRS and state departments of revenue expect self-employed individuals to make tax deposits throughout the year. Employees see tax withheld from each paycheck and remitted to the agencies via their employers. For the self-employed, that responsibility falls on the individual taxpayer and is expected to be paid in quarterly installments throughout the year.

The IRS and state agencies charge an underpayment penalty when filing returns if these deposits are

not made through a backward-looking formula. The IRS underpayment penalty rate is currently 8%, while the Louisiana Department of Revenue’s is currently 9.5%. These rates have increased significantly in the last 2 years because of the Federal Reserve raising benchmark rates. Be sure to stay up to date on your quarterly payments to avoid these harsh underpayment penalties.

Few enjoy paying taxes, but paying penalties on top of income taxes is essentially paying more taxes!

Aspects of the economy are constantly changing and impacting your practice. It is important to recognize when these changes present opportunities and to position yourself to seize them. Interest rate levels are one of many cogs in the wheel of business ownership. Don’t squander your chance to turn a potentially negative situation into a positive!

Understanding Staff Attrition in Dental Practices: Identifying Causes and Solutions

mployee turnover is a never-ending challenge faced by dental practices, impacting both patient care and financial stability. Although attrition isn’t a unique issue to the dental industry, what is unique are the strategies that a practice can implement to improve the ongoing hamster wheel of recruiting, hiring, and training. While some factors contributing to staff attrition are obvious, we’ll take a closer look into the subtler triggers that lead to employee turnover.

1. Inadequate Communication and Feedback Channels In dental practices, the significance of effective communication and accessible feedback channels cannot be overstated.

When there are insufficient communication channels and limited opportunities for staff to voice concerns or offer suggestions, that environment can result in adverse consequences.

For instance, imagine a situation where your office manager identifies a recurring issue with cancellations and the receptionist is having a hard time keeping up with confirmation calls of future appointments and filling those canceled appointments.

However, without a clear avenue for your receptionist to share her challenges and ideas for resolution, her frustration will escalate, and the ongoing cancellation problems will continue.

To counter this, you can cultivate an environment where open dialogue is welcomed and encouraged. By implementing regular staff meetings, suggestion boxes, and systems for anonymous feedback, you can tap into the collective expertise of your team, boost staff morale, and ultimately improve the quality of patient care you provide.

2. Insufficient Professional Development Opportunities In a survey conducted by the American Dental Association (ADA), approximately

30% of dental professionals expressed dissatisfaction with the level of professional development opportunities provided by their practices. This discontent doesn’t just influence personal career advancement, but can also have repercussions on the overall patient experience and the competitiveness of your practice.

Taking a comprehensive approach is key to overcoming the above challenge. Creating tailored development plans, investing in diverse training programs, and supporting continuing education courses demonstrates a commitment to staff growth. If budget is a concern, in-practice mentorship programs, cross-training initiatives, and regular feedback systems foster skill transfer and address evolving needs. These strategies not only enhance professional development but also ensure your team is well-equipped to deliver the best patient care possible, ultimately contributing to the long-term success and reputation of your practice.

3. Unequal Workload Distribution Ensuring an equitable distribution of responsibilities within your practice is essential for cultivating a motivated team.

When tasks are not allocated fairly and transparently, it can lead to feelings of

inequality among your staff members. It’s crucial to institute comprehensive systems that prioritize the assignment of tasks according to individual competencies and regularly evaluate workloads, making necessary adjustments when required. You can do this by: • Assigning tasks based on the specific skills and expertise of each team member.

For example, dental hygienists may excel in patient education, while dental assistants may be proficient in chairside assistance. Assigning tasks accordingly ensures that each team member is working in their areas of strength, which not only improves efficiency but also boosts morale as team members feel confident and competent in their roles.

• Regularly assessing workloads to make sure that no team member is unfairly burdened or underutilized. This involves tracking the time spent on different tasks, patient appointment volumes, and the complexity of procedures. If you identify disparities, make adjustments to distribute tasks more evenly, which can prevent burnout and frustration among team members.

• Being prepared to adapt and redistribute tasks based on changing circumstances. For instance, during a surge in patient appointments, redistributing administrative tasks or extending appointment scheduling hours can alleviate stress on the team. Conversely, during quieter periods, team members may have the opportunity to engage in continuing education or focus on long-term practice improvement initiatives.

These systems act as safeguards for maintaining high team morale and nurturing an environment where every team member feels genuinely valued and enthusiastic about making their optimal contributions.

4. Lack of Recognition and Appreciation Recognizing and appreciating your team members’ contributions can shape their level of engagement and commitment. When you neglect to acknowledge the efforts and achievements of your staff, it inadvertently creates an environment where team members may feel undervalued.

Implementing a system that acknowledges outstanding performance creates a sense of belonging, and ultimately,

strengthens the bond between you and your staff. A few ways to start doing this right away include implementing a monthly “Employee of the Month” award and regular shout-outs on social media or your website, showcasing the staff’s accomplishments or newly acquired credentials.

5. Inadequate Conflict Resolution Inadequate conflict resolution can harm team dynamics and patient care.

Unresolved conflicts lead to lower morale and reduced productivity. In a Dental Post 2023 Salary Survey, it was reported that “31% of the dental hygienists surveyed are considering a job change and one in four said their driving factor was a desire for a more positive work environment.”

To address this, you should encourage open communication, provide conflict resolution training, and establish a structured conflict resolution process. Promoting empathy and mutual respect among team members also helps prevent conflicts, creating a positive work environment that benefits both your staff and your patients.

Recognizing and mitigating staff attrition issues in your practice is imperative for maintaining a thriving and successful clinic. Identifying the root causes of attrition and implementing effective solutions

can significantly enhance the overall work environment at your practice, leading to greater job satisfaction among your staff and improved patient experiences.

New Year, New LDA Events

his year, we’ve worked extra hard to make sure our events are relevant to our members. In just one year, we hosted a sold-out course to satisfy the new DEA requirements (and scheduled a second event for 2024!), provided new opportunities for dental assistants to become EDDAs, and organized a 2024 sedation course to meet the new LSBD regulations. All these events now exist in our yearly calendar in addition to the events you already know and love. Let’s explore the upcoming year of LDA Events.

DEA Course | January 12, 2024 Our first DEA event sold out! We’d hate to disappoint our members who couldn’t make it to the first event, so we scheduled another course for January 12, 2024, that will also satisfy the federal DEA C.E. credit requirements. This is most likely the last DEA C.E. event we’ll host. If you haven’t earned your accredited DEA C.E., now is the time! This course will be presented via ZOOM but when uploading it to C.E. Broker you will put it under “personally attended” so that it counts toward your in-person hours.

Bowden Leadership Conference | January 19, 2024 The L.H. Bowden Leadership Development Conference trains LDA leaders to work effectively within the association. Registration is free, and attendance is very important for all component officers, LDA officers, ADSA officers, directors, committee members, and council members. The LDA is offering mileage reimbursement to these officially invited attendees, and hotel reimbursement (one night) for those traveling from 100 miles away or more.

Sedation Course | March 22, 2024 The updated Moderate Sedation Review course now reflects the new regulations issued by the LSBD and is a one day, 6 hour “update” course for those renewing their moderate sedation permits. To address everyone’s needs, we are making it two different courses with the first one focused on the entire office with an emphasis on oral sedation. Our course will review patient assessment, team members’ roles, emergencies during sedation visits as well as review of the drugs, techniques and troubleshooting cases based off Dr. Ben Record’s 20 years of sedation dentistry. The end of the day will be open for case discussion from his practice and yours. The second course will focus primarily on parenteral (IV, IM) sedation techniques and more.

Summer Education Conference | June 12-15, 2024 The LDA Summer Education Conference is a family-friendly and tax-deductible trip*. In conjunction with the 39th annual Giant Omelet Festival in Abbeville, the Vermilion Parish Lâche pas students (French immersion) painted a wall mural of the Vermilion Iris. LDA President-Elect Dr. Lance Savoie helped to sponsor the beautification service project in conjunction with Vermilion Parish After Hours Rotary Club.

Bayou Houma Family Dental hosted its 5th annual Freedom Day event. The staff, along with local Drs. Pedro Cuartas, Lorie Moreau, and the HFD Doctors and Hygienists, provided cleanings, fillings, and extractions for active military, retired, veterans, and their family members at no charge as a way of saying “thank you for your service.”

Apparently, talent in dentistry isn’t the only thing that’s handed down through generations! LDA member Dr. Ronnie Hermes is the start of a long lineage of excellence in baseball and football.

His son Justin followed in his footsteps, as did his grandson. His grandson, Aubrey Hermes, recently attracted the attention of the South Caddo Parish Journal for his outstanding record in both sports.

This year, Aubrey caught 50 passes, made 10 touchdowns, and is a top football player in the state. He is also a two-time all-state baseball player.

Daylight Saving Time and Global Warming?

with a novel solution and it is truly amazing. They have invented Powdered Water. This unique process and product can and will have many positive effects on our world.

Water in powdered form will be easier and lighter to transport and store. Powdered water will make our lives easier in times of emergency and can save lives by providing a source of clean water almost immediately. Annoying water vapor can be all but eliminated, allowing our world to become cooler and less humid.

Granted, there are a few bugs that the U.S. Patent office pointed out. Perhaps the biggest problem our research lab needs to overcome is the issue of turning powdered water into its liquid form. I mean, if you have powdered water, what do you mix with it to turn it back into water?

Our LDA Research Lab is confident that it can eventually overcome this hurdle. Since it was a really small group seminar, I suggested that maybe we could just eliminate DST completely. Our researchers didn’t think that was a good idea. It seems to me that DST has created more problems than it has benefits. It’s annoying to have to change our clocks back and forth, and at my age, I’m not always sure what time it is anyway. I’m also convinced that the extra hour of daylight is killing my grass.

One good thing I walked away from this seminar with was the little piece of paper showing I had 8 hours of C.E. Now, if it will just count as clinical C.E., perhaps I can return next year and give an update within the LDA Research Facility.

I

was running low on C.E. for this year, so, in a panic, I signed up for an 8 hour seminar on the causative effects of global warming that so many of you have been complaining about, especially during July and August in Louisiana. This seminar was not advertised well, so the three of us that attended it at the LDA Research Facility in Bunkie had the run of the place.

This program was basically explaining the theory that Daylight Saving Time (DST) is a major cause of elevated world temperatures. But what the heck is DST and why do we have this system in place for 8 months out of the year (March-November)?

There is a debate about who actually came up with the idea of saving daylight, but the concept seems to originate from Benjamin Franklin in 1784 (All this from a guy who liked to fly kites in thunderstorms).

But it wasn’t used until World War I, in Europe, to cut the use of artificial lighting so troops could conserve fuel for the war. The US didn’t standardize the system until 1966, when it passed the Uniform Time Act. Does DST actually work? This debate is not totally conclusive. While it does help reduce some energy use for lighting, our own LDA research has shown that more energy is needed for heating and cooling during this extra hour. And of course, increased energy usage is thought to be a cause and effect of global warming due to the resulting increase of greenhouse gases in our atmosphere. LDA research has shown that, regardless of all the greenhouse gases floating around out there, the gas that is causing the most problems is water vapor.

Apparently, water vapor has been around for a long time, but in modern times it is at an all-time high.
