Wellness Tip Summer is the perfect time to get the necessary amounts of vitamin D which help boost your mood, energy and immunity. While there are trace amounts of vitamin D in food, the best way to meet recommended levels is by exposure to the sun. Sunscreen is important, but it can interfere with the absorption of vitamin D. Just 15 to 30 minutes each day is adequate unprotected exposure to gain the necessary vitamin D. So get outside and embrace the sunshine.

Honoring Our Nurses At a reception gala during National Nurses Week in May, Lake Health paid tribute to all of its nurse team members and directed special recognition to 16 nurses nominated for Lake Health’s 2018 Nurse of the Year Award. On December 6, 2017, Desiree Miller, 35, was surrounded by family in a birthing suite at TriPoint Medical Center in Concord, awaiting the delivery of her third son. Without warning, she felt nauseated and her heart began to pound. Her family called the nurse who responded immediately.

Desiree was pale, semi-conscious, gasping for air and couldn’t see. It was 4:01 pm. She doesn’t remember anything after that. The nurse laid her flat, initiated oxygen and hit the “code blue” button.

“My family told me it was four o’clock—the end of the shift for the surgery team—and some of them were already on the elevator heading out when they heard the code called over the PA system. They stopped and came back to save my life! Doctors and nurses came from all different directions, took their places and fit together like pieces of a puzzle,” says Desiree.

Life:

A rare condition

Although not confirmed until much later, during labor Desiree suddenly developed a condition called amniotic fluid embolism (AFE). AFE is characterized by the acute and rapid collapse of the mother and/or baby as a result of an allergic-like reaction to amniotic fluid in the maternal circulatory system. Occurring in pregnancy or shortly after delivery, AFE is extremely rare (1-12 cases in 100,000 deliveries), is not predictable or preventable, and carries a high mortality rate. Those who survive often have health deficiencies.

By 4:02 pm, the room was filled with impassioned caregivers, including OB/GYN James Guerrieri, MD; the obstetric staff; a code team; anesthesiologists; and respiratory therapists. A “code pink” was called as more specialists rushed in.

A Picture Worth a Thousand Words Excellence and teamwork came together to save this mom and baby in dire need.

Together they fought to keep Desiree and her baby alive as they prepped her for an emergency C-section.

At 4:17 pm, Camden Dez Tomaric was delivered with no heartbeat and not breathing, but was revived. Still in surgery, Desiree went into severe hemorrhagic shock. Her blood was not clotting properly, and she was at risk of dying from blood loss.

Despite the odds, surgery was completed. Once stabilized and on life support, Desiree was taken to the Intensive Care Unit. There additional physicians and specialists were consulted, and advanced treatment was initiated.

Although the odds were stacked against it, Desiree and Camden had an amazing recovery, are alive and well, and have no physical or neurological deficiencies from the traumatic event.

“Because of the skill, expertise and outstanding teamwork, we have a great success story. We are a community health system and provided exceptional care to this mom and baby as a result of the team coming together,” says Angie Quirk, RN, MSN, director of Women’s and Children’s Services.

“I am alive and well today because of Lake Health. If not for their swift action, knowledge and compassion,

I would not be here today. I cannot tell you what it is like to live another day because of all of them. How do you repay someone for saving your life? And not only one life, but two,” Desiree says with heart.

STROKE SCENARIO

The number 1 risk factor, signs, recognition, response, treatment and success all came together in one patient story.

November 20, 2017 was just another day on the job for 57-year-old drywaller and outdoorsman Richard Dulac.

He had just come down from his stilts and filled a bucket of water.

Then everything changed. While lifting the bucket, it bumped the sink edge and spilled. He called to homeowner Jacqueline Azbill for a towel. When she brought it, she found Richard hanging over the counter.

She asked what was wrong, but he couldn’t tell her. Thinking he was hurt, she suggested he sit in a chair. Weak, unsteady and unable

to get to a chair, she had him sit on the floor. Then she asked him to stick out his tongue. It stuck out crooked.

was ready for Richard when he arrived and immediately performed a CT scan of his brain.

“I knew immediately that he was having a stroke and told him so,” says Jacqueline. “I learned the signs of a stroke from a posting on Facebook, and I recognized it right away.”

“Rapid evaluation and treatment are critical for any chance at restoring normal function,” explains emergency medicine physician Jay Carter, MD. “Most stroke patients have a small blood clot blocking the blood flow to part of their brain. If they recognize the symptoms and get to the Emergency Department soon after symptoms begin, they may be a candidate for the drug tPA (tissue plasminogen activator). The drug helps break up the clot, restoring

A neurological exam, the CT scan, an EKG and more blood tests confirmed that Richard’s symptoms were caused by a blood clot in his brain and not by other conditions. He had regained his speech while at the ER but still could not move his left arm, hand or leg.

Dr. Carter, neurologist Patrick Tessman, MD, and a radiologist agreed that Richard was an excellent candidate for tPA. So, after presenting the benefits and risks to Richard and his wife Andree, all agreed it was his best option.

Less than one hour after ER nurse Melissa Hickman, RN, began administering the tPA treatment, Richard suddenly noticed that he could use his left arm, hand and leg. Their functioning was back to normal!

Concussions

Head injuries require specialized evaluation, care and recovery Sure, the risk of concussions worries athletes, coaches and parents, but anyone can experience a blow to the head resulting in concussion. Whether in sports, on the job, or from a fall or accident, concussions require quick evaluation and proper treatment. Without the right care, recovery can be hindered and sufferers can have lasting impacts in thinking and performance. That’s why Lake Health offers state-of-the-art concussion care at the Mentor Wellness Campus.

“The Concussion Clinic was created by Lake Health’s sports medicine staff to care for student athletes,” says Brian Juriga, DO, “but we provide evaluation and care for anyone with a potential concussion due to head trauma.”

Testing for concussion The staff uses an array of advanced tests including a computerized neurocognitive test, balance assessment, a visual coordination exam and a full head and neck evaluation. If imaging is required, some recommended tests can be performed on site. Based on the results, Dr. Juriga sets up the plan of care.

“We provide evaluation and care for anyone with a potential concussion due to head trauma.”

Treatment and recovery Recovering from a concussion takes time and requires careful monitoring by a trained individual. The Concussion Clinic partners with local schools to ensure that recovering students progress into the learning environment safely and with special accommodations if necessary. When it’s safe to return to athletics, the Concussion Clinic team connects with the in-school athletic trainer to ensure the athlete follows medical standards and reenters the game safely. Dr. Juriga provides the clearance required by Ohio law for students age 19 and younger when recovery is complete.

“It’s our intent to keep your kids in school as much as possible and to minimize the impact a concussion has on your child’s academic and future athletic performance,” explains Dr. Juriga. “The same principle applies in our commitment to employees returning to work after an injury, so we work with employers, too.”

On an evening in December 2017, Dawn Newsome, RN, was enjoying a fun dinner out with her daughter and a few friends. When Dawn prepared to pay her bill, the waitress handed her a note and told her that $25 of the bill had been paid for by another patron of the restaurant.

Opening the note, Dawn read, “Merry Christmas, Dawn!

Thank you for the great care you gave my mom!” The message was from the daughter of a patient Dawn had recently cared for in Same-day Surgery at TriPoint Medical Center.

Excited to see and personally thank the generous patron, Dawn asked the waitress where the woman

was sitting. The waitress responded that the patron had already left—she didn’t want to bother Dawn—she just wanted to do something nice to recognize the great care Dawn gave her mother.

In 21 years at Lake Health, Dawn has spent most of her career as a medical/surgical floor nurse. She served briefly in Care Coordination but returned to her real passion of hands-on patient nursing when she joined the Same-day Surgery team. There her duties include the preparation of patients for surgery, care after surgery, discharge preparation and the general comfort care for the patient and family throughout their stay, skills at which she excels.
