
Appointment Cancellation Policy For Procedures
We require that you give our practice 1 weeks notice in the event that you (or your family) need to reschedule or cancel your procedure date. This allows for other patients to be scheduled during that time. If you miss the appointment without contacting our office within the required time, this is considered a missed appointment, and a $200.00 fee will be applied to your account that must be paid before rescheduling your procedure. This fee will be charged to the patient, not the insurance company. As a courtesy we send reminder phone calls and emails. If you do not receive a reminder, this policy still remains in effect
We understand that unforeseen emergencies occur and you may not be able to keep your appointment. If you should experience extenuating circumstances, please contact our office to discuss and we can waive the fee. We thank you for your understanding and cooperation. The main reason for this policy is to provide continued availability to ALL of our patients.
