regnancy and childbearing are journeys that carry a lot of mixed emotions.

Every pregnancy is unique and has its own challenges. The universal agreement all expectant mothers have is the successful delivery of their respective newborns into the world.

Kenyatta National Hospital earlier this year celebrated a major milestone with the launch of its newly refurbished Labour Ward.

This came as a relief for expectant mothers and a huge boost for maternal healthcare including Linda Mama, which provides a package of basic health services accessed by all in the targeted population based on need, positioning the Hospital

Located on the ground floor of the Tower Block, a spot check by Newsline to the Ward revealed a

beehive of activities taking place.

animated conversations. They are excited. After all, they are about to bring in a new being into the world.

These are indeed good times for them.

“We receive expectant mothers as referrals from other facilities or walk-in patients. This means that every month, we assist at least 1200 women to deliver, most of them being cesarean section cases which need specialized care like surgery,” says Mrs. Raheli Mukhwana, the Assistant Chief Nurse in Charge, Labour Ward.

Mrs. Mukhwana further explains: “This, most of the time forces us to open a satellite theatre. This especially happens in cases where two women are required to undergo a C-Section at the same time. The ward only has two theatres. There are also cases whereby resuscitation is needed and the Critical Care Unit which is located at the postnatal ward’s area will help take care of about four patients.”

“Our team here is multi-disciplinary, we work with nutritionists, blood transfusion unit, pediatricians to ensure the well being of mothers before and after delivery and team spirit is the motivating factor behind our hard work,” she goes on.

Mrs. Anne Nyaga, an Assistant Chief Nurse at the Labour Ward weighs in.

She points out that upon arrival of the expectant women, they undergo a registration process, which costs Sh1200. Of this, opening a file costs Sh300 while Sh900 is for triage and consultation.

“She will then be given a code for payment after which she will be taken for triage. The mother will also get an admission booklet for the post-natal

“At the triage room, a fetal monitor will be hooked to the mother’s belly to check baby’s heart rate, and measure contractions. If it is not an admission case, she will be advised on how to go about the pregnancy and sent back home to wait for the right time to have the baby,” she goes on.

“Our ward is a busy area. On a normal day, we have between 20 to 25 C-section cases per day and around 25 vaginal births. This means at least 45 children are born here per day. In some cases we can have between 27 to 100,” she continues.

Mrs. Nyaga further credits the joy that comes with the arrival of the newborns as another motivating factor for the team.

“The cry of the little ones when they come to the world is a moment of joy for us,” says Mrs. Nyaga with a smile on her face. “There is job satisfaction that comes with it.” She adds.

Despite all, there are some cases whereby mothers and babies do not make it to full term pregnancies due to complications arising from pregnancies or during birth.

Mrs. Mukhwana, however, encourages women to go for prenatal care since any case arising will be detected early enough to limit them.

This is also added by the fact that KNH is a Referral hospital, meaning most of the complicated cases are being sent to the ward, in such cases, the staff strives to save lives as much as possible.

She further thanked the management for the renovation work done recently, which has seen a spike in the number of patients being admitted to the facility.

“The environment here is conducive with a lot of lighting and the ambiance makes the process bearable for the mothers to be. There is a sense of belonging which they feel once here,” she concludes.

In the Universal Health Coverage Agenda spirit, it is important to note that delivery service is free and mothers are encouraged to register for NHIF since it caters to all the other costs of treatment.

Those under Linda Mama also get the services for free thus having a newborn does not have to be a costly experience.

KNH BOARD MEMBERS TOUR HOSPITAL PROJECTS

By Winfred Gumbo A section of Kenyatta National Hospital Board members last week embarked on an inspection tour of projects being undertaken by the hospital. The site visit was led by Board Chairman Mr. George Ooko.

Also present were board members Dr. Richard Kamau and Mrs. Peris Onsarigo.

The team also included Chief Executive Officer Dr. Evanson Kamuri, Senior Director Corporate Services Mr. Carylus Odiangó, Director Affiliations and Institutional Development Dr. Tom Menge, and Director Legal Services Mr. Calvin Nyachoti.

They first toured the Cancer Treatment Centre, before visiting the Burns Management and Paediatric Emergency Centre where they were given a progress report on how far the projects have reached. “Most of these projects are at the highest stages of development and will soon be operational.” Said Dr. Kamuri On his part, the Board Chairman

thanked the KNH staff for the commitment they have shown towards the realization of the projects. “We need to keep up with the spirit so that we are soon able to cater to our patients who are our

main priority.” He said Once complete, these projects will be able to decongest the Tower Block which already has a huge number of patients and will help improve specialized healthcare.

The Big Interview All you need to know about Pregnancy and Covid 19

That the Covid 19 crisis has fundamentally changed many things on how we go about life is now not in doubt.

Hospitals too have had their challenges as a result of the virus, prompting them to come up with ways through which to undertake their operations smoothly.

At Kenyatta National Hospital’s Obstetrics and Gynecology Department, Newsline sat with Senior Assistant Chief Nurse labor ward Mrs. Raheli Mukhwana who shed more light on this issue: –

Q. Can the virus be transmitted to the foetus or the baby after birth?

Q: Why are pregnant women considered a vulnerable group?

A: Even before Covid 19, a pregnant woman usually has low immunity, thus making her more susceptible to other diseases. I must add that in the beginning, we used to separate a child from their mother and take them to New Born Unit. Later, we learned that the babies were not developing major symptoms. Q: How has the pandemic affected operations at the Obstetrics and Gynecology Department?

A: The pandemic caused panic, confusion, anxiety to both the health workers and their patients too.

Everyone was worried, not only for their safety but their families and loved ones as well. At the onset, there wasn’t much information about the disease. What causes it? How is it transmitted? Is there a cure among others. We were all reliant on the pressers by Health CS Mutahi Kagwe to get information about new infections and what we are required to do to stay safe.

At KNH, the number of patients visiting our health facility reduced tremendously. This, I believe, was precipitated by the less knowledge or lack of information about the virus.

There was a stigma around this disease, with many associating it with being admitted to the Critical Care Unit or worst still, death.

On the other hand, the KNH received lots of referrals from other health centers. This was because, at the time, it was the pioneer institution for isolation of the Covid 19 positive cases.

Q: How are you coping with the pandemic?

A: The hospital has been providing adequate Personal Protective Equipment (PPEs) to its health workers. This has indeed gone a long way in boosting their confidence while on duty. At first, we approached this pandemic with a lot of caution, fumigating all spaces, ensuring our hands were sanitized or washed with soap and water, worn

full PPEs amongst others. Now things have changed. It’s not that we are not doing all that, we do. We have lived with the virus and have an idea on how to operate. What is important at the moment is ensuring that we follow all the set protocols by the Ministry of Health.

We have also reorganized our department. Clinic 66, has, for instance, been reserved to handle suspected cases and patients of Covid 19. The clinic facilitates both normal and C-Section deliveries. This has made it easier for our main wards and theatre to contain and minimize the spread of Covid 19. The hospital has also added more staff on a temporary employment basis, which has ensured our health workers are not working for longer shifts.

Q: Do you conduct Covid 19 tests before admitting patients A. We don’t conduct a test for Covid 19 before admission because our patients are mothers in labor. What we do is a screen at the entry of the

wards before a patient is admitted. We then ask questions like the history of recent travel, fever, dry coughs, and contact with Covid 19 patients. We also check out the client’s temperatures. This helps isolate the suspected cases.

Q: Have you ever had any positive Covid 19 cases, especially among staff?

A: Yes, we have had several cases. In the beginning, they were being taken to isolation Centres. However, things have changed now. We are indeed in the information age and there’s now so much information about the pandemic. For staff members who get exposed and have mild symptoms, it is recommended for them to just undergo home-based care.

Q. Do you still offer Health Education to your patients?

A. Yes we do. However, we have changed the way we go about it.

Before Covid 19, patients sat in groups. They were then given lessons on how to breastfeed, proper nutrition as well as demystifying a lot of myths that surround childbirth, labor, and aftercare of the baby.

However, with the advent of Covid 19, we personalized the sessions to an

individual patient. This has taken a toll on our health workers as they have to give the same information to every patient. We came up with a labor ward hotline number, which patients can call in case of anything.

This has helped solve many problems which could have otherwise been brought by crowding in our wards.

This hotline is still active and we can be reached on 020-2711515. We have also encouraged our patients to read a lot to get empowered.

Additionally, Newsline had a sit down with a breastfeeding patient; Beatrice Awuor, a 28-year-old new mother admitted at KNH ward 1A. She has been regularly attending her antenatal clinics at KNH. Q: How has the journey been during this pandemic?

A: It has been a unique experience compared to my previous pregnancy.

First, I had reduced visits to the hospital due to the pandemic. I was not a high-risk patient but at some point, my blood pressure rose. The situation changed as a result. I was then forced to increase the frequency of visiting the hospital.

Q: Did you have antenatal classes during hospital visits?

A: Yes I did. And the nurses were kind enough to take me through the whole process of how I should take care of my young one. We are still being educated on the aftercare, especially on avoiding things that may expose us to Covid 19.

Q: Were you tested for Covid 19?

A: No, but I underwent screening and luckily I was categorized as a low-risk patient.

Q. Did you at any point during your pregnancy have symptoms similar to those of Covid 19?

A: Yes, I remember I had fever, general tiredness different from that of pregnancy, and my blood pressure was high.

I went to the hospital and upon undergoing a few lab tests, Covid 19 was ruled out. This was a major relief to me.

Prompting Advanced Patient Care Through CMEs

By Marian Moraa Clinicians at Kenyatta National Hospital (KNH) Othaya have undergone training on how to handle head injuries.

The training, which is part of their Continuous Medical Education (CME) process, was chaired by Dr. Omar Ahmed Nasir; a renowned Neurosurgeon at KNH-Nairobi and lecturer at the University of Nairobi.

“Head injuries are commonly caused by a violent blow to the head from incidents such as car or motorbike accidents and intoxication-associated accidents. Therefore, it is important to equip clinicians with adequate head injury management skills because if it is not well handled, there may be serious outcomes such as permanent disability,” said Dr. Omar.

“Brain is time. Therefore, it is critical to handle head injury cases promptly; because a little delay can make a situation irreversible,” he adds.

According to Dr. Ahmed, KNH receives an average of 250 head injury cases every month.

KNH Othaya management recognized a gap in head injury management in the region due to a lack of enough medical expertise and the expensive costs associated with it.

Clinicians receiving a lecture on head injury management from Dr. Omar Ahmed Nasir Facilitating CMEs like these are crucial to the prosperity of healthcare providers. It will allow our practitioners to learn and discover viable ways to improve patient care.

Moreover, it will improve the effectiveness of service delivery in the ever-changing landscape of universal health care provision.

The festive season is upon us!

Everyone is already feeling festive, with the Christmas and New Year celebrations just around the corner.

In many countries, Kenya included, Christmas is a time whereby families come together to make merry. Many people travel from cities, back to the villages to join their extended families in their celebrations.

Others opt for exotic holiday escapades to various world destinations.

There is the preparation of various delicacies and cuisine.

The aroma spices up the air which are accompanied by intricate decorations of Christmas trees at various malls, gifting at shopping outlets to mention but a few.

This year’s celebrations will be held on Friday, December 25.

At Kenyatta National Hospital, preparations for the event started as early as last week, where our very own Santa Clause – moved around various

wards and offices, dishing out gifts and spreading the Christmas spirit across the hospital.

He started by surprising some Board Members, who were on an inspection tour of the various development projects going on across the hospital.

José Feliciano’s Christmas rendition of ‘Feliz Navidad’ engulfed the air as the Santa handed over gifts to the surprised but visibly happy Board Members. This quickly turned into a brief song and dance session.

Later on, the group arrived at the Accident and Emergency Department whereby the patients, medical staff, support staff, and the security team were handed over gifts.

The smile on the patient’s faces explained the joy they had despite some of them being in pain and receiving medication.

As the team toured the hospital, the cheerful spirit was replicated by members of the public who joined by pulling various moves to the Merry Christmas song.

The team then moved to the kitchen to gift the staff

who tirelessly work to ensure the patients eat the right meals during their medication. A brief visit of the children’s cancer ward and the Finance department culminated the day’s tour. One thing was clear, there is joy in giving, the children never hesitated to join Santa for a dance as he handed them gifts.

Despite it being a festive season, it is important to note that this year’s Christmas Holiday comes at a time whereby the world is facing the challenges of Covid-19. The majority may be unable to take part in the usually socialized festive spirit, traveling upcountry to be with loved ones may be difficult since some of them are elderly and are highly exposed to contracting the virus.

This calls for the need to observe the Ministry of Health guidelines by avoiding unnecessary travel thus reducing them from exposure.

Additionally, we should never forget to keep social distance and wear masks all the time. Lastly, hand hygiene should always be observed and in the case whereby one suspects a case of exposure or symptoms, they should visit a health facility for testing.

Relief for Nyeri babies with twisted feet as KNH-Othaya begins Club Foot Management Julius Ita

Kenyatta National Hospital-Othaya Orthopaedic unit has embarked on a journey to bring relief to newborn babies, from a condition if not detected and treated early, will scar a child for life.

When a child is born, it is every parent’s prayer that he or she is well, but that’s not always the case.

Talipes equinovarus commonly known as Club foot is a condition in which a newborn’s foot or feet appear to be rotated internally at the ankle.

“Essentially a patient will require at least 5 weeks of 5 rounds of serial casting to correct the foot, although it’s wholly dependent on the patient’s progress. Thereafter they would be given a special shoe known as a Dennis Brown Shoe,” says Ms. Emma Akinyi, HoU Orthopaedic Trauma Technologist.

Commonly, a doctor recognizes clubfoot soon after birth just from looking at the shape and positioning of the newborn’s foot though this can also be diagnosed through a prenatal ultrasound.

However, diagnosis based on ultrasound alone produces a 20 percent false-positive rate.

Ms Emma added: “From case studies, Mt. Kenya region has a high number of club foot cases and so far we have attended to three successful we started the clinic.

According to research done by the University of Nairobi in 2017, at the Kenyatta National Hospital (KNH) in Kenya, an average of 260 children are diagnosed with CTEV annually.

“Club foot is not a curse nor has your child been bewitched. It’s a deformity that can be corrected if detected early,” said Dr. Mwaura, Orthopaedic surgeon. “My advice to parents is,
