Kudos to all that have made all this work. From the Board of Management led by Chairman George Ooko, to Chief Executive Dr. Evanson Kamuri, EBS and the entire KNH and Mwai Kibaki staff for the relentless work put in place to make this a reality. We believe that healthcare services in this region will greatly improve as referrals will be channeled to this facility thereby decongesting Kenyatta National Hospital and other referral hospitals in the country.

The hospital’s main mandate is to focus on managing non-communicable diseases which pose the greatest health burden in this county and the region. The facility is already playing a crucial role in promoting access to specialized medical services in the region and will also facilitate the implementation of Universal Health Coverage (UHC) as envisioned in the Big Four Agenda.

The clinic serves both adult and pediatric patients, is located in Clinic 17, and runs from Monday to Friday from 8 am to 5 pm. The success of this clinic hinges on support from the KNH management, the Kenya Haemophilia Association (KHA), and donation of factors by the World Federation of Haemophilia (WFH).KNH Director, Medical Services Dr. William Sigilai says the collaboration will bring immense benefits to both the hospital and patients at large.

5. Daily review and factor administration of patients admitted to the wards

KNH hemophilia and bleeding disorder clinic is one of the clinics that carry a bigger burden of patients with a rare condition and because of that, the management after a successful engagement launched the clinic in 2014. This was through the support of donors to be able to provide comprehensive care to patients with hemophilia and bleeding disorders.

3. Review of existing patients who present with bleeds and offer initial management (icepack, analgesic, splinting of limb where necessary as well as on-demand factor administration)

Hemophilia is an inherited bleeding disorder in which the blood does not clotThisproperly.moreoften than not leads to spontaneous bleeding following injuries or surgery. Blood contains many proteins called clotting factors that can help stop bleeding. People with hemophilia have low levels of either factor VIII (8) or factor IX The(9). severity of hemophilia that a person has is determined by the number of factors in the blood. The lower the amount of the factor, the more likely it is that bleeding will occur which can lead to serious health problems. In rare cases, a person can develop hemophilia later in life. The majority of cases involve middle-aged or elderly people or young women who have recently given birth or are in the later stages of pregnancy. This condition often resolves with appropriate treatment. On Friday 28th July 2022, Kenyatta National Hospital officials hosted a team from the World Federation of Hemophilia (WFH) led by their President, Mr. Caesar Garrido to engage in collaboration on the management of the disease.

The hospital team led by Dr. Sigilai –Director Medical Services took the guests round to the Special Hematology Lab and Parasitology Lab to check on the progress of their operations in managing hemophilia and the hemophilia clinic. “We are grateful to the KNH team for facilitating the circumcision of hemophilia patients in March, 2022 said Mr. Garrido as he presented a gift to Dr. Sigilai. Dr. Sigilai also appreciated the team for their effort to support the hospital in the management of Hemophilia.Later, the Head of Unit Physiotherapy - Mr. Kennedy Mogire accompanied the WFH group to the CME (Continuous Medical Education) session.Speaking during the CME, Ms. Angela Forsthe - a representative from WFH talked about the two types of Hemophilia; Hemophilia A which occurs due to a lack of clotting factor VIII.“This type of hemophilia is four times more common than hemophilia B. Of those, more than half of people with hemophilia A have the severe form. Hemophilia B occurs in around 1 in every 25,000 males born worldwide and is an inherited bleeding disorder. With this condition, the blood does not clot as it should, which can result in spontaneous bleeding and bruising after surgery or other injuries. This means that the person tends to bleed for a longer time after an injury and is more susceptible to internal bleeding,” she said. Some of the common symptoms of Hemophilia include but are not limited to; bruising hematomas (which is when there is bleeding into the muscle or soft tissues), bleeding from the mouth and gums, bleeding after circumcision, blood in the stool and urine, nosebleeds that are frequent and difficult to stop, bleeding after vaccinations or other injections and bleeding into the joints. Diagnosis of hemophilia involves a review of symptoms, a clinical evaluation, a review of personal medical history, blood tests, and other diagnostic testing. ‘’If a person has bleeding problems, or if a doctor suspects hemophilia, a physician will ask about the person’s family and personal medical history. A doctor will then perform a physical examination. Blood tests can provide information about how long it takes for the blood to clot, the levels of clotting factors, and which clotting factors, if any, are missing. Blood test results can also help to identify the type of hemophilia and its severity,’’ advised Dr. Forsthe. Though no cure exists for hemophilia, doctors can successfully treat the Treatmentcondition.focuses on replacing the missing protein and preventing complications. It involves giving or replacing the clotting factors that are too low or missing. Doctors can derive clotting factor treatments for replacement therapy from human blood or synthetically produce a form in a laboratory. Synthetically produced factors are called recombinant clotting factors. Specialists often consider recombinant clotting factors as their first treatment choice because they further reduce the risk of transmitting infections that can be present in human blood. However, modern screening techniques have decreased the likelihood of disease transmission from human samples. There are two main forms of replacement therapy.

12/8/2022 ISSUE 17 | Kenyatta National Hospital Newsline 11 patients with hemophilia and other bleeding disorders

Prophylactic therapy: Some patients will need regular replacement therapy to prevent bleeding. This is called prophylactic therapy. Doctors typically recommend regular treatment for people with severe forms of Hemophilia A. Demand therapy: This will stop the bleeding on demand. People living with mild hemophilia may only need to demand therapy, which is a treatment that doctors provide only after bleeding begins and remains uncontrollable.WFHthrough Dr. Angela Forsthe wrapped up the visit with a series of donations including; goniometers, tape measures, kinesiology tapes, theraband, braces, splints and Coban (stretch) wraps. The KNH Management thanked WFH for their continued effort to support KNH.
