A project years in the making, we are proud to partner with Jubilee Park & Community Center and Parkland Health to expand access to healthcare and wellness services in Southeast Dallas. The new clinic offers primary care, dental and mental health services for people of all ages.

“Five years ago, JFS initiated a meaningful partnership with Jubilee Park aiming to promote affordable and culturally responsive mental health services to the community. MEMBER SPOTLIGHT: CHRIS’S STORY

Chris struggled to hold a job for over 10 years when he began volunteering regularly in the PLAN Clubhouse culinary unit. He was able to create lasting friendships and develop a routine.

In 2019, he began in a transitional employment position at the JFS Resale Shop. After a successful period of responsibility there, with the help of Clubhouse staff Chris was able to gain long-term employment at a local grocery store.

By The Numbers

As an accredited Clubhouse model organization, members and staff join together with a common goal of running the Clubhouse and being part of a community that celebrates meaningful relationships, gainful employment, educational achievements and social connections. The PLAN Clubhouse has proven to be a transformative environment to help with reintegration into the workplace by creating a “work-ordered day” voluntary structure that promotes healing, recovery and the crucial elements of independent living. Clubhouse members have the opportunity to prepare nutritious lunches, attend social activities and volunteer. The Clubhouse is a place of belonging and friendship, and acts as a launch pad for a better life for those for whom mental illness has taken a toll.

COLLABORATIVE CASE MANAGEMENT & PEER SUPPORT

PLAN Members benefit from access to Case Management-- securing access to medical and psychiatric care, navigating benefits, housing referrals and coordination of care with family or caregivers.

Members also are able to receive Peer Support services to encourage and promote wellness, stability and independence. Peer Support Specialists have experience in dealing with mental health issues and have specialized training to help their peers move past their own difficulties.

The clinic is projected to serve 3,500 patients in its first year and close to 7,000 patients per year after. This partnership and coordinated approach will directly address this issue and provide a best practice solution that will make this community and these families emotionally and physically stronger as a result.
