THE MERCK MANUAL: DELIVERING GLOBAL MEDICAL KNOWLEDGE
First published in 1899 as a small reference book for physicians and pharmacists, The Merck Manual grew in size and scope to become one of the most widely used comprehensive medical resources for professionals and consumers. As The Merck Manual evolved, it continually expanded the reach and depth of its offerings to reflect the mission of providing the best medical information of the day to a wide cross-section of users, including medical professionals and students, veterinarians and veterinary students, and consumers. Since its creation, The Merck Manual was built to travel well. Compact, easy-to-use, and comprehensive, it was designed to deliver trusted medical information wherever it was needed. Albert Schweitzer carried a copy to Africa in 1913, and Admiral Byrd carried a copy to the South Pole in 1929. These early expeditions underscored what’s become a longstanding commitment from The Merck Manual to ensure healthcare professionals and consumers around the globe have access to current, credible and trusted medical information. In the decades since, The Merck Manual has enhanced and expanded its efforts to deliver medical knowledge to all corners of the world. While the print edition became a go-to resource for countless physicians and patients, The Merck Manual expanded its reach by making all that material plus additional videos and resources available for free at merckmanuals.com and msdmanuals.com in nearly a dozen languages. In 2015, The Merck Manual went further to share trusted medical knowledge around the world with the creation of the Global Medical Knowledge 2020 project. With this ambitious and far-reaching initiative The Merck Manual is committed to making the best current medical information accessible to three billion professionals and patients on every continent. In order to accomplish that goal, The Merck Manual has dedicated apps for medical professionals and consumers, which are available in the world’s most popular languages. Each app is available at no cost and can be downloaded for offline use. Both the websites and the apps come with no ads, registration requirements, or other barriers to access. The Merck Manual has committed to bring these resources to communities around the world that need them most. According to World Economic Forum data, nearly half the world’s population has no access to the internet. In the least connected countries, fewer than one in five are connected. Through partnerships with organizations such as Books for Africa, more than 200,000 copies of The Merck Manual professional and home editions go to nongovernmental organizations (NGOs) for distribution to physicians, nurses and community health workers throughout developing countries. The Merck Manual is distributing print editions and tablets preloaded with apps to bridge the digital divide in parts of the world that need them most. These manual resources are authored by more than 350 medical experts and leaders in their field, including many Jefferson physicians in several different specialties. Since 2010, Dr. Leonard Gomella has served as the Urology Editor for the manual. Throughout the years, other Jefferson Urology faculty have joined the esteemed The Merck Manual’ s contributor list.

Jefferson’s Department of Urology was founded in 1904 as the Department of Genitourinary Diseases with its first dedicated Chair, Orville Horwitz, MD. This makes Jefferson one of the oldest identified urologic disease centric departments in the United States. While many physicians were traditionally trained in “Genitourinary Surgery” in the early 1900’s, a defined Urology residency was established at Jefferson in 1946 by Dr. D.M. Davis, one of the well-known figures in modern urology. Our Department of Urology archives indicate that Dr. Davis produced the first book from our Department in 1953 titled “Mechanisms of Urologic Diseases”. Since the time of Dr. Davis, the scholarly products such as books produced by our faculty has increased greatly as the Department transformed from predominantly volunteer to more full-time medical college employed faculty. Beyond journal articles, our Jefferson Urology faculty have been involved in a wide variety of published books in the field of medicine. While many of these books are urology centric, some are medical books outside of the specialty of Urology. In 2021, we reviewed our Departmental library and faculty files to identify medical books authored/edited by our faculty. Hundreds of chapters have been written by our faculty over the years and have contributed greatly to our scholarly contributions and are not included here. For books included here, our faculty member must have a prominent role in the published book as editor/associate editor and be identified in that role on the cover. We have included only books published by medical publishers that would be appropriate for inclusion in a medical school medical library. Select journals are included if the periodical is published in book form.
A famous quote by our namesake Thomas Jefferson is, “I cannot live without books...” He made this statement shortly after the tenth and last wagon carrying his library left Monticello for Washington, D.C. Jefferson had sold his library to Congress to replace the congressional library that was destroyed when the British burned the United States Capitol on August 24, 1814. Congress, he felt, could not function without access to a proper reference library, so he promptly offered his own. (http:// blog.monticelloshop.org/?p=2030, Accessed December 10, 2021). Just as in Jefferson’s time, books have been a mainstay of medical education and patient care for hundreds of years. Over the years, Jefferson Urology faculty have authored or edited nearly 2 dozen different titles with many published long term through numerous editions and others translated into foreign languages.
While print books are still a major focus of medical publishers, online content is growing in this area. Many of the books noted here by Jefferson Urology faculty are currently available both in print and online or e-book versions, with the electronic version trend likely to continue into the future. This online trend is seen with our department’s involvement with the famous Merck Manual on page 22 of this 2021 Year in Review. In contrast to the U.S. medical readership, print books remain extremely popular outside of the US. Here is a sampling of books from our Sidney Kimmel Medical College Urology faculty. While the list is mostly in alphabetical order we start with the oldest book first.
Mechanisms of Urologic Disease This was the first book published by any Jefferson Urology faculty member. “Mechanisms of Urologic Disease” by Dr. DM Davis was published in 1953 by W.B Saunders in Philadelphia, with a purchase price of $4.50. A book review was published in JAMA on November 14, 1953. The reviewer noted: “This readable little volume gives a bird's-eye view of urologic problems with emphasis on the physiological processes and pathological changes involved. In contrast to most treatises on urology, instead of dividing the subjects on an anatomic basis, the author appropriately treats the subjects on the basis of eight different types of pathological changes.”

The following are excerpts from a review of the second edition by Dr. Paul Lange from the University of Washington and published in JAMA March 2011: “This excellent book does superbly what it intended to do: Quickly provide the reader with essential facts and action items needed to care for patients with almost any urologic problem….every urologic patient care environment should have a hard copy or the online version within easy reach. I recommend it highly.” Dr. Gomella edited all three editions with plans underway for the 4th edition.

The text is for urologists, radiation oncologists, medical oncologists and medical physicists.” This was also published in Japanese.
Chemotherapy and Immunotherapy for Genitourinary Malignancies: A Primer for Urologists and Advanced Practice Providers

This book is designed to familiarize clinical practitioners in systemic therapy options and medical management of urologic malignancies including prostate cancer, bladder and upper tract urothelial carcinoma, and renal cell carcinoma. The book is a resource for urologists, uroradiologists, medical clinicians and family practitioners alike, familiarizing its audiences with systemic therapy regimens for urologic malignancies, as well as their expected outcomes and side effects. A focus of the book is directed towards the role of advanced practice providers in administering this care.
Gomella and Haist's Clinician’s Pocket Reference

The “Clinician’s Pocket Reference” was originally published as “So You Want To be A Scut Monkey” by the University of Kentucky College of Medicine in 1979. The book was originally developed by students at the University of Kentucky. In 1983 the name was changed to the “Clinician’s Pocket Reference” when the 4th Edition was published by Appleton-Century-Crofts, Norwalk, CT. The book is a general reference for medical students and other health care providers for guidance at the start of clinical training. It has been a medical student “go to” reference book for many years and has been published by a series of legacy publishers (Appleton and Lange, now McGraw Hill). It has been referred to as the “manual of manuals” for learning patient care. The latest 12th edition (2022), has been renamed “Gomella and Haist’s Clinician’s Pocket Reference” in recognition of their long standing editorial leadership of the book.
Over the years, various editions of the “Clinician’s Pocket Reference” have been translated into 6 different foreign languages. The Portuguese language translation of the fourth edition was the first in 1983. Here are just a few examples of the translations. Clinician’s Pocket Drug Reference

Handbook of Urological Diseases In Children

This manual provided a quick reference on pediatric urologic problems for medical students and physicians in training. It was authored by Dr. Ricardo Gonzalez, Professor of Urology at Jefferson and the AI Dupont Children’s Hospital. Laparoscopic Urologic Surgery

Laparoscopic surgery began to expand in the early 1990’s. The laparoscopy program in urology at Jefferson was initiated by Dr. Gomella in 1990 with guidance from Dr. Craig Winkle, Vice-Chair of Obstetrics & Gynecology. “Laparoscopic Urologic Surgery” was the first color atlas dedicated to this rapidly evolving field. This book provided a guide to the use of laparoscopic techniques in urologic surgery. It provided essential information on patient selection and perioperative management, on laparoscopic instrumentation and video systems, and on basic techniques including microlaparoscopy and use of lasers and electrosurgery. The book also described a variety of laparoscopic urologic procedures several years before the introduction of laparoscopic radical prostatectomy and robotic assisted laparoscopy. The book was also published in Italian.

Recovering From Prostate Cancer

This paperback book was the first written for the general public that specifically focused on the topic of prostate cancer. Dr. Gomella partnered with Philadelphia Inquirer science writer John Fried. The book antedated others written by high profile experts in the field.
Smith’s Textbook of Endourology

Dr. Bagley served as Associate Editor on the first edition of this book now considered to be the standard endourology reference. The editors of this first edition have been referred to as the “the founding fathers of endourology.”
Surgery on Call

This book was primarily used by surgical house officers to provide rapid information on common surgical problems. The first edition was published in 1990 by Appleton and Lange and subsequently by McGraw Hill. It was released in an Australian English version and was translated in to several foreign languages with the Portuguese version shown here. This book was part of a larger “On Call” Series from Appleton and Lange/McGraw Hill. This series included Internal Medicine, OB/GYN, Pediatrics, Critical Care and Emergency Medicine. In addition to this book, Dr. Gomella served as the editor for the entire series. The Clinics Atlas of Office Procedures: Office Urology
Hirsch IH, Guest Editor 2002, WB Saunders, Philadelphia, PA This was part of a Urologic Clinics of North America series that focused specifically on in-office based procedures in Urology. Topics included prostate biopsy, cystoscopy, urodynamics and others. Dr. Hirsch served as guest editor.

The following comments were noted in a 1985 review in the British Journal of Surgery: “Bagley, Huffman and Lyon are internationally recognized authorities on endourology, the term now commonly used to cover all forma of urological endoscopy including percutaneous renal surgery and ureterorenal endoscopy…”…” it is the best book of its kind….” “The expense of the book ($95) is easily justified by its contents and lavish production. It would form a useful and upto-date edition in the library of any clinician involved in urological practice.”
