MSHA Safety: Bulldozer accident prevention alert A non-fatal accident occured when a bulldozer overturned on an embankment, rolling onto a bench below. Low temperatures and frozen ground created the condition, which led to the accident. Best Practices 1. Conduct adequate examinations of working areas especially during adverse weather conditions. Additional examinations may be needed where frozen ground exists;

2. Train examiners to know the hazards of frozen ground (equipment can slide and sudden ground failures can occur); 3. Correct hazardous conditions before equipment is operated or ground is travelled upon; 4. Operate equipment within its design limitations; 5. Clear outslopes of large rock or stumps where track mounted equipment will operated; 6. Install roll over pro-

tection (ROPS) and wear seatbelts at all times; 7. Install adequate lighting on mobile equipment that operates at night or during adverse weather conditions; 1. Conduct pre-operational checks before mobile equipment is operated. Report hazards to management; and 8. Maintain two way radio communications between mobile equipment operators and all person-

nel in the vicinity. A non-fatal accident occurred when a bulldozer broke into a cavity in a raw coal stockpile above the feeder. The bulldozer cab was completely covered and the operator was entrapped for approximately four hours before being rescued. Best Practices 1. Use enclosed cabs with rollover protection in stockpile areas; 2. Install two means of

two way communication in the bulldozer, with one powered by an independent power source; 3. Maintained communication at all times with the feeder control room and the bulldozer operator; 4. Install two 60 minute oxygen generating SCSRs in the bulldozer and inspect them during preoperational checks. Mount the SCSRs overhead and train operators in their use;

5. Install an independent light source; 6. Install 40 psi cab window glass or chemically treat with a proper window support system; 7. Provide a remote controlled stop device in the cab that can shut off reclaim feeders and belt conveyors; abd 8. Mark feeder areas of the coal stockpile with a suspended marker near the center of each feeder.

Bobcat from 3 wheel to lose contact with the ground. The system also senses when wheels encounter obstructions, changes in grade and higher traction levels. Once engaged, as an operator holds a constant low range speed, the system transfers torque to the wheels with the highest traction. One of the most significant benefits of this traction engagement system is its ability to function like a locking differential while retaining the advantages of its limited slip differentials. This feature is especially useful to buildings and grounds crews, farmers and homeowners with acreage who are frequently working on terrain with changing conditions and grades. The traction control feature is an option on the 5600 model and is a standard configuration on the 5610 model. The traction control system works in conjunction

with the Bobcat® utility work machines’ four wheel independent suspension system. The long wheelbase of Toolcat machines allows the independent suspension of each tire more time to adjust between each impact when traveling over uneven ground. The suspension system also maximizes traction by maintaining tire to ground contact while isolating the machine from changes in terrain, resulting in less impact to the frame, cab and attachments. This reduces bounce, spillage and vibration when carrying materials. In addition, the specially designed suspension stops provide support at maximum lift or payload capacity. A non-DPF solution partnered with more torque and protection mechanisms The Tier 4 Bobcat nonDPF solution was achieved by designing an ultra-low particulate com-

bustion (ULPC) engine. The ULPC is accomplished through a specially designed engine combustion chamber that significantly reduces the amounts of particulate matter created during combustion. Therefore, Tier 4 emissions compliance can be achieved without using a DPF. In addition to eliminating the need for a DPF, the new engines used in the 5600 and 5610 Toolcat utility work machines will have a 12 percent increase in torque. This increase in torque is produced over a wide range of engine rpm, allowing operators of all skill levels to better utilize the machines’ maximum performance. Additionally, the new Tier 4 Toolcat machines feature a machine protection system that monitors, manages and shuts down the engine if needed. This system monitors engine coolant and oil temperatures and will manage en-

The 5610 allows operators to use front-mounted attachments, as well as PTO and rear remote hydraulics to power Category 1, three point hitch implements or attachments. New sewer lines installed despite obstacles by Jon M. Casey

McVac's In fall 2013, Baltazar Contractors, Inc. of Ludlow, MA installed 700 feet of sewer line on Millers Falls Road, connecting it to a site on Industrial Drive in Montague, MA. Baltazar’s reputation helped the company win the job on a no-bid, negotiated contract because of an emergency. They were working to correct a problem with a previously install section of pipe that was not properly installed during a project by another contractor. The job started slowly because of high traffic volume. The projected called for 700 feet of 10-inch SDR 35 PVC pipe, in 14-1/2-foot lengths. During the September project, Montague’s DPW superintendent said, “They’re getting the job done and they’re doing it right. The crew is installing the pipe in the best possible way in a workmanship like manner under

high pressure vacuum truck blew a hole in the sewer beneath very tough conditions. The town of Montague lucked out in getting such a conscientious and experienced company (that is) working for a permanent solution to a very bad problem.” The job was challenging from the start for several reasons. The area had a high vehicle traffic volume, there were utility lines overhead, the old sewer trench was still on one side, ready to cave in and the nearby utility poles seemed ready to topple over because of the weight of the cables and wires. As a result, Baltazar’s workers were required to hold up the utility wires with insulated, fiberglass reinforced plastic poles, so that excavators could work without becoming entangled in the lines. Additionally, existing ground water threatened to cause cave-ins, or to float the pipe upward as it was being installed.

the truck, creating a sink hole that threatened to swallow it. Superintendent George Sajdera said the biggest obstacle was contending with heavy traffic, especially when setting the first precast manhole structure in place. That was where they were tying into the old section of functioning sewer. “We would dig for five minutes,” he said, “And then we would have to wait for 20 minutes for backed up traffic to clear.” That was with two police officers directing the daily, heavy traffic for the duration of the job. To keep the roadway on one side and the old sewer on the other from caving into the trench that was 19feet deep, Sajdera’s crew use a steel digging box with added steel plates. The box was 20-feet long and eight feet wide and was externally supported on either side for added protection. During the excavation, Baltazar’s excavator operator made the trench a foot and one half deeper than the pipe elevation, so they could bed the pipe in 3/4-inch washed stone. They covered the pipe with another foot and one half of similar material. The crew guided the pipe into place with laser equipment set up in a downstream manhole. With this process, a worker targets the end of the pipe as a way to adjust the pipe so that the laser beam confirms the pipe is dead center on line and grade. “For reasons of safety, we send just one experience guy into the trench to set the pipe,” Sajdera said. “And everyone wears a hardhat. Even me.” Once the pipe is on line and grade, it’s covered with screened stone fed from a Celco Hopper with an under belly conveyer belt. That is mounted on the Cat 345 D excavator digging the trench. The conveyer directs stone into the trench as needed. That accomplishes two things. First, the stone bedding under the pipe is kept in place, on line and grade. With stone covering the pipe, it weighs the pipe down to keep it from floating. Second, workers place a suction hose in the stone bedding under the pipe then connect it to a pump. That removes ground water during installation, which helps prevent the pipe from floating. The crew kept the trench backfilled, compacted in lifts with an excavator equipped with a hydraulic compactor. To improve the efficiency of the backfilling operation, Baltazar used a Komatsu WA 389 side dump loader capable of dumping backfill material

without disrupting traffic. Since there was no room to cast material as they excavated, Baltazar Contractors use two Mack tri-axle dump trucks to haul material to and from stockpiles. By using two trucks, there was no delay between trips. A second Caterpillar loader, front dump, refilled the Celco hopper. While this was all going on, a skid steer loader outfitted with a rotary broom cleaned up the pavement as the job progressed. Retired Montague DPW foreman, Charles Richotte, who worked on the original installation that was now being replaced, recalled how different the work was done back then. “We were working in the dark ages with an old cable backhoe and no shoring down to 19 feet. Nobody wore hardhats. The ground water turned the bottom of the trench to quick sand and your feet would get stuck in it. And the Transite pipe floated. We had a pump but didn’t use any stone under the pipe. The sides caved in every little while, and I got buried to the waist more than once. It was awful dangerous. Lucky nobody got killed. I’m glad those days are over.”

Baltazar Contractors Inc. use this 20 foot long by 8-foot wide heavyweight digging box to protect employees who need to get into the trench to lay pipe. The box is outfitted with inch thick steel plates driven down beside the digging box for added protection.

We do not yet have a location in this region of the Northeast so this acquisition is filling a great need to serve Mt. Safety Solutions: Dozers Dozer are one of the most common pieces of equipment at surface mines, they are primarily used for clearing and pushing material, although they can be used for grading and compacting loose materials. Dozers are also one of the heaviest and strongest pieces of equipment, accidents that involve dozers often result in disabling injuries or fatalities. It is important to make sure when operating a piece of equipment such as a dozer that a pre-operation

inspection be conducted. Things to check for when conducting a pre-operation include: • Ensure that the equipment is clear of all people and other obstructions; • Check to make sure all bolts, guards and covers are in place; • Check fluid levels; • Check all gauges and indicators for proper reading; • Make sure the hydraulic oil, coolant lines and hoses are in good condition and there are no breaks, leaks or loose fittings;

• Check ladders, steps, handholds and handrails for loose bolts, breaks or missing pieces; • Make sure the blade is down; • Check to make sure horn and backup alarm are in proper working order; • Ensure that brake and steering systems are working correctly; • Make sure windows and mirrors are clean and undamaged; • When mounting or dismounting dozers, it’s important that operators maintain three point con-

tact at all times and use only the steps and handholds provided; • Complete required inspection reports. Report mechanical problems and safety hazards immediately, and most of all, do not operate an unsafe machine. Proceed with caution and follow all surface haulage safety rules and procedures; and • Inspect the fire extinguisher. These are only a few items that should be checked before beginning work in a dozer. It is also

important to make sure you have inspected your work area for possible hazards such as: overhanging rock, loose rock, debris, vertical or horizontal cracks. Seatbelts must always be worn when operating a dozer. Operating mobile equipment such as a dozer should only be done by individuals who have been properly trained on the equipment. When operating a dozer, it is important that the operator take the time to understand the capabilities,

operating ranges and safety features of the equipment. Establishing and maintaining communication with other equipment operators is an important step in helping prevent accidents. Warm mix asphalt use reaches new heights LANHAM, MD - In the latest survey of the use of recycled materials and warm mix asphalt usage by the U.S. asphalt pavement industry, nearly a quarter of all asphalt mixtures produced in the 2012 construction sea-

son were produced using warm mix asphalt (WMA) technologies. The survey, conducted by the National Asphalt Pavement Association (NAPA) under contract to the Federal Highway Administration (FHWA),

found that the 1,141 U.S. asphalt plants queried produced about 86.7 million tons of WMA during the 2012 construction season. This marks a 416 percent increase in the use of warm mix since the survey was first con-

ducted in 2009. A copy of the full survey is available at www.AsphaltPavement.org/recycling . Because WMA is produced at a lower temperature than traditional asphalt mixes, it uses less energy to produce, re-

duces emissions, improves worker safety, and offers construction benefits. U.S. Secretary of Transportation Anthony Foxx commented in January during the 2014 Transportation Research Board Annual Meeting

that the use of WMA is expected to save $3.6 billion in energy costs alone by 2020. Asphalt pavements also continue to use increasing amounts of recycled

It has spurred us to continue to look for new solutions and to put innovations into practice,” said NAPA President Mike Acott. “This survey reflects how the industry is

More than 99 percent of asphalt pavement reclaimed from roads went back into new roads. In the survey, 98 percent of producers reported using RAP in their mixes. The 2012 survey also asked for the first time about the use of ground tire rubber, steel and blast furnace slags, and other recycled materials. Although national estimates of these products’ usage were not calculated, more than one million tons of other recycled ma-

SKF can supply virtually any type of rotating, reciprocating, or static seal, drawing from its catalog of several hundred standard profiles, and further can engineer unique sealing solutions tailored to particular application requirements. Seals can be made from a wide variety of stock materials (rubber elastomers, polyurethanes, and thermoplastics, among others) or from specialty materials, such as SKF ECOPUR®, a thermoplastic polyurethane exhibiting high abrasion resistance, low compression set and excellent strength. Other options include materials complying with specific industry standards and regulations (such as FDA guidelines) and specially formulated proprietary materials.

Customized seal capabilities and quick turnaround are made possible by unique SKF SEAL JET™ technology. This CNC-controlled production system employs proprietary software and special cutting tools ideally developed for machining different polymers. Continuous or molded one piece seals can be machined up to 157-inch OD (outside diameter) and larger versions can be developed using a welding technique allowing onsite fitting and promoting ideal seal performance. Production quantities can range from a single piece to lots of several thousand delivered in less than five days for normal orders. Safety Zone: Attachments Having the ability to change attachments on equipment to accomplish different tasks can make things much easier; how-

ever the increased versatility also brings the need to be mindful of safety consideration due to the changes in stability, ca-

pacity and visibility. Attachments change the way the equipment performs so it is necessary to make sure that everyone

who will be operating the equipment is trained on the proper use of attachments as well as the equipment itself. It is important when operating a piece of equipment that you know the limitations of the equipment. A few hazards that can be associated with the use of equipment attachments include overloading the equipment and roll overs, to reduce this risk one thing that can be done is review the capacity of the equipment and attachment combination before beginning operation. Ron Witt of Catamount Consulting has this to offer on equipment attachment safety: “Equipment attachment safety. Don’t be fooled about “It’s a one man job”... There are a multitude of attachments for mobile equipment and more being designed every day. The width and breath are

amazing. For example, a medium sized backhoe can have up to 9 different attachments that can do a thousand different tasks. That’s just one backhoe from manufacturer. If ever the instruction manual was important, here it is, (other than Christmas). Attachments should be regarded as tools. As such, they can be either very useful and labor saving, or potentially dangerous if not attached and used properly. Any new tool should be looked at as an opportunity to do task training. Instructions on the hazards associated with installation, operation, maintenance and disassembly. Each of these operations will have significant hazards. One of the design trends is to have an attachment simple enough to do by yourself. And while this

may seem cost effective and a time saver, it may lead to problems if things do go perfectly. This is an area that many would argue “it’s a one man job,” and it may be until a difficulty arises. That’s when it’s difficult for some to ask for help. When problems occur, it’s a team effort that gets the one man job back on track. Many times it may go exactly as planned and that’s the hope. But don’t be fooled, if you’re not task trained for a job get trained and get that manual out. Some of their innovations include extending excavator booms for high-reach applications, fabricating a new, second member mounting configuration for a 16,000-pound hammer, and a concrete pulverizer/magnet combination unit. “Our tallest high reach units are able to reach to 95-feet,” he said. “That makes bringing down a structure, a much safer process. Our patent pending top bracket on the INDECO hammer, gives contractors the ability to mount the larger hammer onto a PC 400 or PC 450 sized excavator, giving it more breaking power with less fuel consumption. It offers less expensive mobility as well. Our patented pulverizer/magnet attachment, allows the operator to crush chunks of recycled concrete fortified with rebar, and once the concrete has been broken away from the LaBounty's MDP 50R Mobile Demolition Processor is a new addition to their product line.

metal, the magnet portion of the attachment can be energized to move the rebar and steel material to its own location.” While we were in the equipment service area, we watched as one of the technicians worked on a Terex Fuchs material handler, prepping it for a new paint job following a rebuilding process that is exclusive to CW. “We are working on our own certified rebuild program,” he said. “We can rebuild a material handler and then offer it for sale with a warranty. We take machines that are still in good working condition but with a number of hours on them and rework them to a ‘like new’ condition. That gives a customer the opportunity to buy a reliable, used piece of equipment with several years of use still available to them.” “We also offer our own line of dust suppression equipment that we call the HAWC line of Dust Destroyer. That is in addition to the Italian made, Dust Fighter line for which we are distributors. These units are available in several sizes, depending on the customer’s need,” he said. Company overview CW President, Cam Gabbard, recalled how Company Wrench began as a heavy truck, trailer and equipment business with a 24 hour wrecker and recovery service in 1999 by its founder and owner, Brad Hutchinson. Soon after, Hutchinson hired a full-time mechanic, a co-worker he called “The Company Wrench.” It didn’t take long for that nickname to become the company’s name for which it is known throughout the Midwest and the east. In 2001, John White came on board to do more on the mechanical side of the business. As demand for equipment repair and service continued to come to CW, they expanded their line to include Stanley Hammers, LaBounty Attachments and other lines of attachments, excavators and material handlers. Today, with company locations in ten states, in addition to those already mentioned, their equipment lineup includes Terex Fuchs, Kobelco, LiuGong, SANY, Screen Machine, Gensco, Anvil, and Gehl, just to name a few. Those added to the CW Machine Worx and CW Environmental line of products, gives this 13 year old company a diverse offering of equipment to the construction, and recycling industry. Gabbard said that Company Wrench has always had a “listening ear” and that when customers had requests to make of the company, CW always endeavored to respond in a way to help serve the customers more effectively. By having the willingness to innovate, CW has built a support staff of design engineers and technical people, that has enabled them to fabricate new and useful equipment design changes that help make equipment and end-users more efficient and at the same time, more profitable as well. “We have a full engineering department,” said Gabbard. “Many of our innovations, like the Second Member Hammer and the second-pin under mounting on an excavator boom, offers our customers useful options on equipment that they use every day.
