2016 IPA Annual Report

Page 1

The Iowa Pharmacy Association’s Annual Report to the membership is an opportunity to share the accomplishments, activities and milestones of the association and our members.

Seizing Change Change is inevitable and can take many different forms. When Iowa Medicaid is transitioned to managed care, that change is challenging. When an endowment is established to support innovation in our state, that change is celebrated. When our members come together to create a statewide performance based network, that change is opportunistic. Our ability as a profession to adapt to, influence and even initiate change determines how pharmacy is practiced and how healthcare is delivered. This year was no different. IPA remains committed to providing members with the professional resources and continuing education necessary to maintain the progressive reputation of pharmacy practice in Iowa. This year, we included a BPS recertification track at the Midwest Pharmacy Expo. For the first time ever, board certified pharmacists or those interested were able to attend this level of programming at a state-based regional meeting. Seventy-eight pharmacists from across the Midwest welcomed this change and took advantage of this new and unique opportunity. Unity of the profession within our state is a unique strength and it was best seen this year where change can be the most challenging - the State Capitol. The relationships built with legislators and policy makers saw fruit this year as PBM rules were finally implemented and, in an unprecedented move, the previous Medicaid reimbursement formula was carried over through the transition to managed care. These achievements are due in no small part to the ability of our members from diverse practice settings to come together and deliver a strong, unified message through grassroots advocacy or face-to-face meetings with their elected officials. While we are often forced to embrace change from the outside, as a progressive profession, we relish the opportunity to create it on our own. Initiatives like the New Practice Model are continuing to expand opportunities for pharmacists and pharmacy technicians to deliver higher levels of care to Iowans and other states are taking notice. IPA facilitated the coming together of pharmacists from across the state to begin developing a performance based network that would create a framework for payers and health-systems to pay for services that positively impact patient outcomes. Our willingness to unite and innovate will allow the profession to embrace and even dictate change as healthcare evolves. The innovative reputation of pharmacy practice in Iowa is rooted in the generosity of Iowa’s pharmacy professionals investing their time, talent and treasure to the profession. This year, the IPA Foundation saw record-setting results at the Silent Auction and Eggleston-Granberg Golf Classic, fueling support for student pharmacists and the future of the profession. The establishment of the Outcomes Innovative Pharmacy Endowment by the former shareholders of OutcomesMTM will drive practice innovation in our state for years to come. Through this generosity, we are enabling the innovators and future leaders in our profession to lead change in our profession. Through ebbs and flows of this past year, the strengths of the pharmacy profession in Iowa were on full display as challenges were faced, opportunities seized and new, exciting initiatives were launched. Please take a moment to review the achievements of the past association year. But don’t take too long. The opportunities and momentum gained this year will not last long on their own. Iowa pharmacy must continue to build on the important achievements of the past year. Your active engagement is imperative to the continued success of the profession in our state.

The mission of the Iowa Pharmacy Association is to promote safe and effective medication use to improve the health of patients by serving the pharmacy profession. 2

ADVOCACY

Legislative Day

IPA held its 39th annual Legislative Day with over 200 pharmacists, pharmacy technicians and student pharmacists descending on the Capitol to advocate for the profession. The Iowa Medicaid transition to managed care was a hot topic with members and legislators alike. Members also took advantage of the opportunity to explain the larger role for pharmacy in health care delivery through statewide protocols. In addition to the advocacy activity, student pharmacists from Drake University and the University of Iowa provided 94 health screenings in the Capitol Rotunda for legislators and staff.

PBM Rules

On June 1, the Iowa Insurance Division enacted rules to regulate PBMs in Iowa, the culmination of 2 years’ worth of strong grassroots advocacy by members. The rules implement 2014’s HF 2297 and 2015’s HF 395, both of which were passed unanimously in both houses of the legislature. The new rules give the IID explicit authority to audit PBMs and request information, including pricing methodology for generic drug prices.

PCMA Lawsuit

In September 2015, a federal judge dismissed the final 2 of the 6 counts of the lawsuit brought by Pharmaceutical Care Management Association (PCMA) against the State of Iowa in 2014 over the Iowa’s 2014 PBM bill. PCMA appealed the decision in October. Thanks to the generous support of members and partners nationwide of IPA’s Preserve Pharmacy Practice Fund, IPA continues to be a resource to the Iowa Attorney General’s office as they defend the State. In February, IPA partnered with the National Community Pharmacy Association to submit an amicus curiae brief to the U.S. Court of Appeals federal court in support of the State of Iowa and against the PCMA appeal. The brief outlined compelling legal reasons defending the dismissal and the negative impact of PBM practices that the law addresses.

Iowa Medicaid/ IA Health Link

On April 1, administration of Iowa’s Medicaid program transitioned to three managed care organizations (MCOs) under Governor Branstad’s Medicaid Modernization Initiative, now called IA Health Link. In the months leading up to the transition, IPA worked diligently with the Governor’s office, the MCOs and key stakeholders to address pharmacy concerns. The most critical issue was maintaining the reimbursement rate

4

of AAC + $11.73. IPA was able to achieve an important, hard fought and unprecedented victory by maintaining the current reimbursement formula. IPA has also used this opportunity to assert the value of pharmacy and how the profession can contribute to the program’s goals of providing more care and reducing costs.

Full Congressional Support for Provider Status

All six members of Iowa’s congressional delegation cosponsored the Pharmacy and Medically Underserved Enhancement Act which would grant pharmacists provider status under Medicare Part B. Sen. Grassley introduced the Senate version of the bill, S 314, in January 2015, after working with IPA. IPA continued to advocate the importance of moving this legislation forward.

Leadership Breakfast with Rep. Buddy Carter

Addressing DIR Fees in Washington, D.C.

IPA worked with Sen. Chuck Grassley’s office to circulate a letter in the U.S. Senate urging senators to call on CMS to finalize their guidance from 2015 that would provide transparency to DIR fees in Medicare Part D plans. A similar letter coauthored by Rep. Dave Loebsack of Iowa was circulated through the U.S. Houses of Representatives.

Working with Sen. Ernst on Controlled Substance Disposal

IPA worked with Sen. Joni Ernst on amendment language for the Comprehensive Addiction and Recovery Act that would lessen the restrictions faced by pharmacies that are interested in collecting controlled substances. Though the amendment was not included in the bill, Sen. Ernst advocated for its provisions in an editorial in the Cedar Rapids Gazette.

Opioid Summit

IPA co-presented with the Iowa Medical Society (IMS) at an opioid abuse summit that brought together thought leaders from across the state to address the growing trends in opioid overdoses. The summit was hosted by IMS and the Alliance of Coalitions for Change and included representatives the Iowa Academy of Family Physicians, the Governor’s Office of Drug Control Policy, Iowa Poison Control, the Iowa Healthcare Collaborative, the Iowa Behavioral Health Association, the Iowa Osteopathic Medical Association, and Pfizer among others.

IPA hosted Rep. Buddy Carter of Georgia, the only pharmacist serving in Congress, for the 2016 Expo Political Leadership Breakfast. Rep. Carter shared his perspective as a congressman on several issues including provider status, PBMs, FDA drug approvals, and what needs to happen in Washington, D.C., to address these issues.

Pharmacy Visits

IPA partnered with several members to host pharmacy visits with Iowa’s congressional leaders while they were back in Iowa during the August recess and throughout the year.

ADVOCACY TakeAway Expands to Controlled Substance Disposal

As of June 2016, the TakeAway program has collected 131,000 pounds of unwanted medication since its inception in 2009. In 2016, the Iowa Board of Pharmacy approved additional funding that would allow the collection of controlled substances. Ten pharmacy sites were selected to receive MedSafe units that are DEA approved to accept controlled substances. In addition, envelopes were made available for any pharmacy to purchase at a reduced rate to give to patients, free of charge, to mail in their unwanted controlled medications.

Recommendations for IDPH Healthy Iowans 2020 plan

IPA submitted three recommendations to the Iowa Department of Public Health (IDPH) for consideration in the Healthy Iowans 2020 vision plan. Those recommendations were: • Collaboration among key stakeholders to develop and deploy strategies to reduce prescription drug abuse in Iowa. • Optimize patient outcomes and reduce medication errors during care transitions with pharmacist-delivered medication reconciliation and medication therapy management services. • Broaden availability, coverage, and public knowledge of immunizations to increase Iowa’s immunization rates in children, adolescents, and adults. IDPH will consider these recommendations as it narrows down priority health issues facing Iowans and strategies to address them.

Impact with IPPAC

Thanks to generous donations from our members, the Iowa Pharmacy Political Action Committee (IPPAC) was able to distribute $4,250 to 15 state legislators who have been supportive of the pharmacy profession. Most of the contributions were hand deliver by members in the legislator’s district to further strengthen those important relationships.

Preserve Pharmacy Practice Fund

Through the Preserve Pharmacy Practice Fund, IPA was able to continue to support the legal efforts related to PCMA lawsuit, and subsequent appeal, over Iowa’s PBM law. Through this restricted fund, IPA was able to partner with NCPA to submit an amicus curiae brief in the U.S. Court of Appeals support the State of Iowa in its defense of PCMA’s appeal.

6

Legislative Session The 2016 legislative session was a busy time for IPA, with our staff and lobbyists actively advocating for IPA’s legislative priorities and additional pharmacy bills that were introduced. Board of Pharmacy Bill - IPA supported the Iowa Board of Pharmacy’s bill (S.F. 453) which included the Board’s proposals on nonresident pharmacy, telepharmacy and outsourcing facility licensure, alternate board members, and definitions regarding pharmacy practice. The bill passed with amendments in both chambers and was signed by Governor Branstad on April 21. Naloxone – IPA worked with other stakeholders to include language in the Standings bill to allow pharmacists to dispense the opioid overdose reversal agent naloxone by prescription, collaborative practice agreement or standing order. Additional Prescriptions - SF 2214 allows pharmacists to change the quantity of a prescription dispensed within the total allowable. It passed unanimously out of both chambers and was signed by the Governor on April 6. Drug Information Program Access - SF 2102, which expands PMP access, passed out of both chambers unanimously and was signed by the Governor on April 6. Medication Synchronization – Medication synchronization was a top legislative priority for IPA and a bill was passed in the Senate (SF 2261), but it stalled in the House subcommittee and ultimately did not pass in 2016. IPA continues to work with healthcare payers, legislators and key stakeholders to implement policies that allow patients and pharmacies to easily synchronize chronic medications.

PRACTICE ADVANCEMENT Starting a Performance Based Network

Iowa Health Information Network

Wellmark High Performing Pharmacy Network

New Practice Model

Development was underway for the Iowa Community Pharmacy Enhanced Services Network (CPESN). Luminaries created minimum required services and a culture for a higher quality of care in Iowa. Pharmacies began to join the network and were volunteering to provide the highest value impact for all stakeholders. Community Care of North Carolina (CCNC) received a grant from the Center for Medicare and Medicaid Innovation to help model their CPESN network in Iowa. The Wellmark High Performing Pharmacy (HPP) Network planning was underway to eventually be in effect July 1, 2016. Wellmark listed pharmacies they consider to be providing a higher level of pharmacy care and began implementing a new performance-based prospective payment model for these pharmacies. The four target disease states are asthma, diabetes, hyperlipidemia, and depression.

Iowa Selected for CMS Enhanced MTM Pilot

The Iowa Health Information Network (IHIN) was initiated in a group of pharmacies to broadcast pharmacy use to improve coordination of care across health care settings. IHIN was operated by the Iowa Department of Public Health (IDPH) who was receiving reports from pharmacies to improve functions and address barriers to IHIN pharmacy use. Coordination with software vendors began to eventually integrate and upload pharmacy information into the IHIN. Phase 1 of the New Practice Model (NPM) pilot program completed its first 18 months of data collection in December 2015, while both phase 1 and phase 2 will end in August of 2016. Tech-Check-Tech was implemented in 17 pharmacies for refill prescriptions allowing pharmacists more time to provide patient care services. Phase 1 showed a similar error rate between tech and pharmacist verified prescriptions. A statistically significant increase in the number of patient care services provided and the time pharmacists spend in patient care was shown.

The Centers for Medicare and Medicaid Services (CMS) Enhanced Medication Therapy Management (MTM) Pilot program was announced and will begin in Iowa (1 of 11 states) on January 1, 2017 for Medicare Part D beneficiaries. CMS will be testing strategies to improve medication use and achieve MTM program goals.

Over ten state associations and boards of pharmacy have expressed to IPA their interest in to replicating the NPM’s success. IPA has been a resource to these states including sending Anthony Pudlo, IPA’s vice president of professional affairs, to present to the Idaho Board of Pharmacy.

Practice Advancement Forum

Statewide Technician Curriculum

The first Practice Advancement Forum was held on June 11, 2015 as a collaboration between IPA and the Iowa Department of Public Health (IDPH) to support the statewide implementation of innovative approaches to incorporate pharmacists in the prevention of chronic conditions. National speakers covered the patient care process, provider status, and health information technology.

A statewide technician education program began development with The University of Iowa; stemming from the upcoming changes in eligibility requirements to sit for the Pharmacy Technician Certification Board exam by 2020. The curriculum will meet the new ASHP/ACPE accreditation standards and will seek a single accreditation across all Iowa community colleges.

Practice Advancement Initiative

The Practice Advancement Initiative (PAI), formally called the Pharmacy Practice Model Initiative (PPMI), was a movement sponsored by ASHP to change pharmacist-provided hospital care. Close to 80 percent of Iowa hospitals completed the PPMI Hospital Self-Assessment to measure the alignment of hospitals with recommendations. PAI focused on a multi-disciplinary patient care team with active pharmacy involvement for appropriate medication use.

Trinity Pioneer ACO

8

The Trinity Pioneer ACO MTM program concluded after two years of research. While the program was not significantly different than the Medicare Shared Savings Plan (MSSP) comparison group, the ACO integrated MTM program was associated with significant decreases in hospitalization rates and Medicare costs showing promising results.

EDUCATION

Collaborative Education Institute (CEI)

CEI was founded in 2008 with stakeholders including the IPA Foundation, The University of Iowa College of Pharmacy, and Drake University College of Pharmacy and Health Sciences, and continues to be a leader in providing innovative live and webbased continuing pharmacy education (CPE) activities which assist pharmacists and pharmacy technicians in advancing their continuing professional development (CPD).

Free CPE Buckets

IPA members receive CEI’s Relicensure and Recertification Buckets for free. These buckets bundle all of the required CPE for pharmacist relicensure and pharmacy technician recertification together in one convenient package. During the first full year this was offered, 371 members took advantage of this important and popular membership benefit.

IPA Goes Local

Now in its third year, the Midwest Pharmacy Expo continues to evolve to meet the continuing education needs of pharmacists and pharmacy technicians in the Midwest. In 2016, Expo became the first state-based regional meeting to host BPS recertification CE programming, with 78 pharmacists attending this high level programming. Also new in 2016 was the addition of the Expo Tech Forum for pharmacy technicians and the inclusion of the Heartland PRN Conference. These tracks were in addition to the high quality continuing education and networking opportunities that are the heart of Expo. Over 300 attendees from 17 states earned up to 16.5 hours of live continuing education at the 2016 Expo.

IPA continued to partner with Iowa’s eight local associations to present a patient safety CPE program. The 2016 program also included a brief introduction and presentation from Andrew Funk, PharmD, the executive director of the Iowa Board of Pharmacy.

Advanced Technician Training Module

As a part of the initial resources developed for New Practice Model sites, IPA developed extensive training modules for technicians interested in or preparing for advanced roles in their practice setting, including tech-check-tech and telepharmacy.

The 2016 Expo also featured a keynote from Edward M. Hallowell, MD, a child and adult psychiatrist, leading authority in the field of ADHD, and the New York Times best-selling author of Shine and Crazy Busy.

With over 100 items up for bid, beer and wine toss and the Red Envelope Challenge, the 2015 IPA Foundation Silent Auction raised over $19,000, to support the Foundation’s initiatives for student pharmacy support and practice advancement. That amount broke the Silent Auction record by nearly $4,000!

Karbeling Leadership Award

The 2015 recipients of the Karbeling Leadership Award were Natalie Roy (Drake University) and Alyssa Billmeyer (University of Iowa). This award, designed to recognize student pharmacists for their contributions to professional organizations and the political process, includes a trip to DC for visits with national organizations and Iowa’s Congressional delegation.

Student Pharmacist Support

Eggleston-Granberg Golf Classic

Each fall, the IPA Foundation hosts the Eggleston-Granberg Golf Classic in honor of the commitment to student pharmacists by Max Eggleston and Boyd and Peg Granberg. The 2015 Golf Classic saw a record 106 golfers take over Otter Creek Golf Course in Ankeny and contribute a record $16,000 to the IPA Foundation’s initiatives to support student pharmacists at Drake University and The University of Iowa though scholarships, leadership training and other resources.

The IPA Foundation supports Iowa’s student pharmacists by providing funding for scholarships, professional development programs like the Bill Burke Student Pharmacist Leadership Conference, and attendance at state and national professional events. These initiatives are designed to instill a sense of professionalism, increase awareness of professional issues, improve communication and leadership skills, and establish students as an integral part of the profession. Thanks to the generous support of our members, IPA Foundation provided $19,000 in scholarship to these student pharmacists from Drake University and The University of Iowa for the 2015-2016 academic year: Drake University IPAF Eggleston-Granberg Scholarships Elizabeth Bald Aubrey Johnson IPAF Mike & Terry Pursel Scholarship Andrew Ruplin IPAF Russell E. Johnson, Jr. & Lucille Johnson Scholarship Krista Maguire

RAGBRAI

For the second year in a row, the IPA Foundation sponsored a team of pharmacist and student pharmacist cyclists in the Register’s Annual Great Bike Ride Across Iowa (RAGBRAI). The team logged 462.2 miles during their week-long trek across the state as they promoted the profession, visited pharmacies and provided first-aid and numerous over-the-counter treatments to fellow riders from three mobile pharmacies. Through this unique outreach opportunity, the team connected with pharmacists and pharmacy technicians across the state and demonstrated the value of pharmacy professionals in a tangible way.

THOMAS R. TEMPLE LEADERSHIP ENDOWMENT For the past 28 years, the Leadership Pharmacy program has provided leadership training to over 300 new pharmacy practitioners in Iowa. To ensure that the conference continues on in perpetuity, an endowment campaign was established in 2012 with a goal of $750,000 to be split equally between Iowa and Wisconsin. The IPA Foundation established the Thomas R. Temple Leadership Endowment for the Iowa portion in recognition of the founder of Leadership Pharmacy, Tom Temple, CEO Emeritus of the Iowa Pharmacy Association. To date, Iowa has collected $350,735 of our $375,000 goal! The IPA Board of Trustees approves an annual budget each year in combination with PNI, Inc., IPA’s wholly owned subsidiary. PNI, Inc. owns the Association’s headquarters located at 8515 Douglas Avenue in Urbandale, Iowa. A financial review was conducted by McGowen, Hurst, Clark & Smith, a CPA firm located in West Des Moines. The report was presented to the Board in April with no significant findings. Below is a summary of the association’s revenue and expense by category, for calendar year 2015. Per the review, only 13% of IPA’s income is spent on operations and administration, while the remaining 87% of our income funds association programs and initiatives, directly supporting the mission of the Iowa Pharmacy Association.
