Clinical History
79-year-old male with left plantar foot pain.

Diagnosis: Insertional Lateral Cord Plantar Fasciitis
The plantar fascia arises from the medial calcaneal tuberosity and consists of medial, central, and lateral cords.
The lateral cord of the plantar fascia has an attachment at the base of the fifth metatarsal.
Insertional lateral cord plantar fasciitis results in pain along the lateral midfoot, which can be confused for other pathologies such as peroneal tendinopathy, cuboid or metatarsal osseous injury, or TMT joint arthropathy.
