62 year old male with incidental finding of a soft tissue mass within the posterior left knee on ultrasound for Baker’s cyst.

Diagnosis: Tensor Fascia Suralis Muscle

The tensor fascia suralis is a rare accessory muscle of the knee. This muscle most commonly originates from the distal semitendinosus but can arise from any hamstring muscle and can insert into the medial gastrocnemius, posterior leg fascia, or Achilles.

Sonographic imaging of non-pathologic accessory muscles demonstrates echotexture identical to that of other normal muscles. This is important to recognize such that variant muscle anatomy is not mistaken for neoplasm.
