Dear Friends and Supporters,

We are thrilled to share Hospice Giving Foundation’s 2024 Annual Report with you. This past year has been one of meaningful change, growth, and connection—all made possible by your steadfast support.

This year’s theme, “YOU,” is a reflection of our commitment to placing the needs of our community at the heart of everything we do. How can we fund, educate, and convene to serve you better? This report offers a glimpse into how we’re answering that question. Some of the moments we’re especially proud to include the following:

• The successful leadership transition from Siobhan Greene, a visionary community leader, to Erin White, long-time Monterey County resident and experienced nonprofit professional.

• Over $1 million in grants were awarded to local organizations providing hospice care, palliative care, grief and bereavement support, and caregiving services.

• The launch of The Siobhan Greene Care and Dignity Fund, which has reached its $500,000 goal to provide emergency grants for families facing end-of-life challenges.

GRANTS

Imagine stepping into a room where every conversation carries the weight of a life story. This is the world Hospice Giving Foundation steps into with each agency we support. From hospital systems to grassroots nonprofits, each partner joins us in a shared journey by assisting families in balancing hope and acceptance at end of life.

In 2024, our Annual Grants Program awarded $1,025,000 to 12 agencies, reaching over 12,500 people. These funds support programs providing medical transportation, caregiver assistance, grief services, spiritual care, and more. The accompanying graph highlights how grantees use this support to serve community members.

Hospice Giving Foundation is honored to partner with these agencies as they deliver compassionate, personalized care for those in life’s final chapter. Together, we envision a community where no one faces this journey without resources and care.

Siobhan Greene Care & Dignity Fund

New this year, the Siobhan Greene Care & Dignity Fund will allocate up to $50,000 annually to address the unmet needs of community members at end of life. To date, the fund has provided support for the following:

• A foster youth coping with the sudden death of his father

• A family adjusting to the unexpected loss of a husband and father while facing the threat of losing their home

• A mother caring for multiple children with terminal illnesses after the heartbreaking death of one of her children

HOSPITALS IN OUR COMMUNITY

Hospice Giving Foundation collaborates with local hospitals to strengthen their capacity to deliver supportive end-of-life care. By funding innovative approaches that often require initial seed funding, we help hospitals test impactful programs with the goal of integrating them into ongoing operations. This year included the following:

• Natividad Medical Center hired a bilingual spiritual care provider to better serve Spanish-speaking patients.

• Salinas Valley Health introduced palliative acupuncture, staff-caregiver support, and expanded spiritual care initiatives.

• Montage Health trained a team of volunteers to support hospice and palliative care patients.

These efforts drive meaningful change, ensuring personalized, compassionate care for patients and families.

BREAST CANCER ASSISTANCE GROUP

New to Hospice Giving Foundation, the Breast Cancer Assistance Group has been providing financial aid to local breast cancer patients for over 26 years. With support from Hospice Giving Foundation, the Breast Cancer Assistance Group will expand its reach, offering larger grants to more patients in need. For those undergoing long-term treatment, additional assistance may also be provided.

PARTNERSHIP FOR CHILDREN

A trusted partner since 2005, Partnership for Children provides vital support to families with seriously ill children. They offer transportation to medical appointments and financial assistance for essentials such as groceries, diapers, and car seats. By alleviating financial stress, the organization empowers parents to focus on caregiving, serving as a crucial lifeline for families in our community.

HERE FOR COLLABORATION

MONTEREY COUNTY PALLIATIVE CARE COLLABORATIVE

The Monterey County Palliative Care Collaborative (Collaborative), formed in 2018, fosters meaningful coordination among professionals and institutions caring for patients with serious illnesses. Over the years, the Collaborative has become a community of practice where learning from expert speakers and colleagues bridges gaps in knowledge and skill. Sharing experiences with others engaged in similar work provides a powerful source of healing and support for professionals who serve seriously ill patients and their families.

TThe Collaborative is dedicated to identifying and meeting gaps in end-of-life services across our region and to meet patient and family needs with compassion and understanding. Each year, the Collaborative responds to emerging community needs. This year, ad hoc teams are planning a community film screening to foster end-of-life care conversations and collaborating with local homeless service providers to support advance care planning for unsheltered individuals. Notable successes include the creation of Jerry’s Place, Monterey County’s only social model hospice, which has provided a safe and compassionate space for 30 individuals to die with dignity.

WHY THE COLLABORATIVE MATTERS

In January 2025, members shared their perspectives on the Collaborative's importance:

• “The solo approach doesn’t work. People and needs get lost in systems. Because of the Collaborative, I know who I can call when a patient or family needs support.”

• “I love that we leave agency politics and competition at the door. Once we’re in this room, we’re humans connecting with humans, supporting those who are dying. This allows us to seek upstream solutions together.”

• “Sometimes, you just need your people. Palliative care is my passion, but it can be lonely. Connecting with others who share this mission is a form of self-care.”

• “In healthcare, hospice and palliative care providers are often outliers. Collaboration is key to sustainability and builds my personal resilience.”

WORKSHOPS & OUTREACH

In 2024, Hospice Giving Foundation continued its commitment to empowering our community through a series of workshops designed to deepen understanding and foster meaningful conversations about end-of-life care. These workshops provided a safe and supportive space to explore complex topics such as grief, advance care planning, and navigating difficult transitions with dignity and compassion.

Led by expert facilitators, our workshops engaged participants through collaborative discussions and shared experiences. Notable sessions included advance healthcare directive workshops, helping attendees plan for future medical decisions, and grief support sessions, offering solace and guidance for those processing loss. Some highlights of the year were bilingual workshops to honor and understand the cultural relevance of Día de los Muertos—the tie to building an altar to welcome our loved ones who have died and navigating grief through the building process.

The workshops reach diverse audiences, reflecting our commitment to accessibility and inclusivity. By creating opportunities to learn, share, and connect, these programs strengthen our mission to uplift conversations about end-of-life care and empower individuals to make informed decisions.

Through these workshops, Hospice Giving Foundation has continued to build the path toward a more compassionate community, where every individual and family feels supported during life’s most challenging moments.

HOW TO SPEAK TO CHILDREN ABOUT DEATH: INSIGHTS

FROM ENRIQUE MORALES, LMFT

Talking about death can be one of the most emotional challenges for parents, but these conversations are vital for helping children process loss and build resilience. Hospice Giving Foundation partnered with Enrique Morales, LMFT with Coastal Kids Home Care, to present a Spanish-language workshop at the Hispanic Ministry in Salinas, Cómo Hablarles a los Niños sobre la Muerte (How to Speak to Children About Death). This workshop uniquely fostered open participation from Latino men, breaking traditional “macho” stereotypes, thanks to Enrique’s relatable and compassionate approach.

Why Discuss Death?

Helping children understand death as a natural part of life fosters emotional strength. Honest, age-appropriate conversations are key to avoiding confusion.

Common Misconceptions

Parents often hesitate, believing they won’t have the “right” answers or that children can’t understand. Many parents wait until children bring up the topic, missing opportunities for guidance.

Cultural Influences

In Latino communities, discussions about death often happen only when directly faced with loss, and parents may delay conversations with young children due to cultural discomfort.

How to Explain Death

Use simple, honest language for younger children and create safe spaces for teenagers to share emotions. Parents can model healthy expressions of grief to help children process their feelings.

The Role of Rituals

Rituals like Día de los Muertos offer opportunities for children to explore grief, honor loved ones, and engage in meaningful conversations about death.

Signs Professional Support May Be Needed

Changes in behavior, anxiety, or difficulty expressing emotions may signal the need for therapy or counseling.

Preparing for Loss

When a terminal illness affects the family, open communication is essential. Answer children’s questions honestly, validate their feelings, and reassure them of your support throughout the grief process.

Enrique emphasizes that while discussing death is never easy, it’s an invaluable opportunity to help children navigate grief in a healthy way. Through age-appropriate conversations, cultural rituals, and professional support, when necessary, families can provide children with the tools they need to cope. We are grateful for Enrique’s compassionate guidance and the positive impact he continues to have on our community.

THE GRATITUDE LUNCHEON

CENTERING THE CAREGIVER WITH DR. JESSICA ZITTER

The 2024 annual Gratitude Luncheon, held at the Inn at Spanish Bay, brought together Hospice Giving Foundation’s most loyal supporters, as well as local end-of-life practitioners and service providers, for an engaging conversation about caregiver support, led by physician and filmmaker, Dr. Jessica Zitter.

Dr. Zitter’s discussion centered on her powerful documentary Caregiver: A Love Story, which explores the emotional and physical challenges of caregiving through the story of a woman facing terminal illness and her husband-turned-caregiver. Following the film, former HG Foundation President and CEO, Siobhan Greene, moderated a dynamic Q&A session with the audience, which included members of Monterey County’s esteemed palliative care teams, such as Dr. John Hausdorff of Pacific Cancer Care and Dr. Kyle Youngflesh of Salinas Valley Health.

This year’s Gratitude Luncheon also marked a significant moment in the Foundation’s history with the announcement of Erin White as the new President and CEO, effective April 2024.

HEALING OUR HEALTHCARE HEROES

This past December and January, nearly 150 local healthcare providers and Hospice Giving Foundation staff attended a three-day retreat, Healing Our Healthcare Heroes, at 1440 Multiversity in Scotts Valley, thanks to a generous grant from the 1440 Foundation. Hospice Giving Foundation facilitated the opportunity, recruiting attendees from grantee organizations such as Natividad Hospital, Hospice of the Central Coast, and Coastal Kids Homecare, as well as members of the Monterey County Palliative Care Collaborative.

The Healing Our Healthcare Heroes program offers immersive experiences designed to address burnout, stress, and the unique challenges healthcare providers face. Workshop topics for the cohorts included moral injury, grief and healing, returning to joy and a sense of purpose, resilience, and self-care. Importantly, the retreats provided opportunities for local healthcare providers to connect in a relaxed setting, sharing ideas and experiences. Attendees also had access to 1440 Multiversity’s offerings, including yoga and meditation classes, guided hikes in the redwood forest, and farm-to-table meals.

By participating in the retreat, the team at Hospice Giving Foundation gained deeper connections with end-of-life and palliative care teams in our community and a better understanding of the challenges and opportunities in their daily lives.

THE SCRAMBLE CELEBRATING 40 YEARS OF PHILANTHROPY ON THE GREEN

For 40 years, Hospice Giving Foundation has teamed up with our local golf community to support compassionate care through end of life in Monterey and San Benito Counties. Hospice Giving Foundation hosted The Scramble, our prestigious golf fundraiser, on Monday, August 19, 2024, at the beautiful Monterey Peninsula Country Club. Golfers enjoyed an exciting, celebratory day playing in recognition of our mission to support personal choice, preparedness, and the highest standards of compassionate, dignified end-of-life care for individuals and their loved ones. Events like The Scramble have allowed us to fund direct patient care services, educational programming on advance planning, community outreach, online resources, and professional education. This year, with the support and generosity of our golfing donors, we raised over $180,000, which will go towards supporting end-of-life care in our community. This year’s Día de los Muertos celebration was a deeply meaningful event that united our community in remembrance, reflection, and support. Held on November 2nd, the event featured vibrant altars adorned with marigolds, photos, and offerings that honored our loved ones who have passed. Families, friends, and community members joined together to celebrate the beauty in life, taking part in cultural performances, traditional crafts, and heartfelt storytelling. Thanks to the generous support of our sponsors and donors, the event brought awareness to end-of-life conversations and raised crucial funds to expand Hospice Giving Foundation’s reach into underserved areas. Together, we normalize the conversation about end of life and celebrate a rich culture that honors those who passed away.

TREE OF LIFE

On December 8, 2024, the Tree of Life ceremony brought our community together at Portola Hotel & Spa for an evening of remembrance and unity. This cherished event honors loved ones who have passed, creating a space for healing and connection through light and music.

Guests illuminated the room with candles, each representing the memory of someone special. The ceremony featured heartfelt readings and performances from The Carmel High School Chamber Singers, The Monterey High School Golden Folk Ensemble, and Harmony Rising.

This year was particularly special as we celebrated several milestones: 40 years of Tree of Life, 40 years of the Monterey Marriott hosting our tree, 10 years hosted by Portola Hotel, and 10 years of Carmel High School performing. All of this was marked by a proclamation from Mayor Tyller Williamson.

“Demographers estimate that 109 billion people have lived and died over the course of the past 192,000 years. Given that so many have gone before us, it shouldn’t come as a shock that we will, soon enough, take our turn in this time-honored tradition! And yet, many of us aren’t well prepared to transition from this life with as much thoughtfulness and curiosity as we are taught to live. When I was given the chance 10 years ago to have my choirs provide singing for Hospice Giving Foundation's annual Tree of Life event, I knew I had to make it happen. Supporting the work of those who support end-of-life care is vital work and one that I have personally benefited from in the passing of my own father just 3 years ago. CELEBRATING SIOBHAN GREENE LEADERSHIP, COMPASSION,

AND GROWTH AT HOSPICE GIVING FOUNDATION

Last spring marked the beginning of a series of celebrations honoring Siobhan Greene, who retired from Hospice Giving Foundation on July 1, 2024. Over her remarkable 11-year tenure, Siobhan significantly transformed the Foundation, enhancing its funding, outreach, education, and community collaborations.

Her notable achievements include the following:

• Raising over $1.5 million for the Heal Together Campaign, which provided grief and bereavement training, as well as services throughout Monterey County during the COVID-19 pandemic.

• Expanding outreach to the Spanish-speaking community, particularly through countywide collaboratives like Juntos con Esperanza and prioritizing bilingual and bicultural staff recruitment.

• Co-founding the Monterey County Palliative Care Collaborative, a vital forum where healthcare professionals meet monthly to discuss the state of palliative and end-of-life care in Monterey County, initiate improvement projects, and provide cutting-edge education to elevate community care.

• Overseeing the distribution of nearly $13 million in grant funding to local palliative and end-of-life service providers.

To honor Siobhan’s extraordinary contributions to the Foundation and to our local community, the Board of Directors established the Siobhan Greene Care and Dignity Fund. This fund provides emergency grants to families navigating end-of-life challenges, covering critical expenses such as groceries, transportation, housing assistance, funeral costs, and grief counseling. Designed to address gaps where insurance falls short, the fund delivers timely support to families in need.

With thanks to the community’s incredible generosity through the MCGives! campaign, the fund has achieved its $500,000 goal. Since its launch in September 2024, it has already awarded close to $30,000 in grants to individuals and families in need.

GRATITUDE AND LEGACY

BARBARA AND JOE COLLINS HONOR A MILESTONE

For those who have undergone cancer treatment, reaching milestones in remission is an extraordinary gift. This year, long-time donor and former board member Barbara Collins celebrated her 20-year milestone in remission. Reflecting on the gratitude she and her husband Joe felt for the success of her treatment, they decided to commemorate this anniversary in a deeply meaningful way.

As a board member of Hospice Giving Foundation between 2018 and 2024, Barbara witnessed the establishment of the Jerry Rubin Foundation for Cancer Care, created in memory of Dr. Jerry Rubin,

who passed away in 2018. Dr. Rubin dedicated his life to caring for cancer patients, pioneering new treatments, and advocating for Medicare’s funding of hospice care at the national level. Barbara also saw Jerry’s Place—a social model hospice providing care for individuals with no place to die—grow from a visionary idea into a reality that has since served 30 individuals with dignity and compassion.

Dr. Jerry Rubin was also Barbara’s oncologist. In this convergence of her milestone celebration, her gratitude for her doctor’s care, and his enduring legacy, Barbara and Joe established the Sustainability Fund for Jerry’s Place at Hospice Giving Foundation. This fund ensures the ongoing impact of Jerry’s Place and honors the legacy of the physician who saved her life.

It’s a remarkable story of milestones, gratitude, and giving back.

Over the years, oncologist Dr. John Hausdorff and his colleagues at Pacific Cancer Care noticed that some patients needed hospice care but had no viable place to go in their final weeks of life. Maybe the family wasn’t equipped to care for their loved one, maybe no suitable home existed.

“We talked about how nice it would be to have a place for hospice patients who had no place to be,” Hausdorff says. They looked for a location, but they had no money. It sat as “a great idea,” he says, until a few years later when a “little ad hoc unofficial group” got more serious. They hit upon the idea of leasing beds at an existing care center. The Hospice Giving Foundation provided the funds to launch at the end of 2021.

Hausdorff and his colleagues formed a nonprofit to accept the funds

and named it after their beloved late colleague, Dr. Jerome “Jerry” Rubin. The Jerry Rubin Foundation for Cancer Care used HGF’s grant to lease two beds at About Care Assisted Living in Seaside, and named it Jerry’s Place.

Hospice organizations come to Jerry’s Place to provide specialized care; the assisted living facility provides day-to-day care. As the only social model hospice in Monterey County –a nonprofit option when dying at home isn’t available, that includes collaboration with hospice services and volunteers – Jerry’s Place charges residents nothing.

The first bed was occupied a few days before Christmas in 2021. Since then they’ve served 25 clients, ranging in age from 53-96. The average length of stay is 42 days. Jerry’s Place has two part-time employees and six volunteers.

HGF awarded Jerry’s Place $200,000 for the first year which covered the beds, each costing $7,500 a month, or $90,000 per bed, but now it’s up to the Jerry Rubin Foundation to find funds. They recently got a jumpstart with a $50,000 donation from Barbara Collins, a former oncology patient of Dr. Rubin’s, and her husband Joe.

The nonprofit’s “Big Idea” for Monterey County Gives! is to raise money to lease more beds, hopefully up to six. “If we get more beds, there will be more patients,” Hausdorff says. “Like most things, if you build it, they will come.”

Hospice Giving Foundation’s Board of Directors is a dynamic, committed group of local leaders representing various industries and communities throughout Monterey County. Each Director serves on 1-2 committees and spends at least two years reviewing grants and attending site visits on the Programs and Grants Committee. The Board of Directors also serve as philanthropic leaders at Hospice Giving Foundation, generating community support through signature events, individual outreach, and personal giving.

One of the Board’s treasured practices is reflecting on a “Mission Moment” at the beginning of every quarterly Board meeting. Director Annie Berlin shared her response at a recent meeting:

What is most important to you in terms of how HG Foundation’s grants impact the community?

“Beyond the invaluable services that our non-profit hospice and palliative care providers make available to patients in Monterey and San Benito counties, the ultimate beneficiaries are the patients, families, and caregivers and the choices they make as they begin their journey.

I care about the choice to provide information and begin the conversations about options for care, including the all-important discussion with families about choices for palliative and hospice care. The choices for families to learn, plan, and prepare for care is also a top priority. And perhaps among the most overlooked and under-resourced, are the resources made available to caregivers and their role with patients.

Hospice Giving Foundation makes the choice to support non-profit palliative care and end-of-life providers through grant making in Monterey and San Benito Counties.

It’s all local, it’s all relevant and it’s all accessible.”

Hospice Giving Foundation honored Sarah Hills as our 2024 agency honoree at the annual National Philanthropy Day luncheon on November 15. Sarah dedicated nine years of service to HG Foundation’s Board of Directors, including serving a term as Board Chair from 2019 to 2021.

GETTING TO KNOW ERIN WHITE, HG FOUNDATION’S

NEW PRESIDENT AND CEO

One of the biggest accomplishments of 2024 was selecting a new leader for Hospice Giving Foundation. We are pleased to introduce you to our new President & CEO, Erin White. Originally from Maryland, Erin moved to the Monterey Peninsula in 2004. Her inspiring journey from the world of education while working at Santa Catalina School and York School will aid her in leading the Hospice Giving Foundation in 2025.

From Education to Hospice Care: A Journey of Purpose

“I’m proud to have been part of Santa Catalina School's transformative journey, where we led a $50 million campaign to build a state-of-the-art mathematics and science center for women as well as funding for financial aid. This initiative started during the 2008 recession and wrapped up amid the COVID-19 pandemic, highlighting the resilience and determination of our community.”

On Balancing Career and Personal Life: What do you do in your free time?

“Reflecting on my journey, both in education and now at Hospice Giving Foundation, I've learned the importance of self-care, including eating well, exercising, and nurturing friendships to maintain balance as a mother and professional. Creating a supportive work environment is crucial, and surrounding oneself with like-minded people can make all the difference. Working mothers often face unique challenges, but prioritizing well-being is key to being effective both at work and at home.”

What has been the most challenging aspect of transitioning between these two very different careers?

“I was a philosophy major in college, and I'm just someone who is very committed to ideas of meaning and purpose in our lives, and what it means to live a meaningful life. I do think that questions of our mortality, and the mortality of those around us, really bring us to the crux of the answer to that question. Transitioning from education to working with Hospice Giving Foundation felt natural given my philosophical background and interest in exploring life's meaning.”

What new and exciting things would you like to share that will be happening for Hospice Giving Foundation in 2025?

“The thing I'm most excited about is the Siobhan Greene Care and Dignity Fund which provides emergency grants for families facing end-of-life challenges. Whether it's groceries, rent, or educational support, this fund helps fill gaps that traditional insurance might not cover. It's a testament to the power of compassion and community support during the toughest times. We’re able to fill in the gaps for families that are facing end of life and help them with those challenges that occur that can't be touched or seen by insurance reimbursements.”

“My great highlight this 2024 was to schedule workshops in advance, leave instructions and a plan so Día de los Muertos event and the Marketing and Communications tasks could continue to be done during my maternity leave.”
