ANNUAL REPORT 2022 CARING. GIVING. TOGETHER.

Celebrating

25

years

MESSAGE FROM THE PRESIDENT Dear Friends and Supporters,

President and CEO Siobhan M. Greene Board Chair Bill Tebbe

Preparing our annual report is a time of reflection. We consider all that transpired in the prior year and feel incredibly fortunate to have been part of this work. In 2022 we saw the realization of Heal Together with funding of over $1.5 million. Our grantmaking surpassed $30 million in our 25-year history. Our outreach touched countless lives and we had the opportunity to gather in person and feel the outpouring of support from our community. The agencies to whom we grant funds encouraged us with their kindness. Our wonderful donors stood with us each step of the way. We found hope in simple moments, gratification from gestures large and small, and inspiration in the beauty of life through both difficult times as well as times of joy.

This annual report shares some of the highlights of this year and gives us a platform to express our gratitude. What may not be as evident is the impact of HG Foundation and the agencies we’ve supported for over 25 years. This work is powerful because of the people who work to preserve dignity through all phases of life, who broach difficult topics in the service of easing suffering, and who advocate for everyone to have access to care and services.

Hospice Giving Foundation is a 501(c)(3) charitable organization.

Compassion seems to be the center of it all--Board Directors who devote their time, staff who define ‘small but mighty’, and donors, supporters, and providers who grace us with their generosity of resources, spirit, and wisdom. For over 25 years we’ve been blessed. We are deeply grateful.

LOVE AND COMMUNITY: GIVING TOGETHER It doesn’t take long in a conversation with Vicki and Gordon Rosenberg to see that their love and kindness towards each other extends out to the rest of their world. What else is very clear, is that humor has a big place in their lives. They met in the 1970’s. Vicki, a graduate of Salinas High School, was a nurse at SVMH. Gordon, a walnut farmer, was living in San Ardo at the ranch owned by his family since the 1870’s. Vicki’s work brought her close to people who were dying. But in 1975 when her dad died from cancer, it touched her personally. She shared that it was his wish to die at home, but it couldn’t be managed. Instead, he died in the hospital. Her mother was very clear that was not to happen to her. This began years of family conversations, plans, and discussions about dying—conversations that few families share. Elizabeth Kubler Ross’s philosophy was just making headway in the late 70’s. Vicki and her mom went to hear her speak at a conference in Hayward. That was 48 years ago, and Vicki reflected that it takes time, dedication, commitment, and dollars for change to happen. She couldn’t be with her dad when he died but she was darn sure she was going to be with her mom. Vicki’s mother died in 2020 at her home in Idaho. It was peaceful and on her terms. Most importantly, her daughters were at her side. Vicki and Gordon have been supporters of Hospice Giving Foundation for over 20 years. Vicki served on the board from 2005 - 2011. When I asked her why Hospice Giving Foundation and what was important to them, she reflected on their family values. “I remember the values my mom and dad instilled in me on volunteering and giving back to the community and doing for others. They gave of their time. Being with Gordon and having our monetary resources, we can now give in philanthropic ways to make a difference. The gift of time and the gift of money coming together.” She also emphasized the importance of our outreach, especially in South County communities, as well as the progress the organization has made since her service on the board. As philanthropists, they look for a sense of belonging and photo credits: tom o'neal

connection, a strong staff and board, and personalized involvement. She said she finds all of this in Hospice Giving Foundation. Most recently, Vicki has been a student of Day Schildkret, an artist and writer who devotes his work to creating rituals, processes for grieving, and exploring ways to find beauty in dying as a natural part of life. This inspirational work has given her new tools and perspectives that help her honor those who die. Creating ‘morning altars’ is part of his philosophy that brings meaning to passing and the impermanence of life. This contributed to Vicki and Gordon’s support of Heal Together. It all ties together in this: Vicki and Gordon’s support at its heart is to help families plan and experience a good death. Death is always hard, but it can be dignified when embraced with love, rituals, thoughtfulness, and yes, a bit of humor. All of us at Hospice Giving Foundation are honored by the gifts Vicki and Gordon Rosenberg share with us.

LATE 1970s | Hospice in our community

began as a six-bed cancer recovery facility in Carmel Valley founded by Dr. Jerry Rubin. After various name changes and organizational structures, the facility was named Hospice of the Central Coast.

2007 | Hospice Foundation marks 10 years with two honors in recognition of excellence as a local nonprofit organization, one from KSBW-TV and Union Bank, the second from the Monterey Peninsula Chamber of Commerce.

2010 | More than $17 million in grants

distributed and Hospice Foundation is again named by the Monterey Peninsula Chamber of Commerce as Best Non-Profit.

2013 | Alice Kinsler, President/CEO retires after 15 years of service; Siobhan Greene assumes role of President/CEO in August.

2017 | Expands leadership

role in the community, focusing on increasing services to Latinx communities and supports the formation of the Monterey County Palliative Care Collaborative.

2021 | Establishes a $1.5

MM grant called Heal Together to expand grief and bereavement care.

2014 | Hospice Foundation is renamed Hospice Giving Foundation (also known as HG Foundation).

2020 | Launch of HOPEGives, a community health movement to foster

cultural attitudes on well-being, quality of life, and access for all people to receive dignified end-of-life care and services. In response to the pandemic, HG Foundation implements emergency funding to respond to critical needs of families and facilitates the Skilled Nursing Support Network on a weekly basis.

2022 | Hospice Giving Foundation’s grants surpass $30MM as the organization celebrates its 25th anniversary of promoting dignity, compassion, and excellence in care through end of life.

SUPPORTING THE COMMUNITY WITH EDUCATION To normalize end-of-life care conversations and promote preparedness HG Foundation helps our diverse community through culturally relevant resources, education, and outreach. In 2022 our focus was to emerge back to the in-person world with educational programming and outreach events. Our workshops continued to be available virtually with a variety of topics in English and Spanish, and activities to engage our community. Our new programs included an online Spanish training to Center for Community Advocacy's (CCA) program “Mujeres en Acción” (Women in Action), whereby community leaders learned about the importance of Advance Care Planning to help us deliver the message to their families and communities. Our language and cultural barriers webinar offered guidance, experience, and suggestions to physicians/clinicians to help make the provider-interpreter experience good for the patient when having conversations about end-of-life care.

YOUR GIFTS AT WORK Hospice Giving Foundation's exclusive focus on serious illness and end-of-life care enables us to reach deeply into the community to support important and groundbreaking work. This past year, filled with many accomplishments, included: • Final funding of the two- year Heal Together Initiative, bringing the total investment to $1,521,014 • Funding a startup residential social model hospice program • Extensive outreach and education across the region • Launching a county-wide Advance Care Planning Task Force GRANTS -- AN INVESTMENT FOR END-OF-LIFE CARE AND SERVICES IN 2022 Hospice Giving Foundation awarded a total of $1,733,839 (including the final payment of the Heal Together initiative). Requests from our community partners totaled more than $1.2 million. • The process is designed to ensure transparency, equitable review, and thoughtful conversations with our grantees. We work to promote greater accountability and provide support to help small organizations enact best-practice governance and nonprofit operations. • Every year, Hospice Giving Foundation offers capacity and program support to our grantees, complementing our role as a funder. BUILDING COMMUNITY RELATIONSHIPS Over the years we’ve seen the powerful benefit and take great pride in collaborating with our local system of care providers to explore how to elevate care in our community.

PROJECT TALK

When Erandi Escareño, our bilingual, bicultural Director of Community Outreach & Education joined our team full-time in May 2021, it was the start of more impactful programs. With her leadership, we continue to build stronger relationships with schools, faith communities, hospitals/clinics, public libraries, Spanish media, and social service programs. Other Collaboratives include Juntos con Esperanza and the Monterey County Palliative Care Collaborative. In our work we: • Explore ways to reach communities more effectively • Engage new partners to create meaningful connections, especially relating to culture, religion, and language • Convene, support, and listen, focusing on solutions alongside our partners and communities

Hospice Giving Foundation was selected as one of 75 agencies approved to participate in Project Talk, a 5-year randomized controlled research initiative funded by the National Institutes of Health (NIH). The Project Talk Trial aims to engage underserved communities in advance care planning and empower them to start conversations about their end-of-life goals and wishes. Erandi conducted the project in Soledad with Spanish-speaking participants in February 2023. ADVANCE CARE PLANNING TASK FORCE The ACP Task Force will collectively host a series of educational programs in mid-April 2023, in conjunction with National Healthcare Decisions Day which exists to inspire, educate, and empower the public and providers about the importance of advance care planning.

Hospice Giving Foundation established a county-wide Advance Care Planning (ACP) Task Force to strengthen and improve effective advance healthcare planning across Monterey County. The ACP Task Force includes representatives from the major hospitals (Montage, SVMH, Natividad, Mee Memorial), attorneys/estate planners, fiduciaries, providers of end-of-life care and education, non-profit agencies, faith leaders, as well as consultants who work directly with families. The ACP Task Force works to: • Coordinate efforts for greater community engagement in advance care planning • Improve county-wide systems • Standardize documentation and create better online access • Address the ongoing need for provider education so these conversations are more frequent in a healthcare setting

IMPACT GRANTS

THE SCRAMBLE

This year we were proud to support ‘Jerry’s Place’, a new residential/social model hospice program for those unable to afford care or have no place in which to peacefully die. This project reflects our commitment to access and equity in end-of-life care and we hope to see it grow and thrive. This new nonprofit was established in honor of the late Dr. Jerry Rubin, a local pioneer in hospice care. In keeping with our philosophy as a funder, we will continue to help support internal capacity development as well as provide operational funding.

In Spring 2022, Joe Martello and David Glickman teamed up to make the return of the beloved Golf Scramble a day to remember! With a full field, a clinic with Amy Alcott, and the stunning setting of Monterey Peninsula Country Club, all the ingredients were in place. On April 26th the players showed up with energy and enthusiasm to be back together for this great event. Team Pacific Ag Rentals took first place while other prizes recognized the longest drive, closest to the pin, and placements in the first and second flights.

CONTINUED COMMITMENT

Despite wild winds and weather that tested everyone’s patience and ability, the day was a huge success. Through the generosity of our sponsors, players, and guests, the Scramble raised over $144,000. Watch for a date in the Fall of 2024 for the next Golf Scramble at Monterey Peninsula Country Club.

FASHION + COMPASSION

A DAY OF GRACE

Hospice Giving Foundation’s first-ever, beautiful Fashion + Compassion luncheon was held on August 24, 2022, at Monterey Peninsula Country Club. The room was stunning with brilliantly colored tablecloths and exquisite flowers from Swenson + Silacci. The models gracefully and enthusiastically brought the classic designs from Debra C’s to life and the guests made this stylish event so memorable. This elegant event raised just over $100,000 thanks to the sponsors, fabulous raffle items, our guests, and supporters.

DÍA DE LOS MUERTOS

TO REMEMBER IS TO LIVE AGAIN

Our second annual Día de los Muertos community event was held on November 2, 2022, at Hartnell College in Salinas. This event was in collaboration with Hijos del Sol Arts Productions and Hartnell College. Families and friends gathered to remember and honor their deceased loved ones with a procession, ‘calaveritas literarias’ (poetry), beautiful altars, marigold flowers, live music with a Mariachi and folkloric performances. New this year, community members crafted their offerings a day before during our arts and crafts workshops at Hartnell College. Those offerings were placed before the ‘Ofrenda Mayor’ (Greater Offering), which was curated by local muralist José Ortiz and his team Hijos del Sol Arts Productions. As part of the tradition, community members enjoyed delicious ‘pan de muerto,’ tamales and hot chocolate from La Plaza Bakery and Panaderia Tres Hermanos. This event was successful thanks to our sponsors, community members, planning committee, and supporters.

TREE OF LIFE Our time-honored Tree of Life Celebration was held on Dec. 11th at the beautiful Portola Hotel and Spa, featuring the Carmel High School Chamber Choir and jazz quintet Andrea’s Fault. The musical program was both soothing and uplifting. As Andrea sang ‘Unforgettable’, guests reflected on people in their lives who are truly unforgettable. The Carmel High Chamber Choir was spectacular. Chaplain Robin Wells provided a beautiful blessing and then the tree atop the Monterey Marriot was lit with lights honoring loved ones.

"In our time together we have experienced emotions and memories only music can touch. We have acknowledged the loss of people in our lives who have shaped us into who we are. And in being present, we hold on to that which is most important, and we learn to let go of what really doesn't matter. May you take from this gathering strength for today and bright hope for tomorrow. May you love so much, that in the fullness of time, each goodbye becomes a greeting to a joyful future yet to come." - Robin Wells, MDiv 17

INFORME ANUAL 2022 CUIDANDO. DANDO. JUNTOS.

Celebrando

25

President and CEO Siobhan M. Greene Board Chair Bill Tebbe

Presidenta/directora ejecutiva Siobhan M. Greene

- Robin Wells, MDiv
