The Difference You Make Every patient’s health care journey is unique. Thanks to generous community support, Hoag’s Care Coordination Program streamlines care across the Hoag enterprise, including all 16 of Hoag’s urgent cares and five of its health centers. Coordinators provide vital assistance in navigating the complexities of health care so patients can rest assured that their experience will be as stress-free as possible. For physicians and staff, care coordinators also make a meaningful impact on their capacity to meet patients’ needs. From scheduling and insurance preauthorization to imaging, prescriptions, follow-up appointments, and more, care coordinators serve as liaisons between patients and physicians’ offices to ensure the efficient delivery of care and an unparalleled patient experience.
New Care Coordinator Joins Hoag As Hoag expands to meet the needs of more communities throughout Orange County, our Care Coordination Program is also growing. In 2023, the program welcomed a new coordinator at the Hoag Health Center in Huntington Beach. Having care coordinators onsite provides a high-touch patient experience, reduces call volumes, and alleviates the flow of traffic at the registration desk. With an additional care coordinator joining the team, the Care Coordination Program now includes four care coordinators and a dedicated patient liaison at the Sun Family Campus in Irvine to support care navigation during construction of the expanded campus.
Off-Site Coordination Assistance Expands in the Community In partnership with the Masimo Wellness Center at the global medical technology company’s Irvine headquarters, Hoag recently expanded its care coordination services to the worksite clinic. Staffed by a Hoag-registered nurse practitioner and full-time wellness coach, the walkin clinic offers employees direct access to integrated wellness and medical services, including onsite lab draws, flu shots, screenings, education, and more. Patients needing more care are referred to a Hoag facility for imaging, follow-up exams, and treatment. By collaborating with the Masimo Wellness Center, Hoag’s care coordinators are expediting and elevating the care the clinic provides, removing barriers to referrals and scheduling.
