Caduceus Session 2011 From the Editor Greetings, members of the Faculty. This is the 1st issue of the 42nd Volume of Caduceus, which has been with the Faculty since 1922.

Publishing is never an easy task. Researching, writing, designing layouts, photographing, typesetting - all of them demand time and diligence. Yet, hard as our work may be, words are just words without readers. The Editorial Board sincerely hopes that Caduceus will inspire you in your leisure, and relieve your pressure of studying, one of the few things that remain unchanged after 89 years.

“Infunity”, id est infinite fun, is how we define this issue. A brand-new, refreshing design carries both rigorous insight and delightful bits, such as anecdotes, jokes and crossword puzzles. By this, we hope to break the link between knowledge and boredom.

New layout comes also with history, culture and bonding. The Chinese calligraphy of 「啟思」on the cover, is written by Professor Sum-Ping Lee, the Dean of Medical Faculty, who had once served in Caduceus during his study. His kind help has made what we could dare dream of a reality. No words would capture our unlimited gratitude to Professor Lee. Yes, fellow members of the Editorial Board, it is here. After the long nights of work, our Caduceus is here.

我們需要怎樣的國民教育？ 在狂熱與冷漠之間，「愛國」將魂歸何處？ 司徒漢津 (M15)

五大標竿 事件一 政府提倡增加基本法課程 施政報告建議增加學習《基本法》的時數，除了建 立《基本法》試題庫讓學校作爲試題，並會檢視現行 中、小學的「德育及公民教育」課程，以「德育及國 民教育科」取而代之。

上五件事件，都 與國民教育息息 相關。政府要光明正大推行 「國民教育」，筆者完全舉 腳贊成，但必須先弄清楚， 我們究竟需要怎樣的國民教 育？

事件二 中學教師籲港澳辦多管傳媒 香港一位中文教師王穎與港澳辦主任王光亞會面期 間，大讚香港的國民教育做得不錯，「升國旗和國歌 奏起時表現出應有禮儀，但仍有不足」。她批評香港 傳媒只發放「負面消息」，削弱國民教育，建議港澳 辦對傳媒「多做一點工夫」。其言論惹來傳媒及教育 界極大反響。 事件三 港人對「中國人」認同感下跌 港大民意調查發現市民對於「香港人」認同感為 36％，較半年前增加11％。民意研究計劃總監鍾庭耀 認爲原因是香港運動員在廣州亞運取得佳績以及馬 尼拉人質事件。另外，市民對「中國人」的認同感為 21％，減少7％，鍾認為與內地阻撓劉曉波領取諾貝爾 和平獎有關。 事件四 調查指國歌短片「增國民身分認同」 公民教育委員會2月調查顯示五成受訪者認為《輝 煌里程》國歌宣傳短片能有效加強公衆對國民身分的 認同感。 事件五 人代政協推廣國民教育 香港友好協進會組織50名人大代表、政協委員到學 校主持講座，介紹最新國情發展。

歷史教育： 理性與感性並重 筆者認為，真正的國民 教育，必須強調歷史教育。 歷史教育除了傳授歷史知識 外，更重要的是喚醒年青人 的國家民族感情。我們經常 強調的「國民意識」，其實 包括理性的「文化認知」和 感性的「道德動機」兩個元 素 [1]。所謂「文化認知」是 指個人對國家歷史文化的理

性認識，進而自我反省，謙 卑自持。後者是感性的愛國 情操，國難當前，在「道德 動機」驅使下同仇敵愾，保 衛國土。 教育當局自2000年推 出課程改革後，逾百間中學 選擇切割中史科，將中史與 通識科融合或改為綜合人文 科。世界歷史、地理、經濟 等科目是以理性為本，放眼 世界，但中史強調理性與感 性結合，從祖國的角度探討

歷史。將中國歷史科與其他 科目合併，並非不可探索， 但必須保持中史教育的元 素，同時不能失掉民族精神 和歷史感情的培養。 在新學制下，通識成為 必修科之一，其中「現代中 國」課題分為「中國的改革 開放」和「中華文化與現代 生活」兩部分，對於中華民 族歷史幾乎沒有提及，更遑 論以史為鑒、痛陳國事。香

港八成以上大學生認為自己 對國家認識程度偏低，而現 在中小學生對歷史文化知識 的貧乏，已是不爭的事實。 政府一方面提倡國民教育， 另一方面卻輕視歷史教育的 重要性，要知道國民教育是 建基於歷史。

要求學生熟讀基本法， 學習國旗、國歌，本來毫無 爭議，問題在於特區政府的 所謂國情教育，雖然沒有禁 止討論國家負面情況，但實 際上避而不談。只要看看國 民教育中心和基本法教育網 站，尤其是「教學資源」和 「國情知識」的部分，隨處 何謂國情教育？ 可見歌頌國家成就的內容， 觀 乎 特 區 政 府 自 回 歸 對於內地維權、劉曉波、三 以 來 的 所 謂 「 國 情 教 育 」 聚氰胺毒奶事件，卻隻字不 ， 只 聚 焦 於 「 國 歌 宣 傳 短 提。教育當局雖然經常強調 片」、「國旗和國歌光碟」 獨立思考、理性批判，但對 、「升旗禮訓練」、「推廣 於當代國情，完全是避重就 基本法」、「內地交流團」 輕的「選擇性失明」。因此 等內容。然而，單單靠唱國 國民教育表面上客觀持平， 歌、升國旗、交流團，真的 實質刻意迴避。傳媒擔心國 能夠做到國民教育嗎？ 民教育淪為「洗腦教育」， 絕非毫無根據的政治抹黑。 健全的國情教育，對於 歷史的黑暗面更應該正視， 大家只要看看我們的曾 並以此為鑒。只讓學生看到 特首，對於劉曉波獲頒諾貝 改 革 開 放 三 十 年 的 經 濟 發 爾和平獎的回應是「不予置 展，而忽略文革、大躍進、 評」，這正是國情教育的反 六四屠城的沉痛歷史；只談 面教材。而當我們的建制派 航空科技的進步、北京奧運 議 員 每 年 冷 待 六 四 平 反 議 的成功，而不談法治人權的 案，連保釣人士出海保衛領 落後、貪污腐敗的風氣、公 土也受到當局阻撓，我們到 民意識的貧乏，這些都是極 底能否期望國民教育真正做 危險的做法。 到正視歷史，增加港人的國 民認同？ 時事評論員練乙錚認為 健全的國情教育，必須著重 公民教育 對比，讓學生了解「文宣中 與國民教育 國」和「真實中國」兩者之 公民教育的目的，是培 間的差距 [2]。例如《人民日 養對社會、國家以至世界有 報》強調「我國公民享有言 貢獻的良好公民，強調公民 論出版自由是不爭的事實」 責任與權利，尊重人權自 ，到底是否真實？溫家寶與 由。值得強調的是，真正的 上訪民衆見面，信訪局卻在 公民教育，是一種「自我教 現場搶記者相機刪照片，破 育」，而非「被教育」。 冰之旅實質成效如何？

德國與日本的歷史教育 德國與日本同為二次世界大戰的戰敗國，但在歷史教育的處理手法上迥然不同。 德國教育法明確規定，歷史課本必須包含納粹時期歷史內容，教師必須深入講解納粹 主義中對猶太人的迫害，尤其是關於集中營和大屠殺的暴行。德國執政者直接面對納 粹暴行的態度，與香港一眾高官、親中政客對於八九民運的迴避態度，截然不同。 相反地，日本大部分主流教科書，往往對日軍暴行輕描淡寫，淡化其侵略行為， 部分更鼓吹日本發動戰爭有利。日本文部科學省面對國內外的批評，一直強調日本教 科書並非由國家出版，但基於言論及出版自由，不會過問當中的價值觀和史觀。日本 文部科學省2007年公佈的新高中教科書審核結果顯示，日本教科書對於南京大屠殺的 死亡數字與真實相差甚遠，「關於南京大屠殺罹難者的具體數字有很多種說法，一種 說法稱超過10萬人，另一種說法稱這個數字大約為4萬人」。 有人認爲，德日兩國對於歷史問題處理手法的不同，是因爲德國人有罪疚意識 (guilt)，因此坦承戰爭罪責，並將防止極右分子死灰復燃視為道德義務。而日本有羞 恥意識(shame)，因此不敢正視戰爭罪行。正因爲此，日本人永遠不忘記廣島原爆， 卻不會想到南京大屠殺，而德國人每提起二戰便想起奧斯威辛(Auschwitz)集中營。 在德國，牢記歷史教訓，已經成爲人民的道德義務和執政者的政治責任。正視歷 史，香港做到嗎？

西方民主政體由人民選 出政府，政治權利由人民擁 有，而公民教育的重要性， 是教育人民什麽是權利與義 務，什麽是憲法，什麽是法 治精神，什麽是人人平等、 自由民主等普世價值。這種 公民教育，除了培養人民的 公民責任意識，也讓公民認 識憲法所授權公民的權利。 人民擁有言論、出版和集會 自由，面對專橫和不道德的 政府，可以訴諸法律，並擁 有選舉權及被選舉權，可以 透過選票行使其公民力量。 內地高官在談論自由民

你會稱自己為 「中國人」？ 「香港人」？ 王穎老師其實提出了一 點很值得我們深思：為什麽 香港人的「國民意識好像一 加強就突然間又減弱」呢？ 好像北京奧運國威大振，或 者汶川地震國難當前，香港 人自視為「中國人」的意識 都會上升，然後又會忽然減 弱。 不少推動國民教育的團 體，經常把調查結果解讀為

的政治和文明素質的不足。 歸根究底，這才是造成港人 對中國人身份認同感不足的 主因，並非愛國教育做得不 足，更不是傳媒報道不夠正 面。 支聯會已故主席司徒華 曾經說過：「我們國民身份 認同並不是愛一個政權、一 個政黨，而是愛中國的同 胞，愛中國的大地山河，愛 中國的傳統文化。」培養國 民身份認同只是國民教育的 一部分，最重要的是培養一 顆關心國家的心，了解中國 的政治、經濟、民生，思考 國家的問題，對國家有承擔 感。否則，香港人對於國家 的認同感只會如王老師所 說，漂浮不定。所謂國民教 育，也只會是徒具軀殼。我 們需要的是情理兼備、正視 歷史的國民教育，而不是單 向灌輸、形式主義、配合國 策、政治正確的順民教育。 ■

主等普世價值時，總愛搬出 「普世價值必須符合國情」 這句子用作擋箭牌，提醒中 國人不要被西方價值觀牽著 鼻子走。然而，對公民素質 的道德要求，並沒有民族、 種族或國家差異。這種「公 民自我教育」，確保人民的 公民權利得到保障，正正是 內地所欠缺的。

香港的國民教育不足，呼籲 當局加強國民教育。與其說 這是因為香港國民教育不 足，不如說這是國民教育的 局限。即使是自稱「中國 人」，也不免存在潛在的分 離意識。自稱「香港人」 的，不完全認同的也未必是 自己的中國人身份，而可能 是中共對自由民主的輕視、 內地官場的黑暗、貪污腐敗

亡羊補牢？護士加薪之謎 護士大量流失，嚴重影響醫護行業 謝汶珊 (M15)

少大學生以薪 金的高低來決定 職業的優劣，這是一個現實 的問題，一份高薪厚職，有 誰不希望擁有呢？在此，我 有一個好消息告訴所有護士 們：各醫院為了挽留人才， 都紛紛加薪，以仁安醫院來 說，註冊護士的月薪起薪點 為二萬六千元，而養和醫院 更高達二萬八千多元！可見 護士這行業的「錢途」實為 不錯。

離職呢？我相信原因並不複 雜：辛苦。相信許多人也知 道護士是一厭惡性行業，雖 然護士於培訓期間也深明此 性質，但是，當真正地，日 復日地去工作時，也難免會 感到吃不消。加上近年護士 人手不足，在職護士就更加 忙得不可開交，辛苦程度實 在不足為外人道！許多現任 護士也被壓得透不過氣來， 因而毅然辭職。當大量護士 請辭後，護士人手不足的情 況便每況愈下：大大加重了 在職的護士的工作量，令人 手更加不足，護士們亦愈來 愈辛苦，造成更多人辭職 ──一個惡性循環。

政府消取護士學校計劃。起 初，政府希望將所有受訓名 額撥歸大學開辦護士學位課 程，但政府最後並無將資源 調配到大學，令過去數年新 畢業的護士人數不足，現時 的護士荒就是政府當年種下 惡果。加上私營醫院發展開 始蓬勃，護士的需求更是大 大增加了！但是，剛剛畢業 的護士卻因為護士學校的關 閉而減少，供應減少再加上 需求增加，自然做成了今天 的局面。病人互助組織聯盟 副主席張德喜曾說：「醫管 局當時為節省資源關閉護士 學校，又想崇尚學識，將學 徒制變成學位化，但此舉變 相提高入讀要求，「唔係咁 多人讀得到」（並非所有人 有資格入讀），「政府依家 應該真係知道錯」（現在政 府知道當時做錯了），亦反 映當局短視，只考慮解決眼 前問題，頭痛醫頭。」有鑑 於此，政府近年重啟護士學 校，希望有助舒緩問題。但 是，即使增加學位，也不能 在短期內增加註冊護士的數 目，未能解決燃眉之急。然 而，樂觀而言，在長遠來 說，對護士行業實在有很大 的幫助。 ■

惠澤藥房 非牟利社區藥房先驅 石志榮 (N14)、何麗敏 (N14)

惠

澤藥房於二零零九年由聖雅各 福群會創立，是全港首家非牟 利社區藥房，成立的目的是服務社區的弱 勢社群，減輕他們的醫療負擔，以及教育 大眾藥物知識。我們有幸邀請到發起人社 工岑智榮先生，以及香港醫院藥劑師學會 會長蘇曜華先生接受訪問。

攜手合作 意義深遠 在日常的工作中，岑先生意識到長者 對藥物毫無認知。他說︰「老人家都喜歡 把藥物放進冰箱中，認為可以令存放的時 間更長，反映他們誤以為藥物跟食物的處

理方法無異。其實他們有這種的想法是因 為他們節儉的性格，他們都不喜歡棄掉藥 物，反而會把過期的藥物保存，留待身體 不適時服用。」岑先生繼續說︰「有不少 長者一天需要服用七至八種藥。當他們對 這些藥物有疑問的時候，卻不敢詢問醫 生，因為他們怕阻礙別人看病，不想成為 別人的負累。他們就只好向社工請教，奈

何我們對藥物亦是一無所知。」 在二零零八年尾，得到蘇先生的全力支 持，一系列的藥物輔導得以在社區推行， 活動的成效十分顯著，因此岑先生認為有 需要讓更多的市民認識藥物。經過大約一 年的時間，全港首間非牟利的社區藥房終 於成立。

藥物教育 不容忽視 為了提高長期病患者對藥物的認知，香 港醫院藥劑師學會與聖雅各福群會合作， 為有需要的人提供藥療輔導服務。這計劃 分為小組輔導和個 別家訪服務。在小 組輔導中，藥劑師 和社工起了相輔相 成的作用。 中心的社工會 先透過報章邀請長 期病患者參加，由 藥劑師學會成員給 予專門的藥療輔 導，如一名藥劑師 對十個病人講解服 用藥物的概念，並 鼓勵長者詢問醫 生用藥的情況。 岑先生表示︰「藥物教育是藥房的一大支 柱 ， 教 導 市 民 『 知 藥 用 藥 』 的 概 念 是 很 重要的。」

從藥房走入社區 為隱蔽病者提供藥物輔導 除了提供藥物輔導小組和舉辦藥物教育 講座，中心如何為隱蔽的長期病患者給予

藥物輔導呢？蘇先生說︰「現階段仍有一 些長者不願到中心接受藥物輔導。當他們 向前線慈善義工尋求幫助時，義工們對專 門的藥物知識亦有限，所以『到戶』是必 要的。」蘇先生所提及的「到戶」是指外 展家訪藥物輔導。他和其學會成員定期在 每個月的一個星期六走入社區，為有需要 的病者提供上門藥物輔導。這個服務不但

其實，整個計劃的目的是強化他們對藥物 的認知，即今天常提及的『empower』， 而不是令他們有所依賴。」岑先生繼續補 充說︰「藥物輔導的跟進工作則由社工負 責。他們會提議長者嘗試自行執藥，適當 地放入中心贈送的藥盒中，而社工亦能按 照drug charts來核對。」

惠澤藥房

能對長期病患者灌輸藥物性質的知識，最 重要的是提升他們自我管理藥物的能力。 蘇先生接著分享家訪藥物輔導的經過。 「到訪後，我們會和那些公公婆婆把儲藏 在超級市場膠袋的藥物全找出，幫他們分 類和講解哪些需要服用、哪些藥物已過使 用期限、哪種重複了藥效。」為了更有效 幫助記性比較差的老友記，他們更有一系 列的輔助工具及跟進服務。 蘇先生說道︰「我們會為他們度身訂 做drug charts，教他們如何適當地管理藥 物繼而服用，把服食藥物的概念弄清楚。 10 | Caduceus April 2011

除此之外，中心還設有電提服藥服務， 由地區義工定期致電長者，詢問他們用藥 的情況，然後在各方面衡量藥劑師再到訪 的需要。

藥療服務的成效 當被問及藥物輔導計劃的成效時，岑 先生表示雖然他們沒有實實在在的數字統 計，但是他們的成效可從實務得知的。例 如，有一位婆婆致電到中心報稱這幾年來 服藥後都感到頭暈，雖然他不知道對她的 幫助是否奏效，但仍然提議她把藥帶到中 心來給藥劑師看看，結果藥劑師發現婆婆

服用了雙重份量的降血壓藥。「婆 婆再打電話來道謝，更表示服藥後 再沒有了頭暈的徵狀。隨後，每逢 她遇到有關藥物問題，都會打電話 到中心來查詢，可見她對藥物的意 識已經有所提高。」他安慰地說。

任重道遠 對於服務社區充滿熱誠的藥劑 同學，蘇先生有這樣的建議︰「兩 所大學的藥劑學生都有充足的訓練 和教育，所以知識並不是考慮聘請 的重要因素。反之，參加面試的藥 劑師是否懷著一股熱誠才是首要的 考慮因素，例如我們會留意他有否 參與義工服務，從而去了解他是否 對服務社區服務有所承擔。」 岑先生補充說︰「我認為有兩 點藥劑同學要留意。第一，同學要 清楚自己當初為何選擇藥劑，要清 楚自己畢業後有沒有堅持當初的理 想，只要堅持著自己的理想，才可 以 面 對 將 來 的 種 種 問 題 ， 因 為 在 藥物教育 社會服務機構工作，藥劑師除了處理藥物 外，還必須關心病人的情況，了解他們背 後的問題，千萬不要獨善其身﹔第二，身 為這間非政府藥房的一份子，我們都有責 任走進社區，更要親身上門探訪，提供到 戶服務，所以藥劑同學必須有充分的心理 準備，不會只在舒適的辦公室工作。」

後記 社區上有很多需要幫助的貧苦大眾，但 只有部份人可以得到外界的幫助。惠澤藥 房的成立令受惠人士圈子變得更大，為他 們解決醫療方面的問題。希望將來會有更 多的「惠澤藥房」為大眾服務。 ■

FACEBOOK的錯？ 護理學生捲入失德指控，問題本質到底何在？ 石志榮 (N14)

大護理生抱嬰 自拍被指侵私隱 的報導在各大的報章都能看 到。事緣數個本校護理學院 三年級學生於病房與剛出生 的嬰兒拍照，卻沒有得到嬰 兒家長的同意便把這些照片 上載到Facebook。其後， 一位署名「香港市民」的人 士去信本校護理學院，才揭 發是次事件。 無疑地，學院對此事感 到十分震驚，並立即作出一 連串行動，包括召見有關同 學了解事件經過，以及為一 年級護理系學生增設有關專 業道德的課堂。學院反應之 大是可理解的，畢竟事情牽

動態，有助朋友之間互相了 解，方便用家交換各種的資 訊。如此一來，各種各樣的 資料﹑圖片都很容易一傳 十、十傳百，以致不相關的 人士都可能看到你的私人圖 片或個人資料。大眾傳媒稱 這些常常在社交網站觀看別 人的個人資訊的人為「偷窺 狂」，他們的「好奇心」非 常重，常到處「關心」朋 友，甚至陌生人。誰知他們

會對你的個人資料打什麼主 意、有著什麼不軌企圖。

「萬人迷」 筆者認為除了「偷窺 狂」外，社交網站的「萬人 迷」亦是另一類值得關注 的網絡使用者。例如，他們 在Facebook內「相識滿天 下」，不論是初初相識，或 是點頭之交，他們都一概不 放過，更甚者連朋友的朋友

是「萬人迷」，他 們都有意無意地把 你個人私隱顯露在 別人眼前。就以是 次事為例，不知道 是什麼原因，同學 的相片就完完全全 地暴露在人前，包 括這位署名「香港 市民」的人士、傳 媒、以致全港市 民。另一例子就是 近來興起的「起底組」。 所謂「起底組」就是專門 收集目標人物個人資料， 然後將一切公開的一群網 民；而Facebook就是他們 常常用來找尋資料的好工 具。不管是什麼人，他們 都總是有辦法把目標人物 的個人資料拿到手，再公 諸同好。「起底組」成功 的背後，Facebook或許也 是「功不可沒」。由此而 知，Facebook散播消息的 範圍多廣、速度多快；更重 要的是，傳播的對象沒有界 限、不可預測，任何人都有 機會接觸、甚至盜用。

從 有 關 報 導 可 見，Facebook的保安並不 是那麼完美。事實上，不 單是這零星的報導反映著 Facebook的私隱保障存在 問題，更多更多的意外還在 我們不知情的情況下發生罷 了。除了Facebook外，各 類網站的保安總存在所漏 動，因此我們總不能毫無 防備地使用Facebook等社 交網站，不可以完全信賴 Facebook的保護措施。反 之，用家要懂得自我保護， 上載到網站的圖片需要先經 過篩選，特別是那些牽涉到 自己或別人私隱的圖片。

島第一核電廠事故動搖了國際 社會對核能發電的信心，歐洲 多國包括瑞士、波蘭和意大利等，紛紛宣 佈擱置或暫延發展核能，以重新評估核能 發電的安全問題和潛在風險， 調整長遠電力供應的規劃。中 國總理溫家寶亦要求嚴格審批 新上核電項目、暫停審批相關 開展的前期工作。

禍從「血」出 鮮血的價值 何麗敏 (N14)

血本來象徵著熱情的、漂亮 的、純潔的，亦是珍貴的和與 生俱來的恩物。捐血者內心本著祝福和友 善的心態獻出自己的鮮血，希望使更多有 需要的人得到幫助。經過輸血和治療，病 患者在其祝福的正能量下亦能漸漸地康復 起來。

揭露一: 官方刻意隱瞞事實 與血霸同流 合污 在防治愛滋病的工作方面，中國政府較 著重宣傳性接觸的安全措施和性教育，但 是，為何華中地區卻仍然出現如此多的感 染人數和死亡個案呢﹖官方又不是完全不 知道真正原因不是在性安全上。在一九九 八年，有關部門已頒布《獻血法》，以法 律禁止賣血，這可顯示其實當局一早意會 血液輸送與染上中國愛滋病的人數持續上 升是有一定的關係。但是，他們情願刻意 封鎖消息、上報作假的染病人數。甚至在 二零零一年，當局更一度阻礙從事防治愛

《一萬封信》道出曾有個別人士利用她的 知名度宣傳其組織，令她極為憤怒，她還 說出會以法律途經追究。偽善者滿腦子想 著發愛滋財，完全沒有理會病人及其家人 朋友的感受。如此下去，他們更會連正規 的愛滋病治療也不敢相信，封閉自己，延 誤治療的機會。 揭露三: 醫院血庫存放含病毒血液 由於醫院血庫不足，尤其是一些村落 醫院位於較偏遠的地區，缺乏運送血漿的 設備及運送的所需時間較長。他們大多依 賴村落的採血站來應付緊張的供給，賣血 站內共用針筒的情況普遍，血霸又會把血 漿賣給醫院，然而醫院的檢驗措施極之不 足。所以，如果一個村民有愛滋病毒，無 數的賣血者及輸血者染上愛滋病的機會相 當高。

每一天，醫院需要處理很多不同的病 人，特別是婦產科的生殖手術、剖腹生產 等，輸血治療在所難免。如果輸入含愛滋 病毒的血液，母體和胎兒感染的機會率亦 非常高。本來，家庭增添新成員是喜事， 可是正因為醫院處理血液不當，令不少人 家破人亡。

不少失去親人的家庭採取法律途徑上 訴，但是官員與醫院管理層早已買通，隨 便給那些家庭一些安家費便草草了事。 揭露四: 中國愛滋病患者遭受歧視 最令人擔心的就是病人及其家人的心理 及精神狀況。在中國，一般人都認為病患 者生活不檢點才會染上愛滋病，正常人是 不會有的。他們甚至認為愛滋病可以由普 通的肌膚接觸而傳染，拒愛滋病病患者於 千里之外。愛滋病患者常遭受歧視、被冷 漠對待，其家人亦會受盡冷嘲熱諷。社會 對他們的偏見令他們自我封閉，不敢面對 任何人或接受治療。一些地方政府還下令 封鎖疫情嚴重的村莊，外界難以與他們接 觸或給予關懷和治療。 因為失去親人的痛苦和得不到公平對 待，令一些人精神反常，產生反社會的心 態，對社會施以報復。曾經有一個記者報 導，在鄭州，一家三口的李家，其妻子因 在某間人民醫院剖腹生產，過程中接受輸 血治療，她和兒子其後都染上愛滋病，一 直被鄰居歧視。李太太常責怪自己把家給 拖垮，最後自殺了。李先生卻認為是鄰居 的對待令愛妻自殺的主因，他整天心想著 為妻子報仇，一天他殺掉了鄰居八人。 在中國，不只是患者被愛滋病害慘了， 血霸、官員、醫院管理層等的道德也病入 膏肓。中國防治愛滋病的道路仍然十分漫 長，教育和助學是防治愛滋的良藥，令新 一代對愛滋病有所認知。 ■

Response to the 2011-12 Budget Executive Committee, Medical Society, HKUSU, Session 2011

I

n response to the Budget released by the Financial Secretary on 23rd February 2011, the Executive Committee of Medical Society, HKUSU has a number of suggestions with regard to several public health issues raised.

1. According to the Budget, there are currently 27,041 hospital beds provided in public hospitals. The current Budget proposes an increase of 21 beds to 27,062 in 2011/12. Such increase is insufficient to cope with the increasing demand foreseen in the coming year. While plans to establish new public and private hospitals exist, increased governmental investment would facilitate vetting procedures and subsequently the inception of new medical services. This would alleviate the burden placed on the public sector.

2. According to the Budget, the current median queuing times for first and second priority patients stand at 2 and 8 weeks respectively. Such figures are unacceptable particularly in light of the fact that in 2009/10, the corresponding waiting periods were less than 1 and 5 weeks respectively. The Budget proposes several means to increase the supply of medical services such as increasing the overall number of cataract operations by 3,000 as well as providing Magnetic Resonance Imaging service to 3,400 extra patients. However, the government should address the fundamental issue of losing doctors to the private sector. By employing more health care professionals, improving working conditions and adjusting appropriate wages, experienced doctors can be retained in the public sector. This is believed to immensely improve the quality of service that patients deserve.

3. According to the Budget, the number of community nurses in 2011/12 is predicted to increase by 2.5%, from 388 to 398. As Hong Kong anticipates an aging population, the strain on geriatric resources will continue to exacerbate. Resources to train related health care professionals must be increased in order to catch up with the rising demand for medical services.

4. The government's initiative to increase the duty on tobacco is laudable. While such measure is implemented, health education is a crucial synergist in deterring the public from tobacco use. More funding could be put into public health education, targeting specifically on the health benefits of smoking cessation.

Despite the considerable number of areas in need of improvement, the increased investment in the health care system reflects that the government recognizes the importance of public health promotion. 們的城市正以一個癲狂的速度 變化和發展。四通八達的道路 帶來了都市化和消費主義，街頭巷尾的每 一個角落，充斥著擴音器、霓虹燈和廣告 牌，向我們的感官不斷重複購物的概念。 參天的高樓大廈長成茂林，我們彷彿每 天生活在迷宮中，忘掉了如何呼吸、怎麼 與大自然對話。鋪天蓋地的商業世界從中 心向外緣擴展，虎視眈眈的想要吞噬田園 的恬靜閑適，再建設、描繪成一個又一個 幾乎倒模出來的所謂現代化理想居庭── 便捷的交通網絡、新落成的住宅、完善的 配套、怡人的景致、比市區清新的空氣等 等。

口號，要在整個珠江口灣區大興工木，破 壞生態，將我們的城市景觀扭曲並簡化成 可供遊客消費的「城市客廳」、「香港西 北部濱海景觀區」、「低碳住區」、「文 化觀光村」等等。如此宏大的計劃，涉及 的地區包括新界東北和西九市區，以至大 嶼山和八仙嶺郊野公園，就連米埔濕地保 護區、落馬洲河套區也會受到波及。港九 新界的交通設計、其他房產和新市鎮的規 劃，也會出現許多未知的巨變。 關乎香港的整體面貌、產業、環境── 如此重要之事，諮詢期只有短短十八天， 難道這只是米已成炊、閉門造車假民主的 過場？如此急速融合的代價，是任由當地 居民村落的歷史文化、生活方式、經濟模 式和農耕生活被徒然擾亂。所謂「冰封三 尺，非一日之寒」，香港本身獨特的文化 身份和城市內涵將因此變得越來越模糊。 再者，融合的大概只會是珠三角的富裕階 級、遊客、房地產商──這根本就是資本 上的融合。難道我們只能眼睜睜地看著三 地政府「打造」跨界的主題樂園、內地富 人的遊樂休閒區？難道我們連捍衛、保護

是人類最好的朋友，因為牠們 在開心、傷心時都會陪伴你， 對你不離不棄。協助犬對於殘疾人士而 言，不只是朋友，而是比朋友更親密、更 重要的好幫手。協助犬能協助他們的起居 生活，減少生活上遇到的困難，使他們更 獨立、更自由地生活。此外，協助犬與殘 疾人士的關係親密，無時無刻都會陪伴在 旁，成為他們心靈的依靠，令他們更有安 全感。 協助犬並沒有 特定犬種限制， 但拉布拉多及金 毛尋回犬較為常 見，挑選原則 以性情為主，穩 定、服從是協助 犬所需俱備的條 件，因為他們在 公眾場合必須遵 守該有的禮儀，遇到突發事件亦要保持鎮 定。協助犬分為三類，包括導盲犬、導聾 犬及服務犬。大家對導盲犬並不陌生，牠 們代替視障人士的眼睛，為他們帶路，避 開障礙物。導聾犬代替聽障人士的耳朵， 當聽到門鈴聲、人名呼喚等聲音時，會提 醒聽障者，帶他們到聲音來源。而服務犬 的服務範圍廣泛，主要協助肢障人士，幫 忙拿東西、開關門等。此外，低血糖通報 犬及腦癇犬亦屬於服務犬。

低血糖通報犬 糖尿病患者必須經常注意自己的血糖水 平，一旦血糖水平過低，隨時會昏迷甚至 死亡。部分患者感覺不到血糖的變化，如 年紀較小的患者，無法判斷自身的情況， 便會常常昏倒。低血糖通報犬靈敏的鼻子

能辨認出血糖水平的細微差異，在血糖瀕 臨危險水平時會向患者作出警告，例如： 舔手、把血糖偵測器拿來等。低血糖通報 犬能減低患者經常測血糖的麻煩，突然昏 倒而發生意外的機會，亦可減低患者因血 糖過低在睡夢中陷入昏迷的危險。

腦癇犬 腦癇犬可感覺到主人即將病發，藉著 做出與平常不同的動作，提醒主人有所 準備，例如：用爪子抓、兜圈子奔跑、直 接向著患者吠叫 等，令患者在病 發前能到達安全 的地方，避免意 外發生。只有十 分之一的犬隻有 內在能力感覺到 病發，這些犬隻 亦必須經過數百 小時的訓練才能 成為腦癇犬。有人認為腦癇症患者病發前 的行為或氣味的改變令犬隻有反應，但確 實原因目前仍是個謎。腦癇犬在主人病發 時會去求助，吠叫、找家人或啟動緊急按 鈕，也可能會躺在發作的人旁邊，保護他 的安全。 ■

Life and Death in the Animal World A Reflection on the Everlasting life of Turritopsis nutricula Lily Cheng (M15)

W

hat is life? What is death? A Kannada proverb says, “We will never know what happiness is like if we have never experienced sadness and sorrow.” Life and death are just another face and facet that are relative to each other. Yet, death is a taboo in many cultures and religions. We can only appreciate the essence of life gradually as we understand more about death.

Then, why does death become a taboo that people with different backgrounds tend to avoid discussing in everyday life? The negative connotation of termination of life and the fear for the uncertainty brought by death may be some possible reasons. As a matter of fact, nobody can escape from this final fate of life. Despite the strong reluctance in talking about death, most cultures treat it seriously when it does approach. Funerals with families and friends gathering around are organised in memory of the loved ones. The ceremonies are not arranged only to commemorate the conclusion of one’s physical existence, but also as a way to show respect to the deceased.

Animal’s emotional attachments are, sometimes, unexpectedly strong. It is not rare to see animals

mourning and weeping for their dead counterparts.

African elephants, for example, are known to display such humane acts. Scientists and enthusiasts have done extensive observation and investigation on their behaviour, and spotted out some clues about their death rituals. They would surround the bodies of their family members very quietly after an initial loud rumbling. Burial with dirt, leaves and tree branches after gentle feeling of the remains with their trunks and feet is their means to show concern and protect the bodies of the passed-away. They always return after leaving for food and water for they would stay with the deceased for several days.

Their kindness is not only limited to related companions, but also extends to other animals and even men, protecting them from harm regardless of whether they are living or dead. Sometimes elephants unrelated to the deceased would visit their graves. Many animals, like chimpanzees, in some sense, do have sympathy and are aware of bereavement just as the African elephants do. Through body language expressed with special postures and movements, many display what we humans recognise as “sorrow” towards death. After all, wild creatures are also brimmed with emotions.

Customs in human corpse treatment vary from culture to culture, as a devotion of respect to the deceased. Cadavers are commonly cremated or buried usually with a coffin in Hong Kong nowadays. Muslims simply bury the bodies without using coffins as a way to return them directly to the nature. Similarly, sky burial practiced by the Tibetans allows scavengers, particularly vultures, to feast on the naked corpses purposely left on the mountainside. Sea burial involves the sinking of the dead or the scattering of cremated remains into the sea or ocean. On the contrary, many of the ancient traditions show people’s strong

desire for eternal life. For example, mummification to preserve body skeleton has been performed in Ancient Egypt to prepare the dead for entering their afterlife and even for future resurrection.

Even if the technique to revive youth in humans is well understood, it is yet to be practical. The usage may bring about further moral impact to our already complicated society. Religious beliefs, to a certain degree, provide hopes to calm wanderers and serve as spiritual feeds to mankind.

Catholics and Christians believe that only God would bring believers eternal life in heaven. Buddhism considers afterlife and rebirth as a consequence of the current life. Taoism depicts the deceased as non-vanishing spirits and, therefore, piety for ancestors existed alongside with the tradition of worshipping Chinese gods.

Complexity or Simplicity

Turritopsis nutricula While human beings are still researching on factors related to longevity, the evolutionary secret has already been encoded in the hydrozoan, Turritopsis nutricula. One major reason hindering common living organisms from escaping death is the irreversible and inevitable cell ageing. The critical property of transdifferentiation bringing back undifferentiated cells in mature Turritopsis has enabled it to become virtually immortal.

The complex life cycle of cnidarians involves an asexual development from colonial stage (polyp) to a sexually mature, solitary, adult stage (medusa) commonly recognised as jellyfish. Developing through embryo and larva, union of gametes from the medusae reproduces new colonial polyps. The pluripotent interstitial cells in the medusa give cnidarian an extraordinary capacity for regeneration. The transformation of differentiated cells into another cell type can only occasionally be seen in some organ regenerations. Yet, laboratory studies of virgin Turritopsis nutricula revealed its ability to revert regularly from adult to polyp stage. This potential to rejuvenate normally presented in the life cycle of Turritopsis nutricula could possibly render endless lifespan to this cnidarian species! Even though they don’t seem to have any higher intelligence, they are there, simply living and renewing themselves.

In the search of the beauty of life, death is seen as a graduation from the lifelong battle which marks the beginning of another journey after death. Over the history of mankind, human’s hope for a longer life had never ceased. Our ancestors kept looking for prescriptions to survive despite the price they have to pay. At the end of the day, when you have a step back from the beliefs that deeply entrenched in you, you will find out it is the limited span that makes our life valuable. Is it still necessary for us to search for life in eternity?

Death is the thing that connects us all. It reminds us that what really matters is who we have touched, how much we have given. It makes us realize that we have to be good to one another and embrace each and every encounter. It calls upon our gratitude to treasure what we have rather than to urge for the things – Ecclesiastes 3:1–15 that we lack. There is nothing more natural than having different opinions on such a profound discussion topic like life and death. It, therefore, always remains a food for thought and your answers may turn into a beautiful refrain that you will cherish for the rest of your life. ■

Leeches, the Blood Healers The use of leeches in medicine Stanford Li (M15)

eeches? Yeeee!!! Yuck!!!

Why on earth do these horrifying, gross, bloody monsters exist at all? This is almost everyone’s impression when one sees leeches. However, there is nothing too strange in this vast world. When you learn to take the bad with the good, these critters could also be huge potential healers.

History Let us first look at the long and eerie history of leeches in the medical field. There is evidence that leeches were used all over the world since the Stone Age, while the first documented use of leeches by medical practitioners is in an Ancient Egyptian wall painting. In medieval times, European physicians brought the use of leeches to its peak. Since the saliva of leeches contains very potent anticoagulants and anesthetics, the wounds bit by leeches could bleed for long periods of time without causing any pain. This distinct characteristic allows physicians to carry out bloodletting on patients.

Bloodletting is a nasty-sounding practice of taking huge amounts of blood from the patient’s body.

Medieval doctors believed that this practice could help to cure a large variety of diseases, ranging from fever to hypertension. However, this practice was abused and often misused in many other health problems. Oftentimes these “cures” have proved to be fatal to the patients. Consequently, bloodletting and eventually the use of leeches died out during the 19th century, after the discovery of germs.

Besides the western world, the use of leeches has also been documented in the East. According to Shennong Ben Cao Jing (神農本草 經), leeches can “expel bad blood

and bruises” (主逐惡血、瘀血， 破血瘕積聚). Li Shizhen (李時珍), a famous Chinese medical practitioner in the Ming Dynasty, also believed that dry leeches can be used in medicine to “facilitate circulation and reduce retention of toxins” (消積散結、消腫解毒).

been using leeches in surgery. The hospital introduced leeches about 10 years ago. Currently QMH is keeping 10 leeches in reserve, and they are mainly used in surgeries involving reattachment of fingers.

Approximately 2 to 3 patients use the alternative treatment per year.

The standard procedure is to apply the leech on the wound for 4 to 5

times a day, each time lasting for 10 to 20 minutes. After the leech takes enough blood, it will inflate and detach from the wound. The venous congestion problem is likely to be solved with the continuous use of the treatment for 5 to 7 days.

Besides reattaching body parts, the Department of Surgery of QMH has also used leeches in breast reconstruction surgery on a female patient a few years ago. It is believed that leech therapy will become increasingly popular in a variety of fields in surgery.

As new anticoagulants Imagine there’s a patient with severe DIC (disseminated intravascular coagulation) and something stronger than heparin is needed to prevent infarction. Hirudin, a chemical found in the saliva of leeches, might be the cure.

In the 1950s, German scientist Markwardt successfully isolated hirudin from leeches’ saliva, and used it to synthesize the most effective anticoagulant in the history.

Natural hirudin is an inhibitor of thrombin. It binds to and inhibits only the activity of thrombin forms with a specific activity on fibrinogen, hence hindering the process of homeostasis and blood clot formation. Therefore, hirudin has therapeutic value in the treatment of blood coagulation disorders, skin hematomas and superficial varicose veins.

In some aspects, hirudin has advantages over heparin, warfarin and other commonly used anticoagulants, as it does not interfere with the biological activities of other serum proteins and can also act on complexed thrombin.

For instance, studies have shown that in the therapy for myocardial infarction, hirudin can reduce the mortality rate by 11%, which is way better than other synthetic blood thinners.

For pain relief Ever suffered from any pain

that cannot be relieved by ordinary painkillers? Again, leeches might be the final solution.

German scientist Andreas Michalsen has done an experiment on leeches. He divided 16 chronic arthritis patients into 2 groups, one of which was given traditional treatments and the other treated by leeches. During the therapy, doctors applied the leeches on the affected joint for 80 minutes. 24 hours later, the pain was greatly relieved and the relief lasted for 4 weeks without any side effects. On the contrary, those patients given traditional medication did not show significant improvement. The experiments revealed a stunning fact – leeches might be able to replace analgesics in the future.

果有人問你︰誰是「兒歌醫 生」？你一定會毫不猶豫地 答︰陳以誠！相信大家在孩童時期，或多 或少都會接觸過他的兒歌，如《眾志成城 抗肺炎》、《滅蚊是對》、《耶穌愛世上 小孩》等。陳醫生在一九七七年畢業於香 港大學醫學院，現為私人執業兒科醫生、 香港醫學會副會長及愛健慈善基金會主 席。他經常出席講座及電視節目，並在報 章雜誌上撰寫專欄，以表達對兒童身心發 展的關注。

兒歌醫生 逾四百首家喻戶曉的兒歌，膾炙人口， 大部份市民都對陳醫生有所認識。不過， 想不到他一開始並不是以唱兒歌成名的。 陳醫生回想自己學生時期，已經熱愛音 樂，曾多次參加校內校外的歌唱比賽，并 屢獲殊榮。直到一九七八年，他為突破機 構創作主題曲，並在八十年代五次踏足紅 30 | Caduceus April 2011

館演出，就這樣開展了他的歌唱事業。 那麼，什麽原因驅使他轉型為一位兒 歌歌手呢？陳醫生回答：「這個轉變純粹 是由於成為了兒科醫生和爸爸的緣故。」 身份和角色的轉變，使他更在意小孩的身 心健康發展。「現今的流行曲和以往的兒 歌，根本就不適合兒童。」他慨歎道。受 歡迎的流行曲大多數都與情情愛愛有關， 幾歲大的「豆丁」怎能明白背後的意義？ 看到超級巨聲、殘酷一叮等綜藝節目，一 些乳臭味乾的小孩引吭高歌，怎不叫人痛

心疾首呢？儘管數十年前的童謠題材正面 健康，卻仍不切合現今實際生活環境。以 《 月 光 光 照 地 堂 》 為 例 ， 現 在 很 難 會 有 「呀媽趕插秧、呀爸捕魚蝦、年三十晚摘 檳榔的情況」。 另一個原因是，他認為歌曲是行為糾 正（Behaviour modification）一個重要 工具。陳醫生的歌曲琅琅上口，歌詞中

夾雜不少令兒童有莫大裨益的訊息，當中 包括《願愛大自然》的愛護環境、《孝順 齊歡笑》的敬愛父母、《賽龍舟》的團隊 精神、《飲食新文化》的食物營養等。把 這些知識融入歌曲中，成為親子互動的工 具。不但能使學習過程更輕鬆愉快，而且 能令父母和子女相處得更融合。陳醫生笑 言，只要有人欣賞和好好運用自己的歌 曲，他就心滿意足。值得一提的是，所有 兒歌的詞和曲都是由他一手包辦，絕無半 點假手於人呢！

時間分配 當兒科醫生固然忙碌，同時間兼顧多 姿多彩的歌唱事業就更為繁忙。但是， 他還熱心參與教堂活動，更擔任香港醫學 會副會長、迦密中學校友會會長等公職。 究竟他怎樣分配時間呢？陳醫生直截了當 表示，人人皆能適當運用時間，根本沒有 什麽秘訣，只要多嘗試，就能把握當中竅 門。他表示：「上帝是公平的，賜給每個 人二十四小時。」而他當大學生的時候， 學習到如何善用一分一秒。「我當年可算 是『搏盡』讀書，『搏盡』住hall的。」 他似笑非笑的說。那時候住St John's College，陳醫生十分投入舍堂生活，尤其喜 愛游泳及唱歌。可能這只不過是老生常 談，但陳醫生一再強調，作為現代的醫學 生，儘管學業有多繁重，也要有其他的興 趣。千萬不要看輕非學業方面的發展，因 為適當的轉移視線，不但可以令讀書的效 率提高，更能夠發展運動智能、人際智 能、空間智能等多元智能，對日後成為醫 生有莫大的益處。「我不能否認MBBS的課 程的確有一定的難度，但最重要的是與一 眾同窗知己一塊兒讀書，互相學習，遇到 困難要努力克服，千萬不要隨便drop out。 數十年前的合格率比現在低，不少同學在 醫學院待了超過五年時間，還未能畢業。 不過，很多當年『肥佬』過的同學，現在 卻是賺錢最多，名氣最大的醫生啊！哈 哈！」說罷就咧嘴的笑著。

多元智能 在1983年，美國哈佛大學教育研究 院的心理發展學家加德納提出了多元智 能理論。他認為過去對智力的定義過於 狹窄，未能完全反映一個人的真實能 力。他提出了八種能力： 語文能力，邏輯數理智力、空間智 力、音樂智力、運動智力、人際智力、 自我解析智力、自然科學能力。

香港醫學會 作為醫學生的我們，日後很大機會成 為香港醫學會的一份子。究竟香港醫學會 是一個怎麼樣的組織呢？那當然要讓身為 香港醫學會副會長的陳醫生為我們介紹一 下。香港醫學會成立1920年，其宗旨為 維護和提高香港醫學執業水準，及在香港 所有註冊醫生中培養友好的專業和社交關 係。香港醫學會主要有三個職責：其一是 代表香港醫學界與國際醫療組織聯絡，其 二是服務社會大眾，其三是要為同業組織 悠閒活動。香港醫學會是世界醫學會及亞 洲及大洋洲醫學組織聯盟的一員，與國際 醫療組織及中華醫學會保持良好關係，定 期互訪，交換意見和經驗。香港醫學會肩 負起教育大眾的責任，透過電臺電視節 目、講座、展覽、小冊子等渠道，向市民 宣傳健康資訊。當一名救死扶傷，力起沉 疴的醫生，總會有身心疲憊的時候，香港 醫學會正好擔當著恢復各人心靈的重要角 色。其中的攝影會、管弦樂團、合唱團、 龍舟隊、足球隊等組織，就是為各位櫛風 沐雨的醫生提供喘息的機會，使同行互相 支持。 鮮為人知的是，原來香港醫學會會歌 《維護民康》都是出自陳醫生的手筆，可 見他絕對不是只唱兒歌的兒歌醫生呢！

後記 當談及兒歌這個話題時，陳醫生立即 喜形於色，侃侃而談。還興致勃勃的翻箱 倒籠，把從一九九五年至今所創作的廣東 話、普通話和英語兒歌、歌譜和海報找出 來，作詳細介紹。他還把這些作品送給Soc 房作為農曆新年的禮物呢！ ■

神藥板藍根？ 吳瑞麟 (M15)

過非典型肺炎一役，相信很 多人都會聽過板藍根這種中 草藥。記得零三年的時候，傳聞這種中藥 可以預防肺炎，出現了搶購板藍根沖劑的 熱潮。坊間認為板藍根有預防和抵抗流感 的作用，維基百科有關板藍根的篇幅更指 出，「有充分的證據表明板藍根及其復方 藥物可以預防和治療由病毒引起的呼吸道 感染。」[1]難怪非典的時候板藍根會被搶購 一空！ 根據筆者在PubMed和《中國期刊全文 數據庫》中搜查的結果顯示，有很多的體 外和動物實驗的結果也顯示出板藍根的確 有抗炎、抗病毒、抗菌、增強免疫力、抗 內毒素的功用。再搜查下去結果更加不得 了，板藍根還可以抗愛滋、抗腫瘤！ [3] 看 來板藍根比傳聞中還要厲害！但是近二十 年來，其實還沒有有關板藍根的功用的雙 盲、隨機、陽性藥平行對照進行規範的臨 床研究。 [2] 所以板藍根到底是否在這麼多 方面都有藥用價值，我們還是需要等待進 一步的研究結果。或許板藍根真的有這麼 神，只是需要更加確鑿的證據支持，現在 就只能夠相信中醫多年來用板藍根的臨床

Dr. Google and Cyberchondriacs Why Googling your symptoms is problematic for patients and doctors Andrew Seto (M15)

I

magine you’re a doctor. Your 5-year-old patient has a hypochondriac mom who feels the need to check his child’s temperature every three hours. She arrives at the A&E department with a pile of Internet printouts of all the likely complications she thinks her child may have. You dismiss it as another case of cyberchondria, or as a BBC article aptly describes, the “Internet print-out syndrome”.

If you’re a medical student, you should be quite familiar with the term “hypochondria”, or “health anxiety”. In fact, you might even be one hypochondriac! Every now and then, you get a cough, nosebleed, or hoarse voice that you would normally ignore, but now with your fast growing medical knowledge, you can’t help but worry… could it be cancer?

The thought usually dissipates soon enough. But for hypochondriacs, this is no laughing matter.

When you combine hypochondria with the power of Google or Wikipedia, you get cyberchondriacs. Essentially, “cyberchondria” is the Internet-induced, 21st century version of hypochondria. In 2001, the London Sunday Times called it the “deluded belief you suffer from all the diseases featured on the Internet”.

Something you watched on the TV (such as a half-remembered House or Grey’s Anatomy episode), a dramatic story about your friend’s cousin’s neighbor’s rare disease, or even the most reputable online medical websites are all possible causes of unnecessary panic for hypochondriacs. It could also be triggered by the death of a closed family member or illnesses suffered during childhood years.

And now, thanks to the Internet, it’s much easier to become a cyberchondriac.

The Power of Dr. Google Suppose you wake up one day discovering a weird tingling sensation in your feet. Although that sensation goes away within minutes, it worries you so much that you decide to Google the symptoms of “tingling feet”. One result says multiple sclerosis. Another comes

up with a list including diabetes, stroke, seizures, migraine, hypothyroidism… All of this prevents you from exploring more common and benign causes, such as remaining in the same position for a long period of time.

Even if you’re using reputable medical websites, which have tools that refine symptom searches, they are not diagnostic in nature, and are prone to assume that rare medical conditions are more common than they really are.

This phenomenon, called “availability bias”, happens when people predict the frequency of an event based on how easily an example comes to mind. It is this availability bias that causes people to worry more about plane crashes than car crashes, which are far more common. (The probability of being involved in an airplane accident is only 1 in 1 million, while the chances of dying in a car crash are actually 1 in 5000.)

Information from medical websites often lead users to suspect rare diseases, and because many relatively harmless conditions share common symptoms with serious ones, cyberchondriacs tend to think they have the worst (rather than the most probable) disease.

Microsoft recently conducted a survey that suggests search engines have the power to escalate health concerns.

Take headaches for example. We know headaches can have different common causes, like stress, caffeine withdrawal and dehydration. But if someone were to Google “headache”, there’s a high chance they would find something about rare conditions such as brain tumor, which actually has an incidence of 7 in 100,000. If you happen to be a cyberchondriac, you may find yourself in a frenzied search on brain tumor treatments or even thinking about making a will.

Cyberchondriacs: Every doctor’s nightmare?

Do not underestimate the enormous number of cyberchondriacs who undergo this preoccupying fear of having a serious disease every day. These patients quickly jump to conclusions at the slightest sign of abnormality. They often go against medical advice and tend to reject the notion that their symptoms are caused partly by mental reasons. Some easily get upset or angry when doctors suggest an alternative diagnosis.

The changing role of doctors Some people believe that when patients are more aware of their disease, they may feel more in control over their disease and more compelled to correct it.

Doctors could spend less time answering their concerns and explaining treatment options, thus improving the efficiency of healthcare.

Others are more skeptical of this sort of shared decision-making model. Information from the Internet may be biased, erroneous, too generalized or too specific to be interpreted by patients with no medical knowledge. Some doctors seem reluctant to adopt this change of role, seeing this extra clinical responsibility as “time-consuming” and “problematic”. Others are simply threatened by the notion that their patients know more than they do.

In a world where more patients become experts, the doctor’s role is a changing rather than diminishing one. It lies in the re-education of patients, interpretation of information they’ve acquired online, and directing patients to use reputable sources.

Cyberchondria Every doctor’s nightmare?

Thanks to the Internet, GPs need to spend the equivalent of one day a week persuading patients that their Internet self-diagnosis might be misguided. Not only does this lead to unnecessary visits, waste of time and resources, but it also causes great frustrations for healthcare professionals.

I for one believe that more informed patients mean better patients. If you know what you’re doing, Googling your disease might yield positive results. Surely, as future medical professionals, our responsibility is to embrace this accessibility of information and allow it to empower - rather than agonize - patients. ■

hat’s the easiest way to know someone?

10 years back, the answer would probably range from conversations to electronic mail, but now it has simply boiled down to one word: Facebook. It is a social networking site that does not only display your personal information, but also allows you to track down people whom you only know by name and even get automatic updates on what your friends are currently interested in.

Zuckerberg’s roommate, Arie Hasit, recalls how Zuckerberg spent hours in front of the computer. "He studied computers and psychology," Hasit said. "Despite the fact that he developed Facebook, he continued his studies as per usual. His grades were okay. He was even in a relationship with a girlfriend. During Facebook's initial days, the walls in his room were filled with graphs and charts which showed how many people joined on a daily basis, who used what application, and who has the most friends."

Facebook: the new Google?

Both Facebook and Google started off as functionally distinct websites; one serves as a social networking

site while the other is a search engine. But as more people use Facebook and present more of their personal details online, it indirectly serves as an advertising platform, this time not only generating basic data but also preferences of family and friends.

Say you want to get a new car. You type the car model on Google and you will get search results regarding details of the model. But if you search for the same product on Facebook, you can actually get reviews and comments from your peers; which type of information is more appealing and valuable? It is up to the user to decide, but this definitely poses a threat to the giant search engine company.

Will Facebook simply be a fad that will die out with time or is it here to stay? Nevertheless, it’s safe to say that Mark Zuckerberg has brought about subtle changes in the way we interact, and that school dropouts can still go on about doing amazing things! ■

Frozen Yoghurt, the “Healthy” Ice Cream Think it’s healthy? Think again.

he enjoyment of frozen yoghurt, or “fro-yo” has been a rising trend among both youngsters and adults in the past few years. With a refreshing taste and creamy texture, as well as a huge mouth-watering variety of toppings, it has become a popular alternative to ice cream. I myself pay regular visits to frozen yoghurt stores at least twice a week, whenever I crave sweet comfort food or whenever I walk past a frozen yoghurt shop. However, is frozen yoghurt really as healthy as it is perceived to be? Or is it another gimmick from corporations to earn money and another health trap for customers to fall into?

Let us first look at when and how this “fro-yo” frenzy arose. Frozen yoghurt was introduced in New England by the company Hood, one of the largest and oldest dairy companies in America, under the

The History

Yoghurt’s Story The origin of yoghurt dates back to around 3000BC, with the classical story of a Neolithic (also known as New Stone Age) herdsman. It is said that a nomad rode into the desert with milk-filled goatskin tied to his camel. Eventually the high desert temperature combined with body heat of the camel provided the optimum growth environment for lactic acid-producing bacteria. Unknowingly, the herdsman consumed the semi-solid milk-turned-yoghurt to quench his thirst, and its unique taste appealed to him and his descendants. Thus, yet another discovery was made accidentally by mankind. name of Frogurt in the 1970s. It was considered as the perfect dessert due to its low caloric content and health benefits. result of this, when reduced fat ice cream appeared in the mid 90s, both sales and popularity of frozen yoghurt plummeted. We have to thank Shelly Hwang and Young Lee for bringing back the delicious frozen treat we see nowadays on the streets of Hong Kong. They are the creator of Pinkberry in California that revived the fro-yo frenzy. People drove across towns and waited in line for 20 to 30 minutes just to order a cup of frozen yoghurt, and ever since, frozen yoghurt stores have been appearing and spreading from California to other states and other countries .

Frozen yoghurt gets its unique flavor from strains of Lactobacillus bulgaricus and Streptococcus thermophilus, which are also present in ordinary yoghurt. L. Bulgaricus was first identified in 1905 by a Bulgarian doctor, Stamen Grigorov. Grigorov also showed that this organism served as treatment and prevention to several diseases, such as tuberculosis, ulcers and fatigue. The organism breaks down milk sugar (lactose) to produce lactic acid, which preserves milk by reducing the pH. The low pH

results in milk turning sour (the unique yoghurt taste) and prevents the growth of bacteria that causes food poisoning. Since this bacteria breaks down lactose, people suffering from lactose intolerance can usually consume yoghurt without experiencing flatulence, abdominal pain or diarrhea. This is one reason why fro-yo can be a superior choice compared to ice cream, especially in Asian countries, where lactose intolerance is more common. The other strain of bacteria Streptococcus thermophilus was also known to promote gastrointestinal health as it is also a lactic acid bacteria and contains similar health benefits as L. Bulgaricus.

So fro-yo is every food-lover’s perfect dream-come-true? Not necessarily. According to a frozen yoghurt shop owner, frozen yoghurt is basically the same thing as soft-serve ice cream. He explained that making frozen yoghurt involves adding yoghurt mix powder into a small amount of regular yo-

since fro-yo has really little fat in it, sugar provides the flavor and taste appeal to compensate, and the high sugar content is often masked by the low-fat label in many stores.

Another illusion that people have about fro-yo is that it is a good diet food due to its low calorie content. This is definitely not true as a regular cup of fro-yo totals to 225325 calories per serving, depending on its ingredients and recipe.

And bear in mind that this is pure frozen yoghurt on its own, not including the variety of fatty toppings including oreo, mochi, gummy bears, chocolate chips… etc. People often forget that the toppings themselves are very high in fat content and are not very healthy.

Consumers (like me occasionally) often get lost in the variety of colorful toppings and are under the misconception that as long as it is froyo, no matter what toppings you add to it (whether it is chocolate or any other sorts of junk food), it is still counted as a “healthy snack”.

But if your emotions manage to take over your reasoning next time you pass by a frozen yoghurt store, don't worry, since the calories from frozen yoghurt are mostly from carbohydrates (sugar) so they are more likely to fuel muscles than get stored as body fat, as compared to ice cream calories.

As we've discovered, frozen yoghurt is not a food-lover’s heaven, and the recipe differs from shop to shop, so it is hard for consumers to judge its actual health values due to the presence of deceptive advertisement and loose food laws.

However, you can consider it as a healthier alternative to ice cream, with the similar satisfaction of the traditional comfort food and the freedom to add any toppings to your liking. Health-wise, it is definitely not equivalent to regular yoghurt, so it is midway between regular yoghurt and ice cream. As with most types of food, moderation is the key to a healthy and happy food-person relationship (however, occasional over-dose should be fine though!). ■

，正淅淅瀝瀝的打在窗前；婉慧驀地清醒過來，放下托着腮幫的手，看看手 錶，原來已是十時多了。快要回家了，但解剖學的筆記還沒有讀完，又有很 多不明白的地方。要是這個時候他在，那該多好；不過還是算了吧。婉慧收拾好書包， 怏怏的走了。 第二天，她飛也似的奔出家門。忘了校鬧鐘的她，趕不及八時半的pre-lab talk。她 只有時間換上實驗袍，去參與這學年的第一堂解剖學。她奔進了實驗堂，一陣陣刺鼻的 氣味衝面而來，令她清醒了不少。在她組員的桌上，屍布已被揭開，一個瘦骨伶仃的老 人正靜悄悄的躺着，頭部被一塊砂布蓋着，令人看不清他的模樣。她戴上了手套，感覺 着實驗室裏的密不透風。一刀一刀的切下去，她有點彷彿，隱約聽到鄰桌的同學在大聲 評論：「為什麼我們的那麼肥胖？身上的肥油怎樣除也除不清，真羨慕鄰桌的……」 「那還用說？還有，你沒看到他腳旁的標籤嗎？」 「啊……病人才二十一歲，姓王……」…… 婉慧記得哥哥多年前也是醫學院的新生，在一次解剖學後，他邊吃飯邊告訴家人有關 屍體的詳細情況，有關那破爛的肺、那縮小了的心……婉慧越聽越覺得噁心，但子聰卻 說：「了解病人一身的結構是做醫生的第一步。雖然接觸屍體並不是非常愉快的經驗， 但我們也要為將來的病人盡責。能為醫學鞠躬盡粹的醫生，是最偉大的。」…… 「為什麼你總是在發呆？快點過來幫忙拉開他的皮！」同桌在催促着。婉慧回過神 來，喃喃着「對不起」，投入了眼前的學習。 午飯時間到了，她悄悄的脫下實驗袍，回到她那隱敝的角落──圖書館中唯一一間給 本科生的個人自修室。她依然冷靜，悄悄的想着子聰，哥哥也是二十一歲、姓王、胖胖 的……開學的時候，他帶着她到處參觀校園，告誡她那裏是吃飯的好去處，那裏的衛生 情況欠佳等等。一陣叩門的聲音劃破了房間的寧靜，她連忙開了門，以為是哥哥來找她 吃飯。一句「對不起，我預約了房間」令她驀然回首，慌忙地走出了自己的角落。 婉慧還是繼續想下去：那一天晚上，她和哥哥吵了架，說了很多令他傷心的說話， 還說不再理睬他。他黯然神傷回到房間，和他分隔兩處。翌日，她回校補課，突然接到 媽媽的電話：「哥哥在醫院，你快來。」在病床上，躺着的是面色慘白的哥哥，她後悔 了，很想向他道歉，但這時心電圖由滴滴的聲音變成鳴的長響。他真的和她分隔兩處， 更遙遠的兩處。 一個月瞬間過去了，婉慧儘管仍是努力讀書，但她總是覺得自己曲高和寡，沒有人能 真正和好匹敵。本來，她就是成績優異的高材生，記憶力驚力；平日最能說服她的就只 有她的哥哥。可惜地，現在他不在了，但似乎自己不用全盤明白課文箇中的深意，便能 把全部背誦如流。傷心、自大、冷漠的她，漸漸走出自己頗小的社交圈子。 “The thoughts of the diligent tend only to plenteousness ; but of every one that is hasty only to want.” – Proverbs 21:5

初為港大護理生有感 賴彥積 (N14)

了香港大學數月，親朋戚友送上不少祝福，風光不少。不少人一聽見「香港 大學」便發出敬仰的眼光，有時會沾沾自喜，然而自覺不過是幸運而已。對 於新認識的，會問自己讀哪科倒令自己好過些，好讓他們不至於覺得自己很「神」，然 而有些一聽港大一詞便停止提問，令我覺得他們覺得自己甚「屈機」，但其實自己不過 是嘍囉一名。 入了醫學院，同學混上不少內外全科醫學生，跟中學相比，確是另一片天空。他們大 多成績斐然，從談吐中氣勢已不自覺的滲出四周。有些出自醫學世家，有些是名校生， 有些是富家子女……站在旁己有氣勢磅礡之感。入醫學院撈個醫學生的美名不以為然的 令自己過意不去，略感名不符實。穿上那件「港大醫護」及「HKU Medicine」的衣服 更是風芒太露，更顯自己沽名釣譽。 己所不欲，勿施於人。不想自己給看高，卻不自覺的看高人。心叫自己別用如斯目光 標籤他們，卻不自覺地用了社會一貫的目光看他們。畢竟現實是階級化的。新認識朋友 時，不自覺會問他們做哪行，讀哪系。收到答案時已自然把他們分層了，而大多都以量 化的方式了事，賺多的叫上流社會，分數高的叫才子。甚麼職業無分貴賤，求學不是求 分數，不過是安慰的說法。這就是現實。

現實 活在現實，人不得不現實起來。眼睛彷彿失去了方向，身體要為現實工作，從前的 夢想早已遺忘。不少港大護理系的都因成績未夠理想而選讀，貪它的學校名氣以及日後 收入穩定。相比其他學科，護理的確是不俗之選，收分低，人工高，既專業，又不怕失 業。自問非富家子弟，長大後有家室要養，絕不想當蛀米蟲。入讀護理系不過是折衷的 選擇而已，對將來生活有保障，又不是不想讀的。畢竟自己高不成低不就的分數只能讓 自己入到這些專業的科目，不然我早已讀英文了。 46 | Caduceus April 2011

責任 誠然，自問對護理行業興趣不大。天天見著病人呻吟的模樣，血肉模糊的景象，彷 彿要對生命麻目才能心無雜念的專業救人。要做到對生命麻目可不是自己所能接受的， 我未必有此硬心腸。一個讀OUHK護理系一年級的朋友對我說過：「假如你不是100%想 做護士的話，那就不好做了，否則傷害的不只是自己，而且是生命！」相比之下，自問 自己對護理的熱誠比她少得多。眼見新聞屢屢有護士犯錯，打錯針、燙傷嬰兒、調亂屍 體……會報導的已是較嚴重的，可想而知還有很多個案沒有公開。有時甚至當病人徘徊 在生死邊緣時，自己的一舉一動都是關鍵。要是當上了護士，難保我不會成為兇手。這 個責任，真的太大了！

護士形象 護士是非常神聖的，照顧病人，拯救生命，甚至比醫生更了解病人需要。醫生醫人絕 不能缺乏護士的幫助，護士照料病人亦必需醫生的診症，兩者是相輔相成的。然而，社 會對護士的價值觀卻不怎一回事，有些甚至視護士為「阿四」，對他們呼呼喝喝。有人 一聽到護士一職便嗤之以鼻，先想到的不是救急扶危，而是屎尿痰。也許護士的形象就 從要處理污穢物建立了吧。

他樂之樂 其實到目前為止，讀護理亦有安慰之處。出了社區中心數日，做過大大小小的社區服 務，我見過天真無邪的小孩以及齒搖髮秃老人家給予自己最感動的微笑，每句多謝都是 真摰的鼓勵。施恩之樂遠比物慾之樂來得實在，助人是快樂的。要是能用自身微少之力 量以成就眾人之樂，這是多麼值得感恩的事。

結語 作為港大護士生，衷心希望各同學認真了解自己入讀的原因。也許大家會聽到很多聲 音，這也難怪，只因護理所牽涉的不只是自己的事，而是社會的事。 ■ 「凡事不可結黨，不可貪圖虛浮的榮耀；只要存心謙卑，各人看別人比自己強。」﹙腓立比書 2:3﹚ 「各人不要單顧自己的事，也要顧別人的事。」﹙腓立比書 2:4﹚

The East Asian Medical Students' Conference Asian Medical Students' Association Hong Kong

miles, roars and tears marked the end of the five-day excitement of the East Asian Medical Students’ Conference in Bangkok, the capital of the land of smiles. 39 Hong Kong delegates convened with medical students from truly across the world, from a large array of countries including Australia, Bangladesh, Cambodia, China, Egypt, Indonesia, Japan, Malaysia, Mongolia, Nepal, South Korea, Taiwan, Thailand and the United Kingdom!

We embraced our cultural diversities and were united by our common passion in medicine as we took on board this conference’s theme of adolescent health.

Academically, there were poster and paper presentations on issues pertinent to adolescent health. Congratulations to the Hong Kong team for coming first in the poster competition! Our topic focus was on the correlation between screen time and adolescent health.

Amongst other topics that were discussed included sexual abuse, adolescent pregnancy, myopia, eating disorders and obesity.

Meanwhile, the fascinating cultural performances, cultural workshops, and cultural souvenirs added great colour to each delegate’s experience of the conference.

ule for these five days was so packed that we only slept for a couple of hours each night and bus rides became our nap times. Our eyebags served as evidence of the fun and bustle that we had been up to. Without doubt, the rich exposure and global connections that have been forged will serve us well and are jewels that could never be obtained from class lectures. We also learned to appreciate the wealth of Thai culture through our visits to the Grand Palace and Ayutthaya, as well as by watching the traditional Thai dance and of course, the famous Lady Boys’ Cabaret Show, which proved to be a beautiful spectacle that even culminated in an opportunity to mingle with the lady boys!

At night, we enjoyed the vibrant nightlife on the river cruise and on the streets of Bangkok. We also relaxed our tired muscles with Thai massage.

For the shopaholics amongst us, they must have enjoyed doing community service in Siam, the heart of shopping in Bangkok, and bargaining for discounts with local vendors. The EAMSC 2011 ended in high spirits and we begin to look forward to meeting our overseas friends again in the Joint Conference that will be held in Hong Kong this June. ■

Harry Potter 7 - A Grand Finale Adrian Tam (M15)

A sensational masterpiece. Enough said.

16-months after the release of HP6, the long-awaited final episode of the Harry Potter film series was finally released. And this was worth the delay - a magnificent 146-minute loaded with hand-gripping moments, intense actions and breathtaking surprises.

The young magician, Harry Potter pledged to undertake the quest of destroying Lord Voldemort’s secret to immortality – the Horcruxes.

formation from “childhood fantasy” to maturity as numerous beloved characters, including Dumbledore, were murdered by JK Rowling. It was a pity though, that children were unsuitable to watch the new film now with the new plot being too complex and additional scary scenes.

The last enemy that shall be destroyed is death.

“Hallows: Part 1” was a big stepup from its preceding productions.

Performances were remarkable, effects were impressive; and best of all, nothing dastardly was done to J.K. Rowling's fiction. An excellent interpretation by the Director, David Yates, led the audience to a world of fear and desolation, portraying the general atmosphere gripping the wizard community following the return of Lord Voldemort. The film not only adapted high degree of details from the book, but it also explored the feelings amongst the trio: Frustration, jealousy and uncertainty. The balance between the two was tricky, which Yates played it out nicely.

The new film captured the feeling of exactly what the latter Harry Potter episodes should have been all along: gritty, emotional, and pragmatic. It was a big contrast to “HP1: Philosopher’s Stone”, which portrayed a magical and harmonious environment. Whilst the magic element remained, HP7 was an utter trans-

Undoubtedly, it was a sensible arrangement to split the book into two parts. The film was set in an ideal pace – neglecting unrelated portions yet including emotional scenes and little embellishments, e.g. Dobby’s return & the depiction of “The Tale of the Three Brothers”.

“Hallows: Part 1” also ended with a nail-biting suspense where Voldemort seized the unbeatable Elder Wand – a perfect place to finish off. However, this may not be good news to many Potterheads, as there will be a further 12-month wait before the release of “Hallows: Part 2”.

Compliments also to the three leading actors: D Radcliffe, R Grint and E Watson, for their excellent acting. Not to mention R Fiennes, A Rickman, H Bonham Carter, B Nighy, T Felton and a long list more…

It was fair to say that “Deathly Hallows: Part 1” was a remarkable success – arguably one of the best films in 2010. As the Harry Potter film series comes to an end, this production is certainly a grand finale: Finishing with a resounding bang. ■
