I’m a healthy 62-year-old and read that it is called “cough variant asthma.” I would like to know more about this but can’t really find anything about it, except that it will eventually get worse if it’s not treated. I was checked twice in the past 15 years for asthma, and I wasn’t diagnosed. However, I’ve not been diagnosed with any acid reflux or postnasal drip. -- D.S.
ANSWER:Although most people think that asthma causes wheezing, cough is a very common symptom of asthma. You’ve named the two other most common causes of chronic cough: acid reflux and postnasal drip. It can be tricky to make these diagnoses. Very often, primary care doctors will do a “therapeutic trial” that involves a short-term prescription to see if there is improvement. For acid reflux, it might be an H2 blocker like famotidine (Pepcid), which acts faster than a proton-pump inhibitor like omeprazole. This is often faster, easier, and much less expensive than a diagnostic test like a 24-hour pH monitor (the gold-standard diagnostic test).
Similarly, a physician might give a trial of asthma medication (such as a bronchodilator like albuterol) to see if it stops the cough, rather than immediately testing a person’s pulmonary function with a trial of bronchodilators -- a standard diagnostic test for asthma. Sometimes a pulmonologist will order a “methacholine challenge test” where an airway irritant is given at very low doses to see if it will trigger an asthma response in the lung. Postnasal drip can sometimes be confirmed by a physical exam, but a fiberoptic laryngoscopy is seldom necessary to make the diagnosis. When therapeutic trials are not helpful, diagnostic tests become necessary. I personally order lung testing before and after bronchodilators on all patients with suspected asthma, as the tests give important information beyond a diagnosis. Lung testing can be used to follow the course of the disease and identify whether there may be a separate problem in addition to asthma.

A large part of healthy eating involves choosing the right foods. In addition to choosing the right foods, health-conscious individuals must choose the right methods to prepare those foods in order to maximize their nutritional value.
Cooking methods such as frying can make for delicious meals, but such meals may not be so healthy. For example, each tablespoon of oil used when frying can add more than 100 calories to a meal. When counting calories, men and women should recognize that the way they prepare foods can affect the overall calorie count of a meal.

Dogs love spending time outdoors. Dog owners with yards know that dogs benefit greatly from some exercise in the backyard. While that time might be great for dogs, it can take its toll on lawns.
Dog urine and feces can adversely affect the look and health of a lush green lawn. Nitrogen is essential to healthy soil, but only at certain levels. When those levels are exceeded, the result can be lawn damage. According to The Spruce Pets, an advisory site that offers practical tips and training advices to pet owners, this is what happens when pets frequently urinate on grass. Urine is naturally high in nitrogen, so when pets urinate on lawns, the grass might turn yellow or brown due to the excess nitrogen content. Nitrogen also is present in lawn fertilizers, further exacerbating the problem for pet owners who fertilize their lawns.
In addition to urine damage, dogs can trample frosted grass, contributing to problems that may not become evident until spring, and get into areas like gardens where they wreak additional havoc.

DEAR PAW’S CORNER: Spring is here, and summer is just around the corner. Can you please remind people that leaving pets in a hot car is a deadly mistake? -- Carol in Syracuse, New York
DEAR CAROL: Absolutely. Folks, even if the weather outside is just warm -- not hot -- the inside of your car can become extremely hot when it’s parked in the sun. Even with the windows cracked a bit, the temperature can still rise by 20, 30 and 40 degrees higher than the outside air temperature. So, if it’s a balmy 65 degrees outside, the interior of your car can reach 105 degrees or higher -- enough to cause heat injury or death for a pet.
That’s why 31 U.S. states now have laws banning pets in parked cars. It’s why police and even private citizens, in some areas, can smash a car window to save a pet. And it’s why pet owners who leave their animal in a parked car may face fines and, in some states, prison. Those laws do not make exceptions for cars that are left running to keep the air conditioning on. If the car stalls or has an automatic switch-off, the interior will heat up dramatically even on a cloudy day.

“One glimmering memory with Mom involves: a perilous journey, a snarky roller coaster guide, and a serene stroll along a mountaintop pond.
It was late autumn, that special time of year when even the air itself smells like nostalgia and calm delight. It was her birthday. We went for a long road trip to the mountains. Outside the car window, foliage passed by like trees on fire with their flapping leaves. Lovely, perfect temperature, chilly without being cold. Excitement built inwardly as we approached the mountain. Within an hour, New York fields of harvested produce began to morph into hills blanketed with colorful fauna, including red and sugar maples; elm and birch; hawthorn, oak and chokeberry.
Then these hills grew into full, luscious mountains, informing us we had arrived at last.
We parked and got out to stretch. Teeming masses strolled to and from the facility. We hardly noticed. We were too excited to be bothered, for as we approached the ticket booth, we spotted the lifts, rising beyond sight to the peak. Mom bought out exorbitantly priced tickets (I paid for mine, as she was paying for the gas), and then we were off to those lovely, terrifying lifts. Gondolas, as Mom called them.
Designed with skiers in mind, they keep them running in seasons without snow for those who seek a gentle little thrill. Mom had always wanted to take a lift up a mountain. With a bit of a hitch we seated ourselves on one as it came round, and then we were off. I thought I would be okay, but then we left the platform, and rose. And rose, ever higher, with nothing but a skinny bar which does not lock to protect us. I do not like heights. I did not enjoy being that high, as it stripped me of all control. If the lifts break down, or something goes wrong, you can do nothing at all. You are stuck. Ever seen 2010’s Frozen? Yeah.
But Mom loved it. She found it incredibly relaxing, and for this I was glad to be sitting beside her, for she was at last happy and contented, which meant the world to me. When we reached the top, we decided to get off to take a stroll in the mountain air. Atop this highland there was a small lake, and we promenaded along its fringes, snapped a few iconic shots. Oddly the air was warmer up there. But so nice. We were away from power lines, and Mom was finally, completely, de-stressed. Which made me happy too. I was glad to be there with Mom. But we weren’t done with our little venture. When we got back down, she wanted to take the Sky Coaster. So we used our tickets and got on the coaster. Before leaving the station, a snarky coaster guide snarked at Mom for not using the hand brake properly. But then we were off, she in the car in front of me, I in my own behind her. Like a proper coaster, we ascended for some time, climbing the mountain again. It was soothing and I was very grateful to be ascending while closer to the earth, this time. Once at the top, we began the descent. I hadn’t been paying attention to the snarky instruction guy, so I basically just fiddled around with the hand brakes. Up ahead, Mom zipped through the forest in her car, taking turns whiplash fast, because she wasn’t sure how the brakes worked either.It had something to do with pulling to release them, and releasing them to go faster, or something, which is why we were confused. I’m pretty sure she took those turns without braking at all, which explained the screams she was unleashing. But they were joyful ones. She was having a blast. For my part, I struggled to keep from bumping into her car, finagling my brakes because we were zooming and I had no idea if these coasters would fly off their tracks.
At the bottom, her hair all up and over the place, Mom was beaming. That was a very fine day, indeed.”
