A focus on pets’ weight for the New Year DEAR PAW’S CORNER: I’ve made a resolution that I hope will resonate with many of your readers. My cat, “Gerard,” is 6 years old and has become very overweight. I can barely pick him up! Since October, I have been working with Gerard’s veterinarian to manage his weight, and things are turning around. I hope that this year, readers will focus on their pets’ whole health. Not just checkups, but everyday choices that will give their pets, like my cat, a longer, healthier life. -- Carly B. in Youngstown, Ohio DEAR CARLY: That is a fantastic resolution. Focusing on our pets’ well-being not only benefits them, but positively affects our lives as well.

& Yoga Teacher Training Center Pulmonary Nodules Appear, Leading To Concern About CANCER DEAR DR. ROACH: I am a 76-year-old woman in what I thought was excellent health. On a recent coronary calcium CT scan that was ordered because of my age and slightly elevated cholesterol, an incidental finding was described in the report as “innumerable, tiny noncalcified pulmonary nodules bilaterally.” The largest of these is in the “left lower lobe with multiple additional 2- to 4-mm nodules,” and they recommended “clinical correlation and a consideration of a dedicated chest CT.” I found the report quite scary, especially considering my mother and grandfather died of lung cancer (although they were smokers and I never smoked). My primary physician has ordered the chest CT but wants to wait four weeks. She says that the nodules could be caused by inflammation or infection and might resolve on their own.

My fear is that they’re actually lung cancer and waiting four weeks might be problematic, not to mention extremely anxiety-producing. I would appreciate your input. -- M.S. ANSWER: I certainly understand why you are concerned about lung cancer given your family history, but primary lung cancer is very unlikely. Since there are so many of these tiny nodules, I agree with your doctor that these are highly unlikely to be lung cancer, which is generally only a single nodule.

Your doctor is correct that there are inflammatory and infectious conditions that can cause a similar appearance on a chest CT. Some of these infections are serious (including tuberculosis), but people generally have symptoms. So, I think this is unlikely. Exposure to lung toxins, especially silicosis and asbestosis, are generally occupational, but they are another potential cause.

Some cancers can spread to the lungs. While the appearance of nodules on a CT scan in a person with no known cancer would be unusual, if the CT scan in four weeks shows that the nodules are enlarging, then this would be concerning.

Getting another scan in less than four weeks might not be enough time to see a change.

I am sorry about the unavoidable anxiety in waiting for the scan, but it’s likely that the scan won’t show a change and you will be able to stop worrying.

DEAR DR. ROACH: I was experiencing hoarseness while taking ibuprofen. A friend got intestinal bleeding due to taking it. My doctor gave me 7.5 mg of meloxicam daily, but I’m not sure it’s OK for long periods of time. I’m 73 with osteoarthritis and osteopenia.

I exercise daily and eat a healthy diet, and I have high blood pressure that is controlled with losartan and amlodipine. -- T.M.H. ANSWER: Ibuprofen is generally a safe drug, but it does have the potential for side effects -- some of them being serious.

Gastrointestinal bleeding is a serious one, and it is most common in older women. I would not try a second nonsteroidal antiinflammatory drug (NSAID) like meloxicam in a person who has a history of serious bleeding with any other similar drug.

Hoarseness could be due to an allergy, due to direct irritation of ibuprofen on the throat, or from the worsening of acid reflux, which can cause hoarseness if acid gets all the way into the throat.

Meloxicam is also an NSAID but is chemically in a different class from ibuprofen (Motrin, Advil and others) or naproxen (Aleve and others), which are closely related. It’s reasonable to give it a try, but if it causes hoarseness again, then an NSAID probably isn’t a good choice for your arthritis. You could consider acetaminophen (Tylenol) or topical NSAIDs, which do not get absorbed enough to cause an increased bleeding risk.

Kids have a long history of disliking dental care, which can frustrate parents who know the importance and long-lasting benefits of proper dental hygiene.

While many youngsters may never excitedly run to the bathroom to brush their teeth, parents can try various approaches to get kids to embrace proper dental care.

Start early. Parents should not hesitate to begin cleaning kids’ teeth once the first tooth appears. The earlier dental care is part of a child’s routine, the more likely he or she is to accept it as part of everyday life.

The American Academy of Pediatric Dentistry recommends using a softbristled brush with a small head. Ideally, use a toothbrush designed for infants, brushing teeth once per day at bedtime. The AAPD advises taking kids to visit a pediatric dentist when the first tooth appears, or no later than the child’s first birthday.

Make it a group effort. Young children love to imitate their parents’ actions and behaviors, and moms and dads can use that adoration to their advantage when trying to get kids to brush their teeth. Brush teeth together as a family, making it seem like an enjoyable twice-daily activity.

Get down on kids’ level and show them how to brush. If kids are resistant, engage them in conversation while brushing so they don’t even realize they’re cleaning away.

Involve kids in choosing their dental care products. While parents should be mindful of the type of toothbrush their children use, always sticking with products recommended by their child’s pediatric dentist, let youngsters choose their toothbrush and toothpaste. Kids might prefer a particular flavor of toothpaste, and a colorful toothbrush with a favorite film or television character might make kids more enthusiastic and less fussy about brushing.

Compliment kids after positive dental appointments. When pediatric dentist appointments go well, compliment children and express your pride in them for taking dental hygiene as seriously as they do.

HOLIDAY BAKING POINTERS Baking is a science and the ingredients are used in a ratio to produce a desired chemical reaction while cooking. Do not substitute ingredients, and measure each addition carefully.

Scooping packs flour down — potentially causing you to use more than the recipe calls for. Instead, spoon the flour into your measuring cup gently and use a straight edge to level it out. Even better, use a scale and metric measurements to ensure exact amounts of wet and dry ingredients.

Certain ingredients, like butter and eggs, are temperature-dependent. Cold butter in biscuits helps them to rise up flaky and delicious. Eggs brought to room temperature enable the emulsification process to work more readily. The difference in temperature can mean a completely different chemical reaction, so follow the recipe accordingly.

Purchase an oven thermometer, set your oven to 350 F, the standard baking temperature, and see what the thermometer reads. If it is different, adjust cooking times accordingly or have the oven repaired.

Use unsalted butter. Most recipes will call for unsalted butter so you aren’t adding unwanted sodium to the recipe, affecting dough consistency and flavor.

DID YOU KNOW? The American Heartworm Society advises dog owners to have their furry companions tested for heartworm annually. Such tests can typically be conducted during routine preventive care visits to the veterinarian. The AHS notes that such tests are necessary even for dogs that are taking year-round heartworm prevention medications. Tests can confirm that such programs are working or indicate if they are not effective. The American Veterinary Medical Association notes that antigen tests are the most commonly performed tests. These tests detect the presence of adult female heartworms and they are typically highly accurate. If a test is positive, veterinarians may recommend further testing, including radiographs, a blood profile and an echocardiogram. Such tests can determine a number of things about the infection, including its severity. The AVMA also notes that the additional testing can help determine the best course of treatment. Cat owners should know that it’s more difficult to detect heartworm in cats than in dogs. The AHS indicates that cats are much less likely than dogs to have adult heartworms, making the disease harder to detect in felines. Veterinarians may recommend cats receive both the antigen test and an antibody test, which detects exposure to heartworm larvae. The AVMA notes that more testing may increase the likelihood of detecting the disease in cats.

emma moore How to preserve food to eat local and reduce waste

Emma Moore spends a lot of time thinking about food: where our food comes from, who grows it, and how to preserve fresh, local food to be eaten any time. Moore is the Agricultural Awareness and Literacy Educator at the Cornell Cooperative Extension of Ontario County (CCEOntario) as well as a certified Master Food Preserver and often teaches workshops and visits classrooms to talk about local food and agriculture.

Moore grew up in Medina and tapped into the local farming community from an early age. She began showing rabbits with the Orleans County 4-H as a Cloverbud, the youngest age group, and when she got older, she showed horses too. She earned a degree in environmental studies from the SUNY College of Environmental Science, where a deep focus on food systems shaped her path. Moore now combines that academic foundation with her upbringing in an agricultural community in her work with CCE-Ontario, helping others understand food systems and the value of local agriculture.

foods throughout the year and eliminating waste.” She attended a three-day training that blended book training with hands-on practice in multiple food preservation methods, including freezing, dehydrating, canning, and fermentation. After completing 80 hours of practice in her home kitchen, she was certified to teach classes in food preservation.

It took about a year for Moore to complete her Master Food Preserver certification. She says, “I did a lot of dehydrating and fermenting and canning at home. I love sweet things, so I got really into different jams and jellies.” She’s also well-practiced in fermentation, especially sauerkraut. “I have a very German family, so there’s sauerkraut going at some point in my house all the time,” she says.

CCE-Ontario needed someone who could answer community questions about pressure canning and other food preservation techniques, so Moore decided to take the Master Food Preserver training to fill this role. Learning how to preserve food fit with Moore’s existing knowledge. She says, “I was already interested in food systems, and food preservation helps so much with using local

awesome when you can grow 80,000 tomatoes, but if you don’t know what to do with it, it’s just a little bit sad to see all that go to waste.”

Moore loves her work teaching food preservation, but she says her favorite part of her job is visiting classrooms and helping kids think about where their food comes from for the first time. “With many, many kids, if you ask them where something comes from, they usually say the grocery store or maybe a factory. It’s surprising for them that so much of what we have comes from farms, and I really enjoy being able to open their eyes to that.”

Moore also helps people think differently about who they imagine farmers to be, explaining, “It’s important to know that lots of food can and does come from our communities. When we think of someone who’s a farmer or a food producer, we could think of our neighbor or our mom or ourselves.”

Not every food preservation project has gone perfectly, but Moore’s training helps her understand what went wrong and when something becomes unsafe to eat. Moore says patience is incredibly important in preserving food so it tastes good and is safe to eat months later, but Moore confesses she can sometimes be a little impatient. She recalls one instance when she had to eat a lot of wax beans for a few weeks after an unsuccessful attempt at pressure canning. “I canned a bunch of wax beans and opened the lid too soon,” Moore said. Opening the lid before it’s totally cool can cause liquid to siphon out of the jar and reduce the long-term safety of your food. “It was perfectly safe and fine to [open the pressure canner], but a lot of jars were half empty and not something I wanted to put on the shelf.”

If you’re curious about trying your hand at food preservation, Moore suggests browsing a local farmer’s market. You’ll have a better understanding of what’s local and in season, and you can usually get a lot of produce for a good price. Moore also teaches food preservation classes at the CCE-Ontario offices in Canandaigua and other locations.

Learning how to preserve food is an important way to tap into local food systems and reduce waste. “When we think of food preservation, we’re trying to make something shelf stable,” Moore says. “If you’re a home gardener and you have a million green beans and you don’t know what to do with them, you could make quick pickles or pressure can them and have something from your backyard any time of year.” When you preserve your own food, you know exactly what went into the jar and you can reuse the jars year after year.

Moore plans her food preservation classes around what produce is currently in season. She hopes to help home gardeners, including members of CCE’s popular Master Gardeners program, use what they grow. She says, “It’s

First-Time Hosts The holiday season is a busy time of year. From holiday shopping to readying the home for guests, there’s no shortage of things to accomplish in a short amount of time. Seasoned holiday enthusiasts may be ready to tackle tasks with ease. But for first-time holiday hosts, the prospect of crossing items off to-do lists may require a little extra assistance.

• Do as much prep work in advance as possible. Giving yourself extra time to get things done can help immensely.

Grocery shop one day, prepare food the next, clean the house one day, and set the table or arrange decor on another. Minor task can be left for the morning of the day guests arrive.

• Cook and freeze items. Utilize recipes that enable you to cook in bulk and then store foods until you need them.

Casseroles, baked pasta dishes, freezable soups, and ready-to-bake cookie doughs made in advance will save a lot of time closer to the holidays.

• Stock the pantry. If holiday hosting involves overnight guests, be sure to keep easy items on hand so guests can help themselves. Dry goods or pre-packaged muffins for breakfast, chips or pretzels for late-night snacks, granola bars, or store made croissants can fit the bill. Put everything in a dedicated basket so guests can grab items as needed. Stock the refrigerator with water, seltzer, iced teas, and sodas, and make a full pot of coffee in the morning so guests can pour their own.

• Avoid complicated cocktails. You don’t want to be stuck mixing drinks all evening. Instead, make it simple for guests to help themselves. Wine, beer, premade punches, or even one specialty drink are ideal.

• Consider hiring a caterer. If the idea of a homecooked meal feels like climbing Mount Everest, swap out some items for store-bought alternatives, or work with a favorite restaurant or caterer to secure foods that only need to be reheated and served.

• Free up closet and room space. Guests will be coming with a lot of stuff, particularly if you live in a cold climate.

Make room in the entry closet for guests’ coats and boots.

Be sure you can accommodate suitcases or overnight bags. Set aside an area where guests can deposit their wrapped gifts.

Avoiding Medicare fraud Medicare is a pot of gold for scammers. There is a lot of money to be made by those who successfully commit fraud against seniors.

You, however, can stop them in their tracks by not falling for the scams.

Here are just a few of the scams: Genetic testing is a hot topic for those who want to be screened for a variety of potential illnesses. If you get a call offering genetic testing (or even at a booth at a senior fair), don’t believe the scammer who says that Medicare will pay for it, and that all you need to do is provide your Medicare number. Never give that number out! The only legitimate way Medicare will pay for genetic testing is if your doctor thinks it’s medically necessary and orders it.

Better Function in Bathrooms Often when people develop mobility or dexterity issues, they find their existing bathrooms are not in a convenient location in the house, or aren’t configured with safety in mind. They may need to add a bathroom in a small space or reconfigure the ones they have. When renovating an existing bath or creating a new bathroom where one did not previously exist, homeowners should keep key considerations in mind.

Multiple light sources can help reduce glare and shadows - two dangerous visual effects in bathrooms, where most home falls occur. Flooring material should be slip-resistant and grab bars are essential in showers, bath tubs and near toilets. Shower heads should be on a slide bar that allows the spray to be raised or lowered depending on the needs and height of the user. Showers and faucets should be controlled by levers, rather than knobs, and all showers should have thermostatic controls to reduce the risk of burns.

“The majority of firefighters in the U.S. are volunteers—neighbors helping neighbors in an incredible and rewarding way,” said Hirsch. “We need more people to answer the call to serve.”

Training is provided by the department, and volunteers can serve in a variety of roles, from providing life-saving emergency response services to conducting fire prevention programs and disaster preparedness planning.

Distracted Driver!

I am so very thankful to the kind man, lady & yellow lab in the red pickup truck towing a trailer in Hamlin! I was distracted—not from texting or talking on my cell, but mentally not focused on driving due to a recent death of a family member. I went through a stop sign & cut him off. He skillfully maneuvered his truck to avoid a collision. I apologized but want him to know how very thankful I was of his skill & kindness! I also want to make people aware of how we can become a “distracted” driver due to many circumstances. Please don’t drive if you physically or emotionally aren’t up to it. HAPPY Blessed HOLIDAYS to all, especially my hero in the red pickup truck!

Some folks may be immunodeficient because of cancer, or chemotherapy, and perhaps other conditions as well. These people are still friendly & peaceful; and challenged. It’s bad neighboring to leave your dog chained in the front yard all hours of the day to bark menacingly at everyone and everything, even at 6:45am! It’s also animal cruelty. The poor thing is NEVER walked, just plopped out there to suffer in the sun, rain or snow. Grow up!
