If you get a call offering genetic testing (or even at a booth at a senior fair), don’t believe the scammer who says that Medicare will pay for it, and that all you need to do is provide your Medicare number. Never give that number out! The only legitimate way Medicare will pay for genetic testing is if your doctor thinks it’s medically necessary and orders it.

Unfortunate But True So let me get this straight: I can buy ham in a plastic bag, bread in a plastic bag, milk in a plastic jug, napkins in plastic wrap, salad in a plastic tub, and ketchup & mustard in plastic bottles‚ but I can’t get a plastic bag to carry it all home because that’s what’s bad for the environment? Distracted Driver!

I am so very thankful to the kind man, lady & yellow lab in the red pickup truck towing a trailer in Hamlin! I was distracted—not from texting or talking on my cell, but mentally not focused on driving due to a recent death of a family member. I went through a stop sign & cut him off. He skillfully maneuvered his truck to avoid a collision. I apologized but want him to know how very thankful I was of his skill & kindness! I also want to make people aware of how we can become a “distracted” driver due to many circumstances. Please don’t drive if you physically or emotionally aren’t up to it. Some folks may be immunodeficient because of cancer, or chemotherapy, and perhaps other conditions as well. These people are still friendly & peaceful; and challenged. Hands may transmit any virus, as well as Covid-19, bacteria, fungus, etc. It is now cold & flu season; please cover your sneezes and coughs but ideally not with your hands. And wearing a mask is also mandatory for some of us.

It’s bad neighboring to leave your dog chained in the front yard all hours of the day to bark menacingly at everyone and everything, even at 6:45am! It’s also animal cruelty. The poor thing is NEVER walked, just plopped out there to suffer in the sun, rain or snow. Grow up!

Pulmonary Nodules Appear, Leading To Concern About CANCER DEAR DR. ROACH: I am a 76-year-old woman in what I thought was excellent health. On a recent coronary calcium CT scan that was ordered because of my age and slightly elevated cholesterol, an incidental finding was described in the report as “innumerable, tiny noncalcified pulmonary nodules bilaterally.” The largest of these is in the “left lower lobe with multiple additional 2- to 4-mm nodules,” and they recommended “clinical correlation and a consideration of a dedicated chest CT.” I found the report quite scary, especially considering my mother and grandfather died of lung cancer (although they were smokers and I never smoked). My primary physician has ordered the chest CT but wants to wait four weeks. She says that the nodules could be caused by inflammation or infection and might resolve on their own.

My fear is that they’re actually lung cancer and waiting four weeks might be problematic, not to mention extremely anxiety-producing. I would appreciate your input. -- M.S. ANSWER: I certainly understand why you are concerned about lung cancer given your family history, but primary lung cancer is very unlikely. Since there are so many of these tiny nodules, I agree with your doctor that these are highly unlikely to be lung cancer, which is generally only a single nodule.

Your doctor is correct that there are inflammatory and infectious conditions that can cause a similar appearance on a chest CT. Some of these infections are serious (including tuberculosis), but people generally have symptoms. So, I think this is unlikely. Exposure to lung toxins, especially silicosis and asbestosis, are generally occupational, but they are another potential cause.

Some cancers can spread to the lungs. While the appearance of nodules on a CT scan in a person with no known cancer would be unusual, if the CT scan in four weeks shows that the nodules are enlarging, then this would be concerning.

Getting another scan in less than four weeks might not be enough time to see a change.

I am sorry about the unavoidable anxiety in waiting for the scan, but it’s likely that the scan won’t show a change and you will be able to stop worrying.

DEAR DR. ROACH: I was experiencing hoarseness while taking ibuprofen. A friend got intestinal bleeding due to taking it. My doctor gave me 7.5 mg of meloxicam daily, but I’m not sure it’s OK for long periods of time. I’m 73 with osteoarthritis and osteopenia.

I exercise daily and eat a healthy diet, and I have high blood pressure that is controlled with losartan and amlodipine. -- T.M.H. ANSWER: Ibuprofen is generally a safe drug, but it does have the potential for side effects -- some of them being serious.

Gastrointestinal bleeding is a serious one, and it is most common in older women. I would not try a second nonsteroidal antiinflammatory drug (NSAID) like meloxicam in a person who has a history of serious bleeding with any other similar drug.

Hoarseness could be due to an allergy, due to direct irritation of ibuprofen on the throat, or from the worsening of acid reflux, which can cause hoarseness if acid gets all the way into the throat.

Meloxicam is also an NSAID but is chemically in a different class from ibuprofen (Motrin, Advil and others) or naproxen (Aleve and others), which are closely related. It’s reasonable to give it a try, but if it causes hoarseness again, then an NSAID probably isn’t a good choice for your arthritis. You could consider acetaminophen (Tylenol) or topical NSAIDs, which do not get absorbed enough to cause an increased bleeding risk.

Take charge of your to-go order: Convenience became the name of the game when ordering food during the pandemic. Curbside pickup service was offered as a safety measure, and many restaurants that never previously offered delivery or takeout started to do so to generate revenue at a time when in-person dining was limited if not disallowed. Some restaurants began offering delivery through apps like Grubhub or Uber Eats, and though that may be convenient for diners, restaurants must pay a fee to utilize those apps. In lieu of ordering through a third party delivery service, consumers can take charge of their to-go orders and pick up the food themselves. That saves restaurants the delivery app surcharge, allowing them to increase net profits on the meals they sell.

Wayne County Residents Raise Guide Dogs for People With Vision Loss B Y S O P H I A KO VAT C H

says Cyndi Marvin, who is currently co-raising her 11th puppy with wife, Darleen Farley, in Clifton Springs. Training begins immediately with essential puppy skills such as learning their name, coming when called, and doing their business outside.

Socialization is critical. Pups go everywhere with their raisers, exposing them to any environment, from grocery stores to the doctor’s office. Farley says, “In some ways, puppy raising is twenty-four hours a day, seven days a week because the pups live with the raisers, and we are responsible for their care and wellbeing.”

Raisers and puppies attend regular training sessions with an instructor from Guiding Eyes for the Blind, which uses a specific training program focused on positive reinforcement and building the dog’s confidence. Beth Walker, regional coordinator for Wayne County, finds the training program to be one of the best parts of working with Guiding Eyes. She says, “I find the positive and success-oriented training of both the dogs and raisers to be a good fit with my background as an educator. The methods used are respectful and geared toward creating a dog who can work as a team member.” Walker will get her third puppy through Guiding Eyes shortly.

Darleen Farley, Cyndi Marvin and Zephram Guide dogs help people with vision impairments navigate dayto-day lives, providing both safety and independence to those who need this essential assistance. But guide dogs aren’t born knowing how to guide. Instead, they’re raised by dedicated volunteers who ensure puppies get the basic training and socialization required to be an excellent guide dog. Some such volunteers reside in Wayne County, raising puppies through Guiding Eyes for the Blind.

Puppies are placed with their raisers when they are eight weeks old. “They are cute and somewhat clueless, but already smart,”

command and blocked his person from moving. A speeding car was making an illegal turn. Had they stepped into the crosswalk, it is likely both would have been severely injured or killed.”

It takes a lot of effort to raise a puppy into a working dog, and though raisers are often sad to return them to Guiding Eyes for their next phase, it’s incredibly fulfilling to see the dog take on the role they’ve been learning their entire life. “It’s rewarding to watch your puppy grow, mature, and grasp all you are teaching them with confidence and independent thinking,” says Linda DePuy, a Macedon resident who has helped raise more than 25 dogs since she began working with Guiding Eyes for the Blind in 1987.

As coordinator, Walker makes sure her raisers and other volunteers in the Wayne County region have all the support they need to maintain the Guiding Eyes training for their puppies. She schedules training sessions, helps active raisers find approved puppy sitters to cover vacations, and serves as liaison to the Guiding Eyes’ central offices. Other volunteers help loan out dog crates, compile reports on puppies’ weight and food, and post about Guiding Eyes on social media.

Once the guide-dogs-in-training are between 18-24 months, they go back to Guiding Eyes in Yorktown Heights, NY, for assessments and further training. Not all puppies end up being working guide dogs. Some become firearm or explosive detection dogs, some go into the Guiding Eyes breeding program, and some adopt back to their raisers or to other families. Every raiser is clear that ultimately, the dog decides what they will do with their training. As Walker puts it, “It’s a partnership with the dog.

If you force them, they won’t do the job well anyway.”

The final stage of training takes place when a dog is matched with the person they will guide. The pair trains together for two weeks at the Yorktown Heights facility to build a trusted partnership where the dog can safely guide the person through daily life.

It’s critical to establish that trust right away, because dangerous situations can arise at any time.

Farley says one of the dogs she and Marvin raised saved his person’s life just two days after they graduated from Guiding Eyes: Farley says, “They were walking the neighborhood so Jax could learn the daily routine. They had stopped at a crosswalk. When given the command to go forward, Jax refused the

Farley says, “It is awe-inspiring. To realize that little fur ball who was not even housebroken when he came to us is now capable of safely guiding a person through their daily routine is beyond amazing.” Marvin adds, “I feel pride like a parent when their child does well.”

Guide dogs are placed all over the country and at no cost to the visually impaired person. There’s a long waiting list of people looking for qualified guide dogs, so Guiding Eyes for the Blind is always looking for new potential puppy raisers. Walker says, “Our raisers come from all walks of life.

We have people who raise in all kinds of capacities.”

Farley adds, “Puppy raisers are essential to preparing dogs to guide persons with visual impairments. Without puppy raisers there would be no guides because puppies can only learn how to live in a home by living in one.”

Practice self-control in all aspects of life. Some researchers believe that the self-control necessary to help people maintain their resolutions is like a muscle that needs to be exercised. When selfcontrol is revved up across many different activities, it’s easy to realize one particular resolution.

For example, in an experiment at the University of Albany, researchers asked 122 smokers who were trying to quit to practice extra self-control for two weeks, either by avoiding sweets or by squeezing on a grip strengthener for as long as they could twice a day. Twenty-seven percent of those who were diligent about practicing their self-control exercise successfully kicked their cigarette habit in the following month, compared with just 12 percent of volunteers who didn’t maximize self-control.
