Medications and Pharmacy Questions to Ask You should be able to answer the following questions before taking any new medication. Although each medication comes with instructions, your doctor and pharmacist should be available to answer any or all of the following questions in more depth and in language that is easier to understand.

What is the name of the medication and what is it supposed to do?

You should know the names of all the medications—both prescription and nonprescription— you take so you can inform each doctor you see.

It is also important to know what each medication looks like.

When and how do I take it?

You need to know how often to take your medication, if the medication is best taken on an empty stomach or with food, and if you should take it at the same time each day.

For how long should I take it?

Your prescription order indicates the length of time you should take the medication and whether

refills are available. Skipping doses or stopping medication to save money or because you “feel better” can result in health problems requiring more expensive treatment in the future.

Should I avoid alcohol, any other medications, food and/or activities?

Certain foods or alcohol may also interact with your medications. Some drugs can cause drowsiness and may affect activities such as driving.

Should I expect any side effects?

All medications can cause side effects, but they are not necessarily serious. Your pharmacist will inform you of the most common side effects. If you experience any unexplained effects, contact your doctor or pharmacist.

What if I forget to take my medications?

Be sure you know the answer to this question when you receive the prescription. The decision to take a missed dose depends on the drug. Don’t panic and don’t take a double dose unless you are specifically directed to do so by your doctor.

Is there a generic version of the medication?

Not all medications have generic counterparts. If a generic version is available, the Food and Drug Administration (FDA) has judged it to be equivalent to the brand name product and can save you up to half the cost.

How should I store my medications?

Proper storage ensures a medication’s effectiveness. The bathroom medicine cabinet is not an ideal storage place. Heat and humidity can adversely affect your medication. Most medications require a cool and dry storage location, and some need refrigeration.

Does this medication replace anything else I was taking?

Make sure you understand if this medication is replacing another medication.

Transportation Options Parking and Transportation At Reston Hospital Center, we want you to feel comfortable and prepared for your outpatient visit, hospital stay or surgery. We have a variety of hospital and community resources that can help make your trip to and from the hospital a smooth experience.

Parking (see map on pages 4-5) You’ll find several convenient, complimentary parking options for hospital patients and visitors and for appointments with your physician, radiology, lab services or outpatient surgery, including: • Main Entrance, with free valet service • Self-parking available free of charge in the parking garatge on campus. The garage is located in the rear of the hospital, between the Hospital Center Plaza Buildings and the Parkway Medical Tower.

• Emergency Room parking available in front of the emergency room. *Please note that the hospital is not responsible for items left in vehicles. Hospital security patrols the parking garage regularly.

Reston Hospital Center offers complimentary valet parking for patients and visitors.

Campus Shuttle The campus shuttle is available Monday through Friday from 6 a.m to 8 p.m. to take patients and visitors from parking areas to buildings on campus. The shuttle picks up at stops marked throughout campus (see map pages 4-5) or can be called for a direct pick up at 571-388-8733.

Transportation Home When your doctor decides you’re ready to leave the hospital, you’ll receive a discharge order with instructions for follow-up care, such as medication, dietary requirements or recommendations and your next scheduled appointment with your doctor. You may stop by the hospital cashier to settle your account. Then, you’ll be escorted by a hospital representative to your waiting transportation. You may want to arrange with a family member or friend to take you home or utilize a variety of community transportation options listed on pages 10-12.

Patient Frequently Asked Questions About Acute Inpatient Rehabilitation

Your physician has recommended that you be assessed for acute inpatient rehabilitation. It is very common for patients to have many questions about what acute inpatient rehabilitation is and what it involves. This sheet is designed to help answer some of those questions, and if you have additional ones, please do not hesitate to ask.

Why is this assessment necessary?

Your doctor feels that you will benefit from acute inpatient rehabilitation. An assessment is performed to determine if you meet certain criteria that qualify you to receive this service.

What is acute inpatient rehabilitation?

Acute inpatient rehabilitation is a combination of medical, nursing and therapy care designed specifically for patients with neurological, musculoskeletal, orthopedic or other medical conditions following an acute hospitalization.

What is acute inpatient rehabilitation and why is it important for me?

Acute inpatient rehabilitation is the most intensive and focused rehabilitative care that you can receive. It is only provided in a rehabilitation unit of a hospital or in a freestanding rehabilitation hospital. It is designed to help you obtain the highest level of function so that you can return home safely. It involves close medical supervision by a specialized rehabilitation physician and a multidisciplinary team of therapists and nurses working towards one goal, returning you home safely and as independent as possible.

Who performs this assessment?

A case manager, who is most commonly a registered nurse or social worker, will perform your assessment. They are trained to assess you for rehabilitation. The information they gather is then reviewed by a physician specializing in rehabilitation who determines if you will qualify for and could benefit from acute inpatient rehabilitation.

When is this assessment performed?

In most cases, the assessment is performed today, or as soon as it can be scheduled.

How much does this assessment cost?

This assessment is completely free of charge.

How long does this assessment take and who determines if I am accepted?

The assessment usually takes 45-60 minutes to complete. The decision is up to the rehabilitation physician.

Do I have to travel for this care or can I stay here?

Our facility feels strongly about allowing you to have a choice of where you want to have your acute inpatient rehabilitation. This hospital does not have a program in house, although there is one or more facilities close by affiliated with this hospital that do.

Information on the location, driving distance, directions, and about the acute inpatient rehabilitation program itself can be provided for you. If you have a preference for a different acute inpatient rehabilitation provider, then we are happy to arrange that as well.

Case Management If you are admitted to Reston Hospital Center, you will be assessed by the nursing staff for your ability to care for yourself at the time of admission and following the hospital stay. If you require additional healthcare services after time at the hospital, such as home care or skilled nursing, you will receive assistance with these arrangements from a nurse or social worker in the Case Management Department.

Based on your agreement with your insurance carrier, a Case Management nurse will provide clinical information concerning the medical necessity of your admission and continued state to your particular insurance company. In turn your insurance carrier will approve or deny your hospital stay. The Case Management Department will work together with the rest of the clinical staff to provide you with a beneficial hospital stay.

Our Case Management Department will provide phone numbers for private duty nursing and agencies for patients who need this type of care. It is the responsibility of the patient’s family and/or significant other to make all necessary arrangements, including payments.

A Growing Number of Options Nowadays, independent living communities, assisted living facilities, nursing homes, and the like are catering to more and more people with a variety of care needs.

Home Care (see p. 32) Advance Directives Home care provides a limited amount of assistance with routine activities (such as shopping, meal preparation and getting to medical appointments) to individuals who desire and are able to live independently in their own homes, and who do not require constant medical care.

Many people today are worried about the medical care they would receive if they should become terminally ill and unable to communicate their wishes.

That’s why a growing number of people are taking an active role in their care before they become seriously ill. They are stating their health care preferences in writing, while they are still healthy and able to make such decisions, through legal documents called advance directives. Before deciding what choices about your care at the end of life are best, you should talk over the issues involved with your family and your physician. You can learn more about these documents at caringinfo.org.

Independent Living and Assisted Living (see p. 18)

Often termed “congregate care,” there is a broad spectrum of independent and assisted living care options available to seniors with varying needs. “Independent living” facilities are specifically designed to accommodate social and recreational activities within a community of seniors.

Money Matters

“Assisted living” or “residential care” tends to combine independent living facilities with shared services such as dining, laundry and housekeeping— and in some cases, medication management and other personal services.

They also include some shared living spaces and provide access to social and recreational activities and transportation. Unlike nursing homes, assisted living communities do not provide extensive medical care.

Nursing home care can come at a significant cost. Medicare pays for some nursing home care costs, but generally only for an initial period—not over the long-term as most people need.

AARP recommends that seniors buy special insurance to help finance their long-term care, and most financial planners recommend saving for long-term care as part of retirement planning.

Intermediate Care and Nursing Homes (see p. 27)

Prescription Discounts

Intermediate care facilities provide additional assistance with daily activities beyond what assisted living provides, such as help with bathing and eating.

“Board and care” homes are subsidized group living arrangements that provide help with daily activities to low-income seniors who do not require the level of care of a nursing home. Nursing homes go a step further by providing true nursing care at a full-time level once only available through hospitals.

Unlike at hospitals, however, residents of nursing homes are still free to come and go as they please.

Hospice (see p. 39)

The mission of hospice care is to provide supportive, as opposed to curative, care to terminally ill patients in order to make them as comfortable as possible. It is specifically funded for those whose life expectancies are six months or less. As such, hospice combines care at medical facilities with varying degrees of home care in order to provide around-the-clock assistance and/or monitoring.

How to Weigh Your Options

Your primary consideration in selecting long-term care should be the type and level of care you need. Many facilities specialize in caring for specific health conditions, such as heart problems or dementia. You also need to consider your budget, any existing coverage you have, proximity to family and friends, and overall daily environment.

Because there are so many options in a given area, it helps to collect information on several facilities and compare them. Also, be sure to visit every facility you are seriously considering in person. To help you get started, Medicare offers a comprehensive listing of U.S. nursing homes by state.

Finding discounted prescriptions may be easier now that prescriptions are considered an “essential health benefit” under the Affordable Care Act– meaning that your insurance must cover them. However, there are more ways to save.

• Prescription Drug Coupons - Ask your pharmacist about pharmacy discount cards that can save you another 10 to 25 percent.

• Patient Assistance Programs - Pharmaceutical companies and nonprofit organizations offer grants and discounts to patients in financial need.

Other Practicalities Besides financial considerations, there are other important concerns. Is the nursing home in question close to friends and family who will visit often?

How are certain amenities, such as pharmaceutical services, handled there?

It’s also important to ask which hospitals a facility has contractual relationships with, should residents require hospitalization.

Meet the Volunteer

F

or Roy Horowitz, life has always been about family and helping others whenever possible. “I am a people person, period,” said the Reston resident. “I love people, I think positive and I’m a humanitarian in my own heart.”

Roy worked for several years as the New Jersey representative for an outerwear manufacturing company based out of Seattle, Wash. He and his wife of 58 years, Jean, have two children and five grandchildren.

Although they initially envisioned a retirement spent living along the shore, their children’s relocations to Reston ultimately swayed their decision.

“We decided as we get older, it’s going to be harder for us to keep traveling back and forth,” said Roy. “So why not go where our family’s going to be?”

Since making the move in 1995, they have enjoyed an active lifestyle while watching their neighborhood continually grow. “It’s a great community that offers so much.

You now have a town center with shows in the summertime and free entertainment,” said Roy. “We do

yoga, and I’ve been playing county softball for 18 years.”

For the couple, becoming involved in local philanthropic efforts was especially important after their move. Jean started working with a nonprofit organization called Vecinos Unidos in Herndon, which helps children with reading and homework, while Roy reached out to Reston Hospital Center.

After handling emergency room registration at the front desk for three years, he learned the surgical fourth floor needed volunteers. He saw his calling to have more con-

tact with patients and has fulfilled this role for 15 years.

“Reston Hospital is run very well. I know a lot of the doctors and nurses; they call me ‘Mr. Roy,’” he said. “I have a great respect for the way they handle their procedures.”

And that feeling of respect has been mutual as the hospital has recently awarded Roy with its Annual Frist Humanitarian Award. Established in 1971 by HCA Inc. in Nashville, Tenn., the national awards honor outstanding individuals for their humanitarian and volunteer activities. Roy’s compassionate nature with surgical patients made him a prime candidate.

“I feel by actually speaking to patients and making them feel better, that’s the key to everything,” he said.

Feeling fulfilled from his work and grateful for the honor, Roy, and Jean, continue to focus on what they can accomplish in the future with the hope of inspiring others.

Continuing care communities offer support for changing health needs

C

ontinuing Care Retirement Communities (CCRCs) offer long-term contracts that guarantee lifelong shelter and access to specified health care services. A staff of medical professionals is on hand to provide different levels of care, from independent living to assisted living and nursing care. A member can move up or down this continuum of care for the rest of their life.

Most CCRCs establish requirements for incoming residents based on age, financial assets, income level and physical health. Residents are typically expected to move in while they are still independent to get to know the community. In return for guaranteed lifelong shelter, amenities and health care, residents usually pay a lump-sum entrance

fee and regular monthly payments.

There are three basic types of CCRC contracts.

An extensive contract covers shelter, residential services, and amenities, plus unlimited long-term nursing care without an increase in monthly payments (except for increases related to operating costs and inflation adjustments). An extensive contract spreads the risk of catastrophic health care costs among all residents, so that no single person faces financial ruin. Entrance fees and monthly costs under this contract are typically higher than those under modified or fee-forservice contracts.

A modified continuing care contract covers shelter, residential services and amenities, plus a specified amount of nursing care. After the specified amount has been received, the resident can re-

ceive care at daily or monthly nursing care rates.

A fee-for-service continuing care contract covers shelter, residential services and amenities. While emergency and short-term nursing care is usually included in the contract, access to longterm nursing care is guaranteed only at daily rates. Entrance and monthly fees are lower because residents are responsible for all long-term nursing and health care costs. Most CCRCs provide a variety of residential services that may be covered by monthly fees or be available at extra cost, depending on the contract.

Visit the CCRCs you are considering with a trusted family member. Immerse yourself as much as possible in the daily life, and assess the management’s philosophy and its relationship with residents.

Understand Entrance Fees

M

ost CCRCs require payment of a onetime fee, usually called the entrance fee, before moving to the community.

The entrance fee amount often depends on the level of care at the time of entry, type of housing chosen (with larger units requiring a larger fee) and agreement type. The entrance fee allows the CCRC to offer lower monthly fees and provide for the future needs of residents. State regulations can impact the terms of the entrance fee and in some states, refunds are called rebates. Entrance fees are typically structured in one of three ways: Declining scale refunds, also known as amortizing entrance fees, specify a period of time in which the entrance fee will be refundable to the resident on a declining basis. For example, if an

entrance fee under this arrangement declines at the rate of 1 percent each month, after 6 months 94 percent of the entrance fee is refundable.

Partially refundable entrance fees guarantee a specific percentage of the refund that will be returned within a certain period of time regardless of the term of residency. For example, 50 percent of the entrance fee may be refundable upon termination of the contract or to the estate upon the resident’s death.

he more years you put behind you, the more worries you may have about continuing to maintain your home.

When you do not need medical supervision, but feel like washing your hands of endless homerelated tasks, independent living is an option that can help.

What is Independent Living?

Independent living communities are residential communities for active older adults who want an enjoyable lifestyle free of the worries and trouble of home maintenance. They are similar to any apartment, condominium or single-family development, except that they provide special services,

including security—an important consideration for many residents.

Most independent living communities are fully secured, with staff members greeting and screening all visitors around the clock. Residents can leave home for extended periods of time knowing that their living quarters will be safe.

As many individuals at this stage in their lives are looking to pursue new hobbies and interests with their free time, the activities and amenities available at a community are a major factor for potential residents. Several independent living communities provide a full range of activities such as shopping trips, outings to cultural events and organized gatherings that promote socialization. Some commu-

nities also have tennis courts, swimming pools, activity rooms and other amenities that keep residents active and engaged. Most offer a variety of meal plans, including the option to prepare meals in your own apartment.

Most independent living communities do not offer health care services (such as those provided, for example, by assisted living communities and skilled nursing/rehabilitation facilities), but some independent living communities are affiliated with health care facilities that can provide such care when it is needed. Residents also have the option of purchasing home health care services from outside providers to whom they may have grown accustomed.

Assisted Living Communities Providing supportive services

ssisted Living communities offer housing and a variety of supportive services delivered by qualified staff members. As opposed to independent living communities, health care services are available at assisted living communities. The Assisted Living Federation of America (ALFA) notes that assisted living is “a unique transition from independent living without care” that “helps seniors enjoy an improved quality of life with independence and care combined.”

Community amenities vary widely but can include private units, state-ofthe-art facilities, beautifully decorated common areas, barber shops and beauty parlors, pharmacies, physical therapy services, recreation rooms, libraries, gardening areas, and fitness centers.

Consumers will quickly find that as-

sisted living communities, also commonly called residential care facilities or personal care homes, come in a variety of shapes and sizes.

Who Needs It? Residents of assisted living usually include anyone who needs some help with the activities of daily life. These include eating, bathing, dressing, transferring, toileting, medication management and walking.

Most communities offer three meals a day served in a common dining area.

Many also offer some type of assistance with transportation. Housekeeping and laundry services are standard offerings.

Staff members are available to meet resident needs 24 hours a day. Most communities are equipped with security systems, and emergency call systems are often available in all residences. For residents with more acute health

care needs, such as Alzheimer’s or other forms of dementia, special units and/or specially trained staff members are often available. In addition, communities offer a wide range of social activities to stimulate resident contact.

Choosing a Community First and foremost, consider the needs and desires of the potential resident. Narrow the search by reviewing a list of all the local communities.

hile some individuals are able to continue running their households with little assistance, there are those who may have debilitating physical or emotional conditions that require care beyond what family or professional home care aides can provide.

What is Nursing Care?

At a nursing home, care is administered by professionals under the direction of a physician. Many facilities also offer sub-acute, respite, rehabilitation and other short-term care. Rehabilitation services can be especially helpful for individuals who are recovering from surgery, an illness or any other life-changing event.

These homes serve as permanent residences for individuals who are too sick or

frail to live at home, or as temporary facilities during a recovery period after a surgery or illness.

Who Pays for Nursing Care?

Medicare, Medicaid, private insurance and personal assets are all used under various circumstances to pay for services in a nursing care facility. If a facility is not certified by Medicare and Medicaid, the care will have to be paid for entirely with personal funds.

In general, Medicare pays for skilled nursing care following hospitalization for the same illness or condition at 100 percent of the cost for days 1 – 20. Some long-term care insurance pays for nursing home care; insurance payments are usually a fixed amount for a specified number of qualified days.

The basic daily rate is the standard charge the nursing home bills to all resi-

dents, which covers the fundamental services every resident receives, including rent for the room, housekeeping, meals and general nursing care. It is important to understand all the services and amenities that are not included in the basic daily rate.

What to Look for in Nursing Care Before choosing a home, contact the Long-Term Care Ombudsman Program of your Area Agency on Aging. Ombudsman programs promote the highest quality of life and care for residents of nursing facilities and can help families and staff with inquiries and complaints. Tell the ombudsman which options you are considering and request any information they may have.

A Special Care Residence May Be the Right Option Certification standards for nursing homes and adult care residences do not specifically address dementia special care units. You will need to rely on your own judgment when considering a special care unit.

Visit more than one special care unit and compare them. Use all your senses when you visit — sight, smell and hearing are all important.

Be sensitive to the overall atmosphere and how the staff interacts with the residents.

Ask about the number of staff on duty at all times and find out about any special training they have had.

Generally, the more staff available, the higher the quality of care. Some reports find a daytime ratio of six residents to one staff person to be adequate in a special care unit; oth-

ers feel that a ratio of eight to one can also result in quality care. At night, when residents are in bed, a higher patient to staff ratio may be fine.

Get a written description of the services and programs offered by any special care unit along with the charges or fees associated. This description should clearly explain to you how the special care unit differs from the rest of the nursing home or assisted living facility. It should emphasize special training the staff receives and describe the environment.

Before choosing a special care unit, you should be confident that the higher price charged for care in the unit will actually result in better care for your relative. You may find that your relative can be adequately

cared for in a regular unit.

Finally, talk with someone about special care units. Your state ombudsman, your local chapter of the Alzheimer’s Association or your local Area Agency on Aging are all good resources. They may be able to provide you with a more detailed consumer checklist to help you compare special care units.

All dementia special care units should focus on the individual needs of the resident. Care plans should be developed, implemented and reviewed often. All patients should be treated with respect and dignity in a physical environment that encourages independence while promoting safety. Researching special care residences can ensure your loved one will receive the proper support he/she needs.

Know Symptoms of Alzheimer’s The Alzheimer’s Association Provides the Common Signs Memory loss that disrupts daily life is not a typical part of aging. It may be a symptom of Alzheimer’s, a fatal brain disease that causes a slow decline in memory, thinking and reasoning skills. Every individual may experience one or more of these signs in different degrees. If you notice any of them, it is recommended you see a doctor.

1. Memory loss that disrupts daily life One of the most common signs of Alzheimer’s is memory loss, especially forgetting recently learned information. Others include forgetting important dates or events; asking for the same information over and over; and relying on memory aides (e.g., reminder notes or electronic devices) or family members for things they used to handle on their own.

2. Challenges in planning or solving problems Some people may experience changes in their ability to develop and follow a plan or work with numbers. They may have trouble following a familiar recipe or keeping track of monthly bills. They may have difficulty concentrating and take much longer to do things than they did before.

What’s typical? Making occasional errors when balancing a checkbook.

3. Difficulty completing familiar tasks at home, at work or at leisure People with Alzheimer’s often find it hard to complete daily tasks. Sometimes, people may

have trouble driving to a familiar location, managing a budget at work or remembering the rules of a favorite game.

What’s typical? Occasionally needing help to use the settings on a microwave or to record a television show.

4. Confusion with time or place People with Alzheimer’s can lose track of dates, seasons and the passage of time. They may have trouble understanding something if it is not happening immediately.

Home health agencies are public or private agencies, organizations or parts of agencies or organizations that meet the requirements for participation in Medicare under 42 CFR 440.70 (d). This indicates these federally-certified agencies provide skilled nursing services and at least one other therapeutic service, e.g. physical, speech or occupational therapy; medical social services; or home health aide services, and they also meet the capitalization requirements under 42 CFR 489.28.

An agency may choose to be exempt from state licensure through certification in one of four ways: • The Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Certification;

• Community Health Accreditation Program (CHAP) Certification; • Medicaid Personal Care Certification; • Medicaid/Medicare Certification.

When trying to find a home health agency, ask for their license number. Be specific in asking for the license number issued by the Virginia Department of Health, and not the number for their license to operate a business.

Questions to Ask When Hiring Paid Caregivers If facing this decision, the following questions will help in making a safe and educated decision: 1. If the hired caregiver becomes ill or otherwise unavailable, what alternate arrangements can be made?

2. Are social security, federal and state taxes, and unemployment insurance paid so the family is not legally responsible?

3. Is there verification that the paid caregivers can legally work in the U.S.?

4. If the hired caregiver is injured, who is responsible?

Remember that many homeowners’ insurance policies exclude such injuries via “domestic employee

exclusions.”

5. A re criminal history background checks, state abuse registry checks, and prior work reference checks completed?

6. Is there bonding and insurance in place that would cover any injury to the client, damage, or theft? When an agency says they are “bonded,” family members should be sure that this covers injury or damages related to the caregiver(s) working in the home.

Understanding Home Health Care Benefits When it comes to understanding Medicare coverage, many individuals can find themselves confused by a complex web of information. This especially applies to those who want to know if they are eligible for Medicare coverage when entering a nursing home or receiving home health care. Below is some helpful information on whether you can use your home health care benefits if you have Medicare and what services and products it will cover.

First, you can use your home health care benefits if all of the following applies to your situation. Your doctor must prescribe that you need medical care at home and create a care plan. The care you need has to be intermittent and in the form of skilled nursing care, physical therapy, occupational therapy or speech therapy. The home health agency must be Medicare-certified and you must be homebound, or unable to leave the home unassisted.

Once you have established those requirements, it is important to understand what Medicare will actually cover, which includes the following: • Home health aide services on a part-time or intermittent basis, as long as there is a skilled need • Medical social services • Certain medical supplies • Durable medical equipment (wheelchair, walker, bedside commode, etc.)

Given the services and products that Medicare does cover, it is also valuable to take note of what is not covered. Care that is required 24 hours a day will not be covered. Other services not covered include personal care provided by home health aides such as bathing and dressing, along with homemaker services like cleaning and laundry. Also not included are home-delivered meals.

If you require care that goes beyond intermittent visits for a limited amount of time, or if your needs are not covered by Medicare, an important option to consider is private duty home care. For more information on Medicare coverage, visit medicare.gov.

patient and family may turn to hospice care when the goals of patient care have switched from curing to comforting.

Hospice involves a special system of supportive services, including pain and symptom management, social services, and emotional and spiritual support. Any individual of any age with a diagnosis of a terminal disease may turn to hospice.

How Does One Receive Hospice Care?

Medicare, Medicaid or private insurance generally require a prognosis of six months or less for services to be covered. In addition to traditional medical hospices, there is a non-medical hospice program

in Montgomery County that offers support for patients with a year’s prognosis or less who may choose to continue their curative treatments.

Hospice is unique in that care and support are provided to the entire family, not just the patient. A team of skilled professionals and volunteers work to serve the special needs of everyone involved. Choice is at the center of the hospice approach — a person faced with a life-limiting illness is encouraged to remain in control of his or her life.

As care is usually provided in one’s home, many hospice patients live out their lives in the comfort of familiar daily routines, surrounded by their treasured possessions along with family and friends. However, hospice services may also be provided in long-term care facilities

or in-patient hospice residential settings.

What Services are Provided?

Hospice team members focus primarily on managing the patient’s pain and symptoms with medication, medical supplies and equipment.

Other services, such as speech and physical therapy, are available if necessary. If managing the symptoms within the home becomes impossible, hospice providers can offer short-term inpatient care.

While family members learn how to properly care for the patient, information and support is also available to help everyone deal with the emotional, psychosocial and spiritual aspects of dying. Surviving loved ones will receive bereavement care and Continued on page 40

How Do I Pay for Hospice?

Most services are fully or partially covered by Medicare, Medicaid, private insurers or prepaid health plans. The non-medical services available in Montgomery County are free and delivered by a staff of

trained volunteers. For traditional hospice services, financial assistance is available.

The National Hospice and Palliative Care Organization notes that, “Eighty percent of people who use hospice care are over the age of 65, and are thus entitled to the services offered by the Medicare Hospice Benefit. This benefit covers virtually all aspects of hospice care with little out-of-pocket expense.”

To receive the Medicare Hospice

Benefit, patients must be eligible for Medicare Part A, have a terminal disease or condition with a less than six months prognosis, and sign a document certifying that they are choosing hospice care instead of routine Medicare benefits.

Patients may only receive covered benefits from a Medicare-approved hospice program. In addition to Medicare’s coverage, hospice services are also generally covered by Medicaid or private insurers.

understanding palliative care Palliative care (pronounced PAHLEE-UH-TIVE) helps a person remain comfortable by addressing issues causing physical pain, emotional pain or suffering.

To palliate means to make comfortable by treating a person’s symptoms from an illness. The

goals of palliative care are to improve the quality of a seriously ill person’s life, and to support that person and their family during and after treatment.

Hospice focuses on relieving symptoms and supporting patients with a life expectancy of months,

not years. However, palliative care may be given at any time during a person’s illness, from diagnosis on.

Most hospices have a set of defined services, team members, and rules and regulations. Some hospices provide palliative care as a separate program or service.
