Community Health and Health Equity
Strong and resilient community care begins with a thorough understanding of the population it serves. The breadth and depth of Inova’s community programs is evidence of the thought and care that goes into each program, offering excellence in quality, accessibility and care.
Inova supports the health of the community through a wide range of safety net services including Primary Care.
Inova Cares Clinic for Families (ICCF)
ICCF provides comprehensive primary care services to patients who reflect its culturally diverse community. Inova Cares Clinic for Women and Children (ICCW/ICCC)
ICCW and ICCC provide comprehensive healthcare for families with limited resources. ICCC provides comprehensive pediatric services for newborns up to 21 years of age. ICCW provides

Inova Juniper Program (IJP)
medical care and mental health, substance use and case management services to individuals living with HIV. As a comprehensive resource, IJP also offers prevention and education resources to clients, community members and healthcare teams.

Inova Cares Medical House Calls (IMHC)
The Inova Medical House Calls Program brings primary care to homebound patients who are 65 years of age or greater. The program provides quality care while supporting aging in place and optimizing resource utilization. Inova Pride Clinic (IPC)
Inova Pride Clinic offers a safe and comfortable space committed to providing medical expertise with cultural humility, tailored to the unique needs of LGBTQ+ people ages 12 years or older. In addition to the full range of preventive care, health screenings and chronic disease management, the program addresses LGBTQ+ specific healthcare needs such as pre-exposure prophylaxis (PrEP) for HIV prevention and gender-affirming care, including counseling for youth and adults about health, gender identity and sexuality. inova.org/PrideClinic | Referrals: Inova Pride Clinic FC #REF307

Inova Cares Clinic for Community Bridging (ICCCB)
Inova Cares Clinic for Behavioral Health (ICCBH)
ICCBH provides therapeutic support to those experiencing mild to moderate mental health conditions in an effort to reduce unwanted symptoms and improve wellbeing. As part of Inova’s not-for-profit mission ICCBH offers adult mental health counseling services for individuals approved through Inova Financial Aid or who have Medicaid.

ICCCB is a comprehensive clinic designed to support patients being discharged from an Inova hospital or Inova emergency room who either do not have a medical home or cannot get an appointment with their primary care provider within 72 hours following a hospital admission. Patients are typically followed by a hospitalist or nurse practitioner for one to three visits, while the patient and the care team work together to establish a permanent medical home in the community or transition the patient back to their primary care provider.

Community
Inova Ewing Forensic Assessment and Consultation Teams (FACT)
The Inova Ewing FACT Department provides acute and non-acute forensic exams for victims of sexual assault, intimate partner violence, strangulation, human trafficking and child abuse. This includes evidence collection, assessment, photo documentation of injuries, medication administration for the prevention of sexually transmitted infections and linking patients to resources in the community. FACT examiners also serve as expert witnesses in court proceedings, interpreting injuries and forensic findings.

Inova Lions Eye Clinic
The Inova Lions Eye Clinic, with support from the Virginia Lions Eye Institute Foundation, provides free eye care to uninsured adults at or below 400% of the federal poverty level referred by a community safety net partner. The Inova Lions Eye Clinic provides free comprehensive ophthalmic care, including laser, medical and surgical care for all types of conditions of the eye.

Care Connection for Children (CCC)
Inova administers Northern Virginia Care Connection for Children serving children and youth with disabilities. CCC works closely with families to coordinate care and find the best healthcare and community resources in Northern Virginia. The program offers free care coordination services, information and referrals, education and support for families of eligible children from birth through age 21.

ElderLink
ElderLink is a non-profit partnership between Inova and the Fairfax Area Agency on Aging that promotes the well-being, safety and independence of older adults and caregivers. ElderLink provides case management, care coordination, health and wellness programming and support to caregivers and patients returning from the hospital to their home in the community.

Partnership for Healthier Communities (PHC)
PHC increases access to healthcare for uninsured children and adults in Virginia. A bilingual team provides individuals and families with comprehensive application and enrollment assistance for programs like Medicaid, FAMIS, local safety net providers and Inova Financial Assistance. PHC partners with school districts, community partners and safety net clinics to provide education and targeted outreach. inova.org/PHC | Referrals: Unite Us or Inova PHC webpage
Inova Healthy Plate Club (IHPC)
IHPC works to increase vegetable consumption among children in Title 1 schools as well as those who live in under-resourced areas. Through programs based in school, community and virtual settings, IHPC aims to improve students’ understanding of nutrition and the importance of healthy behaviors. Additionally, the department runs an annual Inova Team Member food drive, food pantries in clinics, and a number of food access initiatives for patients and the community. inova.org/healthyplateclub
With support from the Fairfax County Health Department, Inova Specialty Access Program connects individuals in need of specialty services with providers at Inova and in the community. Safety net partners work with Inova teams to enroll patients in Inova Financial Assistance while Specialty Access team members facilitate the scheduling of appointments with patients and practices.
