It’s What We Do
N
orth Simcoe Muskoka (NSM) is one of 14 Community Care Access Centres (CCACs) in Ontario and is your local provider. When you connect to us, a qualified member of our care team works with you, your caregiver and family to identify care goals and coordinate a care plan that is unique to you. We work closely with hospital partners to support people returning home, and are connected to family doctors, nurse-practitioners, community support service agencies, long-term care homes and other partners to ensure care is provided at the right time, and in the right place to maximize the value of every healthcare dollar. Since CCACs opened in 1997 as notfor-profit, government-funded agencies to provide home care, they have been helping people access the care they need – in their homes, communities and schools. CCACs help people understand their care options and help them connect to programs and services for their needs and the needs of loved ones. This annual report shows how care is being provided in homes, nursing clinics, communities and schools across the region for people of all ages. We will continue to collaborate on improving timely access to care for you and your family members. You can trust, even with health system changes ahead, our focus is on you.
The Health System is Changing There Are No Immediate Changes Taking Place
T
he Minster of Health and Long-Term Care, Dr. Eric Hoskins, released a proposal in December 2015 outlining changes to Ontario’s healthcare system. On June 2nd, Bill 210, Patients First Act, 2016 was introduced which, if passed, will give Ontario’s Local Health Integration Networks (LHINs) a broader role in primary care and responsibility for home and community care services currently provided by Community Care Access Centres (CCACs). As a result,
CCACs will not exist in the future. However, services and care that we provide will continue. We know that the Ministry of Health and Long-Term Care (Ministry) is committed to continued investment in building capacity in the home and community care sector. CCACs have always advocated for improvements in care for all Ontarians. We stay committed to working closely with partners to ensure patients get the best care possible through the future change and beyond. We
are optimistic the proposed changes will support continued improvements in care. Our focus remains on providing safe, quality care to patients we serve. There are no immediate changes taking place. NSM CCAC continues to provide service and receive referrals for care.
