BDA TO DDRB: PAY CUTS HAVE CONSEQUENCES T

he British Dental Association has said that real terms cuts to dentists’ pay will undermine patient care, as part of its oral evidence to the Review Body on Doctors’ and Dentists’ Remuneration (DDRB) for 2016/17. Eddie Crouch, the BDA’s Vice Chair and Chair of its Review Body Evidence Committee, said: “The rhetoric and the reality on dentists’ pay couldn’t be any further apart. As a profession we are not looking at ‘minimal’ rises or even pay ‘freezes’. Any talk of ‘pay restraint’ for dentists is simply a euphemism for deep and sustained pay cuts. All the official evidence tells precisely the same story. We are not ‘all in this together’, and our profession should not be expected to take a disproportionate hit to bail out a cash strapped NHS. “Let’s be clear these cuts have consequences. Patients quite rightly

demand ever higher standards of care, and that requires a well-trained and highly-motivated team, armed with the best in equipment and materials. We are reaching a point where delivering this on ever-diminishing funds will no longer be possible. “Today (16 November) colleagues from all the home nations and crafts spoke with one voice. Their message was simple: we can’t go on like this.”

Public confidence in NHS dentistry is high and quite rightly needs to remain so; Practices are focusing their finances on retaining highly prized practice staff to underpin the dentist workforce, however, some practitioners are not receiving pay increases as a

result; Taxable incomes continue to remain at significantly lower levels than 2008/9; Time spent on non-clinical activity continues to increase putting at risk the time available to see patients and access to NHS services; Morale and motivation continues to be very low across the UK, particularly in Northern Ireland; Dentist retention rates are not as good as they could be and 30 per cent of associates intend to increase the amount of private care they provide; There are problems in recruiting associates in some areas, particularly Wales; A third of practice owners plan to retire in the next five years and six per cent of dentists want to leave dentistry and work in another sector within the next

For an £8.60 fee, Scottish dentists must examine their patient’s mouths for dental decay, missing teeth, existing restorations, tooth wear, evaluate and record their gum condition, perform a cancer screening and disease check of their cheeks, tongue, throat, lips, face, neck, and jaw joints, perhaps take an x-ray, come to a diagnosis, provide a treatment plan, discuss it with the patient and then write up the notes to GDC standards.

KEVIN LEWIS IS TO STEP DOWN AS DENTAL PROTECTION DIRECTOR

K

evin Lewis is to step down as Dental Protection’s Dental Director by summer 2016, having served in the role since 1998. Kevin explained, “18 years is a long time for anyone to lead an organisation such as this. In fact my connection goes back even further than that, as I was a member of the original Board of Directors when Dental Protection Limited was created as a separate company within the Medical Protection Society (MPS) group back in 1989. I then moved across to become a dentolegal adviser in 1992 so it is actually 27 years of continuous involvement – that’s quite scary to think about.

“It has been a huge privilege not only to work in this fascinating field over so many years, but also to work with such phenomenally good people. There is something very special about being in a position to help your professional colleagues through times of difficulty, and one of the pleasures of the job is seeing that commitment and passion running right through the Dental Protection team. Our members can be assured that this commitment will remain undiminished.” Asked about his future plans, Kevin explained that he will still be involved in and around dentistry both in the UK and internationally, commenting:

“In addition to my role as Dental Director I have been part of the MPS Executive Committee for almost 18 years, and for most of those years serving on its elected Council as one of the three executive directors of MPS. There has also been a lot of travel involved so it has been quite demanding and it will be nice to have a bit more time to call my own after next summer. I will certainly be looking forward to that, and also to continuing my writing and lecturing for some time yet.” Commenting on Kevin’s decision to step down, Simon Kayll, Chief

Executive at MPS said: “Kevin has made an outstanding contribution to dentistry and Dental Protection over many years. He is highly respected and will be deeply missed by his colleagues for whom he has been an insightful leader, mentor and friend.” Arrangements for the appointment of Kevin Lewis’s successor will be announced in due course.

DENTAID LAUNCHES FIRST PAY WHAT YOU CAN DENTISTRY SCHEME

I

nternational dentistr y charity Dentaid has teamed up with The Real Junk Food Project Dewsbury to launch a new scheme that will help vulnerable people who find it difficult to access NHS dentistry. The Real Junk Tooth Project will provide emergency, pain relieving dentistry. A pilot project is being launched in Dewsbury on Thursday, December 3 and, if it is successful, could be introduced in other towns and cities in 2016.

Dentists and support staff are volunteering their time to provide emergency treatment on a ‘pay what you can’ basis. Between 6pm and 8pm people suffering dental pain can turn up and wait to be seen at Dewsbury Dental Centre on Halifax Road in the West Yorkshire town. They will only pay what they can afford. The idea came about after The Real Junk Food Project Dewsbury realised that many of the homeless people, low wage and migrant

workers, and vulnerable people it was feeding at its cafes and drop-in sessions couldn’t enjoy their meals of donated food because they were suffering from toothache. Many had been living in pain for several months. Dentaid is committed to eradicating dental pain around the world whatever people’s circumstances might be. We are working with The Real Junk Food Project Dewsbury to launch the project which is our first in the UK. Dentaid is also looking for

other dentists who would be interested in volunteering their time for future Real Junk Tooth Projects and would love to hear from anyone who would be willing to donate consumables. s soon as you hear people jesting “it’s close to next Christmas than last”, it’s a sure sign that someone in the practice is about to be tasked with organising the annual party. As that time is well and truly upon us now, it’s worth a reminder about the potential tax consequences of seasonal generosity. First off, the Christmas party - or in fact any annual party. Provided you are entertaining only staff and their close family, the full cost is deductible against your trading profits. If you also invite suppliers or clients, then only the staff-related expenditure can be deducted. However, the potential

pitfall is that if the total per-head cost (including transport and accommodation, if applicable) goes over £150, the whole lot – not just the excess – is treated as a benefit in kind on the employee, and must be reported on their P11D. Alternatively, it may be more economical to agree a PAYE Settlement Agreement with HMRC. The £150 is an annual total, so if you have, say, a summer BBQ then this also counts towards the limit. However it’s only the event which pushes staff over the £150 which is taxable. Seasonal gifts to employees should also be given some thought. Cash

gifts or store vouchers are treated as salary and taxable as such. However, HMRC will not seek to tax trivial non-cash gifts such as a bottle of wine or box of chocolates. Their limit of “trivial” remains under wraps, but a common-sense approach should avoid any challenges. Larger gifts (perhaps a whole case of wine?) will be taxable through the P11D or PSA channels as outlined above. Happy Christmas! I

n July, children’s dental health once again grabbed the headlines. The Dean of the Royal College of Surgeons, Professor Nigel Hunt, talked of a “crisis point” and that “it is absolutely intolerable that in this day and age, in a civilised country, children are having so many teeth out due to decay, which is over 90% preventable”. With nearly 26,000 five-to-nine year olds admitted to hospital for tooth decay in England during 2013-14 the figures are indeed grim. He went on to recommend that sugary foods and drinks’ packaging should carry cigarette-style warnings, plus better education for parents and more research into why certain groups are less likely to visit the dentist. We all know that prevention is crucial and the overall message was that, by working together, we can continue to make the necessary improvements in order to head off a crisis and bring these damning numbers down. It is all systems go for children’s dental health, but what happens when they grow up? From early to late teens, everything changes. No more supervised brushing for a start! How can we encourage adolescents to maintain the good oral care habits they learned as children, despite huge pressures – social, financial and psychological – that will

start to act upon them? With a total increase in permanent (yet immature) tooth enamel, oral hygiene is more important than ever for teens. The same can be said for other life stages, such as pregnancy and menopause, research has suggested that rush of ‘sex hormones’ during puberty affects the periodontium, making this an important time for an individual’s periodontal status. As for avoidable factors, a smoking habit usually starts, and gets established, during adolescence. According to Ash, around two-thirds of adult smokers reported that they took up smoking before the age of 18, and over 80% before the age of 20. Smoking, of course, has a plethora of serious dental and systemic health consequences. With more independence, there will be inevitable changes to diet. Faddy eating plans can also be an issue at this time and the impact of eating disorders, which are often initiated during the teenage years, should be noted. Individuals who develop bulimia nervosa may experience acid erosion to the surface of the teeth as a result of frequent vomiting. Anorexia nervosa, can lead to increased caries, xerostomia and osteoporosis due to malnourishment and a lack of essential nutrients.

More freedom to snack and, if they have left home to go to university or negotiate the world of work for the first time, less money to spend on food can lead to unhealthy choices. Energy drinks are popular during examination time as they are marketed as being able to boost energy, decrease fatigue and enhance concentration. However, they are often full of sugar, too. Skipping breakfast – or grabbing something unhealthy on-the-go – is common amongst adolescents. New research has shown that teens are twice as likely to suffer from halitosis if they miss the most important meal of the day. Oral health education directed at teens, which incorporates bad breath as a motivational tool may trigger them into better oral health habits. This is a defining time and no one wants to be known as the girl or guy with smelly breath! With finances, time and changing priorities all likely to be an issue we need to look at simple, costeffective ways to boost our teens’ dental health between appointments. Education about the causes of halitosis, proper brushing techniques and the dangers of smoking, of course are important, but they could add some easy-to-use adjuncts to their routine, too. HOLISTIC LEARNING FOR DENTAL PRACTICE MANAGERS T he Trailblazer Management Apprenticeship in Dentistry provides an extensive workbased programme with a national recognised level 4 qualification – ILM Certificate in Leadership & Management. Due to the nature of apprenticeships, the two-year course keeps students in the working environment with only three or four days taught in college per year. A portfolio of work showing evidence of scenario-based projects, learning activities and assessments is developed in practice and a student mentor supports individuals as they learn. The programme covers Human Resources, Patient Care, Clinical, Marketing, Risk Management, Quality Assurance, Finance and the Dental Industry and other areas, enabling practice managers to marry their existing abilities with new skills. Employers can obtain fully qualified people to run their businesses without huge expense. They are not required to fund the entire apprenticeship, just a third; the other two-thirds is funded by the Skills Funding Agency (SFA). Students must of course provide drive and commitment to the training but they do not pay anything, so

t the BDIA Showcase 22nd – 24th October, B.A. International gave attendees the opportunity to demo their new BA Ultimate Power + and Ultimate Turbines. A completion of a demo using either of the new handpieces qualified the user for a chance to win one of the two turbines in a prize draw. The demo stands proved very popular with attendees and there was a prize awarded at the end of each day of the show. T h ey we re a l s o r u n n i n g a ‘Superhero Selfie’ prize draw, where attendees had to take a ‘selfie’ with B.A. International’s mascot on their stand, then upload this image to the B.A. International Facebook page. The individual was then entered into a prize draw and had the opportunity to win a gift ranging from 1st Prize of an Apple Watch, 2nd Prize a Selfie stick, and 3rd Prize a bottle of Champagne. There was a prize draw at the end of each day and the competition proved to be a great attraction for visitors, with many positive comments referring to the competition. The initiative also gave visitors the opportunity to interact with the company’s stand, which meant a busy day for the B.A. International staff as they entertained questions and inquiries from visitors
