Faircape Health Somerset West provides restorative care and rehabilitation in a purpose-built healthcare facility in the Western Cape.
Sub-acute care addresses the critical phase of recovery and provides a tailored pathway to individuals requiring intensive medical attention and rehabilitation following illness, injury, or surgery.

Our rehabilitation mission is to drive goals and outcomes. We believe that setting clear goals and measuring outcomes are essential components of rehabilitation. This ensures the best possible care for our patients. Our rehabilitation programmes use the International Classification of Functioning, Disability, and Health (ICF) framework, Goal Attainment Scaling (GAS), and the Modified Barthel Index (MBI) to measure the outcomes of each patient.
The ICF Framework:
The framework was developed by the World Health Organisation and is a comprehensive model that helps us understand the impact of health conditions on an individual’s functioning and disability.
We assess the physical and mental aspects of our patients’ health.
We evaluate how these health conditions affect an individual’s daily life and participation in society.
We consider the influence of the physical, social, and attitudinal environment on an individual’s functioning.
Goal Attainment Scaling is a method used to set personalised rehabilitation goals with patients. Instead of relying on generic goals, GAS allows us to work with patients to define specific, achievable objectives that are meaningful to them. Each goal is assigned a numerical score, and we continually monitor progress. This approach ensures that our rehabilitation plans are patient-centred and outcome-driven.
MBI is a widely recognised tool for measuring an individual’s level of functional independence in performing activities of daily living, such as bathing, dressing, and mobility. This index assists in assessing and tracking patients’ progress. Additionally, it can be used to quantify changes in a patient’s functional abilities over time.

Tokai Physiotherapy Inc. is our dedicated physiotherapy practice. The team of physiotherapists approach rehabilitation with the same commitment to excellence intrinsic to Faircape Health. Boasting a team of dynamic physiotherapists with special interests in different areas, they make a significant and positive impact on patient’s lives through care, healing, and empowerment.
Our physiotherapy special interest areas:
• Orthopaedic rehabilitation - Our orthopaedic physiotherapists have undergone rigorous training in orthopaedic-related surgeries and injuries. By working in close collaboration with orthopaedic surgeons, our patients receive a seamless continuum of care, resulting in a faster and more effective recovery.
• Neurological rehabilitation - Our neurology physio’s specialised knowledge allows them to provide cutting-edge care to individuals. By participating in prestigious training, including the Bobath course, they ensure that patients receive effective and personalised treatment.
As dedicated General Practitioners at Somerset West and Bridgewater Manor healthcare facilities, both Dr. Erin Warren-Hansen and Dr. Michelle Bowers bring extensive experience and a shared commitment to compassionate, patient-centered care. Dr. Warren-Hansen focuses on preventive care, chronic disease management, and acute treatment, striving to improve health outcomes through personalized, evidence-based practices. Meanwhile, Dr. Bowers, with a strong background in family medicine, emergency medicine, and psychiatric medicine, is passionate about providing holistic care and enjoys working as part of a multidisciplinary team to support patients along their journey.
The Healthcare Manager plays a key role in ensuring efficient administration of referrals, admissions, medical-aid authorisations and coordinating healthcare services. By working closely with the multidisciplinary team and the administrative staff, the Healthcare Manager and the team are a vital link, connecting specialists, hospitals, and patients, while ensuring seamless operations and a comfortable stay and safe discharge for all patients.
The first point of contact when a patient is scheduled to be transferred to our facility. They ensure that each patient’s journey is seamlessly guided to the next step of their medical care.
Providing care and assistance to patients, registered nurses ensure that needs are met with dignity and respect and that prescribed medications are managed appropriately.
Assisting patients with various activities of daily living, including hygiene and mobilisation, Carers promote and extend a patient’s independence.
A bridge between the patient, family, and the healthcare team, our discharge planner who is a qualified social worker, promotes patient well-being and recovery while minimising the risk of readmission.
Contributing to achieving sustainable nutritional plans for rehabilitation patients, our dietitian implements healthy eating, weight management, nutrition, food service and menu planning.
Communication and swallowing techniques are attended to during rehabilitation with our speech therapists. They assist patients in improving their communication skills, articulation, and voice disorders.
Our registered wound care practitioner provides specialised care to in-patients and those whose wounds need ongoing treatment.

We ensure patient safety and security while monitoring carers and nursing staff with CCTV cameras in all rooms and common areas. Please note that the footage is strictly only accessed by our Medical Personnel.

On admission, the patient and next of kin/contact person will be introduced to the administrative team. The team will relay messages to the rehabilitation team, which includes the doctor, physiotherapists, occupational therapist, speech therapist and discharge planner.
Patients capable of providing feedback to their family on their rehabilitation are requested to do so. The family will be contacted upon admission and thereafter only if the patient becomes medically unstable or in the case of an emergency.
A discharge summary of progress made will be sent to the referring doctor and patient where indicated.
If a patient is unable to provide feedback to their family, the medical team involved will communicate with the family via email. Updates on the patient will be given to the next of kin/contact person as needed.
Please note, discharge times are between 10:00am and 12:00pm.

The visiting hours are from 11:00-12:00 and 15:00-19:00. We understand that family and friends play a vital role in the recovery process of patients, and we encourage support during a patient’s stay.
We allow two visitors per patient. We kindly ask that visitors adhere to the designated visiting hours to ensure a smooth and efficient rehabilitation process for our patients.
Furthermore, we request that visitors wait in the coffee shop should the rehabilitation team need to work with the patient. We understand that this can be inconvenient, but it is crucial to ensure the best possible care and rehabilitation outcomes for our patients.
Please note that the visiting hours are subject to change based on the patient’s condition, treatment plan, or any other unforeseen circumstances. We will notify visitors of any changes in visiting hours as soon as possible.
Should visitors wish to bring alcohol and food items, this needs to be authorised by the patient’s doctor in the facility.

Sub-Acute admissions take place on a referral basis from the hospital.
1. At least 72 hours before discharge from Hospital: Referring Doctor completes

2. Patient Assessment Sister from Faircape Health visits the patient in the hospital to assess the patient. Healthcare manager will confirm bed availability. *Admission is subject to bed availability
3. The administrative team submits to medical aid for approval.
4. Once authorisation has been received, Faircape Health Somerset West staff liaises with the treating doctor to confirm when the patient will be ready for transfer.
5. Once discharge date is confirmed, the patient is seen for assessment within 48 hours before transfer is accepted.
6. The hospital will arrange an ambulance to transfer the patient to Faircape Health Somerset West.

5. Once quotation is accepted, referring Doctor discharges from hospital and the hospital can arrange ambulance transfer to Faircape Health. Ambulance transfer is for own account.

Some medical aids do not cover consumables. If consumables are used there will be an additional charge to your account, payable prior to discharge.

Our professional housekeepers ensure a sanitary environment. They carry out their duties with the highest levels of courtesy and discretion. Housekeeping services include: overall freshening up of rooms and daily linen changes.
Optimum nutrition and deliciously prepared meals are vital for health. Our in-house catering service works in partnership with our group’s dietitian to develop nutritionally balanced meals. Patients and residents are guaranteed inclusive, wholesome and healthy meal offerings.
Special requests to bring in food can be submitted to the Healthcare manager for approval prior to admission. Alcohol consumption is limited & monitored by a registered nurse and subject to MDT treatment plan.
The cost of your stay at the sub-acute facility includes meals prepared by our 5-star chefs and includes:

I was admitted for 21 days and I did not think I would ever be able to get up from my bed again. The staff’s patience and compassion during my recovery was a blessing. It was a glorious day when I was able to sit out in the sunroom after my second week.
