Care System: Pre Hospital Services in our care system are activated to complement partner and patient needs
On-Demand Urgent Medical Care:
DispatchHealth provides urgent and high acuity care in the comfort of home, reducing unnecessary ambulance trips and costly visits to the emergency room. DispatchHealth calls this on-demand, same day or next day service Acute Care. Acute Care produces medical cost savings and improved patient experience.
Care Team: A DispatchHealth physician assistant or nurse practitioner and a medical technician, virtually supported by by an emergency medicine or internal medicine physician.
Care System: Hospital
Services in our care system are activated to complement partner and patient needs
In-Home Hospital Alternative:
DispatchHealth provides hospital-level care in the comfort of a patient’s home. Such patients typically present with general medical conditions that could otherwise result in a hospital admission. DispatchHealth calls this hospital alternative service Advanced Care. The Advanced Care team reduces unnecessary and costly hospital stays, which improves hospital capacity and throughput while producing superior outcomes for the patient.
Care team: Led by a board-certified hospitalisttrained internist, supported by a team of Advanced Practice Providers, a 24/7 nurse command center and other care givers as needed.

Care System: Post Hospital

Transitional Care/SNF at Home:
DispatchHealth provides a variety of transitional care services aimed at readmission reduction to support the management of patients following a visit to the emergency room, hospital, or skilled nursing facility. DispatchHealth also partners with hospitals, payers, or skilled nursing facilities to support discharges and the transition home, or potentially provide more ongoing care in the home as an alternative to a skilled nursing facility and/ or other high-cost post-hospital management. At DispatchHealth, these services are called Bridge Care, ED to Home and Extended Care.
Care team: A DispatchHealth physician assistant or nurse practitioner plus a medical technician, virtually supported by a board-certified emergency medicine or internal medicine physician, or a board-certified hospitalist-trained internist, supported by team of Advanced Practice Providers, and a 24/7 nurse command center.

Care System: Outpatient Support

Last Mile Care/Population Health:
DispatchHealth is uniquely positioned to extend a partner’s reach into the home in a variety of ways. Whether supporting a partner’s Population Health initiatives or leveraging Last Mile Care to complement virtual services, DispatchHealth meets patients in the comfort of their home.
DispatchHealth identifies and addresses social determinants of health, provides portable x-ray and ultrasound services, point of care lab testing, in-person assisted virtual visits, or other health and wellness assessments.
Care team: Extending partner care into the home with medical technicians, sonographers, radiologic techs, radiologist, nurses, or other providers as appropriate based on patient need.

Our clinical capabilities are integrated to support the system of care in the home
DispatchHealth’s flexible and expansive clinical capabilities can safely support care across the healthcare continuum to keep patients healthy at home. Not only can DispatchHealth provide an array of last mile services including portable x-ray and ultrasound, point of care lab testing, procedures and medication administration if needed, but patients can also benefit from the nurse triage and remote monitoring capabilities staffed through a 24/7 command center that can intervene clinically, address social needs, and coordinate after care. These capabilities are integrated to support all services delivered by a multidisciplinary team of technicians, nurses, advanced practice providers and physicians employed by DispatchHealth with experience in emergency medicine, hospital medicine, family medicine and more.
