Who qualifies for Advanced Care? Adult individuals who are experiencing an acute medical illness—such as cellulitis, kidney and urinary tract infections, chronic obstructive pulmonary disease exacerbations, heart failure exacerbations, and many other conditions—can receive our in-home Advanced Care services as an alternative to a hospital stay.
How do patients access the Advanced Care program? There are two primary tracks for patients to access Advanced Care. The first is when a patient is receiving a DispatchHealth Acute Care visit requires escalation to a higher level of treatment. Rather than sending that patient to the hospital, our Advanced Care team will work through appropriate safety protocols, risk stratification, and clinical criteria to ensure the patient can remain and be treated in-home. The second way is for a community provider to call our intake team directly to discuss evaluation and admit into the program. We’re proud to offer our Advanced Care services as a safe and cost-effective alternative to a traditional hospital admission.
What do Advanced Care services include? We provide Advanced Care in two phases over a 30-day episode of care, all of which is built into a single payment bundle. Multi-faceted safety layers are embedded in our Advanced Care services, including front-end risk stratification and environmental safety screenings. Emergency escalation procedures are also established and reviewed to ensure the patient and their team understand the protocols.
How is an Advanced Care team staffed? Each DispatchHealth Advanced Care team includes a hospitalist, an advanced practice provider, a care coordinator, and a registered nurse with hospital experience.
How does DispatchHealth Advanced Care partner with primary care? Coordination with primary care is a cornerstone of our Advanced Care program. The DispatchHealth Advanced Care team collaborates with primary care providers through active daily huddles, tech-enabled two-way communication, and shared clinical progress notes in order to optimize a patient’s care plan. DispatchHealth also offers emergency-trained Acute Care teams, which can provide support care for patients should a concern arise.
