Obstructive Sleep Apnea

O

bstructive Sleep Apnea is a common disorder.

If you have OSA, it means that your airways collapse partly or all of the way while you are sleeping. Muscles of upper airway relax when you fall asleep. If you sleep on your back, gravity can cause tongue to fall back. This narrows the airway and can reduce the amount of air getting in the lungs. Narrowed airways causes snoring by making tissue in the back of throat vibrate. Apnea is when the airway is completely closed, so breathing temporarily stops. The airway can collapse repeatedly during the night. Air cannot get in to your lungs, resulting in lack of oxygen. You wake up so you can start breathing again. Frequent awakenings at night can cause sleepiness during the day. Lack of oxygen and frequent nighttime awakenings have negative consequences for your health.

Signs of Obstructive Sleep Apnea 1. Loud or frequent snoring.

2. Choking or gasping while you sleep.

3. Morning headaches.

4. Daytime sleepiness or tiredness.

5. Trouble concentrating.

Who is at risk for OSA?

You are at increased risk if you are: 1. Overweight.

2. Hypertension.

3. Male with a neck size of 17 inches or more.

4. Female with a neck size of 16 inches or more.

5. Male over the age of 40.

6. Female over the age of 50.

Not all people with OSA have risks factors. Children can have OSA as a result of large tonsils or narrow airways.

Veteran’s Day Ceremony

The City of Boca Raton will honor local veterans at their Veteran’s Day Ceremony. Featuring veterans groups, city officials, music by Ft. Lauderdale Highlanders and Boca Raton Community High School Band; drills by Boca Raton Community High School NJROTC; and Boca Raton Police and Fire Honor Guards.

But what happens when you want to transport the motorcycle? The scourge of the true enthusiast, there’s myriad damage that can and often --does occur when trying to get it up or down a ramp, and who needs that headache before a road trip? Longtime entrepreneur Dan Scott of Lighthouse Point, paired up with fellow motorcycle enthusiast and “steel man” Sam Graffeo and together the duo created a unique trailer to solve their problems and lo and behold --their company, Rampless Motorcycle Trailers was born.

“I saw the design and was completely captivated. I contacted Dan in 2015 about purchasing the patent from him when he received it. He wanted to do something with it other than just sell it. I asked him to come see my steel company and that was the incubator that spawned the conversations that ultimately lead to a partnership,” explained Graffeo, a Parkland resident.

We opened Rampless Motorcycle Trailers in April of 2016,” says Graffeo, who has worked in the steel industry since 1980, opening his own steel distribution company, SG Steel Services Co. in Ft Lauderdale, in 1994.

He added, “Our trailers are the safest, easiest and quickest way to transport motorcycles. There are no ramps involved in our trailers. The entire floor of the trailer lowers to the ground to load and unload. Your feet are always touching the ground or floor of the trailer for complete control, which provides a worry-less experience in the process.”

Since the beginning, the pair honed the design to make it seamless, safe and nearly effortless. Their trailers, which are patented and DOT compliant, have a six-month warranty, and are designed to ingeniously lower flat to the ground, which eliminates the need for a ramp while staying connected to the tow vehicle. All the wiring they use is internal to the frame. Front wheel chocks are made by Condor Products, which is rated No. 1 nationwide.

Their trailers come standard with black or white rims but upgrades to custom wheels and radial tires are available for the true aficionado.

“If someone has a customized bike, say a special

paint color or theme, we offer them a variety of colors they can select. If someone’s motorcycle is long or extended further than any stock bike, we can build the trailer to accommodate that bike as well,” he added. “We make trailers to accommodate one or two motorcycles, ATVs and trikes and we custom build to suit nearly every need. If a client doesn’t see it in our selection, we’ll build it for them.”

They also sell accessories, such as upgrades on their tires and wheels, spare tires and spare tire brackets, and stone guards to fit the trailers.

Besides creating a product that “ramps up” the experience (pun intended) for fellow motorcycle enthusiasts, Graffeo and Scott both believe strongly in giving back to their community. Back To School Last month’s column addressed the American Opportunity and Lifetime Learning Credits available for taxpayers who pay qualified education expenses.

While credits are generally more favorable than deductions, there are numerous tax provisions that grant tax deductions or other benefits for education related expenses. In most cases, taxpayers cannot utilize both credits and deductions in the same tax year.

In addition, the various deductions have different phase-outs based on modified adjusted gross income. The deductions themselves may apply to different types of expenditures. Below is a quick summary of some of the available education expense deductions and other benefits that may be useful. Coordinating the use of credits, deductions and other tax benefits can be a complex matter. It is important to consult your tax advisor to maximize your benefits.

1. Student Loan Interest Deduction: Interest expense paid on student loans for dependents is deductible up to $2,500 of interest per year on qualifying student loans. Married filing joint phase out begins at $130,000 of gross income, with full phase out after $160,000 of gross income.

2. Use of Retirement Funds For Qualifying Education: IRA funds can be accessed to pay qualifying education expenses on a penalty free basis for taxpayers under age 59 ½. The 10% early distribution penalty will not apply, to the extent funds are used for qualified education expenses.

3. U.S. Savings Bond Series EE and I: Interest income can be excluded to the extent funds are used for qualified tuition and fees. These funds must be used for the taxpayer, the spouse, or dependents.

Married taxpayers must file a joint return to secure the exclusion. A phase out applies to married filing joint taxpayers, with adjusted gross income starting at $116,300, with full phase out at $146,300.

4. Above the Line Deduction for Qualified Tuition: An above the line deduction for up to $4,000 of qualified tuition is available for taxpayers with modified adjusted gross

income (“MAGI”) up to $130,000. If MAGI is between $130,000 and $160,000, the maximum deduction is $2,000.

Over $160,000 of MAGI, no deduction is permitted.

5. Qualified Tuition Plans (Section 529): States sponsor Section 529 Plans. Each State has its own rules and regulations.

Generally, all income grows tax free with distributions for qualifying education expenses non-taxable as well. There are no Federal deductions upon contribution, but many states do provide meaningful deductions.

6. Education Expenses Incurred as Part of a Trade or Business: Enhancing one’s skills in a current trade or business can result in an important tax deduction. So long as the education enhances skills, rather than enables one to enter a new trade or business, the expenses may be deductible. If expenses are deductible, all related expenditures, such as travel, over-night stays, and related meals, might also be deductible. These expenses would qualify as excess itemized deductions reportable on Schedule A. Despite all this complexity in determining which education credits and deductions provide the best results, meaningful tax savings can be achieved. Because each of the above-noted provisions have their own separate rules and regulations, it is important all taxpayers incurring education expenses become familiar with the provisions, so they can maximize their benefits as they incur today’s high cost of education.

Few of us have been taught to listen because so few of us have been listened to.

First as children, then as adults… we live in a society of impatience, distraction, of multi-tasking and self-serving agendas.

We are in such need to be heard, to share what we have to say, to explain our side of the argument-- to show what we know.

We enter conversations with our point or angle in mind. Sometimes it’s as if you let the other person speak just so you can get your turn to say what you need to say. To be right or to win.

When we don’t listen, we all lose.

Did you know that most conflicts and challenges we experience are caused by ineffective listening which translates into miscommunication?

So how do you know what kind of listener you are? Let’s take a look at the three levels of listening: 1. Subjective Listening: Also known as “selfish listening” because the listening is based on the agenda and needs of the listener. Whatever is said is heard through the experience of the listener.

This type of listening rarely satisfies the person who is speaking.

Speaker: I’m so upset that I missed my son’s soccer game.

Listener: Yeah, I missed my daughter’s play once and I felt awful.

2. Objective Listening: Also known as “selfless listening” because the listening is completely focused on the speaker.

There are no thoughts about how it relates personally to the listener and can be very effective. It allows the speaker to feel heard.

Listener: Seems like you’re really disappointed.

3. Intuitive Listening: Also known as “powerful listening” because the listener is hearing all components and intuitively connecting with the speaker’s real message. The listener is not only paying attention to the words, but also tone, emotion, and energy, as well as what is not being said. This level of listening creates the deepest connection with the speaker.

Speaker: I’m so upset that I missed my son’s soccer game.

Listener: Being so upset is totally understandable. It seems like like you’re feeling really down on yourself about it and what it may mean about everything you are trying to balance right now.

In life, we all listen at all the levels. After all, you’re human. But by being more aware, you can take steps to reach for those deeper levels.

I will say this, to powerfully listen is to hear with the intention to understand. To powerfully listen we must be silent, we must be present in the moment, we must pay attention, we must be curious, we must be open-minded and vulnerable.

And most importantly, we must kick our EGO to the curb.

Pineapple Parfait Total time: 10 minutes Servings: 1 1 jar (23.5 ounces) Dole Pineapple Chunks 2 tablespoons Greek yogurt 2 vanilla wafers, crushed Measure 1/2 cup drained fruit.

Spoon pineapple chunks into dessert glass. Spoon yogurt over pineapple. Sprinkle crushed wafers on top.

Tropical Rum Raisin Dessert Total time: 30 minutes Servings: 4 1 jar (23.5 ounces) Dole Tropical Fruit, drained, juice reserved 1/2 cup Dole Seedless Raisins 1/4 cup spiced or coconut rum 2 cups vanilla ice cream or frozen yogurt, divided 4 teaspoons toasted shredded coconut, divided Place fruit in medium bowl and set aside. In small bowl, combine raisins, rum and 1/4 cup reserved juice. Allow raisins to soak at least 20 minutes until plump. Drain off liquid.

Combine fruit and rum raisins; spoon into serving bowls. Top each bowl with 1/2 cup scoop of ice cream and top each with 1 teaspoon toasted coconut.

Tip: 1/2 teaspoon rum extract and 1/2 cup reserved juice may be used instead of rum.

Asian Chicken Lettuce Wraps

Total time: 15 minutes Servings: 1 1/2 cup refrigerated cooked chicken breast strips 2 Bibb or Boston lettuce leaves 1/2 cup Dole Mandarin Oranges, drained 2 teaspoons slivered almonds 1 tablespoon light Asian salad dressing Divide chicken equally inside two lettuce leaves, making two wraps.

Add 1/4 cup of Dole Mandarin Oranges and 1 teaspoon slivered almonds into each wrap.

Serve lettuce wraps with light Asian salad dressing for dipping.

Orange Chipotle Avocado Toast

Total time: 10 minutes Servings: 1 1 slice whole wheat bread 1/2 avocado, diced or smashed salt pepper 1/3 cup Dole Mandarin Oranges, drained 1/2 teaspoon chipotle peppers in adobo sauce, finely chopped 1/4 lime Lightly toast whole-wheat bread and top with diced avocado. Season with salt and pepper, to taste.

In medium bowl, toss oranges with chipotle peppers.

Spoon oranges over avocado toast and finish with a squeeze of lime.

The Snowbirds Are Coming!!

It’s that time of year again. The traffic is more congested, lines are longer at grocery stores, restaurants are packed by 6pm… The winter visitors, aka “snowbirds”, are arriving in full force! This happens every year in Florida and who could blame them? If I lived up north, I would also make the trip down here to beat the cold.

The winter visitors bring a lot of business with them. One business in particular that seems to stay busy during the season is the emergency restoration business. Every year during this season, our mold remediation workload seems to double for the months of November and December.

This is due to a number of things. I would say the majority of our mold remediation work is related to the A/C unit.

Proper maintenance of an A/C unit is very important, especially in Florida where they run all year long.

Here are some tips to save you money and help prevent any damage to your property: - Make sure you have someone physically checking on your property at least once a month. A good property manager or a friend can do this. Some of the best property managers will go as far as to send you updated pictures of your property.

- You can prevent mold from developing on your property by using a programmable thermostat. Have it programmed to run your AC at 72° for two hours in the morning and 85° the rest of the time. This will prevent mold by removing moisture from the air during cooler hours.

- Use portable dehumidifiers. Be sure you do your research on them and buy the correct number of dehumidifiers you would need to dehumidify your entire house/condo.

- One of the best investments I made was a NEST Thermostat. It is referred to as a “learning thermostat” because it can create a schedule, its proven to save energy, and you can control it from a smart phone. I highly suggest investing in the NEST thermostat or one similar to it.

- Flush out you’re A/C drain line before you leave for an extended period of time. This will save you a lot of heart ache in the long run. Nothing worse than arriving at your home, A/C is working properly, but you have water damage due to a clogged drain line.

- I always suggest having you’re A/C coils cleaned. Your A/C has been running all year long and probably hasn’t had the filter changed. If your filter hasn’t been changed

all year, the coils start to act like a filter attracting dirt, mold, and other contaminants.

These are just suggestions, as I am not a licensed A/C technician. These are tips that I have picked up over my tenure and tips I’ve utilize. If you have any questions concerning air conditioning, I recommend you talk to a reputable A/C company.

Boca Helping Hands Enriches Lives With Resources That Offer A Hand Up

This philosophy has guided Boca Helping Hands (BHH), a HERE non-profit organization that assists BYLINE neighbors in need, for nearly 20 years.

For people facing financial crises, BHH steps in with immediate aid. Dollars devoted to this program help prevent eviction, utility shut-offs and other emergency situations.

Founded in 1998, BHH started as a soup kitchen in a local church. Today, it’s a thriving, $2 million operation that provides a wealth of food, financial, medical and job-related resources for individuals and families throughout the community.

Like food services, the financial assistance program is designed “to supplement, not sustain,” Swedenborg explains. Boca Raton residents who apply must document their needs and usually receive a onetime payment, reinforcing BHH goal to lend a hand up, rather than a hand out. “We don’t enable,” says Swedenborg. “Our mission is to instill dignity and foster independence.”

Feeding the Hungry BHH welcomes up to 200 guests a day for hot meals, which are served at the Remillard Family Resource Center, BHH spacious home at 1500 NW 1st Court. Lunch is offered Mondays through Fridays, and family dinners are scheduled Thursday evenings. Patrons include people of all ages, from all walks of life--the elderly, single moms with kids, veterans, the unemployed and the working poor.

Preparation for the Workforce Excellent job skills are key to gaining independence. To help clients build these skills, and better compete in the marketplace, BHH offers both mentoring and training.

The program is unique among community meal programs. Guests can choose from a wide selection of healthy, upscale fare, and enjoy their meals in the comfort and comradery of a pleasant, wood-trimmed dining room that resembles a small café. Menu items are lovingly prepared, and beautifully presented, by a caring team of volunteers. Even special occasions, from St. Patrick’s Day to the Kentucky Derby, are celebrated with specially themed dishes, dining room décor, and staff dress.

“Everything, from the food to the atmosphere, reflects our passion for our work and respect for those we serve,” says Development Manager Karen Swedenborg.

Through the program, hot meals are delivered to the homebound as well. Over 5,000 pantry bags, filled with a variety of groceries, are also distributed monthly to qualified, low-income residents of Palm Beach County. Additionally, the program’s backpack service, which benefits 1,400 boys and girls from ten schools, ensures that children are well fed on the weekends.

Keeping BHH warehouse and kitchen shelves fully stocked is a huge undertaking.

Canned, frozen, and fresh goods—1.8 million pounds annually--are donated by individuals and foundations, corporate and community food drives, and food recovery partners. At present, these partners include 100-plus stores and restaurants, from Publix, Winn-Dixie, Target and Walmart, to Whole Foods, Trader Joe’s and the Capital Grille.

The mentoring program develops job readiness by coaching clients, one-on-one, on writing resumes, submitting online applications, conducting job searches, and practicing proper workplace etiquette. Classes to improve English and computer literacy are available as well.

Job training centers on jump starting clients’ careers, through fast-track courses for lucrative positions as hospitality workers, home-health aides, commercial drivers, call center representatives and help-desk technicians. The program is highly successful, both in completion and job placement rates, says Swedenborg. “Students enrolled to become home health aides, for example, rarely drop out, and 85 to 90 percent find work after graduation.”

Health Care Services Added This year, BHH expanded its spectrum of services with medical and dental care.

Joining forces with another non-profit, Genesis Community Health, BHH established a clinic at 30 SE 6th Street in downtown Boca, where people from across the county receive physical exams, lab tests, and other primary care procedures. The ultimate goal is to treat 3,000 patients a year.

To complement the health clinic, a dental clinic, housed on Crawford Boulevard, opened in September.

Sound Fiscal Footing Funding for BHH many operations spans individual donations (40 percent), to dollars garnered through foundations and fundraising. The organization hosts two signature fundraising events a year, Bowling for Bread, part of the Boca Chamber Festival Days calendar, and Monopoly & Casino Night, highlighted by a Monopoly competition.

Grant monies, which totaled $665,000 in 2015, also contribute a fair amount to BHH revenues.

Texting: A Real “Pain in the Neck” Ever suffer from neck pain after sitting in front of the air conditioner? Well...that is just a myth, but neck pain from texting has become so common, it has officially earned its’ own medical diagnosis: Text Neck.

Neck and upper back pain have become increasingly more prevalent since cellphones and texting have become such large part of the American lifestyle.

Studies have shown that the average American spends an average of 4.7 hours per day on the cell phone, with the greatest percentage coming from teens and young adults. This has resulted in a staggering increase in the number of complaints related to neck, upper back and upper extremity complaints in individuals in their teens and 20’s. The prolonged use of the cell phone for texting causes a significant change in posture, with the head shifting forward and the neck flexing downward resulting in a reversal of the normal physiologic curvature of the cervical spine. This repetitive behavior can lead to irreversible changes in the posture of the individual, which then causes weakening of the neck and upper back muscles leading to increased pressures across the discs. Ultimately, this weakens the discs in the neck and spine, making them more vulnerable to bulging and herniation. This then leads to compression of the nerves causing pain and weakness of the arms and upper back. As time goes on, these changes become progressively worse and harder to correct.

So what is the answer to this vexing problem? Obviously, we are not ready to “cut the umbilical” with our phones, therefore we must begin to aggressively “self treat” and, from time-to-time, may need more professional treatment, ie, chiropractic or physical therapy to avert any more invasive options in the future.

So, what can someone do since we cannot give up our cell phones? First, spend less time texting and more time talking, which in turn improves communication skills.

When one cannot talk, text while holding the phone at eye level, thereby, holding the head up and avoiding excessive neck flexion. As for our children, limiting their texting (not an easy task) and keeping them active in sports. This will help them to condition their posture and maintain core strength. Yoga and Pilates also have proven to be quite successful.

Several strengthening and stretching exercises have also proven to be very advantageous.

Bring the chin down while maintaining the neck in extension and performing isometric contraction of the neck muscles. Next, standing with the back against the wall, pull the shoulders and head back touching the wall while contracting the neck muscles. Yet another is while lying prone on the floor or the bed: lift the head and the chest off the bed while contracting the neck and upper back muscles. Sore back? Try arching the lower back and lifting the legs and feet off the floor while contracting the lower back muscles. An “inversion table” readily found on-line can also be quite effective.

Finally, never hesitate to seek out professional help if that pain persists, but especially if you are experiencing any nerve pain, weakness, or altered sensations.
