Dear Member, RCN Those 78% of members who are also RCN members will have been shocked to hear the news that The RCN, from July, will exclude aesthetic nursing from their insurance cover. Whilst we have no expectation that the RCN will change its stance, we will be asking for further information on why and how they reached this momentous decision, which we feel sends out a very negative message without the necessary detail to qualify it. Insurance groups have reported that nurses tend to be more cautious than other clinical groups in the treatments they undertake; only 14% of all paid claims since 2009 were related to treatments undertaken by nurses. The leading medical insurance group reported that very few complaints made by patients were against nurses in the past 5 years (none for chemical denervation and less than 10 for dermal fillers). They speculated that this was due to the relationship fostered between nurse and patient and how nurses manage expectations and complaints. We would benefit from a full understanding of WHY the RCN consider aesthetic nurses to represent such an unacceptable risk, what percentage of total paid claims and for what reasons? Do the RCN mirror the experience of other insurers or does their experience differ, and why? We do need to examine the practice that leads to claims and consider how we, as an organisation, need to direct our standards and learning to reduce risk to the public, also to protect the reputation of nurses who DO practice safely, professionally and ethically. You may all take a look and form your own conclusions, but those who have volunteered to help develop guidelines will make good use of the survey and all your comments to produce consensus guidelines. As clinicians, it will always be your decision who to treat and how, as long as your decision is informed by best practice and safety standards and influenced primarily by the best interests of the individual patient. Surgical Thread Lifting There has been some discussion and postings on social media recently on the topic of nurses carrying out surgical thread lifting using reabsorbable sutures. Though the scope of nursing practice is not limited by definition, it must be undertaken with due consideration of a sound evidence base of safety and efficacy and the nurse must be able to evidence competency. The BACN is not satisfied that any of these criteria have been sufficiently explored, and certainly the necessary Competencies have not been defined or agreed. Nurses should not accept manufacturers or distributors standards without thorough scrutiny. The availability of insurance cover does not reflect in and of itself an endorsement or justification to undertake any procedure in the absence of clear, independent and professional consensus guidelines. It is unclear as to who has made the decision that this procedure is appropriate for nurses to undertake. We must not rely on commercial companies and those who are paid by them to make this decision - they will often err on the side of profit. Although it may be purported that NIP’s would be in a position to treat complications such as infection or allergic reaction, nurses would not be in a position to address facial nerve or parotid duct damage or serious adverse events without advanced surgical support.

Health Education England Update As you are aware, Health Education England (HEE) is working with regulators and royal colleges to conduct a review of the qualifications required for non-surgical cosmetic procedures. This is one of the outcomes of the ‘Review of the Regulation of Cosmetic Interventions’ led by Professor Sir Bruce Keogh. To inform this review, HEE have asked interested parties to contribute to a ‘Call for Evidence’ and will be holding workshops providing an opportunity for all stakeholders to contribute to the review and will bring together a variety of stakeholders including:       

Professional Associations Regulatory bodies Royal Colleges Practitioners Course Providers Insurers Publishers Other stakeholders and organisations.

The BACN will be submitting a joint response with BCAM to the call for evidence (which anyone may participate in see our website News section and click on the link). We are delighted to be collaborating with our medical colleagues. We will also be around the table at a series of expert group meetings with HEE and will keep you informed of any progress made. Administering POM’s It has been brought to our attention that there are members who are attempting to solicit supplies or toxin, bypassing prescriptions and face to face assessment, and more worryingly, supplying toxins without a prescription. Whilst we do appreciate how challenging it is to provide a service to patients requesting toxin and how difficult it is to access the V300, such behaviour is illegal, in breach of your professional Code of Conduct and puts you at risk of prosecution and losing your registration. Such behaviour plays into the hands of the media and those who see Aesthetic Medicine as a field which attracts the greedy with no regard for legislation, regulation, safety or ethics. The BACN are working extremely hard to present our members as nurses who strive for best practice standards and to promote aesthetic nurses as safe, professional and ethical. We would not hesitate to suspend any member found to be in breach of NMC Standards and our own Code of Conduct. I would ask members to ensure they have read our Code of Conduct and to please contact us if they need any help, advice or support.
