The majority of health center operating funds come from patient revenues including Medicaid, Medicare, commercial insurance, patient fees, and other resources. Health centers have access to other programs, including the Prospective Payment System (PPS) for Medicare and Medicaid patients; the 340B drug
program; free vaccines from the Vaccine for Children program; provider loan repayment through the National Health Service Corps (NHSC); and access
medical malpractice coverage through the Federal Tort Claims Act (FTCA).

UNDERSTANDING SLIDING FEE DISCOUNTS

• Health centers are required to offer their services on a sliding fee scale (SFS) for low-income patients with or without insurance, and they cannot turn away patients for an inability to pay.

• Patients with incomes between 100-200% of the current FPG are charged on a sliding fee scale and patients below 100% FPG may only be charged a nominal fee.

BUILDING A SUSTAINABLE WORKFORCE

“My experience with the CareerStep MA Training program gave me access to attend school that I otherwise wouldn’t have had. I am a young single mom and needed a cheap fast track way to get an education and still be able to support my family. The MA Training program provided me with that. My experience going through the program was good. I had a wide variety of support from Career Step, AUCH, and my employer, Carbon Medical Service, to help me through the process. They were all very accessible to me and my direct supervisor helped to keep me on track. It was important for me to finish the course for both personal and professional reasons. It has given me the skills to excel at work and pushed me to further my education.”

Medical Assistants (MA) are an anchor of patient care in America’s health centers. The value of a qualified MA cannot be overestimated, as they can enhance productivity, drive quality improvement, and advance patient care. However, hiring and keeping qualified MAs is a workforce challenge due to the high demand for MAs at small and large health centers alike. Adding to this challenge, MAs in Utah do not have to be certified, which can sometimes lead to gaps in medical knowledge.

To address this challenge, Utah health centers partnered with AUCH to undertake a pilot project utilizing CareerStep’s online training platform for MAs. Through this project, as many as 25 Utah health center employees will become Certified Medical Assistants. This project provides economic opportunity for the learner and improved patient care for the patients they serve.

VALUE BASED CARE

Our value is caring for the whole person.

Caring for the whole person means offering comprehensive care in a coordinated and proactive manner. Utah’s health centers provide whole person care by offering medical, dental, behavioral health, substance use disorder (SUD) services, and affordable pharmaceuticals. Addtionally, health centers address social determinants of health by providing social supports, health education, assistance with insurance coverage, and healthy lifestyle coaching.

We focus on comprehensive primary and preventive care, including immunizations and screening exams, and proactive management of chronic illnesses such as diabetes, asthma, heart disease, lung disease, and cancer. Preventing illnesses and managing chronic disease creates value by providing care in the least costly setting and keeping people out of emergency rooms and hospitals.

We work with each patient to get well, be well, and live well.

The whole person care model recognizes that physical illnesses may co-occur with or exacerbate mental illness and SUD. Patients with these conditions may experience worse health outcomes than expected because of the interplay between the conditions. For example, people struggling with depression may be less able to engage in managing their chronic medical conditions than people without depression. Co-occurring conditions are optimally managed by providing integrated care. Integrated care happens when primary care and behavioral health clinicians work together and closely collaborate with patients and families to address all aspects of the patient’s health.

VALUE THROUGH TECHNOLOGY

Utah health centers are building capacity for success in value-based payment models through the use of enhanced technology to improve the quality and efficiency of care through actionable data. In Fall 2021, UTECH implemented an interoperable population health management platform designed to facilitate care transformation, drive quality improvement, aid in cost reduction, and simplify mandated reporting.

At the close of 2022, twelve health center organizations have successfully implemented the platform.

The platform, Azara DRVS, enables health centers to more efficiently organize patients into groups by risk —allowing care teams to more effectively manage them. In 2022, Utah health centers used this platform for a targeted outreach campaign to improve their patients’ cancer screening rates. The campaign led to increased cancer screening rates, saving lives and lowering long term health care costs.

INTEGRATED CARE TRAILBLAZERS

SHIFTING TO MEET PATIENT NEEDS

Family Healthcare, which operates five clinics in St. George, Cedar City, and Hurricane, has been integrating behavioral health into their medical care settings since 2015.

In that time, their behavioral health team has more than doubled, currently consisting of eight fulltime Behavioral Health Consultants (BHC), who work shoulder to shoulder with 14 medical providers.

By shifting their approach to behavioral health care, moving away from traditional therapy, and increasing immediate access with brief interventions, Family Healthcare has seen improved patient outcomes from diabetes to depression.

For over 35 years, the Association for Utah Community Health (AUCH) has represented Utah’s health centers and their patients. As Utah’s federally designated Primary Care Association and Health Center Controlled Network, we provide more than 12,000 hours of training and technical assistance to Utah’s health centers and affiliate members each year.

AUCH’s Community Health Services division is home to the Utah Healthcare Corps, an AmeriCorps program, and supports more than 20 CHWs making a difference in the community by providing vital health education and connections to health and social resources to improve patient lives.

In support of our members and the people they serve, AUCH helps reduce barriers to health care through health promotion, community engagement, education, and policy analysis.

AUCH receives federal, state, and private program support to improve the quality of and access to comprehensive health care services.
