BEHAVIORAL HEALTH CARE

The integrated behavioral health model consists of a team of primary care and behavioral health clinicians working together with patients and families to provide patient-centered care. This care is designed to address mental health and substance use conditions, health behaviors and chronic illness, life stressors and crises, and stress-related physical symptoms. The integrated care approach gives health center patients the opportunity to become actively engaged in comprehensive health care.

RESEARCH INITIATIVES + QUALITY IMPROVEMENT

COLLABORATIONS POPULATION HEALTH MANAGEMENT

Health centers partnered with AUCH, the Huntsman Cancer Institute Center for HOPE, and UDOH through a grant with the National Institute of Health (NIH) to implement population health management and patient outreach tools, intended to drive the reach, uptake, and impact of COVID-19 testing and vaccinations among Utah’s health center patients.

DECREASING COLORECTAL CANCER MORTALITY Health centers partnered with AUCH, the Huntsman Cancer Institute Center for HOPE, UDOH, and the American Cancer Society through a grant with the Centers for Disease Control (CDC) to increase colorectal cancer screening rates and ultimately decrease colorectal cancer incidence and mortality among Utah health center patients.

INCREASING HPV VACCINATION RATES Health centers are developing a partnership model with AUCH, the Huntsman Cancer Institute Center for HOPE, UDOH, and the American Cancer Society North Region to increase adolescent HPV vaccination rates in five rural Mountain West states. Partners will convene leadership from the five state health agencies and professional associations to explore barriers, review evidence, and create action plans that address geographic disparities, increase HPV awareness, and increase HPV vaccination rates.

TOBACCO CESSATION Health centers have an ongoing partnership with AUCH, the Huntsman Cancer Institute Center for HOPE, and UDOH through a grant from the Patient-Centered Outcomes Research Institute, to evaluate text messaging, phone counseling, and electronic health record (EHR)-based interventions and how they impact tobacco-using patients who enroll in Quitline treatment and successfully quit.

PAIN MANAGEMENT FOR PATIENTS WITH CHRONIC BACK PAIN Health centers partnered with AUCH and the University of Utah, through a grant with NIH, to evaluate telehealth strategies that provide non-drug options for patients with chronic back pain. The primary goal of this project is to provide patients with an accessible alternative to opioids, thereby reducing the risk for dependence and/or misuse.

Utah’s health centers were prepared to address COVID-19 and its impacts since the early stages of the pandemic, focusing on keeping patients healthy, as safe as possible, and out of the emergency departments.

Mobilized quickly to maintain and increase access to medical, dental, and behavioral health services via telehealth.

•

Utah Navajo Health System, Inc. (UNHS), Family Healthcare, Wayne Community Health Center, Inc., FourPoints Health, Green River Medical Center, Utah Partners for Health, Mountainlands Family Health Center, Midtown Community Health Center, and Fourth Street Clinic, partnered with Utah Department of Health (UDOH) to expand testing capacity to Utah’s most vulnerable and at-risk populations and communities.

•

Utilized Community Health Workers (CHW) to address social determinants of health and provide assistance to those affected by COVID-19.

Utah Navajo Health System

From March 2020 to February 2021, UNHS administered more than 9,000 COVID-19 tests and over 2,000 vaccinations to the Navajo Nation, which had one of the highest infection rates in the country.

Fourth Street Clinic

In response to the pandemic, Fourth Street Clinic set up multiple tents to transform the clinic parking lot into a fully functional clinic.

Their dedicated staff performed more than 4,300 tests from March to December 2020.

In June 2020, the mobile clinic began testing clients at Salt Lake’s three Homeless Resource Centers and other community partner locations to mitigate the spread of the virus in community living situations – performing nearly 5,800 tests from June to December 2020.

In total, Fourth Street Clinic performed over 10,000 COVID-19 tests from March to December 2020.

FINANCIAL IMPACT HEALTH CENTER PROGRAM GRANT FUNDING In 2019, Utah health centers received over $41 million in grant funding from the Health Resources and Services Administration (HRSA) to provide comprehensive health care services on a sliding-fee scale to lowincome patients. Utah health centers discounted $84 million in services to their low-income and uninsured patients resulting in a $43M deficit. The centers must recoup this deficit through other forms of revenue, including payments from Medicaid and Medicare, commercial insurers, contracts, and philanthropic grants.

COVID-19 FUNDING FOR FEDERALLY QUALIFIED HEALTH CENTERS In 2020, Utah’s health centers were awarded more than $16M in funding from the U.S. Department of Health and Human Services for COVID-19 preparedness, response, and testing. This funding was imperative in protecting clinics from closures, preventing reductions in staff, and allowing health center patients to maintain access to care.

IN MEMORIAM OF H. JACK GEIGER Jack Geiger, MD, broadly recognized as the father of the health center model of care in the United States and one of the foremost leaders in the struggle for equality, social justice, and human rights, died December 28, 2020, in New York City. Sponsored by Tufts University and armed with grants from the Office of Economic Opportunity, in 1965, Dr. Geiger, Dr. Gibson, and Dr. Hatch opened the nation’s first urban and rural health centers at Columbia Point, Boston and Mound Bayou, Mississippi–inspiring the growth of HRSA’s Health Center Program. Today health centers serve over 30 million people in more than 14,000 communities in every U.S. state, territory, and the District of Columbia. Dr. Geiger’s selfless dedication to serve humanity in areas of greatest need will live on in the legacy of the Health Center Program.
