Your key workers have a special responsibility to support you. You will have your own personal care plan which outlines your individual requirements (physical, social, emotional, cultural, spiritual etc.) so that all the team are aware of and respect your needs and wishes.
You and whoever you choose, such as a family member or friend, will be involved in setting up this care plan, along with relevant professionals. It will be updated regularly to ensure that your care needs and choices continue to be met within the home.
Your key worker will always liaise closely with your next of kin to ensure that all your requirements are met.
