Welcome to Sunnymeade We are pleased to launch a new supported living scheme in Chard, Somerset. Sunnymeade provides person-centred supported living across three selfcontained flats, for individuals with learning disabilities, mental health needs and/or autism spectrum conditions.

Brought to you by Agincare Enable, we work together with local authorities, integrated health boards and professionals, enabling individuals to live the life they want as independently as possible.

Three self-contained flats refurbished to a high standard for comfort and independence.

What we offer: Supported living services, outreach, community-based services and respite care.

Support for adults with learning disabilities, autism spectrum conditions, mental health conditions, complex needs, physical impairments, and epilepsy.

Person-centred care plans that focus on developing independence.

Implementation of a positive behaviour support (PBS) approach which promotes inclusion, choice, participation and equality of opportunity, enhancing quality of life.

One ground-floor flat, with two located on the first floor, each with its own front door, living room, kitchen, bedroom, and bathroom.
