WELCOME

As an employee of Boone Health, Unum is pleased to continue providing eligible Boone Health employees the opportunity to enroll in Unum’s voluntary individual short term disability insurance. Whether you are currently participating or looking to enroll for the first time, use this guide to learn how this program works, how to enroll or make changes to your existing policy, who to contact for questions, and much more!

LET’S GET YOU STARTED

What is voluntary individual short term disability insurance and how does it benefit me?

If you are looking into this insurance for the first time, it’s important to know this is a completely voluntary program that pays in addition to Boone Health’s short term disability plan to help provide additional income protection above the 60% benefit currently being provided.

If you become injured or ill due to a covered off-the-job disability or covered pregnancy, this program pays up to 40% of your monthly base salary to protect your income. When you are on a leave of absence, the Unum voluntary individual short term disability insurance helps ease the financial stress and protects your income. It’s also important to know, YOU own the policy so if you leave Boone Health or retire you can take it with you. Unum will bill you directly for the same premium amount.

HOW IT WORKS

BOONE HEALTH PAID SHORT-TERM DISABILITY

Covers 60% of your base weekly earnings.

BOONE HEALTH PTO

Available PTO will supplement 40% each day until your PTO is exhausted or you return to work.

VOLUNTARY INDIVIDUAL SHORT-TERM DISABILITY INSURANCE

You can buy up to 40% of your monthly base salary.

Employees are required to use available PTO during the Boone Health STD 5-day elimination period and to supplement their STD benefits in accordance with the current Boone Health PTO Policy. ®

2023 PROGRAM DESIGN (effective 11.01.2023)

Unum provides Boone Health employees the opportunity to enroll in the voluntary individual short term disability insurance. PLEASE NOTE: If you have an existing policy, you can continue your coverage.

BENEFIT PERCENTAGE AMOUNT

The percentage amount of coverage based on your monthly income.

BENEFIT MINIMUM AMOUNT

The minimum amount of coverage based on your monthly income.

BENEFIT MAXIMUM AMOUNT

The maximum amount of coverage based on your monthly income.

ELIMINATION PERIOD

The elimination period is a period of time that an employee must be disabled before benefits are paid.

BENEFIT DURATION

If you become disabled, this is the maximum amount of time you can receive benefits for a covered disability.

6 months (180 calendar days).

MENTAL HEALTH

PREMIUMS

Mental health coverage is included and will provide a benefit equal to 50% of the policy benefit amount with the same 7-day elimination period up to a 6-month duration period. If you reside in VT or CA, mental health is covered at 100% of the policy benefit amount, up to a maximum benefit period of 6 months.

Premiums are based on your age at the time your voluntary policy goes into effect and the selected monthly benefit amount. Please see below details on how to get a quote:

HOW TO ENROLL

NEW PARTICIPANTS

Unum will guarantee the issuance of a voluntary individual short term disability policy to the following new Boone Health employees:

• if you started working >24 hours a week on or after September 9, 2022, or

• if you were hired on or after September 9, 2022

All other applicants will not be guaranteed issuance and will be asked health questions (medical underwriting).

EXISTING PARTICIPANTS

If you are currently participating in the Unum voluntary individual short term disability insurance, and would like to make changes or ask a question about your policy, please call 1.800.214.6184 and one of our Advisors will assist.

ENROLLMENT AND CHANGE OPTIONS FOR EXISTING PARTICIPANTS

OPTION 1 - KEEP YOUR EXISTING POLICY “AS-IS” NO ACTION NECESSARY

OPTION 2 - KEEP YOUR EXISTING POLICY THE SAME AND ENROLL IN ADDITIONAL COVERAGE. YOUR COVERAGE BETWEEN ALL EXISTING AND NEW POLICIES CANNOT EXCEED 40%.

OPTION 3 - DECREASE EXISTING COVERAGE

HERE’S WHAT YOU NEED TO KNOW:

• If you have an existing Unum voluntary individual short term disability policy in place, you can keep your coverage the same and continue your payroll deductions.

• Mental health coverage is included when enrolling in new coverage.

• If you currently have a 60% benefit amount under your existing policy, you have the option to change this policy to a 40% benefit amount.

3 ENROLLMENT AND CHANGE OPTIONS FOR EXISTING PARTICIPANTS

As an existing participant in Unum’s voluntary individual short term disability insurance, you have the following options during this enrollment time period. To make changes to your existing plan/or enroll in an alternative plan design, you must call 1.800.214.6184 during the enrollment time period and an advisor will complete your application over the phone.

OPTION 1 – KEEP YOUR EXISTING POLICY — NO ACTION NECESSARY

If you want to keep your existing policy, no action is necessary.

OPTION 2 – KEEP YOUR EXISTING POLICY THE SAME AND ENROLL IN ADDITIONAL COVERAGE. YOUR COVERAGE BETWEEN ALL EXISTING AND NEW POLICIES CANNOT EXCEED 40%.

You may elect coverage up to 40% of your monthly base salary to a maximum of $5,000. (Please Note: This is the maximum benefit amount for all your Unum policies combined).

• A new policy will be based on the 2023 plan design.

• The new policy will be issued based on your age at the time your new policy becomes effective.

• The new policy amount will be subject to a new pre-existing condition period.

• Evidence of Insurability (EOI) is required for existing policyholders who are enrolling in the new plan design.

• For details, please see policy provisions on the last page.

• There will be no impact to your existing coverage if you add coverage. Unum will issue you a new and separate policy under the new plan design.

OPTION 3 – DECREASE EXISTING COVERAGE

• You may reduce your existing benefit amount; however, the elimination period and the benefit duration cannot be changed. For example, the elimination period cannot be changed from 14 days to 7 days and the benefit duration cannot be changed from 180 days to 90 days.

• Your existing policy will continue to be based on the age you were at the time your policy was originally issued (i.e. original effective date) and you will not have a new pre-existing condition period.

FREQUENTLY ASKED QUESTIONS

HOW DOES THIS VOLUNTARY INDIVIDUAL SHORT TERM DISABILITY PLAN THROUGH UNUM COORDINATE WITH MY BOONE HEALTH SHORT TERM DISABILITY PLAN?

Your Unum voluntary individual short term disability policy does not coordinate with the Boone Health short term disability plan. Rather, your policy will pay in addition to the Boone Health short term disability plan. This program is not intended to replace existing coverage, but rather pay in addition to the Boone Health short term disability plan.

WHO CAN ENROLL?

All regular full-time and part-time employees working 24 or more hours a week (48 hours or more a pay period) are eligible to participate (excluding Directors and above, and Boone’s Medical Group Physicians and Nurse Practitioners and all employees that are eligible under Boone’s salary continuation policies). Coverage is available to all eligible employees ages 17 to 69 who are actively at work.

IF I LEAVE BOONE HEALTH OR BECOME INELIGIBLE, CAN I CONTINUE MY COVERAGE?

Yes. If you leave Boone Health or lose eligibility, you can continue your coverage. Unum will bill you directly.

CAN MY SPOUSE OR CHILDREN BE COVERED?

No. Unum’s voluntary individual short term disability program only covers you as the employee if you were to experience a covered disability.

ARE BENEFITS TAX-FREE?

Yes. Benefits are tax-free because you pay the premium with after-tax dollars, under current laws.

HOW DO I FILE A CLAIM?

POLICY PROVISIONS

Pre-existing condition limitation — If you have a pre-existing condition within a 12-month period before your coverage effective date, benefits will not be paid for a disability period if it begins during the first 12 months the policy is inforce.

A pre-existing condition is a condition for which symptoms existed (within 12 months before your coverage effective date) that would cause a person to seek treatment from a physician or for which a person was treated or received medical advice from a physician, or took prescribed medicine. The determination on whether your condition qualifies as pre-existing will be based on the date of disability and not the date you notify Unum.

Pregnancy — Is considered the same as any other covered illness after the policy has been in effect for 9 months. The available monthly benefits will be paid upon fulfillment of the elimination period. Benefits will not be paid if the insured individual gives birth within nine months after the coverage becomes effective. However, medical complications of pregnancy may be considered as any other covered sickness, subject to the pre-existing condition limitation.
