Welcome to the YCCO Community Resource Fair!

Service Integration Team

What is Service Integration (SI)

SI is a way to facilitate collaboration among community partners to provide coordinated resources and discuss local needs.

Collectively, SI community partners work to improve overall health and well -being of Yamhill County residents by matching resources to clearly defined needs while avoiding duplication of service.

SI oversee a pot of funds that are dedicated to supporting community needs that traditional means of resources could otherwise not cover.

What We Do: 3 Components to a SI Meeting

1.Team Business

Discuss SI District Balances

• SIT has 7 districts in YC

• YC host 6 districts

• West Valley is shared with Polk Co. Hosted by Abby Warren

SI has two types of Funds

• Urgent Funds

• General Funds

Max SI Financial Assistance

• $400 for most SI request

• Exceptions apply to Housing Stabilization needs while district has funds

2. Resource Sharing

Resource Questions:

Community Partners brainstorm cases We discuss barriers clients may be facing what local resources could assist

Resource Sharing: Opportunity to let the SI team know of any exciting new resource in the community.

Partner announcements: SI partners share a brief update on the changes in the agency or classes or workshops they may be hosting in the month.

3. Agency Presentation

Introductions:

Agency Presentation are 15-20 minutes.

Quick SI Guideline

• Share the services that your agency offers.

• Describe the population do you serve.

• Discuss application/referral process

• Share hours of operation.

SI Supported Solutions

Most SI requests will fit one of the Supported Solutions and will be put to a vote by the SI district where funds were requested.

Basic Needs: Food, Clothing, Rx’s and personal items

Extracurricular: Fee’s materials, clothing or shoes needed to participate in extra-curricular activities

Education: Classes, tuition, fees literacy programs

Youth Development: Leadership / learning opportunities not part of the regular school programs

Community Development: Events & Activities

Urgent Needs: Emergency Housing, Utility Assistance

Urgent Request Criteria

A need that if not addressed immediately could result in serious consequence to the client.

Utility Shut off or Eviction Notices

Urgent request will bypass the SI Team vote and approval's are at the discretion of the SIT Specialist and EL Director.

For General SI request we ask that you make every attempt to present case at the SI team meeting for the district you are requesting the funds

Things to Remember

• Please complete the SI application in its entirety.

• Note there must be a plan of sustainability.

• Email any documentation that supports your request.

• Depending on the SI request additional documentation may need to be completed; W9, letter from management co etc.

• We are not a first line funding source.

• SI requires 2 attempts for community assistance with an outcome. Approved, denied, willing to leverage .

• Only one SI request per client per SI calendar year.

MISSION AND VISION

HOW/WHY DID FAMILYCORE COME TO BE?

• Background: Many families in Yamhill County face significant challenges that impact their ability to thrive. These challenges include accessing healthcare, securing quality childcare, finding adequate housing, and obtaining nutritious food.

• Purpose: FamilyCore was created to address these challenges head-on by providing a comprehensive network of services and resources tailored to the needs of families with young children.

• Vision: Our vision is to nurture strong, resilient families where every child has the opportunity to grow up healthy and succeed. By offering integrated support, we aim to create a community where families can easily access the resources they need.

• Impact: FamilyCore seeks to make a lasting impact by improving the overall wellbeing of families, reducing stressors, and promoting healthy development for children ages 0-5. Our holistic approach ensures that families receive the support they need to thrive in all aspects of their lives.

How to request services

• Complete the Request Form:

• Fill out the request form available on the page.

• Answer each question to the best of your ability, providing as much detail as possible about the family's needs and circumstances.

• Form Submission:

• Once you have completed the form, submit it online.

• The submitted form will be directed to me for initial review.

• Review Process:

• I will review the submitted form to determine if the family qualifies for assistance.

Some reasons a family might not qualify include:

• They are not in our service area.

• Insufficient information provided in the form.

• The family is already receiving assistance from another agency.

• Presentation to Community Agencies:

• If the family qualifies, I will present their case to our network of community agencies.

• These agencies will evaluate the case to identify which one(s) can best meet the family's needs.

• Two county parenting education hub through the Oregon Parenting Education Collaborative (OPEC)

• Provides evidence-based parenting education and opportunities for connection and empowerment

Yamhill Early Child Education

• Free early learning for children in Yamhill County at or below 200% of the FPL

• Preschool Promise locations in McMinnville, Newberg, and Dayton

• Head Start locations in McMinnville, Newberg, Lafayette, and Sheridan (130% FPL)

• OHP members are automatically eligible for Preschool Promise

• Referrals to other public preschool programs or IFSP services in Yamhill County are available as needed

Parent Leadership Council & Early Learning Council

• Yamhill Community Care’s Parent Leadership Council (PLC) is comprised of parents from Yamhill County. These parents advise the Early Learning Council regarding challenges and priorities for families in Yamhill County.

• The Early Learning Council works collaboratively to develop coordinated systems that are child-centered, family-friendly, culturally, and linguistically appropriate, and community-based to meet the needs of Yamhill County families.

• Together these committees approach family resiliency and school readiness by focusing resources on the intersection between health and education.

• The Parent Leadership Council also provides a forum to support the development of leadership and advocacy skills among members. It is also a means to developing intentional community, though shared activity.

Safe Family Volunteers are...

Host Families: Screened and approved by Safe Families, Host Families take in children for short periods of time.

Family Friends: Provide support to families in need (e.g., mentoring and transportation) and Host Families (e.g., babysitting and providing meals).

Resource Friends: Provide various types of goods and services to families in need to help parents get back on their feet.

Family Coaches: Are trained to provide resources and family-like services such as mentoring and emotional support to families in crisis. They also ensure that kids are well cared for in Host Family homes.

Safe Families Churches: These are faithbased communities that support Safe Families as a ministry of their church. There are three church levels: Participating Church, Lead Church, and Community Lead Church.

YCAP Energy Services

ENERGY SERVICES

YCAP energy services is committed to helping people stay warm and safe.

The intent of energy assistance is to provide financial support to eligible households to offset residential heating and cooling costs. Findings confirm that there have been dramatic increases in home energy costs that impose a disproportionately larger burden on fixed, low, and lower middle income households.

YCAP recognizes that adequate home energy/heating and cooling is a necessary aspect of shelter, and that deficiencies in home heating and cooling pose a threat to life, health and/or safety.

BILL ASSISTANCE

LIHEAP

(LOW INCOME HOME ENERGY ASSISTANCE PROGRAM)

Assistance for senior and disabled households begins in October, with general public being placed on a referral list starting in December. Assistance may be directed to the primary heating or cooling source in the home.

OEAP

(OREGON ENERGY ASSISTANCE PROGRAM)

Is for PGE customers only. OEAP qualifying criteria includes income eligibility. Priority status may be given to households with no power or a shut off notice.

OLGA

(OREGON LOW - INCOME GAS ASSISTANCE)

Is for Northwest Natural (NWN) customers only. OLGA qualifying criteria includes

(A) Income eligibility

(B) Active account with NWN.

ELIGIBILITY

Assistance is available to renter and owner occupied housing that are below 60% of Oregon’s Statewide Median Income.

YCAP Housing Stabilization

Housing Stabilization

Housing Stabilization at YCAP offers a variety of services and resources to the community including: one-time financial assistance, shelter placement, supportive housing programs, case management, and other supportive services. Our programs are targeted towards lowincome households, and individuals and families who are homeless or at risk of homelessness.

SERVICES INCLUDE:

•Self-sufficiency and goal-setting information

•Referral services

•Financial Assistance that can be used for work related needs, transportation, education, ID, licenses and birth certificates

•Case management to assist families in achieving and maintaining stable housing

•Assistance searching for and obtaining housing

•Shelters for homeless families with children and single women

•Affordable housing

•Rental assistance for homeless and low-income households and those at risk of becoming homeless

Homebuyer Program

Affordable homeownership/loan opportunities for low-income (4065% AMI) families or individuals living or working in McMinnville, Amity, Carlton, Lafayette, Sheridan, Willamina or Yamhill. Program requires Need for Better Housing, Willingness to Partner with Habitat, and Ability to Pay

Home Repair Program

Resource for low-income (<65%

AMI) homeowners in need of minor repairs, ramp, rails/grab bars or other accessibility modifications within program budget. Requirements include up to date mortgage, homeowner’s insurance and property taxes.

ReStore Donation-based store offering building supplies, furniture, appliances, paint and more for 50-70% below retail prices.

Donated goods defer thousands of pounds from local landfills. Proceeds from sales go toward building more affordable homes.

Referral Process

Homebuyer Program Application period based on construction and fundraising timelines, which change frequently.

Average 1-2 times per year.

Interested parties and referrals should review website for basic qualifications and then email or call MacHabitat staff to be added to inquiry list.

*Note – this is not a waiting list. Our staff contacts the inquiry list to alert them of open application date ranges when these are decided along with outreach to community.

Home Repair Program Applications are currently being accepted! Applications are available to download from our website and printed versions in our office. Required documentation to submit is listed on application. Inquiries about specific projects can be sent to Repair Program staff after qualifications are reviewed.

ReStore Store is open for shopping Tuesday – Saturday 10am-4pm.

Donation hours/days can be found on our website/social media and are subject to change.

ReStore can accept/recycle household paint for the Oregon PaintCare Program!
