Governor Evers Proposes Greater Scrutiny of Health Insurance Companies
In his biennial state budget address to the Wisconsin Legislature, Governor Evers has proposed a number of provisions aimed at cracking down on health insurance companies who have high rates of denying or restricting patient care.
The Governor is proposing a number of solutions including audits of companies with high denial rates, limits on the use of pre authorizations, greater transparency of denied claims and a new office within WI OCI to handle consumer health care complaints. The Governor’s budget address marks the official start of the 2025-2026 state budget process which happens in the odd year of the two year session of the Wisconsin Legislature.
The Governor included his health insurance reform ideas as part of the Wisconsin Office of the Commissioner of Insurance’s budget package. Here is what he is specifically proposing within WI OCI.
Health Insurance Claims Denial Audits
The Governor recommends establishing a framework for auditing high rates of health insurance claims denials among insurers offering plans in the state over which the office has regulatory authority. To improve the coverage of health insurance plans, the office would audit insurers with claims denial rates above a certain percentage, as determined by the office, and require corrective action plans based on the audit findings. The Governor also recommends granting the office the ability to enforce corrective action plans through fines, forfeitures or other mechanisms to ensure compliance within a certain date of receiving the corrective action plan.
Prior Authorization Exemption Requirements
The Governor recommends establishing a framework to streamline and expedite coverage approvals for medical procedures, allowing insured individuals to receive prompt determination regarding coverage for their care. The office would develop parameters for when services are exempt from requiring prior authorization by a health insurer. Providers who receive prior authorization approvals above a certain rate would be exempt from requiring prior authorization for services provided or recommended to them.
Prior Authorization Transparency
The Governor recommends requiring health insurers to provide consumers with a list of treatments and services covered by health plans offered by the health insurer which require prior authorization.
Public Intervenor Office
The Governor recommends creating a Public Intervenor Office within the office to provide information and assistance to health insurance consumers to ensure they receive the best possible claim outcomes they are entitled to under their insurance policies. Consumers whose health insurance claims are denied would be assisted with appeals and other legal actions to pursue coverage for health care services.
The GOP controlled Legislature has already announced that it will remove policy proposals like Evers’s health insurance reforms from the state budget.
Last session, the WCA joined with a number of other health care provider groups to support AB 507 which among other things would have prohibited health insurance companies from requiring pre authorization for the first 12 visits. This legislation will likely be reintroduced this session and the WCA will support it.

Beyond Adjustments: Why Eliminating Toxins in Home and Personal Care Matters for Clinical Outcomes

As chiropractors, we recognize that the body has an amazing, innate ability to adapt to stressors and restore health.
We support patients through structural adjustments to remove interference from the nervous system, allowing the body to function optimally. However, in today’s world, our patients face constant toxic exposure from their home environments and personal care products—factors that can significantly undermine the clinical results we strive for.
The Toxic Burden: A Hidden Barrier to Healing
Modern personal care products, household cleaners, and even everyday plastics contain a cocktail of chemicals that disrupt endocrine function, overload detoxification pathways, and contribute to systemic inflammation. Endocrine disruptors such as parabens, phthalates, and bisphenol-A (BPA) mimic hormones, throwing off the delicate balance of the body's regulatory systems. Chronic exposure to these toxins contributes to metabolic disorders, immune dysregulation, and neurological stress—issues that we frequently see in our offices.
Chiropractic philosophy teaches us to seek out and correct the contributing factors of dysfunction. Adjustments restore proper nervous system function, but if a patient’s biochemistry is burdened by toxins, their progress may be limited. Encouraging patients to eliminate toxic exposure from their daily lives can enhance their healing and long-term health outcomes.
Practical Steps for Chiropractors
Educating patients about toxin-free living doesn’t have to be overwhelming. Here are a few ways we support our patients at The Wellness Way:
1. CONSIDER THEIR ENVIRONMENT
Provide patients with a simple checklist to assess their personal care and home products. Highlight common toxins to avoid, such as synthetic fragrances, parabens, sulfates, and petroleum-based ingredients.
2. OFFER SAFE ALTERNATIVES
Guide patients toward cleaner options. Brands that use organic, plant-based ingredients can help reduce toxic exposure. You can even stock or recommend vetted non-toxic products in your clinic.
3. SUPPORT DETOXIFICATION
Encourage hydration, proper nutrition, and supplements that assist in detoxification. Chiropractic care already supports detox pathways by enhancing lymphatic flow and nervous system function.
4. EDUCATE THROUGH WORKSHOPS & CONTENT
Host workshops or provide handouts on non-toxic living. Many patients simply don’t know the impact of their daily product choices. Providing them with knowledge empowers them to make healthier decisions. Starting April 2025, we will host our first ever Detox Challenge for patients and our social media audiences. Reporting Deadline: March 21, 2025
UPDATE: FinCEN Not Issuing Fines or Penalties in Connection with Beneficial Ownership Information
FinCen is back but has announced that it will not issue any fines or penalties, take any other enforcement actions against any companies based on any failure to file or update beneficial ownership information (BOI) reports pursuant to the Corporate Transparency Act by the current deadlines.
No fines or penalties will be issued, and no enforcement actions will happen, until a forthcoming interim final rule becomes effective and the new relevant due dates in the interim final rule have passed.
This announcement continues the Treasury’s commitment to reducing regulatory burden on businesses, as well as prioritizing under the Corporate Transparency Act reporting of BOI for those entities that pose the most significant law enforcement and national security risks.
No later than March 21, 2025, FinCEN intends to issue an interim final rule that extends BOI reporting deadlines, recognizing the need to provide new guidance and clarity as quickly as possible, while ensuring that BOI that is highly useful to important national security, intelligence, and law enforcement activities is reported.
FinCEN also intends to solicit public comments on potential revisions to existing BOI reporting requirements. FinCEN will consider those comments as part of a notice of proposed rulemaking anticipated to be issued later this year to minimize burden on small businesses while ensuring that BOI is highly useful to important national security, intelligence, and law enforcement activities, as well to determine what, if any, modifications to the deadlines referenced should be considered.

WCA Insurance Advocacy Committee
The WCA Insurance Advocacy Committee is continuing to collaborate and have discussions with different payors on a regular basis. Here are the most recent updates:

WCA met with WPS’s Medical Director and Claim representatives in August of 2024. Discussed were the changes that WPS put into place during 2024. Examples include, no longer covering Maintenance Care, no longer covering some therapies for certain conditions, and a reduction in fees. During this discussion we were told that these changes were made across all professions and all claims. WPS did refer to past OIG chiropractic articles, billing for services that do not meet medical necessity and lacking adequate documentation. The WCA Help Desk is still receiving feedback from members that does not align with the explanations we discussed. We have reached out to WPS for another meeting to discuss ongoing member concerns.

WCA had a phone call with Elevance Health, formerly Anthem and members of the Carelon Pre-Auth team, this past November. The discussions were based around the pre-auth process, the Carelon portal and guidelines used to make determinations surrounding un-attended therapies. We will continue to have discussions and work with Elevance Health. The WCA Help Desk would like to have future discussions surrounding denial of E&M services when performed on the same day as Manipulation.

Dr. Kay Fogeltanz, WCA Insurance Advocacy Committee Chair continues to reach out to Optum’s Senior Clinical Director seeking clarity on processes surrounding denied claims. Most recently we have contacted them with member questions surrounding the “Navigate” plans in Wisconsin and not needing a referral to see a Chiropractor in Wisconsin. We will continue to contact them with member questions and concerns as they arise.

Tammy McKeown, CT. continues to have monthly check ins with Fulcrum’s Senior Manager of Provider Network Development. During these monthly check ins they discuss member concerns, claims issues, policy and or contracting concerns and any upcoming changes to guidelines or billing. Fulcrum attended Fall Convention and was very collaborative working with provider members in person and gathering network feedback.
Please continue to contact Help Desk with any Insurance questions or concerns. The committee will continue to establish collaborative relationships and open dialogue with our contacts.
CMS Releases 2025 Medicare Physician Fee Schedule
The Centers for Medicare & Medicaid Services (CMS) published the calendar year (CY) 2025 Medicare Physician Fee Schedule (MPFS). Providers will realize a 2.83% cut in reimbursement fees for 2025. In addition, the 2% sequestration fee cut is still in place. Congress did not take action to stop these cuts before adjourning for the year. The Medicare Part B deductible for 2025 is $257.
The American Medical Association and the American Chiropractic Association have filed complaints with CMS, saying that inflation and rising overhead costs have created a burden for physicians. Historically, Congress has intervened at the beginning of the calendar year to lessen the looming cuts. We will keep you updated if/ when Congress intervenes.
Due to the current uncertainty, we are recommending the following:
• Participating Providers: If you are a Medicare participating provider, we encourage you to continue to bill Medicare at your normal fees. Your bills will be processed based on the appropriate fee schedule once released.
• Non-Participating Providers: If you are NOT a participating provider for Medicare, we recommend that you submit your bills with current fees until we know if Congress will intervene. Based on current information, you can process your claims for services after January 1, 2025, using the currently published 2025 rates and inform your patients that additional amounts may be owed or require refunding.

Immunity, Longevity and Healthy Aging

The US population continues to age rapidly, with about 18%, or one in every six Americans 65 years old or older.
By 2040, 22% of the population will be 65 or older. And people aged 90 and over are the fastest-growing segment of the American population.1 As the U.S. ages, we need to look not only at lifespan, the number of years a person lives, but also quality of health as we age. Longevity is living a long and healthy lifestyle, essentially free of the chronic diseases and disabilities associated with aging. And in the desire for a long and healthy life, the role of our immune system cannot be overstated. Today, the average person in the U.S. can expect to live to 77.5 years but spend only 66.1 years in quality health.2
How influential are genetics?
Some people inherit genes that may shorten their lives or lead to early disability or dementia. We know, for example, that carrying a variant of the apolipoprotein E4 (APOE4) gene can increase the risk of Alzheimer’s disease (AD). About 25% of the U.S. population carries one copy of APOE4, which roughly doubles the risk of developing AD. People who carry two copies of APOE4 homozygotes are more likely to develop early AD (starting at age 55) and have a 60% lifetime risk of developing AD dementia by age 85.3
Your genes aren’t always your destiny. They may load the gun, but lifestyle pulls the trigger.
A recent study looked at 353,742 adults who participated in the U.K. Biobank project between 2006 and 2010 and whose health was tracked until 2021. Researchers used a polygenic risk score to determine a person’s overall genetic predisposition to a longer or shorter lifespan and found regardless of genetic risk, an unhealthy lifestyle is linked to a 78% heightened chance of dying early. However, a healthy lifestyle can offset the effects of life-shortening genes by 62%.4
In practical terms, someone age 40 at high genetic risk of a shortened lifespan could extend their life expectancy by several years if they adopted a healthy lifestyle (not smoking, regular physical activity, moderate alcohol, healthy weight, adequate sleep and good diet). The earlier your patients start to make healthy choices, the more significant the improvement.
Look at people with the APOE4 genetic variant, which predisposes them to AD. About 20% of the population carry the gene, but they account for about 40 to 65% of AD cases. In a prospective cohort study of 29,000 older adults in China, about 20% of participants carried the APOE4 variant.5 When researchers looked at memory loss and cognitive decline over 10 years, they found those with the APOE4 variant who followed a healthy diet and lifestyle had the same notably slower rate of decline as those who didn’t carry the variant and followed a healthy diet and lifestyle. A healthy lifestyle offsets the inherited risk of the gene variant to the point where the risk is the same as not having the variant. So, you can mitigate genetic factors with lifestyle.
Lifestyle changes: A look at diet and nutrition
Diet is the most influential place to start. It’s never too late to make dietary changes that increase longevity. A recent study looked at the diets of more than 100,000 participants spanning 30 years.6 The participants were in their 40s when they joined the study. Participants who followed healthy diets from their 40s onward were 43% to 84% more likely to be functioning well both physically and mentally at age 70 compared with those who did not. The diet most likely to lead to healthy aging had higher intake of fruits, vegetables, whole grains, unsaturated fats, nuts, legumes and low-fat dairy. The ties between diet and healthy aging remained strong even when researchers accounted for physical activity and other actors.

The Mediterranean diet
Study after study7 has shown the Mediterranean diet pattern is an excellent way to support immune health. It provides balanced and varied nutrition that includes plenty of good fats, polyphenols, vitamins and trace minerals, such as selenium and zinc, all crucial for an active immune system. The diet helps maintain a healthy lifestyle, decreases the risk of metabolic and cardiovascular disease, reduces inflammation and promotes a healthy gut microbiome.
Protein
Older adults should consider adding more high-quality protein to their diets to support the immune system within the Mediterranean dietary pattern. Protein is essential for building and maintaining immune cells and antibodies and repairing damaged cells and tissues, particularly as the immune system ages. Current recommendations are 1 to 1.3 grams of protein per kilogram of body weight per day, slightly higher than the recommended amount of 0.8 grams per kilogram for younger people.
Zinc, selenium and magnesium
We often overlook the role of nutrients, such as zinc, selenium and magnesium, in building strong immunity. Zinc, for example, is considered a trace mineral. Still, it’s essential for cytokine signaling and the proper functioning of the pathways that trigger T cell and B cell proliferation in the face of infection. As a co-factor for many enzymes involved in immune cell function, zinc also plays a complex role in balancing pro- and anti-inflammatory immune processes. Zinc deficiency leads to overproduction of pro-inflammatory cytokines, reduced T-cell production and increased susceptibility to viral infection.
Omega-3s
People with high levels of omega-3 fatty acids live almost five years longer than those with the lowest levels.8 Low omega-3 levels also increase the risk of sarcopenia or age-related muscle loss.9 Omega-3 supplementation and dietary changes to include more fatty fish, nuts and seeds may help raise the index to a more favorable level.
Before making specific dietary or supplement recommendations, suggest patients have an omega-3 index test, a simple blood spot test to determine the percentage of EPA/DHA in their red blood cell membranes. A score of 8% or higher is ideal, yet most people in the U.S. score 4% or below, putting them in the highest risk zone for cardiovascular disease.10
Peptides
Age-related changes in immunity mean healing from injury and recovering from illness can be slower for older individuals, which can lead to inactivity, loss of fitness and poor nutrition from loss of appetite, which can then start a slow slide into worsening health. Select peptide supplements help speed healing and a return to regular activity. Peptides, short protein chains containing no more than 55 amino acids, are easily absorbed by the body and quickly enter circulation.
One highly effective peptide supplement is body protection compound 157 (BPC-157), sometimes called the “wolverine” supplement.11 This synthetic peptide contains only 15 amino acids and was isolated in 1993 from a larger protein in human gastric juice. BPC-157 stimulates the body’s natural healing process by reducing inflammation, promoting tissue regeneration and new blood vessel formation, and possibly upregulating growth factor production.12
Final thoughts
You play a pivotal role in the healthcare continuum, particularly in enhancing immunity and promoting healthy aging, by focusing on the body’s innate healing capabilities. Your expertise lies in employing noninvasive techniques that support immune function and fostering a holistic environment conducive to long-term health and wellness. As DCs, we are uniquely qualified to translate aging research into recommendations for our patients that not only target immediate issues but also contribute to the prevention of chronic conditions. Improving diet and assuming a healthy lifestyle can have a powerful impact on overall health and longevity for patients of all ages, but the sooner improvements start, the sooner they are felt and the more impact they have on longevity. Every patient encounter should be seen as an opportunity to provide accurate, actionable information about the importance of a healthy diet and lifestyle for longevity.

ACA LEGISLATION UPDATE
Congress Reintroduces Bill to Increase Medicare Coverage of Chiropractic Services
Bipartisan legislation that would increase patient access to Medicarecovered services provided by doctors of chiropractic was reintroduced January 16 in both the U.S. House and U.S. Senate.
The Chiropractic Medicare Coverage Modernization Act (H.R. 539 / S. 106) would reduce barriers to care and enable seniors and other beneficiaries to more fully access the chiropractic profession’s evidence-based, non-drug approach to managing pain and improving musculoskeletal function.
The legislation achieved historic support in the last congressional session, laying a strong foundation for the current bill, which to date has 75 cosponsors – 64 in the House and 11 in the Senate.
The new bill was introduced by Reps. Gregory Steube (R-Fla.) and John Larson (D-Conn.) and Sens. Richard Blumenthal (D-Conn.) and Kevin Cramer (R-N.D.). Initial cosponsors include Sens. Tammy Baldwin (D-Wis.), Chris Coons (D-Del.), Steve Daines (R-Mont.), Martin Heinrich (D-N.M.), John Hoeven (R-N.D.), Amy Klobuchar (D-Minn.), Jerry Moran (R-Kan.), Mike Rounds (R-S.D.), and Jeanne Shaheen (D-N.H.).
For the past 50 years, and with no scientific or policy basis, Medicare has limited beneficiaries to coverage of one chiropractic service, manual manipulation of the spine. Those requiring additional services have been forced to seek them from other types of providers, delaying care, or to pay out of pocket.
While adding no new services to Medicare, proposed new language would bring Medicare coverage into alignment with other federal programs and private health plans, giving seniors access to additional covered services in their chiropractor’s clinic, such as other nondrug therapies, evaluation and management services, and diagnostic imaging. The allowable covered services would be determined by a chiropractor’s state licensure.
“The prevalence of musculoskeletal conditions such as back, neck and joint pain increases with age. The opioid crisis has emphasized the importance of offering non-drug options for pain management, and treatment
guidelines today support their use as a first line defense against common musculoskeletal pain,” said American Chiropractic Association (ACA) President Leo Bronston, DC, MAppSC.
“We are thankful to the sponsors and cosponsors of this bill for standing with us in support of improving access to non-drug chiropractic services that can help seniors reduce or eliminate the need for riskier and costlier alternatives such as prescription pain medications and surgery,” added John Falardeau, ACA senior vice president of public policy and advocacy. “Musculoskeletal pain, led by spinal disorders, costs the U.S. healthcare system $874 billion per year and is the most common cause of severe long-term pain and disability.”

Chiropractic Residency Program Delivers Exceptional Results

Last year, the WCA printed an article on the creation and development of the Aurora Health Care Chiropractic Residency Program. This program offers a structured, comprehensive curriculum that includes clinical rotations, interdisciplinary collaborations, and scholarly activities. The residency provides an opportunity for chiropractors to develop their clinical skills, gain experience in a wide range of healthcare settings, and contribute to the advancement of chiropractic knowledge through scholarly activity and research.
The results of first 2 academic years of this program include:
• Residents were exposed to a broad patient population with 5,161 documented complex cases with co-morbidities defined within Mental Health, Post-Surgical, Musculoskeletal and Neurological categories. Category breakout included 2109 musculoskeletal cases, 1504 Neurological cases, 962 Post-Surgical Cases and 586 mental health cases.
• Each of the 4 residents have completed Milestone Assessments and Live Clinical Performance examination above the threshold established for proof of competency.
• Each resident completed Chart Stimulated Recalls to the level established as a threshold for successful residency completion.
• Each resident successfully completed the required reading list to include discussion of relevance with faculty.
• Chiropractic residents delivered competent management of 3728 patient visits without reporting incident of safety concerns.
• Residents successfully completed procedural competencies related to both clinical and operational procedures.
• Surveys completed during course of residency included patient perception of resident, specialty rotations perception of resident, resident perception of program, resident perception of faculty, and resident self-assessment.
• Chart review thresholds were met by each resident which demonstrated competence in patient charting.
• Each resident completed their core program interdisciplinary rotations in Radiology, Neurosurgery, Orthopedic Surgery, Interventional Pain, Physical Therapy Rehabilitation, Sports Medicine, Family Medicine, Internal Medicine and Non-Interventional Pain to level of established threshold or great and each resident chose some elective rotations in which to participate.
• Each resident successfully completed the required scholarly activities to include learning module completion, QI project participation, case study completion and presentation, and recommended reading list.
• Each resident participated in Aurora GME scholarly activity and research learning presentations achieving threshold levels of attendance.
• Each resident completed CITI training and received certificates of completion.
• Each resident delivered scholarly presentations to the threshold level established for successful program completion.
• Resident graduation rate was 100%.
• Post residency placement and employment rate was 100%.
• Residency program application rates have steadily increased.
• CCE accreditation was received in August of 2024.

New Wisconsin Educational Requirement for Chiropractors

Professional Boundaries & Ethics Training
Last year, the Wisconsin Chiropractic Examining Board [WCEB] passed a new rule which requires Wisconsin chiropractors to obtain 2 hours of training in professional boundaries and ethics during each educational biennium. The rule is effective beginning with the 2025 -2026 educational biennium, and 2 hours are required in each successive educational biennium.
The WCEB last required professional boundary training in 1992; however, that was a one-time requirement. The new rule requires professional boundary and ethics training each biennium.
The new rule, Chir 5.01 (1) (i), states that qualifying professional boundaries and ethics courses may include any of the following subjects:
1. Sexual misconduct as defined in s. Chir 6.02 (7)
2. Ethics in billing or business management practices.
3. Risk management.
4. Professional boundaries in clinical practice settings.
5. Any other topic that the Board or the Board’s designee approves as being related to the subject of professional boundaries and ethics.

Professional and sexual boundary violations continue to be a significant problem in healthcare in general and chiropractic in particular. The practice of chiropractic has many unique features which predispose to significant exposure for boundary violations with patients including: the use of hands-on techniques, very close physical proximity of the doctor to the patient, patients who are seen regularly over time, the informal/friendly environment of many chiropractic offices, solo practices, small or no staff presence, home offices, late or unusual office hours, etc.
Despite strict enforcement, and increased education, regulatory agencies continue to field significant numbers of complaints of professional and sexual boundary violations involving chiropractors. When finally disclosed, these violations can be catastrophic for the chiropractor, sometimes resulting in license sanctions, including license revocation. In the more egregious cases, the chiropractor may be sentenced to jail, or be assessed significant monetary fines or judgments. The cumulative effect of these publicized boundary violation complaints is also extremely negative for the public perception of the chiropractic profession as a whole.

Taught by Dr. Jeff Wilder, the course, “Professional Boundaries 2025-2026”, will review the climate of sexual abuse in the United States, in healthcare, and specifically, in chiropractic. The psychological factors peculiar to doctors and patients who are involved in boundary violations will be reviewed. Attendees will learn special responsibilities of a chiropractor as an employer, and will discuss topics such as gowning, treating minor children, how to deal with an aggressive or flirtatious patient, why consensual relationships are not legal, and many other relevant topics. A major focus of the course is to develop proactive steps that can be taken by the chiropractor to reduce the likelihood of a patient complaint of a boundary crossing or boundary violation.

Pelvic Obliquity Through the Chiropractic Lens

Charting
a course for correcting pelvic obliquity seems like a task for a Chiropractic 101 class when considering its simplest definition:
“A postural
abnormality
occurring when the pelvis tilts to one side.”
The reality is it can be more complex and require we use our full set of tools. Furthermore, while this may be a common presentation in your office, the chiropractic perspective on this and other neuromusculoskeletal issues is rarely the first to reach the public. The competing information a potential patient is exposed to when researching subjects like pelvic obliquity will likely add roadblocks to their path to chiropractic care. Therefore, your care plan needs to address all clinical components and be well-supported by patient education.
As the immediate base of support for the spine, the pelvis directly influences the effectiveness of your chiropractic care. The most common postural distortion of the pelvis is anterior translation. Often this distortion is caused by overpronation; when the arches collapse, the weight falls to the midline, the feet flare out and the body’s weight is pushed forward. This phenomenon presents itself overwhelmingly as asymmetrical — contributing to obliquity in the pelvis — and the margins are so small they may go unnoticed. A healthy population typically has a normal pelvic obliquity angle of 0–5.6°. Slight pelvic obliquity may be common and normal. This can also cause a bilateral stretch-weakness of the iliopsoas muscle bilaterally. Pelvic obliquity can cause abnormal posturing, which can increase stress on the spine, sacroiliac joint and hips. It can also cause pain in the buttock region, hip region, lower back or into the lower extremities.
A common chiropractic approach to correction would be to clear spinal subluxation before addressing any related areas like the hips and lower extremities as needed. This may take place within a visit or over the course of multiple visits. first instinct, but most do not share this integrated view of the body. Unfortunately, the traditional linear approach to examination and diagnostics can create a funnel overly focused on symptoms.
Viewed from the public’s perspective, I believe you’ll find the world is hungry for chiropractic. A quick search yields the following possible causes for pelvic obliquity:
• Muscle imbalance
• Unsuitable seating
• Leg length inequality
• Scoliosis
• Hip contracture
• Gait abnormalities
• Spine surgery
• Trauma
The traditional approach people have been taught through conventional medicine is to follow each of these leads individually, but a chiropractic assessment and treatment plan has a pathway to address all of them. Let’s unpack some of the components of pelvic obliquity with a chiropractic solution in mind.
Muscle Imbalance
Tight or weak muscles in the front or back of the pelvis, can cause misalignment. In the chiropractic model, the vertebral subluxation complex is the first consideration and explains functional muscle imbalance from nervous system interference. Muscle coordination is based on timing that’s measured in milliseconds. Even a partial loss of reception can disrupt muscle action timing, leading to a tug-of-war between muscles struggling to maintain balance. Psoas reactions are common to protect against SI joint instability, particularly involving the SI ligament, which is also vulnerable to dysfunction of the arches. In our exam, look at individual ranges of motion as well as symmetry of motion, a big part of our indicator systems that guide specific adjustments. This approach is critical in assessing spinal and pelvic dynamics.
Leg Length Inequality
Leg length inequality can cause pelvic obliquity. While planning how to correct asymmetrical pelvic tilt, there may be complicating factors, such as naturally occurring leg length discrepancies, which make compensatory misalignments a functional necessity. One of the most common findings that can introduce asymmetry to the legs and pelvis is excessive foot pronation (hyperpronation). In addition to occurring bilaterally, it usually presents asymmetrically. Extremity adjusting and custom flexible orthotics can balance the pedal foundation and minimize serial distortion, so your adjustments hold longer and better.
Hip Contractures
Hip contractures can develop when the normally elastic connective tissues in the hip or knee are replaced by inelastic fiber-like tissue, which can prevent normal movement. Additionally, connective tissue contracture can affect neutral positioning, further compromising posture. The care plan may need to include ways to influence connective and soft tissues to hold and provide stability. By correlating the spine with hip and lower extremity fixation and misalignment, we can identify the whole-body patterns that cause pelvic obliquity and simultaneously map out areas that need support to recover.
Unsuitable Seating, Including An Ill-Fitting Chair
Sitting down should offer a chance to recover from activity, but unfortunately, many people have to perform from an office/desk environment, and the choice is more of a necessity. Whatever postural abnormalities you bring to the chair can quickly be magnified if the chair can’t provide support and promote balance. After corrective chiropractic adjustments are given, holding adjustments will require expert guidance on posture in all common positions, such as sitting (proper chair selection and use), standing (flexible orthotics and shoes) and sleeping (positioning and pillow use).
Final Thoughts
Each of these mechanisms involves the dynamic relationship between the spine and extremities through the pelvis. You can likely trace the pattern leading throughout the lower extremity. In these cases, we need to be resourceful and get as much support and function from the entire kinetic chain as possible. We can teach movement efficiency and build on posture-based behavior to help people manage these common abnormalities, even complex cases like pelvic obliquity.

Bruce was born in La Crosse, WI to Howard and Carmen Barge on October 11, 1949. He married Pamela M. Barge on August 23, 1997 in Onalaska, WI. Together with his wife, they own Barge Supply and Nutrition. Bruce was an avid outdoor sportsman. He loved getting out to fish, hunt deer, pheasants, and ducks. Each year he would bring home a buck on opening day and get enough pheasants for the family Christmas dinner. He always joked that he would make his small Silky dog into the world’s best duck hunter. As a golfer, he was striving for his third hole in one (because 2 wasn’t enough) and telling jokes to whoever was near.

Dennis was born on April 7, 1956 in Burlington, WI to Robert C. and Mary Ann (Schwabenland) Bulgrin. He married Susan G. Golden on December 2, 1978 in Manchester, Missouri. Professionally, Dennis was a Doctor of Chiropractic. He was a member of the Wisconsin Chiropractic Association and served the Nekoosa, WI area for many years. He took great pride in the relationships he was able to build with his patients, and the care he gave to so many people over the years. In his spare time, Dennis was a family man. He enjoyed doing many things with his family including, playing video games, watching travel shows, and gracing them with a movie quote or a joke whenever the opportunity arose.

Paul Charles Maurer, age 65, current resident of the Town of Norway and formerly of Oak Creek, passed away at his home on January 2, 2025 due to Glioblastoma. Paul was a resident of Oak Creek, Wisconsin, for 25 years before moving to the Town of Norway, where he spent 7 years. A chiropractor for the Aurora Healthcare Edgerton Clinic, Paul retired in 2024. He was formerly a member of St. Matthew Catholic Church and currently of St. Thomas Aquinas Church. Paul was a loving husband to his wife, Sue, for 36 years, a father to three wonderful sons, and an incredible friend. A published author, he additionally enjoyed running, long distance cycling, and working on the family’s hobby farm.
