Are We Meeting Your Needs? As an organization committed to providing the best possible care to all our patients, it is important for us to know what your hospital experience was like. A “Patient Questionnaire” may be mailed to you at home after your discharge from hospital. Your comments are very important to us as it will help us to monitor the quality of our services and provide ideas on how to improve our patient care. If you have any concerns about your care we encourage you to discuss these directly with the Manager or Director of the service concerned or contact the Patient Relations office at (416 or 905) 494-2120 extension 32474.
