General Information Health Card Your Ontario Health Card is a very important piece of your personal information that should be carried with you at all times. You will be asked to present your Health Card upon registration. If you do not present your valid Health Card number to admitting within thirty days of your visit, you will be billed for services. If you have additional private insurance please provide this information upon admission.

Billing for Services Not Covered by OHIP For your convenience, we offer credit card payment as a convenient way for patients to pay for uninsured services or items that are incurred in the Emergency Department. This option offers patients the benefit of settling their account immediately with a copy of the transaction to take home for their records. Payment for all other outpatient procedures is collected upfront before the procedure. Payment can be made by cash, cheque, Visa, or MasterCard in the Business Office, Monday to Friday, 8:00 a.m. to 4:00 p.m. For your convenience, a banking machine is located at the front entrance of the Hospital.

What are uninsured services or items? Here are examples of services not covered by OHIP. • Ambulance transportation other than transfers between hospitals • PSA – prostate blood screening test • Fiberglass casts • Cast sandals • Cervical collars • Shoulder immobilizers • Clavicle splints • TED stockings • Tensor bandages • Crutches • Wart / skin tag removal • Circumcision • Preferred accommodation www.sjghel.ca
