Welcome to St. Joseph’s General Hospital Elliot Lake St. Joseph’s General Hospital has provided care for the residents of Elliot Lake and the North Shore since 1958. Over the years, we have evolved to keep up with the advances in health care and continue to meet the needs of our changing community. Elliot Lake was once a bustling mining town and is now a flourishing retirement centre. We are proud to offer you and your family convenient access to services not available anywhere else along the North Shore.

Information regarding St. Joseph’s Manor, our 64 bed Long Term Care Facility, and the Oaks Substance Abuse Treatment Centre, both managed under the Corporation of St. Joseph’s General Hospital, are also included.

St. Joseph’s General Hospital Elliot Lake Corporation The Corporation of St. Joseph’s General Hospital Elliot Lake consists of three unique and separate business units: 1) St. Joseph’s General Hospital Elliot Lake is a 54 bed full service acute care facility including a 24-hour Emergency Department, an Intensive Care Unit, Operating Room, Obstetrical, Outpatient Services and Hospice Suite.

2) The Oaks Centre is a residential substance abuse treatment centre comprised of two services: Withdrawal Management Services (Detox) and Residential Treatment Services (Camillus Centre)

3) St. Joseph’s Manor is a 64 bed long term care facility.

General Information Health Card Your Ontario Health Card is a very important piece of your personal information that should be carried with you at all times. You will be asked to present your Health Card upon registration. If you do not present your valid Health Card number to admitting within thirty days of your visit, you will be billed for services. If you have additional private insurance please provide this information upon admission.

Billing for Services Not Covered by OHIP For your convenience, we offer credit card payment as a convenient way for patients to pay for uninsured services or items that are incurred in the Emergency Department. This option offers patients the benefit of settling their account immediately with a copy of the transaction to take home for their records.

Payment for all other outpatient procedures is collected upfront before the procedure. Payment can be made by cash, cheque, Visa, or MasterCard in the Business Office, Monday to Friday, 8:00 a.m. to 4:00 p.m. For your convenience, a banking machine is located at the front entrance of the Hospital.

What are uninsured services or items?

Visiting Policy The Hospital’s visiting policy ensures the well being of our patients, minimizes the risk of infectious diseases coming into the facility from the community, particularly the flu, coughs and colds, and assists in maintaining a confidential environment.

Only 2 persons may visit any one patient at any given time EXCEPT palliative and critical care patients.

No one with new symptoms of respiratory infection including cough, shortness of breath or fever shall visit patients in the Hospital.

Anyone doing so will be asked to leave.

No child under 14 years may visit unless they are visiting their immediate family (parents, grandparents, and siblings) and are closely supervised by an adult.

Visitors shall use waterless hand antiseptic upon entering and leaving the building. Dispensers are at Hospital entrances and at the elevators.

Visitors are asked to be as quiet as possible in rooms and corridors.

Visitors of patients who are on isolation are required to follow Isolation Procedures. Ask the nurse for instructions before entering the patient’s room.

Visiting Hours General Inpatients Two visitors at a time between 10:00 a.m. to 9:00 p.m.

Intensive Care Visiting is restricted to immediate family, one person at a time, no more than five minutes in any hour.

Palliative Care Visiting hours are unrestricted, but no more than two visitors at a time.

Emergency Department For confidentiality reasons, visitors are not allowed in the Emergency Department unless required for the safety of the patient such as paediatric or confused patients, in which case only one visitor will be allowed with the patient. All other visitors are to remain in the waiting room. www.sjghel.ca

Outpatients / Day Procedures One person may accompany a patient who is attending an Outpatient Clinic or Day Surgery.

Public Washrooms Public washrooms are located on: • The first floor close to the Gift Shop • On the ground floor between the elevators and the classroom

Compliments / Concerns The employees, physicians and volunteers take great pride in the care delivered to our patients and their families. We encourage you to share your experiences and feedback with us. Through this feedback we can best address any concerns you may have in a timely manner and also work on improving our processes. The unit managers are available and interested in hearing from patients and families. Charitable Donations Donations are used to purchase replacement equipment (capital equipment) as well as new types of equipment not previously available at the Hospital. The Ministry of Health does not fund capital equipment. Income tax receipts are provided for all donations. Donations can be made in the Business Office Monday to Friday, 8:00 a.m. to 4:00 p.m. and at Switchboard after 4:00 p.m. and on weekends / holidays.

The Hospital accepts donations from many sources: • People donate in honour of the care received by a patient during their stay at St. Joseph’s General Hospital.

• Many families choose to honour the memory of loved ones by selecting the St Joseph’s General Hospital or St. Joseph’s Foundation Elliot Lake as the recipient of memorial donations.

• Some donors like to give annually in order to support St Joseph’s General Hospital or St. Joseph’s Foundation Elliot Lake.

• Service groups and local clubs donate the proceeds from golf tournaments, dances, and many other special events.

St. Joseph’s Foundation Elliot Lake is a registered charity with a mandate to raise funds to support the charitable activities of the Sisters of St. Joseph of Sault St. Marie in Elliot Lake and surrounding area. The charitable works of the Sisters of St. Joseph of Sault St. Marie include the Health Care Mission of St. Joseph’s General Hospital, St. Joseph’s Manor and The Oaks Centre Elliot Lake.

Patient Safety St. Joseph’s General Hospital Elliot Lake wants you to be involved in your care. We ask that you read this booklet thoroughly and ask your health care provider questions pertaining to your care. Be involved, your health depends upon it.

Health-care interventions such as the prescribing and administering of medications, surgical procedures, laboratory tests require interactions between health care providers and technology to be completed safely and effectively. Our staff are dedicated to providing safe patient care.

Adverse events can and do occur in hospitals, which may result in disability or even death.

Management and health care professionals at St. Joseph’s General Hospital are committed to providing the best possible care to all our patients. We believe in the dignity of each person, social responsibility

and a reverence for life. We are committed to the healing ministry of Jesus and hence we believe patient safety is a top priority. We promote a culture of patient safety.

As a patient you and your family must be involved in your care This means taking part and being involved in every decision about your care. This also means asking members of your health care team questions, so that you can make informed choices. You and your family should understand as much as you can about any medical problem you have (your diagnosis), treatment or procedure that you will undergo or medicine you should take. Before you see a member of our health care team, plan the questions you want to ask and write them down. We recognize that when you ask questions, you may not understand the answers you get the first time. Please ask again.

Tell our staff about your past illnesses as well as your current condition You know the most about your health. Please tell our staff everything you can, even if you think they already know and even if you think it is not important. Please tell us if: • you are taking any medications • you have had a recent visit to the hospital or to see your family doctor • you have seen another doctor or gone somewhere else for health care • you have an ongoing illness like diabetes, high blood pressure or heart disease • there is an illness that runs in your family, such as high blood pressure, asthma, or cancer • you have an addiction such as smoking, drugs, or alcohol Please remember that this information is held in the strictest confidence.

Bring all your medications with you to the hospital This includes any vitamins, herbal remedies or over the counter medications. Some medications combine with each other in your body and produce bad reactions. To protect you, our staff must know about everything you take. This allows us to have the best www.sjghel.ca

possible information when decisions are made about your care. It also enables us to identify medications that may be causing your current illness. Know why you are taking the medications prescribed to you. If you are admitted to the hospital, send your medications home with a family member.

Tell our staff if you are allergic to any medications or foods If you get sick, members of our health care team may have to act quickly. Before they give you any medication, they need to know if you could have a bad reaction to it. That’s why you should tell them in advance about any allergy or reaction you have ever had to any medication or food. A “reaction” may include rashes, headaches, trouble breathing, and feeling unwell.

Make sure you know what to do when you get home from the hospital When you are getting ready to go home from the hospital or after a medical treatment ask as many questions as you can to make sure you understand: • whether you will have to pick up a prescription before you go home • the type of care you will need at home. Do you require homecare?

• what symptoms to watch for and who to call if you feel unwell • what medications you must take, how to take it, and any side effects • what foods you may eat and not eat • when you can go back to normal activities, such as work, school, exercise, and driving • what follow-up appointments you will need and who will make them

Hand Hygiene Hand washing, when done correctly, is the single most effective way to prevent the spread of communicable diseases. Ensure that your healthcare provider washes their hands before they make contact with you. For the protection of yourself and others, please utilize the alcohol based hand sanitizer located throughout the building.

STATEMENT OF PATIENT’S RIGHTS AND RESPONSIBILITIES St. Joseph’s General Hospital Elliot Lake, is committed to providing quality health care for all residents of Elliot Lake and the North Shore.

PATIENT’S RIGHTS Every patient has the right to be treated with courtesy and respect.

Every patient has the right to privacy and confidentiality.

Every patient has the right to be informed of his/her condition, diagnosis, treatment and prognosis in a manner which is understandable to them.

Every patient has the right to give or refuse consent to treatment, including medication, in accordance with the law and to be informed of the consequences of giving or refusing consent.

Every patient has a right to expect that members of the Health Care Team will communicate with one another in order to ensure continuity of care.

Every patient has a right to know who is providing his/her care and who is the doctor in charge of his/her treatment.

Every patient has the right to ask questions and to receive a response.

PATIENT’S RESPONSIBILITIES Patient rights carry with them certain responsibilities. We ask each patient and/or representative to respect the following obligations: Each patient and/or representative agrees to provide the Health Care Team with all information relevant to the diagnosis and treatment of the patient.

To facilitate the timely provision of care consistent with a patient’s wishes, each patient should provide the Hospital with the name of a representative that can be contacted in the event that he/she is unable to express his/her wishes.

Every individual is strongly encouraged to prepare a Power of Attorney for Personal Care. www.sjghel.ca

Each patient agrees to follow the treatment plan that was developed on his/her behalf.

Each patient accepts personal responsibility for the consequences if he/she decides not to follow the treatment plan.

Each patient agrees to be courteous and respectful of other patients, visitors, and members of the Hospital’s Care Team. This includes recognition that the needs of other patients and/or their families may sometimes be more urgent.

Each patient agrees to respect hospital property, policies and procedures.

Each patient and/or guardian agrees to reimburse the Hospital for all charges not covered by the Provincial Health Care Plan or private insurance.

• Your Ontario Health Card and all relevant private insurance information which covers semi-private or private accommodation.

• A list of medications and dosages that you are presently taking.

Include those you buy without a prescription and all herbal medicines.

• Housecoat and non-slip footwear • Toothbrush, toothpaste, comb, hair brush, body lotion, shaving supplies, and any other toiletry item you use daily • Facial tissues, sanitary items • Pen or pencil • Your cane, walker, wheelchair or other assistive device that you use.

• If you are admitted and your vehicle is in the parking lot, please arrange to have someone pick up your vehicle and return it to your residence.

• Valuables such as jewellery, cash greater than $20.00, or credit cards. Leave these at home, or with a family member or friend.

• Electrical devices such as TVs or computer.

Discharge Planning for discharge will begin at the time of your admission.

Your health team, in collaboration with you and your family, will determine when you are ready to go home. You will be given specific instructions to follow once you leave the Hospital. You may also be given a prescription for medication. For best care, have all of your prescriptions filled by the same pharmacy.

Discharge time is 11:00 a.m. If you are being discharged home, please make arrangements for a family member or friend to pick you up by discharge time so that other patients requiring admission can be accommodated. If your pick up is delayed, you may be asked to pack your belongings and wait in a chair so that housekeeping staff can prepare the bed and room for the next patient.

For Your Safety General Safety If you wish to leave your patient care unit, please obtain permission from the staff at the nurses’ station before leaving and inform the nurses upon your return.

Fire Safety You may hear one of our regular fire drills, “Code Red” during your hospital stay. If you are on your patient care unit when you hear the alarm, please return to your room and keep your door closed. If you are in another area of the Hospital, please stay where you are until the “all clear” signal is given. Please follow the instruction of our staff. They will provide you with direction if further action on your part is required.

Security Hospital staff, and volunteers are easily identified by their name badge. If you observe suspicious activity or individual(s), have lost www.sjghel.ca

personal items, or have any security concerns, please notify a staff member immediately.

St Joseph’s General Hospital Elliot Lake (SJGHEL) has the responsibility to protect its property as well as the health and safety of its patients, residents, staff, physicians, volunteers and visitors. As such, St Joseph’s General Hospital is monitored by a video surveillance system to help accomplish this goal.

Smoking Smoking is not permitted in the Hospital or on any Hospital Property. Please note: Provincial Offences Officers have the authority to fine individuals if they do not abide by the no smoking regulations.

If you wish to stop smoking: • Smokers Helpline Referral Service is available upon request (1-877-513-5333 www.smokershelpline.ca) • Nicotine Replacement Therapy is available to inpatients upon request with a Doctor’s order.

Hospital Services Banking Machine An automated bank machine is located at the main entrance of the Hospital.

Cafeteria Our cafeteria is located on the ground floor and welcomes visitors and patients. It is operated by hospital staff and offers a wide selection of hot entrees, soups, sandwiches, and desserts at reasonable prices. The cafeteria is open from 7:00 a.m. to 1:30 p.m. Hot breakfast is served from 9:30 a.m. to 11:00 p.m., hot lunch from 11:30 p.m. to 1:30 p.m. Any visitor wishing a hot dinner tray may order one by calling the kitchen at ext. 2212.

Chapel The chapel is located on the main level of the Hospital, opposite the administration offices. Directional signs are near the elevator. The Chapel is open from 8:00 a.m. until midnight.

Confidentiality / Privacy St. Joseph’s General Hospital is committed to respecting privacy concerns of all patients and staff in accordance with the Personal Health Information Protection Act (PHIPA) which gives Ontarians control over the collection, use and disclosure of their personal health information.

Request for personal health information must be directed to the Medical Records department. Record retrieval and photocopying are subject to a fee.

For inquires regarding privacy issues pertaining to the Personal Health Information Protection Act (PHIPA) or the “lock box” option, contact our Privacy Officer by phoning 705-848-7182, extension 2441.

Electrical Equipment The Hospital prohibits patients from bringing electrical equipment such as fans and televisions into the hospital. In special circumstances, electrical equipment, which is CSA approved and has been inspected by our Maintenance Department, will be allowed. Please advise your nurse of any electrical equipment you have with you.

Environmental and Linen Services Your room and bathroom will be cleaned routinely by a member of our Housekeeping Department. Your bedding will not be changed every day during your Hospital stay unless it becomes soiled.

Gift Shop The Gift Shop is located near the main Hospital entrance. All proceeds from sales are donated back to the Hospital. Snacks, newspapers, magazines, parking tokens, and gift items are available for purchase. The Gift Shop is open daily.

Hours are posted on the door of the Gift Shop.

Interpreters To ensure clear communication between patients, family, and staff, the Switchboard maintains a list of personnel and volunteers who are able to provide interpreter services.

Lost and Found Lost and found items are kept in the Emergency waiting room, near the main entrance. The Hospital is not responsible for items that go missing. We recommend that patients not bring large amounts of cash, jewellery, or personal momentos to the Hospital.

Mail Incoming mail is delivered by Hospital Volunteers. Outgoing mail may be deposited in the Canada Post mail box located outside the main entrance. Postage can be purchased from Switchboard.

Parking The Hospital utilizes a Pay As You Go parking system. The gate opens as you approach it. The cost to leave the parking lot is $3.00, payable by Loonies, Toonies, or parking tokens. Tokens are available for purchase from the Gift Shop at $2.75 each. Upon approval, individuals parking at the Hospital more than once a day may purchase blocks of 20 tokens for $30.00 from the Business Office, Monday to Friday, 8:00 a.m. to 4:00 p.m.

The Business Office will only sell the tokens at the discounted price to individuals in possession of an approved request form. Speak to the nursing staff regarding this form.

Numerous handicapped parking spots are available just inside the parking gate. Additional handicapped metered parking is available at the front of the Hospital.

Security regularly patrols parking at the Hospital. Parking enforcement is maintained under City of Elliot Lake Bylaws 03-13 and 02-18. Instructions are printed on the back of the ticket for payment options or to dispute the ticket.

Spiritual / Pastoral Care Spiritual and religious care is available for patients and family members 24 hours a day, 7 days a week through our Hospital Chaplain, community clergy, and pastoral care volunteers. Any member of the health care team will be pleased to assist you in making the necessary arrangements.

Telephones A pay phone is located at the main entrance, and near the elevators on the main floor of the Hospital.

A telephone is located at each patient bedside with the exception of the Intensive Care Unit. Dial “9” for an outside call.

All long distance calls must be charged to your calling card or third party billing. Dial “0” and the switchboard operator will assist you with your long distance call.

Televisions Television rentals are provided by the Hospital. A staff member doing daily TV rounds will provide you with rental rates. This occurs in the late afternoon.

Vending Machines Vending machines with cold drinks and snacks are available in the cafeteria on the ground floor.

Volunteers / Community Services St. Joseph’s General Hospital is fortunate to have many dedicated and diverse volunteers both directly and indirectly affiliated with the Hospital. If you would like to get involved and give back to your community here are some suggestions: The Hospital Auxiliary raises funds for capital equipment. For information regarding their main activities, the President of the Auxiliary can be contacted through Administration at 705-848-7182, ext. 2238.

The Gift Shop is entirely run by volunteers. Speak to the volunteer on duty to contact the Gift Shop Coordinator, or dial 705-848-7182, ext. 2345.

Volunteers assist in the movement of patients and information throughout the facility. The Coordinator can be reached at 705-848-7182, ext. 2410.

Elliot Lake Palliative Care Program is a separate entity with its own board of directors. The mission of the Elliot Lake Palliative Care Program is to assist clients by providing a trained, caring, friendly volunteer to support them and their family during the palliative stage of a terminal illness. Following a careful selection procedure, volunteers attend a 30 hour training program in which they receive instructions in the Palliative Care Philosophy, skills in effective communication, coping with stress and dealing with the bereavement process. Volunteers are active in both the community and the Hospital and offer bereavement support services. Contact the Administrator of the Palliative Care Program at 705-848-7182, ext. 2235 for more information.

Pastoral Care Volunteers offer spiritual and religious care and are organized through their respective clergy. Volunteers visit patients from their parishes as well as assisting patients in attending services held in our hospital’s chapel. The Pastoral Care Manager can be reached at 705-848-7182 ext 2410.

St. Joseph’s Manor St. Joseph’s Manor opened in November 2002. It is a sixty-four (64) bed Long Term Care facility physically adjoining St. Joseph’s General Hospital. Individuals must be assessed by the Community Care Access Centre Coordinator in your area and qualify under the Ministry of Health and Long-Term Care guidelines to take advantage of the services provided. Accommodation rates are set by the provincial government and are available on the government website: www. health.gov.on.ca/en/public St. Joseph’s Manor provides a home-like environment for individuals whose daily care requirements are too extensive for to be met in the community by family and professional caregivers. Your physician may continue to provide your medical care if desired.

The Recreation Department provides high quality leisure experiences that meet the social, emotional, physical, cognitive and spiritual needs of all residents. Volunteers and local entertainment groups are an integral part of the Recreation Department.

The Oaks Drug and Rehabilitation Centre The Oaks Centre is an accredited treatment facility sponsored

by St. Joseph’s General Hospital. It is comprised of two services: a withdrawal management program (Detox) and a residential treatment program (Camillus Centre). The Oaks Centre is a co-ed facility and it is handicap accessible. Both the Detox and Camillus receives professional support from a Certified Medical Addiction Specialist.

Other ancillary service providers housed at the Oaks Centre are the Algoma Community Care Access Centre, the Counselling Centre of East Algoma and The Elliot Lake Methadone Clinic.

Withdrawal Management Services (Detox) The primary mission of withdrawal management is to offer nonmedical crisis intervention, detoxification, and stabilization to Ontario residents 16 years of age and older.

Withdrawal management accepts referrals from numerous sources including medical professionals, the legal system, families and friends, and self. Potential residents must be medically stable, nonviolent, lucid, and not suffering from an active psychiatric disorder.

The Opiate Tapering program offered by the Medical Addiction Specialist has received much recognition from Employee Assistance Programs and others across the province.

Amenities include handicap accessible rooms and a self-contained unit for individuals who wish anonymity and/or privacy from residents or visitors.

Chemical Dependency Program - Adult and Young Adult The applicant’s admission information will be assessed by a team to ensure the participant is physically, emotionally, and psychologically able to participate actively in all phases of the program. The applicant shall provide, prior to admission, a doctor’s referral form if required, a form indicating psychological stability if required and Standardized Assessment Tools.

Each client, with his/her counsellor identifies problems to be addressed during / following treatment and develops a treatment plan, with target dates to resolve these problems as well as an aftercare plan. Each client is required to participate actively in his/her program of recovery.

Family / Co-dependency Program This program is designed to assist men and women suffering from the effects of another person’s chemical dependency, a spouse or other family member who is a recovering or still practicing alcoholic.

The family program addresses the physical, emotional, social and spiritual aspects of co-dependency. It features lectures, videos, group therapy and limited one-to-one counselling sessions.

The applicant’s admission information will be assessed by a team to ensure the participant is physically, emotionally, and psychologically able to participate actively in all phases of the program. The applicant shall provide, prior to admission, a doctor’s referral form if required and a form indicating psychological stability if required.
