he kids are out of school. Family vacations.

Barbecues. Gardening and yard work. Hiking in the mountains. Playing in the water. It’s summertime!

proportion of “CE Junkies” in our organization. It isn’t until the days get darker, colder, shorter, and wetter that my inner nerd starts looking for CE. But it’s worth it to take a little time right now to look at the CE opportunities coming up this Fall. Classes fill up, and early bird tuition discount deadlines go by when you don’t pay attention. As the saying goes, when you snooze, you lose!

When the sun comes out here in Washington, I just want to go outside and play. Sometimes I yearn for the days when a dentist could just hang a sign on his front door that says “Gone fishing.” (Although it’s more likely that I’d be hiking in the Cascades.)

In addition to planning out your CE, don’t forget to RSVP for the Gala on October 4th. And plan on coming to the Oral Cancer Walk on October 12th in Kirkland… even if it is a little colder and wetter than in the summer. ■

Structuring A Strong Team When Building A Practice

Building a practice is a pivotal point in every doctor’s career.

Even the most prepared individuals should have a strong team around them while planning to bring their vision into reality. The components of that team; the General Contractor specializing in dentistry, the real estate Broker, the Designer, the Banker, the Equipment Representative, and the Technology specialist, all play important roles that complement each other and are crucial to the success of a project. Relying on the expertise of others in the earliest stages allows a doctor more time to focus on building a new business, while also helping that business flourish in the best foundation possible.

Within the General Contractors scope is the largest fiscal portion of building a practice, which is why having a contractor that specializes in dental projects is so important.

Producing a budget that is all inclusive requires knowledge of the very unique infrastructure demands of a dental practice.

These details, such as water solenoid valves and purity inspections of medical gas lines, can affect the budget, but more practical problems can also arise in the layout of chairs, operatory lights and televisions, creating very low tolerances and requiring precise, experienced builders. Choosing a general contractor that understands what is required at the offset can also help foster trust, minimizing changes and challenges during the project.

Almost as important as trust within a contractorclient relationship is communication. A contractor that is willing to challenge outside forces in order to support their client throughout a project, and address issues that arise in a timely manner is essential to keeping a project on track. Coordination between the entities, within the team and outside of it, such as with the city and material suppliers, should be a regular activity of the contractor. As well as updating the team as to where the project is and where it is going.

Construction is challenging, so having a team captain that is willing to take charge and lead a project in a positive direction is key to that project’s success.

Knowing if an empty space has the infrastructure necessary for a dental practice can be difficult to ascertain, which is why having a knowledgeable Broker on your team from the onset is imperative. Figuring out what is critical to a client or practice as it pertains to the geographical, marketing, and other aspects, can screen out spaces, saving a significant amount of time. An experienced broker should also understand the need to negotiate with the landlord in order to get additional tenant improvement allowances and free rent periods.

Extracting design details from the mind of a client is not the only role of a Designer in the early stages of a practice. A designer with dental knowledge will be able to deliver plans that are efficient in their design as well as follow legal requirements such as code compliance and patient privacy laws. Setting up the flow of an office during the design phase can also support physical operations once the practice is running.

Understanding the fiscal parameters of where a project initiates is the basic role of the Banker in a dental project. A banker who also plays an active role not just at loan signing, but also during construction will help to protect the investment and keep the momentum of the project flowing forward. Finding the right type of loan program that is most suited to a Doctor now and into the practice’s growth projections, will benefit the practice well after construction is over.

Deciding on equipment may not be the first decision a doctor needs to make when building a practice but having an Equipment Representative’s support in the planning stages to identify the style of dentistry and the needs of the practice will help produce a budget that is accurate and still provides the necessities for the practice. Identifying things like left handedness and additional procedures performed can allow designers to more accurately plan the flow and allocate adequate space for the doctor’s specific style of dentistry.

Many doctor’s do not choose their Technology Specialist in the beginning of a project, but their pre-planning can help define the system operations of the business. Systems vary greatly in their depth as well as cost, and a specialist can create a perfect custom set-up. They can also work with the equipment rep and general contractor in order to plan for the in-wall infotech infrastructure necessary for the practice. ■

Harmonize & OptiBond Universal Harmonize & OptiBond Universal Clinical Case Studies - Anterior Restoration Dr. Scott Coleman

The use of tooth-colored composite materials in today’s dental practice is a given. The improvement in our ability to confidently bond to both enamel and dentin has certainly been a paradigm shift for our profession. The improvements in composite materials to meet the different requirements needed for posterior restorations and anterior restorations have been a challenge.

Case 1 Case 1 is a class IV in tooth #8 to restore the incisal, distal corner. I used a selective etch technique to prepare the tooth, and then followed with an application of OptiBond™ Universal adhesive and cured with the Demi™Ultra curing light. A layer of Harmonize dentin shade A-3 was applied lingually and cured; I then followed with Harmonize enamel shade A-2 with additional curing. The restoration was contoured using 12 fluted carbide burs and ProGloss™ disc and cup polishers. Polishing paste was not needed. The translucency and luster blended beautifully.

Total time: 20 minutes

The biggest issue is creating a composite that can handle posterior wear and meet the demands of anterior esthetics in the same material. I have found that Harmonize

by Kerr works remarkably well in both uses.

Conservatively restoring anterior teeth to a high level of esthetic with composite material is commonplace in my practice. In my experience, the non-sticky malleability of Harmonize™ Universal Composite is a pleasure to work with and I am always satisfied with the end results. When restoring just specific areas of a tooth, matching the existing structure is a big part of creating a restoration that has a true “chameleon” effect and looks natural.

Harmonize & OptiBond Universal Restorative Case Study

Case 2 Case 2 is tooth #11 with a failing composite restoration and recurrent decay. The composite and decay were removed without anesthesia using a CO2 Solea® Dental Laser. OptiBond Universal was applied and cured with the Demi Ultra curing light. Harmonize dentin A-3.5 was applied and built up from the lingual, followed by enamel A-3 shade. Trying to blend a transition color from an existing porcelain crown on the bicuspid toward the natural color of the lateral is challenging. The surface was finished with fluted carbide burs and ProGloss polishers. No additional polishing paste was used. The final luster and surface texture created using Harmonize resulted in a very natural finish. Total time: 40 minutes

Case 3 Case 3 illustrates multiple teeth restored with Harmonize. Diastema closure with restoration of class IV defects was carried out using a total etch technique followed by OptiBond Universal, which I routinely use for its minimal film thickness as well as its clear appearance. The base restorative material was Harmonize dentin shade A-1 followed by enamel B-1 and a final layer of Harmonize Translucent Blue. We finished shaping the material using twelve fluted carbide burs; and then we used disc and cone-shaped ProGloss polishers to create the desired luster. We corrected the gingival height discrepancy using a diode laser. Total time: 45 minutes

Case 4 Case 4 demonstrates the use of Harmonize to restore multiple anterior teeth. Utilizing Harmonize dentin A-2, enamel A-1, and translucent shades with OptiBond Universal as the adhesive, teeth #7, #8, #9, and #10 were restored and finished with fluted carbide burs and ProGloss polishers. The patient’s smile was restored in a single one hour appointment.

PIEZOTOME CUBE –

ACTEON North America has launched the Piezotome CUBE (figure 1), a brand new ultrasonic bone surgery device that will change the way dentists think about atraumatic extractions. One of the most common procedures dentists perform are tooth extractions. Although they are a common procedure, they can present some unique challenges for general dentists and specialists alike.

The main goal of any tooth extraction is preservation of the socket, bone, and surrounding structures.

Traditional methods of extraction such as forceps and rotary burs alone can lead to complications such as fracture of the buccal bone, loss of surrounding bone, and soft tissue injuries. Also, patients can experience significant swelling, pain and discomfort post operatively.

Piezotome CUBE can help dentists make their extractions more atraumatic and predictable.

The high frequency, ultrasonic vibrations of the CUBE only disrupt the PDL and bone and will not damage soft tissue so it is very effective even with ankylosed teeth. This eliminates having to cut a surgical flap in the gingiva preserving the periosteum.

The extremely thin extraction tips (Figure 3), preserve more bone, even the thinnest of bone in the buccal area to facilitate immediate implant placement. Lastly, your patients will thank you due to reduced pain, swelling and trauma post operatively.

Extractions with immediate implant placement are rapidly becoming more and more popular for the following reasons.

Recent studies have shown patients lost 50% of crestal width in a 12-month period (6.1 mm average) after extractions1 Two thirds of which (3.8 mm; 30%) occurred in the first 3 months1-2 (figure 2).

Also, if patients can get the implant placed in the same visit as the extraction, it saves them another visit.

Preserving the socket is paramount for performing immediate implant placement after extractions.

Glazed surfaces are not smooth at all even though they may look that way.

See the two pictures below - using a 3D Laser Microscope you can see the rough surface of a glazed surface on an e.max® abutment as opposed to the polished surface. Even with the polished surface picture at 10x magnification, the surface variations are minimal. Glazing techniques differ which can leave the restorative surfaces quite rough; this is one way the opposing dentition can be worn leading to potential failure.

Studies are available that show glaze wears away over time exposing the rough surface of the restoration and

this is most important in the occlusal surface. Once the rough surface is exposed, further wear of the opposing dentition occurs. This is not new information - polished restorations last much longer and prominent third party organizations have shown this over the years with zirconia being a big part of these findings.

Many labs and docs still prefer to glaze and this will not change. That’s not the issue - glaze can be great at filling porosities in the restoration and for aesthetics. Sometimes it’s a little too glazed and shiny so polishing can help to aesthetically bring that mirror-like finish back a bit - it just depends on the case. Aesthetics aside, the occlusal surface must still be polished smoothing out that rough surface so when the glaze has gone, the restoration’s occlusal anatomy is still very smooth.

from aluminum oxide polishing paste to diamond polishing paste, impregnated rubber wheels to impregnated flexible spirals. So what is the right product to use? That all depends on what restorative material you are using.

Restorative materials are different in their composition and so naturally they need to be finished differently. For instance, Zirconia and Composite are two completely different materials with very different properties and therefore need to be treated differently when finishing.

These choices also change on a case by case basis. Perhaps a mirror-like finish is not ideal in certain cases as it doesn’t match the adjacent teeth.

Why did you join the Washington AGD MasterTrack program?

Later, when I was serving in the USAF one of my mentors in the 99th Dental Squadron at Nellis AFB, Dr. Mike Sherrill, told me more about the AGD. With a better understanding of what it was about, I immediately joined. I loved the emphasis on continuing education and the camaraderie that comes with membership.

I saw MasterTrack program listings in the WAGD newsletter, but was a bit reluctant to sign up due to the distance from Walla Walla. My Walla Walla colleague, Dr. Steve Karmy, signed up for MasterTrack and would return from his sessions to tell me how fantastic it was and that I would really enjoy it. With his raving reviews, I signed up.

When I finished my military service and returned home to Walla Walla I updated and transferred my membership to the Washington AGD.

It was everything he said it was… and more! The handson participation courses are my favorite aspect of MasterTrack, and the attendees quickly develop friendships that make the sessions even more enjoyable. A hidden gem within MasterTrack is the Sunday case presentation time. This is an incredible mode of education where we get to learn from each other, presenting cases and discussing them, earning CE along the way. The professional yet relaxed, friendly atmosphere at MasterTrack keeps me coming back year after year and I believe it is the best value for CE that I have discovered when I consider all the outstanding hands-on courses I get to participate in while learning in a room filled with friends.

As a dentist in Eastern Washington, do you find it harder to attend quality CDE courses?

It is no secret that with a smaller population there are generally fewer CE courses available in a given area, and more rural Eastern Washington is no exception when compared to more densely populated Western Washington. In the past it may have been more challenging to find the types of dental courses I wanted to attend, but this is no longer the case. It takes less than an hour to fly from Walla Walla to Sea-Tac and about 5 minutes to get from the airport to the Educational Center. There

anyone who hasn’t already done so to register for a course there to see what an outstanding place it is to learn in. Its close proximity to Sea-Tac International Airport is a fantastic convenience for people who don’t live in Western Washington.

So the Education Center’s location near the SeaTac airport been a determining factor for why you have attended Washington AGD courses?

Absolutely! It is a brilliant location. Each time I attend courses at the Educational Center I am reminded of this. Even more impressive to me is the number of people from out of state who have discovered the same thing. Some of our most active MasterTrack attendees are flying in from far away places because our proximity to the airport makes it so easy for them to essentially “commute by plane” to attend courses. The location of the Educational Center has really opened up the possibility of more active involvement with the Washington AGD for those of us who live far away from the greater Seattle area.

Dr. Edwards serves on the Washington AGD Board and is a participant in the MasterTrack program. He was awarded his MAGD in 2018. ■

Following Tooth Extraction, the Maintenance of the Original Bone Volume is Paramount.

But what parameters need to be considered? An experts opinion.

DANIELE CARDAROPOLI, SCIENTIFIC DIRECTOR PROED, INSTITUTE FOR PROFESSIONAL EDUCATION IN DENTISTRY, TORINO, ITALY AN ABSTRACT: The healing process of post-extraction sockets is related to a series of biological events, including the formation of a coagulum replaced over time by a provisional connective tissue matrix, woven bone, lamellar bone and bone marrow.1 The development of the alveolar process is directly related to the presence of the natural dentition, and it follows different degrees of atrophy after tooth extraction. Mainly, the limited supply of cells coming from the periodontal ligament causes resorption of bundle bone. Also, when the cortical bone plate is thinner, the alveolar bone is resorbed crestally and more on the buccal than lingual side.2 Clinically this histological remodeling results in horizontal and vertical ridge resorption.

In the first six-months following tooth extraction, the alveolar ridge can develop a mean horizontal width resorption of 3.8mm and a mean vertical height resorption of 1.24mm. 3Moreover, one year after tooth extraction the ridge can lose up to 50% of its original horizontal width.4 Starting with biological, anatomical and clinical principles that rule the dynamics of post-extraction site healing, it is essential to attempt to maintain the original volume of the ridge. It is a common experience that delayed post-extraction sites present inadequate bone volume for proper implant placement. Therefore, if an implant placement is planned, additional ridge augmentation procedures are essential for counteracting ridge loss. Slowly

resorbing biomaterials seem to be most effective in maintaining the initial three-dimensional volume of the ridge.8 After extraction, the alveolus should be debrided and thoroughly rinsed with saline to decontaminate the site, before filling with a biomaterial. The ideal biomaterial should be biocompatible, osteoconductive and provide slow resorption to compensate for inevitable remodeling of the bundle bone. Deproteinized bovine bone mineral possesses these characteristics: It preserves on average 93 % of the initial crest volume at four-months followup, the alveolus volume is virtually preserved, and histologically it consists of approx. 26 % new bone and 18 % residual bovine bone granules.5 Typically, after the placement of the biomaterial, the extraction socket is sealed using a resorbable collagen matrix such as Geistlich Mucograft® Seal. This bilayer, porcine, collagen matrix protects the underlying bone graft and promotes secondary healing of soft-tissues without the risk of infection. Full closure of the soft-tissue usually occurs between the third and fourth week following extraction. Four to sixmonths after Ridge Preservation an osseointegrated implant can easily be inserted into a preserved bone ridge, with appropriate crest volume and ideally healed soft-tissues.10 A bone crest preservation technique is applied to maintain the pre-existing hard and soft-tissue anatomy, provide a stable crest volume and optimize functional and aesthetic results that simplify clinical procedures… For the complete article, please email: news@geistlich-na.com. ■

UWSOD AGD “StudentTrack” Kicks Off With Inaugural

CROWN PREPARATION COURSE

W

ashington State is home to one of the most active AGD chapters, the Washington Academy of General Dentistry (WAGD). Through the WAGD’s guidance and mentorship, the University of Washington AGD Student Chapter (UWSOD AGD SC) has created some innovative opportunities for students to experience all the AGD has to offer.

Under the leadership of President David Chi and former President Evan Mitchell, the UWSOD AGD SC has created 3 CE opportunities

and 1 social event per quarter. The University of Washington School of Dentistry Graduate Prosthodontics program and its Director, Dr. Van Ramos, provide a prosthodontic lecture by one of the UWSOD Graduate Prosthodontics Residents.

This is a “win-win” for the student chapter and the Pros program.

Attendance continues to grow and now the UWSOD AGD CE opportunities are being extended to dentists in the community.

On June 1st, 2019 the UWSOD AGD StudentTrack kicked off

with “Crown Preparation 101: From Analog to Digital”. Through support from Komet USA, Bien Air, Kettenbach, Planmeca, Burkhart, Garfield Refining, Diashine and Pascal. Students were generously provided preparation burs and acrylic adjustment kits by Komet USA. Crown Preparation 101: From Analog to Digital is a guided handson course that attempts to create a logical workflow from seating the patient through buildup, preparation, impression/scan, provisional and finally releasing the patient.

Participants were given the opportunity to put the Planmeca Emerald scanners through their paces.

Exposing students and young dentists to a systematic workflow, created confidence and improved efficiency in the participants. Crown Preparation 101 targeted 2nd, 3rd & 4th Year students but also had 5 general dentists attend the course. In the future the UWSOD AGD SC plans to take this course “on-the-road” to other dental schools and meetings.

Future UWSOD AGD StudentTrack course offerings will include Dr. Jihyon Kim teaching composite restorations based on the Bioclear Technque. Dr. Dzuy Nguyen will expand students’ experiences with endodontic treatment.

More digital dentistry is coming and the UWSOD AGD SC will continue to have access to the WAGD Fellowship and MasterTrack course at the WAGD Educational Center. ■

CROWN PREPARATION 101: FROM ANALOG TO DIGITAL SYNOPSIS:

Today’s dental school curriculum is packed full of courses and requirements.

Often students and new graduates feel as if they would benefit from additional education opportunities.

Crown Preparation 101: From Analog to Digital is a hands-on course that attempts to create a logical work flow from seating the patient through buildup, preparation, impression/scan, provisional and finally releasing the patient. By developing a systematic approach, we create efficiency which will turn the fourvisit crown preparation & final seat, into a comfortable two visits!

Participants will perform at least one posterior and one anterior crown preparations. Other topics including dental materials and sonic instrumentation will be presented as time and the will of the group allows.

OBJECTIVES: 1) Introduce the student or “newer” dentist to a logical crown preparation workflow for both traditional impressions and digital scanning.

2) Teach evaluation of the “big picture”; what are the parameters we need to consider when planning a buildup and crown.

3) Stream line crown preparation times with guidelines and efficient rotary instrumentation including the use of electric handpieces.

4) Introduction of sonic instrumentation to create ideal draw and glass smooth margins.

5) Illustrate tissue management protocols from packing cord, retraction paste and laser.

6) Digital workflow to obtain ideal margins and clearance.

7) Provisionalization options and guidelines.

TARGET AUDIENCE:

This course is targeted towards 3rd and 4th year dental students and the “newer” graduate. This course is based on years of clinical experience and observation of students. Discussion will include patient management strategies.

Experienced clinicians may find this course “too basic” for them but very appropriate for their young associate.

APPLYING FOR PACE APPROVAL Approval for CE Credits from the Academy of General Dentistry (AGD).

The Academy of General Dentistry (AGD) Program Approval for Continuing Education (PACE) was created to assist members of the AGD and the dental profession in identifying and participating in quality continuing dental education (CDE). PACE approves program provider organizations, not speakers or specific courses. The program provider approval mechanism is an evaluation for the educational processes used in designing, planning, and implementing CE. Approval by the AGD does not imply endorsement of course content, products, or therapies presented, nor does this approval imply that a state or provincial board of dentistry will accept courses. Approved program providers are expected to comply with all relevant state and federal laws. CE offered by approved program providers will be accepted by the AGD for Fellowship/Mastership credit.

GET THE APPLICATION

Go to www.agd.org Click on the Education tab and then PACE Apply/Renew Tab to get the application. To qualify for PACE approval, all organizations should have offered a planned program of CDE activities for at least 12 months.

Note:Â If your organization has not offered a planned program of continuing dental activities during the 12 months immediately preceding the application date, you may apply for a maximum of one year approval.

FEE

AGD Members: If you are an AGD Member or Associate Member (Specialists are Associates) the FEE is $195 for 4 years. Please make your check out to Washington AGD. AGD Members do have access to the AGD membership database.

Non-AGD Members: If you are NOT an AGD Member or Associate Member the FEE is $595 for 4 years. Please

make your check out to Washington AGD. Non-AGD Members do not have access to the AGD membership database. If you would like to become an AGD member, please submit the enclosed membership application along with the membership dues. Please make your check out to the AGD.
