A Time to Be Thankful This time of the year usually finds an increase in Practice Management Articles relating to what you, as the leader of your business, need to implement in order to be successful in 2017.

What these authors occasionally overlook is the need for you and your staff to reflect upon the many positives accomplished by your office. As the primary Eye Health and Vision Care Provider you saved any number of patients from not only vision loss but in many cases from undiagnosed systemic diseases. Without the patient with early onset diabetes seeking his or her vision care provider their longterm systemic problems would have become devastating.

The list of those patients with systemic diseases you diagnosed and treated or referred is quite deep. When you stop and reflect upon the 2500 or more patients you examined during 2016 please take a few minutes and bring to mind the role you played in them living happy healthy lives. I know the focus the general public places upon Doctors of Optometry relates to Vision Correction – not even ‘Eye Health and Vision’.

However, we know (or should know) the true role the many years of training, the ongoing clinical continuing education, the National Board Examinations and now Board Certification Examinations our profession plays in the delivery of health care.

Therefore, rather than an article about setting a ‘Business Plan’ for 2017 – this year through this brief article I am requesting you take a few minutes and remind yourself and your staff of the very important role you play as a Primary Entry Point Health Care Providers within the US Healthcare System! As a Doctor of Optometry, you provide more than a majority of the Comprehensive Eye Health and Vision Care examinations within the United States. The public relies upon you and your education each time you enter your examination rooms and review with them their health and vision history!

Our profession provides each of you with an opportunity to provide every patient seeking your care with the very best in Health Care – not merely Eye Health and Vision Care!

I would challenge you to let your staff know how much you value and honor this privilege! My sense is they will follow your lead and begin to reflect upon how great they felt knowing your office saved any number of patients in 2016 from greater health problems simply because you and your team cared for them!

Yes, I understand you must focus upon your - Balance Sheet, Income Statement, Fixed Costs, Variable Costs, Chair Costs, Cost of Goods Sold etc… yet I believe it as important during this Holiday Season to recall the may positives accomplished by you and your team will other than the very important task of providing the best Vision Care possible to your patients.

Spending a few minutes discussing this aspect of your practice will add something that those figures on a spreadsheet will not – Positive Morale!!

Once you and your team know and discuss the many positives that providing Eye Health and Vision Care AND your role in diagnosing Systemic Health anomalies affords your patients your team will be reminded of just how thankful they should be to participate with you in your role as a Primary Health Care Provider!

I also believe, with the above discussion in mind, a review of the routine practice financial information will become a more worthwhile and accepted exercise by all on your team!

I hope each of you holding the degree of Doctor of Optometry – especially during this time of the year recognize how important you are to each patient you treat.

Additionally, how thankful you and your staff should be for the privilege of providing your training and expertise each day of the year to each patient seeking your care!

On a personal note, as I reflect upon what I am grateful for during this time of the year - one thing I know for certain is how thankful I am to have chosen the profession of Optometry and to hold the degree Doctor of Optometry!

Additionally, I never would have imagined how thankful I would be to have my daughter - Jennifer C. Mallinger, OD and my niece - Leah C. Mallinger, OD à both hold the same degree!

MERIT-BASED INCENTIVE PAYMENT SYSTEM 2017 will bring big changes for all of us in the form of MIPS: the Merit-Based Incentive Payment System. If the name is foreign to you right now, no worries as you still have plenty of time to prepare. The MIPS program will require doctors to report quality measures, EHR use, clinical practice improvement activities and advancing care information to receive the highest reimbursement from Medicare. This new program begins January 1st of 2017 you may choose to participate one of four choices.

The good news is you can wait until the last quarter of 2017 to start and still earn a potential 4% increase in your Medicare reimbursement for 2019. First let’s begin with which doctors are excluded from participating and will not receive the potential 4% penalty in 2019. Any doctor who does not bill Medicare charges of $30,000 or more in a calendar year, as well as all doctors who see 100 or fewer Part-B Medicare patients in a calendar year will not be required to meet the program criteria. You are also excluded if you participate in a qualifying advanced payment model like an accountable care organization (ACO) which is a healthcare organization characterized by a payment and care delivery model that seeks to tie provider reimbursements to quality metrics and reductions in the total cost of care for an assigned population of patients. A group of coordinated health care providers forms an ACO, which then provides care to a group of patients.

Fortunately for all of us who are not excluded, we are being offered four choices on how we choose to participate.

OPTION 1 is full participation for 90 or more days in 2017 with the opportunity to earn a 4% increase in Medicare reimbursement for 2019. That means reporting 6 quality measures(PQRS); 1or 2 clinical practice improvement activities that demonstrates a role in overall public health initiatives; and 5 objectives and recording a performance score for advancing care information(Meaningful Use) on 9 measures. An added bonus may be available for “exceptional performers” who report beyond the expected level.

OPTION 2 is partial participation for 90 days or more in 2017 resulting in reporting on one quality measure(PQRS), or more than one improvement activity, or more than the required measures in the advancing care information performance category(Meaningful Use). This option results in a small payment increase on Medicare reimbursement for 2017.

OPTION 3 is test participation that requires you to report one measure in the quality performance category; or one activity in the improvement activities performance category; or report the required measures of the advancing care information performance category. This option results in no penalty on Medicare payments in 2017 however you will not receive any bonus either.

OPTION 4 in no participation if you choose not to report in 2017 any measures or improvement activities resulting in a 4% decrease in Medicare reimbursement for 2019.

For those of you who are AOA members I suggest you begin the process of registering for the new registry being offered for free to AOA members called AOA More. This registry will help you report your quality measures and clinical practice improvement activities. Reporting quality measures with AOA More also earns doctors bonus points. There is no cost for AOA members to use AOA More and non-members can use the registry for an $1800 annual fee.

The turnover of employees is one of the most expensive components of running a small business. Whether you are replacing an existing staff member or adding to your staff, how you manage that selection process can be easy or very painful.

Determine up front exactly what your operation needs in the new hire by considering a few questions: • Are your immediate needs full or part time?

• Do you require prior experience as a technician, vision plan/medical biller or optician?

• Do you require specific language skills?

• Are you willing to hire for personality someone that will mess well with the existing team that has no experience in your arena? If so, that requires the patience to train from scratch that new person.

• Do you have a current job description that spells out exactly the expectations and responsibilities for that position?

Where in your neighborhood do you need to advertise for employees?

• Local newspaper or job board • Craigslist • Zip Recruiter or other similar online HR solutions • Approach local vocational schools or community college placement services • As you see patients each day and inquire about their occupation, would any of them make a good addition to your team?

• Local professional society website • Buying Group website Laws in your state or city are specific to exactly when benefits or unemployment are to be paid after hiring. Some states do not allow provisional employees. Some state require benefits be paid within weeks of a new hire. If it is legal, consider offering your new hire a six to twelve week internship with no benefits and if that works out, move them into a full time position.

Nothing is more frustrating and expensive than recruiting an applicant, training them for several weeks and having them be a bad fit and need to be replaced. Make the effort up front to identify the applicant that has the skill set and personality to fill that spot on your team.

ELIMINATE REPETITIVE TASKS IN 2017 Repetitive tasks such as addressing recall postcards and making reminder phone calls zap energy and creativity from your staff. Derek Hamilton, O.D., owner of Wells Branch Vision Care in Austin, TX, believes activities like these should be automated to create efficiency and staff growth.

How to Hire Your Unicorn Every business in eye care is looking for their competitive advantage – something to differentiate them from the rest.

Carrying unique frame lines is a great way to distinguish one optical from another, while investing in high-end technology can be effective to separate one clinic from another. But in all businesses (not just in eye care), the real difference can be created with the people that run and operate the business. To illustrate this point, let’s look outside of our industry. When top athletes leave their teams to join another franchise in professional sports, the entire dynamic of the team (and therefore product) changes. One team may get better while another becomes worse. This same idea can translate into every office in the world. Building a strong team will help improve your overall product offering to your patients.

When building your team, it’s important to understand the type of people that you want to employee. Your qualifications for the role should far extend experience or education, and focus also on the type of people you want to have work for you. Often, businesses hire out of necessity, not out of opportunity. When hires are made out of necessity, the interview process is typically short with only a few applicants.

You’re then just hiring the best that applied, rather than the best available. Finding the best available all starts with a well thought out recruitment strategy.

Recruitment is an art, not a science and should be carefully crafted with each position. Let’s take an Optician for example – the strategy to hire your Unicorn Optician (my definition of a unicorn is someone that you’ve only heard about, but not actually met – someone magical that can transform your business in a very positive way – someone typically very hard to find) should be very different to finding your Unicorn Receptionist or Office Manager.

Determining the qualifications of your Optician should start with a list being created of what the position actually requires (this is where the experience and education piece comes in).

The next step requires thought into adding to that list every quality that would make that Optician exceptional. Think back to the best Optician you ever worked with and list out their qualities (think of it as making a recipe). Lastly, finish your list with the qualifications that would make someone…

anyone… the perfect piece to your team puzzle. Every business has a different DNA, a strand of personality that makes them unique and make sure the qualifications of your DNA is listed in that job qualifications.

Once your list has been created, it’s time to write the most compelling job posting you’ve ever read. Gone are the times of bleak, boring, templated job descriptions. Those types of job descriptions attract people currently looking for a job and those people typically just circulate. To cultivate a winning team, you need winning players and to get winning players, you need to sell them on a winning environment and situation.

Take the time to really show off who your business is with your job description. Share your mission or vision statement, share your leadership philosophy, share the company culture, share the perks and fringe benefits that come along with the job.

Don’t be afraid to list your deal breakers in your job posting as well (the Unicorns will see this as a signal of a clearly defined corporate culture, which is craved by every Unicorn).

Lastly, make sure the right people see your job posting.

Market it appropriately through social channels, job boards, local forums, etc. Ask your network to share your job posting with theirs. Do not rely on posting a Wanted sign in your front window or on your website that expect people to find it.

If you really want to hire the best, invest in creating a strategy that will attract the best.

When it comes to recruitment, think different than you have before. Take the time to understand who you are looking for and don’t settle for mediocrity. Craft a strong message in your job posting and blast it out for the world to see.

MULTI-GENERATIONAL MARKETING– ONE SIZE DOESN’T FIT ALL When Coca Cola was first introduced in 1886, the marketing for it was pretty simple. “Delicious!

Refreshing! Exhilarating!

Invigorating! The new and popular soda fountain drink, containing the properties of the wonderful Coca plant and the famous Cola nuts. . .”

A banner across a store entrance, an ad in a newspaper or magazine, or word of mouth. That was it. And as we all know, it worked.

Marketing has evolved from the one-size fits all version Coca Cola used back then to the multi-generational marketing game we have today. To play this game, you need a marketing strategy that touches each of the generations who use your products and services: Baby Boomers, Gen X’ers and Y’ers, Millenials, etc. Where to begin?

KNOW YOUR PATIENTS To understand who your current and potential patients are, you need demographic and sociological information based on where your practice is located, and who lives in the area. A number of useful resources are available to help you do this, including City-Data [www.city-data.com] and The U.S. Census Bureau’s American Factfinder [https://factfinder.census.gov/faces/nav/ jsf/pages/community_facts.xhtml]. Focus your marketing dollars on patients in the area surrounding your practice. If your practice is in a college town, align your marketing and product push accordingly. The idea is to identify who you’re talking to so you can determine the best way to reach them. Building a persona for each demographic makes this easier.

MARKET TO EACH GENERATION Get to know your target audience’s shopping habits. Tilt your brand and messaging to align with their preferences . Which generation prefers subscription services over shopping in a store? And how best to “talk” to each generation? The Greatest Generation (born 1940-1945), for instance, likes a “don’t worry; we’ll take care of it” approach. And they like print.

Contrast that with Millenials (born 1982-2004), who generally mistrust marketers and focus on digital communication.

It’s not just what you say, it’s how and where you say it. Which demographic prefers Snapchat to Facebook? Who prefers text message to snail mail? The older crowd is more comfortable with a brochure in hand than a smartphone. But just because the younger crowd is more comfortable with their mobile devices doesn’t give you the whole picture.

In an article that appeared in Adweek [http://www.adweek. com/news/technology/infographic-here-are-devices-yourtarget-audience-using-164026], Joline McGoldrick, research director at Millward Brown Digital, is quoted as saying, “It is easy to stereotype and say the best way to reach millennials is on mobile, but that is not always true. As the analysis shows, device usage varies from generation to generation based upon what the activity is.” Another good resource for learning how the different generations think, behave, and spend was published by Gannet NJ [http://www.gannettnj. com/knowledge-center/digital-marketing-insights-marketingacross-generations-week-of-2-5-16/]. According to the article, millenials are harder to reach and “. . . keenly aware of marketing messaging and easily tune it out.”

PLAY THE GAME First create a plan. Figure out what messages, discounts, promotions, etc. you will be offering for the year. Think about creating some special holiday-related offers, for instance. Then delegate. Maybe you have some people on staff who can post regularly to social media and monitor what’s working and what isn’t. Track your progress on all fronts—be it social media, print ads, etc. Make adjustments as necessary. Keep doing what works; let go of the rest.

Multi-generational marketing doesn’t have to be costly. Sure it would be nice to offer the various generations precisely what they like best, but if you don’t have the marketing dollars to do that, find characteristics they have in common—and market to those.
