SPRINGFIELD — Intellectual and developmental disability service organizations are bracing for potential cuts to Medicaid and Medicare from the federal government under congressional Republicans and President Donald Trump.

About 3.9 million Illinoisans are enrolled in Medicaid. Of that total, 44% of Medicaid recipients are children, 9% are seniors and 7% are adults with disabilities, according to the Illinois Department of Healthcare and Family Services.

“We’re very concerned. We don’t see what the path is right now,” Illinois Association of Rehabilitation Facilities CEO Josh Evans CEO said. “And so our mission is to continue to educate our members of Congress that this is not just a program that is ripe with payments, it’s serving people.”

IARF is an association of community-based providers that serve children and adults with intellectual and developmental disabilities and serious mental illnesses in Illinois. Community providers focus on inclusion in a smaller community that offers more independence when providing care and some community providers help their residents find employment.

What’s To Be Done About Blocked Crossings?

If you had to drive across the train tracks between St. Charles Road and Main Street on Saturday, you may have been one of the dozens of commuters forced to detour due to Union Pacific Railroad freight trains stopped cold on the tracks, unmoving and, like the railroad, impervious to our collective groans and sighs.

The problem of idling freight trains in the west suburbs seems to have worsened over the years. Last year, the problem got so bad that River Forest Village President Cathy Adduci told Wednesday Journal newspaper that she sought an in-person meeting with railroad officials to address the “‘significant increase’ in reports of idling trains from residents.”

Last May, the problem of idling freight trains prompted the Highland Park City Council to unanimously approve an ordinance “requesting that rail carriers immediately cease unnecessary diesel locomotive idling and excessive horn use to work toward resolving a number of community concerns,” the Record North Shore reported.

One Highland Park city official wrote a memo stating that residents and local businesses were regularly submitting complaints about “ongoing excessive noise and pollution from diesel locomotive freight and commuter trains” that idle throughout the day, during the evening, and overnight.

The Record North Shore reported that Highland Park officials explained that “throughout the past several years, freight train activity has increased in the region, and in Highland Park, specifically, yearround,” adding that the increased activity “has also led to freight trains stopping, staging, idling, waiting, and parking for long periods of time.”

Earlier this year, Dixmoor Mayor Fitzgerald Roberts told the Chicago Tribune that “trains stop in the village at least once a day, often sitting anywhere from 30 minutes to more than 12 hours. He said a train sat idle for more than 12 hours Jan. 14, starting about 9 a.m., when he woke up, and still obstructing the crossing well past 10:30 p.m., when he went to bed.”

William Wong, a spokesperson for the Federal Railroad Administration (FRA), told Tribune reporters that people experiencing delays due to train track crossings blocked by unmoving freight trains can call a phone number that’s posted on the “blue Emergency Notification System sign located at every grade crossing to report the issue.”

He also recommended people report blocked crossings “through the Federal Railroad Administration’s blocked crossing incident reporter web portal available at railroads.dot.gov/blockedcrossings.

Multiple people told the Tribune that they’ve never received any response from the FRA after calling and reporting blocked crossings online. The Tribune added that Wong said “the agency will take action if they receive a ‘significant’ number of complaints about a particular crossing within a two-week timeframe.” The data also helps “determine where the administration’s resources and efforts can be best directed to help address local blocked crossing issues.”

Why do freight trains idle and block crossings?

The Congressional Research Service (CRS) explained that freight train operators are reluctant to turn off engines because they generate “thousands of horsepower and take an hour or more to warm up before they can start pulling rail cars.” That’s why they decide to leave them idle while stopped. Also, like a car during winter or summer, the engines may provide heat or air conditioning for the crew, “which may be aboard even if the train is idling.”

The CRS explained that trains block crossings because when trains are “moving at different speeds or in opposite directions along a single-track rail line, one train will usually

have to wait on a side track or on the nearest two-track segment until the other train has passed; if a road crosses these tracks, the crossing will be obstructed until it is safe for the train to proceed.

“In many cases, these waits are planned and meant to be brief, but unforeseen delays due to an oncoming or passing train can mean a stationary train must remain in one place for an extended period. Railroad crews, bound by [work] limits, may not be permitted to move the train, which then must wait until a fresh crew can relieve them.”

There are a limited number of ways to address idled trains and blocked crossings. Building overpasses at high-traffic locations is costly and time-consuming. Environmental Protection Agency (EPA) standards require any locomotive or engine built after 2015 to have “idling control systems that shut off engines after 30 minutes” to mitigate pollution, Gray Television’s InvestigateTV reports.

“But experts who testified before the Senate say very few of them have been made, so much of what remains on the rails is old, with outdated technology.”

Whatever the cause, freight train idling has consequences. InvestigateTV cited studies conducted by the California Air Resource Board and the University of Southern California that have found “people living near rail lines and rail yards have lowered life spans, increased asthma, heart and lung disease rates, and a risk of cancer because of the exposure to particulate matter that comes from train emissions. Experts say millions of people in low-income communities of color are especially affected.”

What can we do about it?

In Illinois, there’s a bill—SB1513—making its way through the General Assembly that says “no rail carrier shall cause, suffer, allow or permit the unnecessary foreseeable idling of a diesel locomotive within 1,000 feet of a residence, business, school or hospital for a continuous period of time longer than 30 minutes in a manner such as to cause or contribute to a condition of air pollution.”

We’ll see what comes of it. Meanwhile, perhaps you should call your state representative and/or state senator and express your support for the legislation.

What’s the bigger picture?

The problem of blocked crossings and idling freight trains reveals precisely why we need the government to work in the best interests of regular people. We need federal entities like the EPA and state legislatures to have the power to hold private entities like railroads accountable. Otherwise, the most we’d be able to do about potentially life-threatening nuisances like idled locomotives is throw up our hands.

Trump Administration Blocks More Than $400M In Funding For Illinois Health Programs

Nearly $2B in federal funds is not being sent to Illinois, according to Pritzker’s office

SPRINGFIELD — President Donald Trump’s administration is pulling back $477 million from Illinois that was to be used to prevent infectious disease and treat mental health and substance abuse issues.

The Illinois Department of Public Health announced last week the federal Centers for Disease Control and Prevention is taking back $125 million allocated to IDPH and 97 local health departments for infectious disease prevention. Another $324 million appropriated by Congress

for “future use” in preventing and treating infectious disease also is being blocked.

Separately, the Illinois Department of Human Services was notified this week that $28 million from the U.S. Department of Health and Human Services for mental health and substance use disorder treatment would be terminated. The funds had been appropriated by Congress in the American Rescue Plan Act for 77 Illinois community organizations providing mental health and substance abuse treatment services to residents.

“This decision to terminate already awarded federal funding will cause immeasurable harm and disruption to the health and safety of the people of Illinois and generate larger expenses in the longer run,” Gov. JB Pritzker said in a statement.

Director Sameer Vohra said in a statement. IDPH has been using the funding to track the spread of diseases, invest in lab upgrades, build up the public health workforce, and improve vaccination efforts.

A report published last week by Trust for America’s Health, a non-profit research organization, ranked states into one of three tiers based on their preparedness for a public health emergency, and Illinois ranked in the middle tier.

Illinois is one of 47 states with detailed “surge plans,” that outline IDPH’s ability to handle a sudden surge in testing demand and implement changes like reallocating staff and acquiring supplies, according to the report.

Over the past decade, IDPH published several surge plans including an “Ebola Virus Disease Preparedness and Response Plan” in 2019 and a “Pandemic Influenza Preparedness and Response Plan” in 2020.

A surge plan is a “critical indicator of a state’s emergency preparedness” that demonstrates a state’s capabilities to tackle a crisis, the report said.

It noted that seasonal flu vaccination rates also give an insight into the state’s emergence response capabilities. According to IDPH, about 28% of Illinoisans received a flu shot for the current season.

IDPH also is keeping an eye on the spread of bird flu with the now-ceased federal funds. Although the CDC says bird flu does not currently pose a threat to humans, there is concern that interaction between the seasonal flu virus and the bird flu virus might create a new strain that could start human-to-human transmissions and increase the risk of a pandemic, Vohra said in a February interview with Capitol News Illinois.

Legal and budget impacts

Funding for IDPH’s and DHS’ programs was appropriated by Congress but is now being rescinded by executive agencies. The Illinois Attorney General’s Office has already sued the Trump administration over other spending cuts, including on disaster funding, clean energy grants and teacher preparation grants.

Funding for IDPH’s infectious disease prevention programs was appropriated by Congress in the 2020 CARES Act — a law signed by Trump designed to help businesses, individuals, and health care institutions access funding during the height of the pandemic.

Funding for DHS’ mental health and substance abuse program was appropriated by Congress in the American Rescue Plan of 2021 — a wide-ranging stimulus bill signed by President Joe Biden that included funding for programs intended to alleviate effects of the pandemic.

“The State of Illinois will do everything in our power to restore this vital federal funding and continue to invest in common sense public health solutions to keep our state safe and healthy,” Pritzker said.

A judge issued a preliminary injunction blocking Trump’s broad federal spending freezes in January, but Illinois and other states have continued to be notified that money already destined for the state’s use is being rescinded.

The $477 million being pulled from IDPH and DHS is part of roughly $1.8 billion in federal funding the state is expecting to receive but remains in limbo. It’s also the latest example of the state losing money that’s already being put to use.

“I’m going to do whatever it is that I can do, but I can’t come up with $8 billion to keep a federal program going in my state,” Gov. JB Pritzker said in an interview with The Contrarian last week. “I can spend hundreds of millions of dollars to try to provide free healthcare for people who are most acute, but people are going to die because of what they’re doing.”

Prtizker proposed increased funding in the developmental disabilities division at the Department of Human Services, DHS, in his proposed 2026 fiscal year budget. This would include funding to continue placements of individuals who qualify and want to live in community-based settings and for new placements under a 2011 federal court order the state has struggled to comply with.

The Ligas Consent Decree requires states to provide care options in integrated community settings for Illinoisans with intellectual and developmental disabilities who request community-based services.

Despite Trump’s claim that he would not make cuts to Medicaid, congressional Republicans’ budget resolution would almost certainly result in shrinking funding for the program.

Trump has vowed not to cut Medicaid benefits, but he has also said his administration will go after “waste and fraud” and cited tens of billions of “improper payments” in entitlement spending as the target for trims.

“You need to be careful in terms of how you’re looking at Medicaid, whether it’s focused on ways you can try to eliminate fraud, abuse and improper payments, which we all support, by the way, (but) major substantive changes to Medicaid will have a downstream impact on disability services,” Evans said.

Service providers worry the budget cuts proposed in a United States House budget resolution could jeopardize access to medical care for people with disabilities in Illinois and across the United States who rely on Medicaid. The budget proposal calls for $2 trillion in budget cuts, making it likely that Medicaid and Medicare will be impacted, Evans said. All 14 Illinois Democratic House members of Illinois’ congressional delegation voted against the resolution.

“I think some people assume that the cut automatically equals cost savings, but that isn’t necessarily the case,” said Kelly Berardelli, CEO of southwest suburban-based disabilities nonprofit Sertoma Star Services. “Just because the funding is cut doesn’t mean the need is gone, and a lot of people we serve are from the most vulnerable populations, so they’re go-

ing to still need services and supports.”

Sertoma Star Services serves more than 1,500 children and adults with intellectual and developmental disabilities in the Chicago area and Northwest Indiana. The organization receives most of its funding from Medicaid, and many of the people using their services rely on Medicaid for access to care.

“Any cuts to Medicaid have the potential to reduce the quality of life for the people we serve,” Berardelli said.

Evans agreed.

“Disability services in Illinois are primarily exclusively funded through Medicaid,” he said. “There’s no private pay, there’s very little to no Medicare. It’s all Medicaid.”

If access to community-based care is slashed by Medicaid cuts, people will seek care through institutionalized facilities, which tend to be large facilities run by the state with a focus on medical care, or in some cases, be hospitalized. This could cause Illinois to further violate the Ligas Consent Decree.

According to Berardelli, people with intellectual and developmental disabilities living at home could lose access to respite care if

Medicaid funding is decreased in Illinois. Respite care is temporary care from a professional who is not the recipient’s primary caregiver, which is usually a couple of hours in a day or week.

More than half of those who receive care from Sertoma Star Services live with a family member over the age of 55, making the threat to respite care particularly concerning, Berardelli said. If these people cannot get respite care, they may not be able to live at home and will have to be placed in institutionalized facilities, more full-time care away from home.

While some may seek placements at community providers, there are already long wait times and a shortage of community providers of care for people with intellectual and developmental disabilities.

“Cuts to Medicaid would, I think, inevitably increase that waiting list,” Berardelli said. “Progress has been made over the past several years, and we would definitely see that progress reversed if there were cuts to Medicaid.”

Behavioral health services would also be impacted as Medicaid helps both to fund service providers in addition to insurance cov-

erage for services such as mental health care and addiction treatment.

“The majority of our member organizations who provide behavioral health services are straight Medicaid,” IARF senior vice president of behavioral health policy and advocacy Emily Miller said. “Very few accept private insurance and so you would decimate the community with these drastic cuts that are being proposed to the Medicaid program.”

Cutting federal funding would also cause many health industries to compete with one another for funding. If there is a more limited pool of funding for health provider programs, not every specialized program would get the funding they need.

In the state fiscal year that ended in June, Illinois received over $20 billion in federal Medicaid funding, which made up about 62% of the total funding for Medicaid programs in Illinois, according to HFS.

“If there’s a major change where we see a dramatic loss of dollars, that means we’re going to have to be lobbying against one another in the healthcare and human services space for a more limited amount of resources,” Evans said. “We cannot be put in that position.”
