UHUK Conference 2024 Delegate Pack

Page 1

WELCOME LETTER

Dear Delegate,

Welcome to the UHUK 2024 Conference and Awards Ceremony! It’s an honour to welcome you all to this year’s gathering, and it’s especially meaningful as my final UHUK Conference as Chair. This event promises to bring together our diverse experiences, ideas, and innovations to advance the care we deliver for our communities.

As you prepare for the next two days, please take a few moments to read this letter and explore the agenda. The packed schedule highlights not only each presentation’s focus but also introduces the expertise and backgrounds of our speakers. With this overview, you’ll be well-prepared to navigate each session and make the most of this exceptional conference.

This year’s conference holds special significance, with the NHS at a critical crossroads. The urgency for change has never been greater, and so too is the opportunity to create meaningful, lasting transformation. The UHUK community, grounded in NHS and social values, is uniquely positioned to drive this change. Together, we’re proving how urgent and integrated care providers are essential to sustaining the NHS and enhancing patient outcomes. This conference celebrates that commitment as we share achievements, best practices, and our unwavering dedication to excellence in patient care.

Our theme, “Sustaining the NHS: Is Virtual Care the Solution?” reflects one of the most timely and impactful discussions facing healthcare today. Over these two days, we’ll explore whether digital and virtual care solutions can effectively sustain NHS services, examining their potential to deliver safe, efficient, and accessible care across communities. In a time when Wes Streeting has launched a public consultation to gather insights and experiences to “help fix our NHS,” and the Darzi Review has highlighted the need to shift NHS resources toward community-based care, we have a unique chance to contribute new ideas and champion solutions that align with these priorities. The Darzi Review's call to reallocate resources from hospitals to community care, for instance, aligns directly with the core of our work as a UHUK network and challenges us to lead the way in finding viable, community-focused solutions.

A special thank you goes to our sponsors, whose partnership enables this conference and its mission. Their expertise and innovations will be on display throughout the event, showcasing their critical contributions to healthcare. I encourage each of you to connect with our sponsors and explore how their solutions can support and enrich the work we do. Their support is essential to our shared goals and embodies the spirit of partnership and collaboration that drives UHUK forward.

Reflecting on my journey in urgent care, which began with my appointment to the Liverpool GP Co-operative Board in 1998, I feel a profound gratitude for the experiences, connections, and shared achievements we’ve cultivated. Working alongside each of you has been an honour and a privilege. Our progress together has built a stronger, more responsive healthcare system, and I am deeply grateful for your support and collaboration over the years.

Looking ahead, it will be exciting to see how our discussions and insights from this conference will inform the future of our sector. Thank you all for your commitment, and I hope the next two days are as inspiring and thought-provoking as the work you do daily. Enjoy the conference, and may it fuel the ideas, partnerships, and innovations that shape our future.

Thank you for registering for what promises to be an outstanding occasion. I want to acknowledge the incredible talent each of you bring to the conference, that will really make it a valuable and memorable event. Please keep your feedback forms at hand, and I look forward to seeing you.

Warm regards

Through our decades of experience in providing urgent care services, our subsidiary, CLEO Systems, is leading digital transformation in urgent, emergency and outpatient care through the development of innovative clinical systems for use within the health service.

Established in May 2011 by a group of Cornwall Practices, Kernow Health are a GP owned provider organisation and deliver a number of NHS contracts across Cornwall, these include Cornwall 111 Integrated Urgent Care Service, School Immunisation Programme, Children’s Eating Disorder Service and Cornwall Training Hub [formally CEPN].

These contracts support the visions and values of the organisation, and also ensure that we are able to reinvest any profits made back into the company; again, with the intention of supporting GP practices across Cornwall.

The principal activity of the company is a social enterprise focused on securing the future of Cornish General Practice. As a Community Interest Company, we will use our resources to improve healthcare in the communities we serve.

Covering the whole of Cornwall, with a resident population of 565,000, KHCIC has developed into a trusted healthcare system partner.

London Central and West (LCW) started in 1994 as a local GP co-operative, and has evolved into one of London's largest and most successful providers of integrated urgent care services covering a population of about 3.7 million. In 2014, LCW became a Certified Social Enterprise/Community Benefit Society and invests any surplus in delivering and improving services to patients.

LCW was the first London provider to go live with a 24/7 Integrated Urgent Care service covering North Central London.

We have increased our skill mix of clinicians to include GPs, Advanced Nurse Practitioners, Nurses, Paramedics, and Pharmacists, ably supported by our expanded non-clinical support teams.

We work in partnership with Commissioners, Acute and Community Trusts, GP Federations, PCNs and other health and social care organisations in the development of services, including:

 North West London IUC from November 2020 in partnership with the London Ambulance Service and PPG: this includes a 24/7 Clinical Assessment Service run jointly with PPG

 Two Urgent Treatment Centres (UTC) located at Charing Cross and Hammersmith hospitals with a co-located GP practice of 10k patients

 Inner North West London Single Point of Referral for the Community Independence Team

 Care home / telemedicine service delivering an enhanced response to 140 care homes in North West London.

The Partnership of East London Co-operatives (PELC) delivers a range of high-quality primary and urgent care services to North East London. As well as managing a successful, long-standing out-of-hour service for 1.1 million patients, we have implemented an effective subcontract infrastructure to deliver a 111 call centre -resulting in improved performance and a more refined customer journey. PELC also play an extensive role in supporting and improving A&E Depts. and Urgent Treatment Centres (UTCs).

We have some of the highest utilisation rates at one of the busiest emergency departments in the country. Additionally, we are developing a new urgent treatment centre model at the front door of the ED in order to administer the appropriate care to patients in a more timely and effective manner.

Our GP and wellness services have also been integrated into HMP Chelmsford. There, we are not only improving the day-to-day life of prisoners, but also giving GPs an opportunity to enhance their clinical skills by engaging in prison healthcare.

We champion effective process, positive workplace environment and innovation in order to deliver the best possible results for our patients.

Salford Primary Care Together are responsible for driving forward collaboration and supporting the local practices to pool their services, staff and resources so that all GP practices in the neighbourhood are strengthened and improved. This also means connecting general practice with other professionals and support in local neighbourhoods, such as district nurses, social workers, mental health workers, voluntary organisations and local community groups.

This means people will have better access to the advice, expertise and support they need to be as independent as they can, with the opportunity to see a wider range of health professionals and access the full range of activities in their area. Local people will require fewer hospital admissions and visits as there will be more skills and support available within the community they live.

In addition to our work with GP practices, we directly provide primary care services, including the Ordsall and Claremont Primary Care Diagnostic Unit and the Salford Wide Extended Access Pilot.

We are a Community Interest Company, which means we exist to improve the health and wellbeing of Salford people and all our resources will be dedicated to doing that.

SELDOC Healthcare - We’ve been getting people better for nearly 25 years and we are never going to stop. We deliver healthcare services to more than 2 million patients in primary care and play a vital role in local healthcare systems. We were established in 1996 and made a commitment to work to improve the quality of care to local patients. Since then, we have grown into new areas including urgent care, clinical support for emergency services and medical care at home. Patients access our services either directly or through NHS 111 as part of local Integrated Urgent Care provision. In addition, we support patients to stay well at home rather than needing a hospital stay and offer a range of services for GP practices including telephony, home visits and remote GP appointments.

We work closely with local GP surgeries, NHS commissioners, partner organisations and other healthcare professionals and have built a reputation as being a fantastic, operationally smart healthcare services provider. Our core purpose is to deliver local healthcare you can trust – we care about our patients’ healthcare and our values underpin everything we do.

Shropshire Doctors Cooperative Ltd (Shropdoc) is a not-for-profit company established in 1996. It provides a range of services to 600,000 patients throughout the whole of Shropshire, Telford & Wrekin and Powys and deals with over 200,000 calls and over 50,000 patients per year. The range of services provided by Shropdoc means they are operational, in various forms, 24/7 365 days a year and now includes North Wales and Staffordshire.

The core of their services remain the GP Out-of-hours service but planned diversification means they also deliver Care Coordination (SPA) to Powys, Shropshire and Staffordshire, telephony Physiotherapy triage, 24/7 Oncology helpline to Lancashire and Shropshire, patient appointment booking, daytime clinical triage, and various workforce initiatives including Urgent Care Practitioners who are being deployed in-hours to Practices not just OOH. Shropdoc has a subsidiary organisation called Caradoc which provides some non-core services, including a successful purchasing consortium which has also now extended to benefit UHUK members.

Shropdoc received an 'Outstanding' rating from the CQC in 2017.

Suffolk GP Fed CIC

Suffolk GP Federation CIC is a not-for-profit Federation owned by 58 GP practices. As well as supporting its members it provides a suite of services including Out of Hours, ED streaming and Extended Access in Suffolk.

The Federation also directly manages a GP surgery and provides a portfolio of community services.

The Federation was established to enable primary care to ‘work at scale’. We do this by:

Facilitating practices working together in an open, democratic and transparent way

Addressing issues which are optimally solved by collaboration (rather than by individual practices)

Providing a management infrastructure, skills and expertise that an individual practice would find uneconomic to employ.

www.suffolkfed.org.uk

Herefordshire’s Out-of-Hours GP Service is operated by Taurus Healthcare on behalf of Herefordshire General Practice.

Herefordshire General Practice is a collaboration of GP providers who work together to enable a stronger, independent General Practice for the future. It comprises all 19 GP surgeries in Herefordshire (and one in Tenbury Wells, Worcestershire) based in five Primary Care Networks, the Local Medical Committee and the GP Federation (Taurus Healthcare).

A key aim is to offer 24/7 patient care in the right place with the right professional at the right time. We do this through traditional in-hours, GP surgery-led care together with additional services provided by the Federation’s clinical arm – including the GP OOH Service. This is facilitated by joined up patient records as EMIS is used across Herefordshire General Practice (including in the OOH service) to enable continuity of care and the best possible service for patients.

www.herefordshiregeneralpractice.co.uk

Western Urgent Care (WUC) provides GP OOH services across the WHSCT area which covers the counties of Derry, Tyrone and Fermanagh. We cover a range of urban and rural areas across five clinical bases and for a population of approximately 300,000 patients. This will include a small number of patients from the Republic of Ireland living along the border areas.

We also provide GP OOH support for a small number of hospices and palliative care wards within the local area. This work compliments the Marie Curie Service we host providing Nurses to patients on the Palliative Care Pathway during the out of hours period.
