Step-by-Step Guide to Complete the Re-Enrollment Contract

STEPS TO COMPLETE THE ONLINE RE-ENROLLMENT CONTRACT STEP 1 - Before you begin, you will need: § §

UCLA Lab School portal account login information for each parent Note: Every parent/legal guardian listed on your child's profile needs to log in separately and sign the Re-Enrollment Contract, COVID Addendum, and acknowledge that you've read the Parent Handbook before it can be successfully submitted.

STEP 2 - Log in to your school account.

STEP 3 - Click on “You have: 1 Contract to Review” on yellow banner

STEP 4 - Click on "Review"

STEP 5 - Make sure your child's information is correct in their profile and click "NEXT"

STEP 6 - Acknowledge that you've read all of the Terms and Conditions & UCLA Lab School Policies

STEP 7 - Acknowledge that you've read the Parent Agreements.

STEP 8 - Acknowledge that your child has read the Student Agreements.

STEP 9 - Acknowledge that you've read the Parent Handbook.

STEP 10 - Make the decision to re-enroll or not re-enroll.

If you are applying for tuition support, you must sign this contract first to receive a new contract with the tuition reduction.

STEP 11 - If you are re-enrolling, pick your payment plan.

If you have chosen YES, and are NOT re-enrolling, you may skip the next steps and go to the signature page.

STEP 12 - Acknowledge that you've read the Enrollment Withdrawal and Leave of Absence deadlines.

STEP 13 - Acknowledge that you have read the COVID Addendum by typing your initials next to each of the 23 provisions and signing your name at the end.

LAST STEPS: Sign the final page.

Review the contract.

Pay the non-refundable deposit (let us know if you are applying for tuition support

Receive a confirmation.
