Post Vessel Incident Actions Background Following a Marine Incident, there are several internal and external actions that are required. The purpose of this alert is to reinforce what the requirements are and the relevant post incident actions.

Duty to Preserve the Scene In the event of a vessel collision, HCF has an obligation to ‘preserve the scene’ until AMSA and OTSI have granted permission to for the vessel to be moved. The vessel can only be moved if there is an immediate requirement to prevent loss of life, injuries, loss of vessel or further damage. Otherwise please wait for approval to move the vessel from Operations.

Post Incident Actions Actions following specific vessel incidents and situations are detailed in each vessel class VERP and Emergency Action Guide Flipchart. In addition to the actions detailed in the VERP and Flipcharts, vessel crews should also attempt to record the details of all passengers using the Customer Incident Log (or any other available means). This will ensure appropriate customer care and follow-up can occur.

Notification Requirements Following any incident (collision, grounding, submerged object strike, close quarters, pollution etc), there are a number of reporting obligations that need to occur: • Sydney Ports VTS – Immediately via VHF 13 or (02) 9296 4003. • HCF – Immediately via GRN / Telephone to the Controlling Officers (who will notify the Incident Manager if required). • HCF – Safety Report Form submitted to your Line Manager by the end of the shift. • AMSA – By completing the AMSA Incident Report form by the end of shift and submitted at the same time as the Safety Report Form (SHEQ Team will pass onto relevant stakeholders within 72 hours). • When in doubt if the incident is reportable or not, report to VTS and complete a SRF.

Conclusion Following these reporting requirements will ensure HCF can respond in an appropriate and effective way. It is the responsibility of the individual(s) involved to report all incidents. Complying with this information will ensure HCF meets the reporting and escalation obligations under the relevant legislation.
