magine a trip to the drug store to fill a prescription for a blood pressure or cholesterol medication and leaving with a drug tailor-made

just for you. No worries about adverse drug reactions, side effects or whether the medicine will work. Such a day is not too far off, says Dr. Zvi Loewy, professor and chair of the Department of Pharmaceutical and Biomedical Sciences at Touro College of Pharmacy.

A pioneer in the field of genomics for decades, Dr. Loewy was among the first experts in DNA fingerprinting for law enforcement, recovery and issues of paternity. Now he is leading a program at TCOP in pharmacogenomics, the study of how a person’s genetic makeup, or genotype, impacts his or her reaction to medication.

Using the ever-increasing amounts of genomic data collected from large populations to evaluate various approaches to disease, drug applications and development, researchers now better understand how an individual’s genetic makeup influences responses to disease, medical conditions, and the drugs that treat them. As a result, the field of pharmacogenomics has emerged as key in identifying drug therapies that promise an entirely new personalized approach to medicine. This new field of science is expected to dramatically alter what and how we take medications for simple and complex illnesses and conditions.

Touro is also expanding its reach to the broader medical and pharmacy community with programs for working pharmacists and physicians interested in developing a foundation in pharmacogenomics and how patients can benefit from these new approaches to diagnosis and treatment.

“Through both our curriculum as well as the pharmacogenomics experts we are attracting to Touro, we are providing our students with exposure to this cutting edge area,” explains Dr. Loewy. “Our goal is for students to be prepared so they can land jobs and take part in this exciting new phase of pharmacy.”

TOURO IS PREPARING STUDENTS FOR GROWING FIELD TCOP is preparing its students for this growing field through course offerings and rotations. Studying pharmacogenomics at Touro involves learning its foundations, including a basic review of mechanisms of gene expression and gene regulation, along with genetic concepts and history.

During rotations, students can pursue additional exposure to pharmacogenomics. The school has teamed up with Genewiz, a global genomics service company involved in DNA sequencing, gene synthesis, molecular biology and bioinformatics. At Genewiz, students gain real world work experience and Genewiz staff are now serving as guest lecturers at Touro.

At Touro College of Pharmacy, professors and scientists like to think out of the box. Recently, a team of faculty members including Mariana Babayeva, M.D., Ph.D., Haregewein Assefa, Ph.D., Paramita Basu, Ph.D. and Zvi Loewy, Ph.D. did just that when they collaborated on a research project investigating possible therapeutic benefits of marijuana for Parkinson’s disease. Marijuana, also known as cannabis, is now approved for medical use in 29 states and is believed to have therapeutic effects for chemotherapy-related nausea and vomiting, decreased appetite due to HIV/AIDS, chronic pain and muscle spasms.

50 MILLION PARKINSON’S SUFFERERS WORLDWIDE The Touro research lab, with its focus on natural products for medical purposes, specifically to enhance the quality of life, hoped to identify a condition where marijuana’s therapeutic effects might have a positive impact.

As part of their research, the team considered difficult-to-treat disease states, among them, Parkinson’s disease. A progressive disorder of the nervous system impacting the motor system, Parkinson’s is the second most common neurological illness in the United States after Alzheimer’s disease, impacting one million Americans and over 50 million people worldwide.

An examination of over 300 scientific papers on marijuana from all over the world uncovered evidence that marijuana not only reduces tremors caused by Parkinson’s disease, but that it may also help with other debilitating

symptoms including insomnia, pain, and depression. In addition, the Touro researchers found preliminary evidence that marijuana has anti-inflammatory and antioxidant properties that could slow the progression of the disease. Their findings were published last winter in the peer-reviewed journal, Parkinson’s Disease.

MULTIPLE BENEFITS The research team, which has worked together for many years at Touro, both as teaching colleagues and on collaborative research efforts, was excited by their findings. “We discovered that the chemical complexity of marijuana provides multiple benefits for Parkinson’s that affect different symptoms of the disease,” explains Dr. Babayeva.

With pain affecting nearly 50 percent of the patients with Parkinson’s there have been significant drawbacks to the use of pain medication, including addiction. Explains Dr. Basu, “Marijuana has been found to relieve pain in other diseases and should be studied for Parkinson’s.”

In this progressive disease, a person’s brain slowly stops producing dopamine, a neurotransmitter that plays an important role in the movement and many other physical, cognitive, and physiological functions. Current medications target motor symptoms, but have limitations and haven’t shown to slow the progression of the disease.

“Compounds in marijuana bind to dopamine receptors and help to reduce the effects of the dwindling dopamine. Essentially the compounds replace the normal compounds that are adversely affected by Parkinson’s,” explains Dr. Assefa. “In addition, we found laboratory evidence showing that marijuana has anti-inflammatory and antioxidant affects that may prevent neuron damage.”

For the researchers, marijuana’s therapeutic effects seem promising. “Cannabis may provide a viable alternative or addition to the current Parkinson’s treatments. More research is needed to provide data on the efficacy and safety and its molecular components,” says Loewy.

A

comprehensive review of the scientific literature on opioid pain management found that people with a certain gene mutation do not respond as well to opioid pain medications and may need to be managed differently in order to get relief.

Though there are standard dosages for opioids, studies have shown that patients react differently to them.

“Some people don’t seem to respond as well, and doses need to be raised in order to have adequate pain control, but that increases the risks of side effects, including addiction,” said Priyank Kumar, Ph.D., assistant

professor and head of laboratory research at the Touro College of Pharmacy, who conducted this research with Drs. Zvi Loewy and Maureen Sullivan.

The researchers looked at about 60 studies over a ten-year period to examine a known gene variation, called a single nucleotide polymorphism, or SNP, found on an opioid receptor.

The studies showed that people with a difference of one neucleotide—they had an A118G, rather than the standard A118A —had a decreased sensitivity to opioids. The studies showed that people with this polymorphism were also more susceptible to side effects including addiction.

Dr. Kumar and his team are advocating for genetic testing for this polymorphism in patients before those with chronic or serious pain are

Someone with this polymorphism could be put on a different opioid other than morphine, such as fentanyl. “It will help us in designing a better pain management regime for the patients with A118G polymorphism,” said Dr. Kumar. “Genetic testing may explain and predict many of the clinical responses seen with opioids medications, and may help the clinician identify those patients at genetic risk of opioid misuse and addiction” he said.

CHANGING LIVES TOURO PROFESSOR INVENTS NEW MEDICAL DEVICE TO HELP MILLIONS

P

rofessor Zvi Loewy, who heads the Department of Pharmaceutical and Biomedical Sciences at Touro College of Pharmacy and has served as VP of Research at two major pharmaceutical companies, invented the first “all in one” system to help diabetics measure glucose levels. This innovative medical device has received approval from the Food and Drug Administration and will soon be hitting the market.

Professor Loewy, can you explain how this new device works and how it is different from what is currently available?

This device is being commercialized by Intuity Medical, and it is unique in that it is an “all in one” system. Other products currently marketed require the patient to have on hand a piercing device, chemical strips and a small reader. This device, called the POGO system,

is a self-contained 3-in-1 product. It’s much more user-friendly and provides for better test accuracy.

When did you first think of this invention and what steps did you take to bring it from idea to reality?

I conceived of it several years ago when we were exploring the creation of novel devices for different healthcare applications including pointof-care glucose testing. We broke the glucose testing process into the three steps, optimized each step independently and then designed the unit that would “put it all together.”

How do you think this will impact the lives of those with diabetes?

The product provides for ease of use and portability while providing high quality results. Hopefully, people will use it according to the frequency recommended by their healthcare provider. Control of diabetes is linked to glucose monitoring several times daily. Because our system uses thin needles that are less painful, it will be much easier for people to monitor regularly.

What advice do you have for students who are interested in pursuing inventions and patents?

Think “out of the box”, try to address a technical challenge that has an unmet market need. Share your ideas with friends and colleagues who can help you build upon it.

Who would you say is your own professional mentor?

Today it is my close network of scientists and engineers, colleagues that I have had the privilege of working with on different projects throughout my career.

Are you working on any other inventions that will improve quality of life and health?

Yes. We are currently developing new chemistries for oral care applications. The objective of the new chemistry is to change the distribution of bacteria in one’s mouth. Essentially, we reduce

the “disease- causing bacteria” and allow non-disease bacteria to replace them. One can envision the new chemistry functioning similar to “probiotics” but on an oral device including braces, mouth-guards and dentures.

What opportunities exist at Touro for students interested in researching and developing medical devices?

The beauty of the diverse and comprehensive Touro College and University System is our ability to collaborate on a multi-disciplinary level. In my lab I offer opportunities for undergraduates, pharmacy and medical students to conduct fundamental research on a variety of projects. The more disciplines we involve in the research, the more ideas we generate and the greater the potential for breakthroughs. My vision is that our collaboration will bear fruit in the form of many new inventions that will improve health and lives.

TAKING UP RESIDENCE TOURO & MT. SINAI JOIN FORCES TO FURTHER POSTGRADUATE OPPORTUNITIES IN PHARMACY

I

n pharmacy, it’s common knowledge that postgraduate residencies have become an increasingly popular way to start careers after graduation from pharmacy school.

They provide training under experienced preceptors and valuable experience with direct patient care. Residents can further hone their skills in hospital settings and gain a competitive advantage in the job market. Postgraduate year-one residencies (PGY1) are often followed by postgraduate year-two residencies (PGY2), which are more specialized.

It’s also well-known that unfortunately, demand exceeds supply for the residencies, which are filled through the American Society of Health-System Pharmacists’ national matching service. In 201617 however, Touro College of Pharmacy and Mt. Sinai joined forces and helped close the gap when they opened two new PGY2 spots – one in infectious diseases and the other in critical care.

“Touro is committed to furthering the profession by providing funding for PGY2 residencies,” said Dr. Roopali Sharma, the faculty member who serves as residency coordinator for Touro.

“These residencies provide systematic training in advanced areas of pharmacy practice so pharmacists are prepared for cutting-edge patient care, as well as academic or other specialized positions.

They increase the residents’ depth of knowledge, skills, and levels of expertise in medication therapy management and clinical leadership in their areas of focus.”

Additionally, the PGY2 residents are prepared for board certification in their specialties. In critical care, certification is available now and in infectious diseases, certification is expected to be approved shortly, Dr. Sharma said.

The two new Touro-Mt. Sinai residents chosen for the 2017-2018 academic year are Dr. Nicole Davis for the PGY2 in critical care and Dr. Adeola M. Okoduwa for the PGY2 in infectious diseases.

Born and raised in the Bronx, Dr. Okoduwa obtained her PharmD at Hampton University School of Pharmacy in Hampton, VA. She recently completed her PGY1 residency at Montefiore Medical Center.

“DOING EXACTLY WHAT I LOVE” Dr. Davis, who was born and raised in Honea Path, South Carolina, obtained her PharmD from the South Carolina College of Pharmacy in Columbia, SC and recently completed her PGY1 residency at Mt. Sinai.

“With the PGY2 position being so new to Touro and Mt. Sinai, there is enormous opportunity not only to build the program, but to promote the practice of critical care pharmacy and be a valuable resource to my team,” said Dr. Davis.

“I am excited for a year of experience doing exactly what I love and becoming a specialist in critical care.”

Dr. Davis follows in the footsteps of Diana Gritsenko, PharmD, who had the inaugural PGY2 residency during 2016-17 and recently began working as a multispecialty clinical pharmacist at Yale-New Haven Hospital with a focus in critical care. She came to Touro from The Brooklyn Hospital Center, where

she served as Chief Pharmacy Resident after graduating from St. John’s University.

Said Dr. Gritsenko, “I thoroughly enjoyed my residency experience! I spent the year learning to serve my critically ill patients and optimize their pharmacotherapy. It is a truly rewarding experience being able to come in every day and do what you absolutely love.”

Additionally, she taught Touro students, and with the help of her preceptors and mentors, she created three publications during the year: a journal club webinar, and presentations for meetings and symposiums.

GAINING EXPERIENCE ON THE ACADEMIC SIDE Last year’s PGY2 resident in infectious diseases was Dr. Arsheena Yassin, who came to the program from Englewood Hospital and Medical Center where she did her PGY1 residency. Born and raised in Guyana, South America, she earned her PharmD at Rutgers University. In July she began work as a clinical pharmacy specialist in infectious diseases at Mt. Sinai St. Luke’s hospital.

Her PGY2 prepared her well, she said, providing her with rotations in adult pediatric infectious diseases, solid organ transplantation, oncology, HIV, and microbiology and antimicrobial stewardship. She also gained experience on the academic side, lecturing and teaching students at Touro.

“I was able to see how dependent the multidisciplinary teams are within a hospital on such a pharmacist and also see the diverse roles we play in various settings,” said Dr. Yassin. “The experience was unique and diverse and provided me with the experiences necessary to be a clinical infectious diseases pharmacist. There are limited residencies, especially PGY2s in infectious diseases and even fewer that provide both clinical and academic experiences like this one through Touro College of Pharmacy.”

Faculty Welcome NELLY ADEL, PHARMD, BCOP, BCPS ONCOLOGY EXPERT CHOSEN TO HEAD PHARMACY PRACTICE DEPARTMENT

DR. NELLY ADEL joined Touro in 2016 as associate professor and chair of the Pharmacy Practice Department. Previously she was a director of the PGY2 Oncology Residency Program and manager of Clinical Pharmacy Services at Memorial Sloan Kettering Cancer Center in New York for 12 years. Dr. Adel earned her PharmD and B.S. from the Lebanese American University in Beirut, Lebanon.

She completed a pharmacy practice residency, PGY1, and an oncology specialty residency, PGY2, at The Johns Hopkins Hospital in Baltimore. Dr. Adel has spoken and published widely and worked on numerous special projects in oncology.

Among her more significant achievements,

she has made meaningful strides in the improvement of patient care. Since coming to Touro Dr. Adel has been working to help Touro become a provider of continuing education programs for Harlem health care professionals, as well as working with the American Association of Colleges of Pharmacy to incorporate oncology into the curriculum of pharmacy schools. Her areas of expertise include chemotherapeutic treatment, medication management, dangerous drug interactions, hematologic malignancies, Bone Marrow Transplants, GI malignancies and breast cancer.

DIPAN B. RAY HAS BEEN WITH TOURO

College of Pharmacy since it opened in Harlem in 2008. He has worked as a practicing pharmacist for over 25 years in a variety of settings – as a staff pharmacist at Montefiore Medical Center; as a research scientist and department manager of research and development at a major pharmaceutical company, International Specialty Products in Wayne, N.J.; and he owned a community pharmacy in Yonkers, where he would stop by after his day job to close up shop and work on weekends. Humble and revered by students and colleagues alike, Dr. Ray is as comfortable in the lab as he is counseling patients or out in the field visiting pharmacy sites to make sure that Touro students are getting the quality educational experience they need – or even joining students on volunteer missions abroad or in the South Bronx, visiting public schools to talk to kids about the dangers of prescription drug abuse.

Dr. Ray, as you and TCOP approach the celebration of your 10th anniversary together, how would you say the role of the pharmacist has changed over the years?

I have been fortunate to have had the opportunity to witness how the pharmacists’ scope of practice has evolved and continues to change. Pharmacists today are more engaged clinically around patient-centered care, providing medication therapy management and offering immunization services in the community setting; working as clinical practitioners as members of healthcare teams in institutional settings; working as drug information specialists in managed care settings; and offering expertise in regulatory affairs, medical affairs and pharmacovigilance in the pharmaceutical industry. Pharmacists are now certified to administer all CDC-recommended vaccines to adults. This change in prac-

tice has provided tremendous benefits to patients, especially our seniors who are at high risk for contracting communicable diseases.

MULTITUDE OF

As Director of Practice Experience, you are responsible for curriculum that makes Touro unique--54 weeks of rotations. This is more time than most schools devote to practice. Why the heavy emphasis on practice?

PROFESSIONAL

We believe confidence develops through practice. Our unique program offers student pharmacists more handson experience in a variety of settings.

With the multitude of professional options for pharmacists continuing to evolve, our goal is to prepare our students for these newer opportunities, better situating them for the latest challenges and career paths.

CONTINUING TO

Our students often say they learn something really critical from their experience in a particular setting. For example, Mir Mahmood, class of 2018, said his rotation at a marketing agency in managed care taught him to think not only as a clinician but also as a marketer, which he found valuable. He explained: “Before I started my rotations, I always thought that knowing the clinical safety and efficacy of a drug would be enough.

However, I learned that the economic impact of drugs is just as vital. Safety and effectiveness are important, but also key is keeping costs down. I learned how to do that, which can result in better compliance and reduction of hospital readmission rates.”

A big part of Touro’s mission is serving the community and the underserved. In training future

We always encourage our students to get involved in community outreach activities like health fairs and health screening camps. During these events student pharmacists counsel patients in their use of medication, perform bone density screenings, diabetic foot examinations and blood pressure measurements. They also talk to patients about the importance of receiving vaccinations to prevent communicable diseases and how to maintain healthy lifestyles.

participant in these events. “Touro emphasizes hands-on experiences and service,” said Duong. “At our Health and Wellness Expo last year, we applied our knowledge and educated patients about disease states. It is important for us as future healthcare providers to give back to the community, especially underserved communities like the Bronx and Harlem.”

How has pharmacy education and training changed over the years?

Pharmacists today are an integral part of the health care team. They receive the best education and training to become drug information specialists and make appropriate interventions in an inter-professional team environment with physicians, nurse practitioners and social workers, all with the goal of achieving better health outcomes for patients. Students complete a national certificate program in pharmacy-based immunization delivery before graduation that allows them to become vaccine providers, broadening the scope of practice for the new pharmacists.

What does the future hold for our student pharmacists?

Our graduates are, and will continue to be, exceptionally well-prepared to take on the many challenges in all areas of the healthcare system, whether in community practice, hospital or managed care settings, the pharmaceutical industry, or other evolving opportunities that may arise. Specifically, new opportunities for pharmacists have developed and are growing in managed care settings, where pharmacists perform drug utilization review and medication therapy management to achieve positive patient outcomes.

In addition, PHS pharmacists spoke about their experiences as officers and described career opportunities. You can find PHS pharmacists in federal agencies, such as the Centers for Disease Control and Prevention, Indian Health Service, Bureau of Prisons, National Institutes of Health, Food and Drug Administration (FDA) and Centers for Medicare and Medicaid Services.

The PHS officer who I thought was a pilot turned out to be Commander Jerry Zee, regional pharmacist for the USPHS Commissioned Corps and while there I got to meet with him at the Center for Medicaid and Medicare. He shared his experiences with the Bureau of Prisons and the Indian Health Service, and I learned about the growing role of pharmacists in managed care. I also visited the headquarters of the PHS, the Coast Guard, the Pentagon, the Naval Support Academy, and American Pharmacists Association (APhA).

C

hristopher Mendoza, PharmD and an active duty Lieutenant with the United States Public Health Service (USPHS), received his Doctor of Pharmacy in May 2016. His dream was to become a federal pharmacist. Upon graduation he became a federal pharmacy resident with the U.S. Department of Health and Human Services, stationed in Gallup, New Mexico, one of the top clinical sites within the Indian Health Service. Recently Chris accepted a new position within the IHS, at Zuni Comprehensive Community Health Center, where he is working as a clinical pharmacist. Chris is passionate about his work with the IHS, which is enabling him to learn and grow as a pharmacist as well as make a difference to communities greatly in need of services. At Zuni, he is very excited about embarking on new projects for the Medical Center, including starting a pharmacy-based clinic and offering services to the Zuni Tribe to improve health outcomes and health disparities.

Why did you choose public health pharmacy?

I chose public health because as a federal pharmacist, I am able to impact communities at large as well as individuals. This past year I was with the Gallup Indian Medical Center (GIMC) for my PGY1 residency training and I was able to work in a capacity that positively impacted the lives of the natives of the Navajo Nation and other surrounding tribes.

What did you do during your residency in Gallup? What are you most proud of?

My residency training at GIMC was very strenuous, yet rewarding at the same time. I was

able to rotate through the various pharmacy-based clinics that GIMC offers. The current clinics include anticoagulation, pain, diabetes, immunization, HIV, anemia/nephrology, and asthma. GIMC also offers outpatient, inpatient and ER services.

I am most proud of my residency project, where I was named the GPRA (Government Performance and Results Act) champion of diabetes nephropathy screening. The Act requires federal agencies to show they are using their funds effectively. Monetary awards of $500,000 are given for specific goals that are met. I was able to reach my goal of increasing the number of patients screened for diabetic nephropathy through a pharmacist-led multidisciplinary initiative, bringing in $500,000 to the rural IHS hospital. This was accomplished by creating policies and procedures that enabled outpatient pharmacists and primary care doctors to more efficiently and effectively identify diabetic patients – for example by making sure correct labs were done, and by other means.

What had the greatest impact on you during your time at GIMC? What did you learn? Did anything surprise you?

The greatest impact was seeing the appreciation of the patients who came to the clinics. It made me realize that my hard work and that of others really made a difference. I learned that through these pharmacy clinics, we are able to increase access to health care and decrease health disparities. What surprised me was when my transfer 30 miles south to Zuni was accepted, the administration was very supportive of starting a pharmacy-based clinic to offer services to the Zuni Tribe to

improve health outcomes. This was already envisioned by the new Chief of Pharmacy at Zuni, but they did not have the staff to move ahead.

Now that I am on board, we have enough people and experience to start the clinic.

Why did you change locations and what are your plans for your time at Zuni?

GIMC unfortunately did not have room to keep me. We are planning to start a pharmacy-based comprehensive disease state clinic, implement a 24-hour pharmacy service and start an Antimicrobial Stewardship Committee. The Committee will ensure proper use of antibiotics that correlate with the sensitivity of the bacteria specific to the Zuni tribe. I also have plans to make Zuni a rotation site for pharmacy students and start bringing them here to work with our patients. I am currently working to have an agreement set up with IHS and Touro so that I can precept students from my alma mater, in addition to other pharmacy schools.

How did your choice of residency impact your career long-term?

Residency matured me as a clinician.

I am able to make recommendations with confidence and be an integral part of a person's health care team.

I plan on implementing the skills I learned to better serve the Zuni Tribe. Currently, I plan on working in this setting for a while. I have an affinity for providing health care for those in need, and working as a federal pharmacist within the IHS allows me to do so. The beauty about being on active duty with the USPHS is that I can move among various federal agencies, such as the Food and Drug Administration, Centers for Disease Control and Prevention, or the National Institutes of Health, which I plan to do as my career progresses.
