La bioéthique, moteur de la recherche ?

Les questions d’éthique liées aux soins de santé, objet de réflexions depuis Hippocrate, Sun Si Miao et Ibn Sina (Avicenne), ont été intégrées au domaine de la «bioéthique» après la seconde guerre mondiale. Cela s’explique d’une part par les expériences médicales réalisées par les scientifiques du régime nazi sur les sujets humains, plus particulièrement les prisonniers des camps de concentration, l’entrée en scène du nucléaire, et d’autre part par les progrès fulgurants de la médecine notamment dans les domaines de la génétique, des transplantations d’organe, mais aussi du clonage et de l’utilisation d’embryons humains. De ce fait, l’alerte a été déclenchée par des mouvements des droits civils quant aux buts et limites de la technologie médicale. Ces mouvements ont accordé une attention croissante sur le déséquilibre entre médecin et patient et sur la nécessité de donner au patient le pouvoir de contrôler ses propres décisions en matière de soins.

Dans un article intitulé : « Contribution de l’éthique à la santé publique », Carl H Coleman, Marie-Charlotte Bouësseau et Andreas Reis tracent les frontières entre les domaines de la bioéthique et celui de l’éthique de la santé publique : « Contrairement à l’accent mis traditionnellement par les spécialistes de la bioéthique sur la relation médecin-patient, l’éthique de la santé publique s’intéresse à la conception et à la mise en œuvre de mesures pour contrôler et améliorer la santé des populations. De plus, elle cherche à voir au-delà des soins de santé pour envisager les conditions structurelles qui favorisent ou empêchent le développement de sociétés saines ».

Dans une perspective mondiale et par delà le champ sémantique, les auteurs relèvent dans les détails les principales questions dans le domaine de l’éthique de la santé publique: Inégalités dans la situation sanitaire, accès aux soins de santé et aux bénéfices de la recherche médicale, action face à la menace des maladies infectieuses, coopération internationale pour le contrôle et la surveillance de la santé, exploitation des individus dans les pays à faible revenu, participation, transparence et responsabilisation.

Ce préambule nous amène naturellement à évoquer l’éthique dans le cadre de la recherche scientifique. Les publications en ligne sur ce thème sont innombrables. Si tous les organismes consultés mettent en exergue la complexité du sujet en développant les éléments connexes, combien nombreux, ils ont le grand mérite, et au-delà des idiomes des uns et des autres, de poser dans la pratique les textes normatifs devant réglementer et réguler la recherche scientifique. De plus, ils

engagent dans le sillage de cette entreprise le rôle des divers acteurs dont les institutions scientifiques et juridiques, les organismes de santé nationaux, internationaux et onusiens, les laboratoires biomédicaux… Certains vont même jusqu’à soutenir l’idée d’une formation impérative en éthique de la recherche, sachant que dans le cadre de l’éthique, le processus de prise des décisions est aussi important que leur résultat (participation, transparence et responsabilisation).

Dans le domaine de la recherche médicale, on a accordé ces dernières années une grande attention au renforcement des systèmes de consentement éclairé et de contrôle par la communauté.

Une fois ces systèmes en place, l’étape suivante sera de mettre au point des mécanismes d’évaluation de leur efficacité. Dans certains pays développés ou en voie de développement, les conseils nationaux de la recherche scientifique (CNRS) se voient confier cette tâche, indépendamment ou en association avec d’autres organismes. Certains pays comme la France se sont dotés d’un comité d’éthique du CNRS, créé en 1994. Il s’agit d’une instance consultative indépendante, le COMETS, placée auprès du conseil d’administration du CNRS. Il a pour missions: *« conduire et développer la réflexion sur les aspects éthiques suscitée par la pratique de la recherche sans occulter les finalités de cette dernière ; * dégager, dans le domaine de l’éthique, les principes qui concernent les activités de recherche, les comportements individuels, les attitudes collectives et le fonctionnement des instances de l’organisme ; * formuler des recommandations concernant la définition, la justification et l’application de règles relatives à l’éthique et à la déontologie de la recherche qui peuvent avoir trait aux rapports des personnels entre eux et avec l’organisme, à la communication scientifique interne et externe, et plus généralement à la responsabilité du chercheur devant l’organisme et devant la société, en particulier dans ses activités d’évaluation, de valorisation de la recherche et d’expertise ; * sensibiliser les personnels de recherche à l’importance de l’éthique de façon à garantir un juste équilibre entre leur liberté intellectuelle et leurs devoirs vis-à-vis du CNRS et de la société.

Pour l’accomplissement de ses missions, le comité noue des relations suivies avec les acteurs de l’éthique tant en France qu’à l’étranger. Il se rapproche du médiateur du CNRS afin de prendre en considération les problèmes éthiques d’ordre général qui pourraient être soulevés par les cas individuels ».

Reste à savoir que le comité n’intervient pas dans les controverses scientifiques et ne traite pas les cas individuels.

Cet éditorial est un appel à l’adresse des enseignants chercheurs qui y trouveront des éléments qui dépassent le choix du sujet et la définition des objectifs de la recherche.

Pour formuler ces idées en termes de loi, il s’agit pour le chercheur de rédiger un véritable contrat de résultats et un contrat de moyens. C’est dans ce dernier surtout que les règles d’éthique s’expriment avec acuité, là où la personne humaine est placée au devant de la scène, dans ses dimensions morales, physiques, intellectuelles et sociales. A ce niveau de la réflexion, la dimension éthique et surtout la formation préalable sont les piliers de la construction problématique et de la démarche à adopter.

Is Bioethics the engine of research?

Ethical issues related to health, subject of discussion since Hippocrates, Sun Si Miao and Ibn Sina (Avicenna), were included in the field of “bioethics” after the Second World War. This is explained in part by medical experiments by Nazi scientists on human beings, particularly prisoners in concentration camps, the emergence of nuclear and also by the rapid progress of medical researches particularly in the areas of genetics, organ transplants, cloning and the use of human embryos. Therefore, the alert was triggered by civil rights organizations warning about the objectives and limits of medical technology. These movements have paid increasing attention to the imbalance between doctor and patient and the need to give the patient the power to control his own healthcare decisions.

In an article entitled “The contribution of ethics to public health,” Carl H Coleman, MarieCharlotte Bouësseau and Andreas Reis draw the boundaries between both fields of bioethics and the ethics of public health: “Unlike the traditional emphasis by bioethicists on the doctor-patient relationship, the ethics of public health is concerned with the design and implementation of measures to monitor and improve the health of populations. In addition, it seeks to look beyond healthcare in order to consider the structural conditions that promote or hinder the development of healthy societies”.

From a global perspective and beyond the semantic field, the authors note in detail the main issues in the field of public health ethics: Inequalities in health status, access to healthcare and benefits of medical research, action against the threat of infectious diseases, international cooperation for the control and monitoring of the health of individuals operating in low-income countries, participation, transparency and accountability.

This preamble brings us naturally to think of ethics in the context of scientific research. Online publications consulted are endless. If all the organizations consulted highlight the complexity of the subject by developing numerous related items, they have the merit to establish the normative practice to regulate scientific research. In addition, they engage the role of various actors including legal and scientific institutions, national health agencies, international and UN agencies, biomedical laboratories, ...

Some even support the idea of mandatory training teaching the ethics of research, knowing that in the context of ethics, decision-making process is as important as the outcome (participation, transparency and accountability).

In the field of medical research, it has in recent years given great attention to the strengthening of informed consent systems and community control. Once these systems are in place, the next step is to develop mechanisms for evaluating their effectiveness. In some developed countries or developing countries, national councils dedicated to scientific research are entrusted with this task, independently or in association with other organizations. Some countries such as France have established an ethics committee of the CNRS, created in 1994. It is an independent advisory body, the COMETS, reporting to the Board of CNRS. It aims to: * Lead and develop the reflection on the ethical aspects generated by the research practice without obscuring the purpose of the latter; * Identify, in the field of ethics, the principles concerning research activities, individual behaviors, attitudes and functioning of collective bodies of the organization; * Make recommendations regarding the definition, the rationale as well as the application of rules of ethics and the ethics of the research which may relate to the relationship between team members with each other and with the organization, internal and external scientific communication, and more generally the responsibility of the researcher to the organization and to society, especially in its evaluation activities, promotion of research and expertise. * Educate staff about the importance of research ethics to ensure a balance between their intellectual freedom and duties vis-Ă -vis the CNRS and society.

For the fulfillment of its duties, the committee is forging relationships with stakeholders of ethics both in France and abroad. He approached the mediator of the CNRS to consider the ethical issues of general view that may arise in individual cases.�

What remains to be seen is that the committee is not involved in scientific controversies and does not address individual cases.

This editorial is a call to address teacher-researchers who will find elements beyond the choice of subject and the definition of the research objectives.

To formulate these ideas in terms of law, it is for the researcher to draw up a proper contract of performance and resources. It is in the latter especially that the ethical aspects speak with acuity, where the human being is placed at the forefront in its moral, physical, intellectual and social dimensions. At this level of thinking, ethical and especially pre-service training are the pillars of the construction problem and the approach to adopt.

Résumé Le coiffage pulpaire direct permet de recouvrir l’exposition pulpaire afin de maintenir sa vitalité. Son succès dépend principalement de la capacité du matériau utilisé à amorcer la réparation du tissu pulpaire et la formation de pont dentinaire. Le MTA (agrégat minéral de trioxyde) est un matériau biocompatible qui permet la croissance, la prolifération et la différenciation des cellules souches régénératrices du complexe pulpaire. Toutefois, malgré ses caractéristiques d’étanchéité, de biocompatibilité et son caractère hydrophile, la manipulation du MTA n’est pas facile (état sableux) et son temps de prise est très long (4h).

Un nouveau matériau, le ciment de Portland modifié (CPM), a été élaboré dernièrement; il possède les mêmes caractéristiques du MTA, en plus d’une manipulation plus aisée (consistance pâteuse) et d’un temps de prise plus court. L’objectif de cette étude est de comparer, histologiquement, la réaction du tissu pulpaire après coiffage direct des molaires maxillaires de rats, en utilisant le CPM ou le MTA.

Abstract Capping an exposed pulp in order to maintain the vitality of the pulp is the main purpose of pulp capping procedure. The success of this procedure depends on the capacity of pulp capping agent to stimulate pulp tissue repair and dentin bridge formation.

Mineral Trioxide Aggregate (MTA) is a biocompatible agent that allows growth, proliferation and differentiation of mesenchymal cells in the pulp. Despite its many advantages, the handling of the MTA and its long setting time (4 hours) oblige the clinician to postpone the restoration to another day. A new product, called CPM (ciment de Portland modifié), has been lately in use. It has the same characteristics of the MTA added to an easier handling way and a shorter setting time. The aim of this study is to compare histologically the reaction of the pulp tissue after a direct pulp capping of rat molars, using the CPM or the MTA. Keywords: Pulp capping – MTA – pulp vitality – cement.

introduction Le concept du coiffage pulpaire se base sur le potentiel de réparation et de récupération du tissu pulpaire.

D’après Aeinehchi et coll. [1], le succès de ce traitement dépend de plusieurs facteurs: -l’âge.

-la santé parodontale.

-la phase d’édification et de maturation pulpaire.

-la taille de l’exposition pulpaire.

-la cause de l’exposition (traumatique, mécanique, chimique, thermique, carieuse ou iatrogénique).

-la contamination bactérienne du site exposé.

-le matériau de coiffage utilisé.

Le MTA (agrégat minéral de trioxyde) a été initialement conçu pour obturer toute voie de communication entre le système canalaire et les tissus entourant la dent. Son utilisation est indiquée dans différentes situations cliniques telles que le traitement d’apexification, des résorptions radiculaires internes et des perforations; il sert également comme matériau de coiffage pulpaire (Accorinte et coll.

[2]).

Le MTA est un matériau biocompatible qui permet la croissance, la prolifération et la différenciation des cellules souches régénératrices du complexe pulpaire en odontoblastes.

Toutefois, malgré ses caractéristiques d’étanchéité, de biocompatibilité et de son caractère hydrophile, sa manipulation n’est pas facile (consistance sableuse) et son temps de prise est très long (4 heures). Un nouveau matériau, le ciment de Portland modifié (CPM) a été élaboré par Khalil I. [3]; il possède les mêmes caractéristiques du MTA, mais de manipulation plus aisée (consistance pâteuse après malaxage) et un temps de prise plus court.

L’objectif de cette étude « in vivo » est d’évaluer histologiquement la réaction du tissu pulpaire après un coiffage direct effectué sur des molaires maxillaires de rats, en utilisant le CPM ou le MTA.

matériels et méthodes choix de l’échantillon Dix rats âgés de cinq mois ont été choisis de l’animalerie du laboratoire de recherche en sciences chirurgicales de la faculté de médecine de l’Université Saint-Joseph de Beyrouth.

Étape opératoire Les rats ont été anesthésiés par injection intramusculaire d’une association de Kétamine (0.5 ml) à 50mg/ ml et de Xylasine (1 ml) à 2mg/ml pour assurer une anesthésie générale.

Les deux premières molaires maxillaires gauche et droite ont été traitées chez les rats soumis à l’expérimentation. Les dents cibles ont été désinfectées à l’aide de gluconate de Chlorhexidine à 0.2%.

Une fraise boule diamantée de 0.5 mm de diamètre montée sur turbine a été utilisée pour préparer la cavité d’accès sous irrigation abondante de sérum physiologique. La trépanation a été entamée au milieu de la face occlusale de chaque molaire jusqu’à ce que la pulpe soit visible par transparence à travers le mince plancher de la cavité.

Une exposition pulpaire a été ensuite créée mécaniquement par la pointe d’une lime C+ 15\100 stérile. La cavité d’accès a été irriguée avec du sérum physiologique et asséchée avec des boulettes de coton stériles assurant l’hémostase au site de l’exposition.

Le travail a été effectué sous un agrandissement de trois fois grâce à l’utilisation d’une loupe binoculaire.

Les deux matériaux (MTA et CPM) ont été préparés juste avant utilisation en mélangeant sur un support de verre de la poudre à de l’eau stérile (3 :1) jusqu’à l’obtention d’un gel colloïdal. L’excès d’humidité a été éliminé à l’aide d’une compresse.

Chez un groupe de huit rats, le coiffage pulpaire direct a été effectué par application de CPM au contact direct de la pulpe à l’aide de la pointe fine d’un fouloir métallique. Le matériau a été condensé localement par la pointe des cônes de papiers absorbants stériles. La cavité a été obturée

par du composite. Chez les deux rats restants (témoins), le MTA a été utilisé suivant la même technique de malaxage et de mise en place.

Soins post-opératoires Les rats ont été transportés dans des cages isolées. Du paracétamol a été ajouté à l’eau de boisson pendant 72 heures pour un effet antalgique.

Durant la période d’hébergement, l’eau de boisson a été changée chaque jour et un régime alimentaire normal a été fourni. Deux rats traités avec CPM n’ont pas survécu. Ils ont été exclus de la suite du protocole expérimental.

Sacrifice des rats Après perfusion intracardiaque d’une dose létale de Kétamine (1mL) et de Xylasine (1mL), les rats ont été sacrifiés à des délais post-opératoires différents: -groupe CPM : deux rats sacrifiés à une semaine du traitement, un rat à deux semaines du traitement et trois rats sacrifiés à quatre semaines.

-groupe MTA : les des deux rats ont été sacrifiés après quatre semaines du traitement.

technique histologique Après sacrifice des rats, les dents traitées ont été conservées avec leur support osseux dans du formol à 10% pendant une semaine. Les spécimens ont été ensuite envoyés au laboratoire d’histologie. Le travail au laboratoire d’histologie a obéi à un protocole bien défini selon la technique Exakt®.

L’observation des coupes a été faite à l’aide d’un microscope Olympus CX 41 relié à un appareil photographique numérique (Olympus E330).

résultats Les résultats des dents traitées avec CPM à 1, 2 et 4 semaines sont présentés dans le tableau 1. Un pont de minéralisation n’a été observé qu’après 4 semaines du traitement.

de minéralisation a été détecté au niveau des 4 dents.

Ces résultats ont été confirmés par l’examen histologique puisque, à 1 et 2 semaines du traitement, aucune minéralisation n’était détectable au contact du CPM (Figs. 1, 2). Par contre, les figures 3 et 4 objectivent la formation et le début de minéralisation du pont dentinaire qui s’est formé audessous du CPM et autour des fragments dentinaires après 4 semaines du traitement.

Les figures 5 et 6 représentent des coupes obtenues à partir de dents traitées par du MTA. Elles montrent le début de formation et de minéralisation du pont au contact du MTA.

discussion Cette étude a pour objectif d’évaluer histologiquement la réaction du tissu pulpaire après coiffage direct effectué avec le CPM, par comparaison au MTA, sur des molaires maxillaires de rats.

Jusqu’à présent, plusieurs espèces animales ont été utilisées dans des protocoles expérimentaux de coiffage pulpaire comme les chiens (Tziafas et coll. [4]), les singes (Pitt Ford et coll. [5]), les rats (Kuratate et coll.

[6]) et les souris (Simon et coll. [7]).

Dans le présent protocole, des rats Wistars âgés de 5 mois ont servi de cobayes. L’utilisation des rats dans un

modèle d’étude se heurte à une difficulté majeure notamment la petite taille de la dentition chez cet animal; le diamètre de la molaire (approximativement 1.4 mm) complique la procédure expérimentale en l’absence d’instrumentation adaptée [7].

Cependant, la molaire du rat présente une similarité morphologique avec la molaire humaine puisqu’elle est formée de trois racines et de trois cuspides [7].

Le protocole expérimental appliqué dans la présente étude est proche de celui adopté par De Lovschall et coll. [8] et Simon et coll. [7]. Ces derniers ont perforé la face palatine au milieu du diamètre mésio-distal de la première molaire maxillaire gauche des souris, à l’aide d’une fraise en carbure de tungstène de calibre 0.4 mm jusqu’au plafond de la chambre pulpaire; ils ont ensuite exposé le tissu pulpaire avec la pointe d’une lime C+ 15/100. Le coiffage a été réalisé avec le MTA, puis la cavité a été obturée par du composite. L’étude histologique des tissus prélevés, après sacrifice des animaux à différents délais, a permis de constater le début de formation d’un pont dentinaire entre le CPM et le tissu pulpaire sous-jacent après quatre semaines du traitement.

Dans notre étude, le CPM et le MTA ont été utilisés pour le coiffage.

Les mécanismes d’action de ces deux matériaux pourraient être similaires

puisqu’ils ont à la base des composants identiques - les constituants principaux du ciment de Portland malgré les modifications spécifiques apportées à chacun d’eux. La confirmation de cette hypothèse nécessiterait des études plus élaborées avec des échantillons plus grands.

Le CPM, comme le MTA, contient des composants de calcium et notamment l’oxyde de calcium en quantité importante [9]. Après hydratation, cet oxyde produit de l’hydroxyde de calcium qui favorise la formation d’un pont calcique séparant le produit du tissu pulpaire vivant.

L’efficacité du MTA dans les procédures de coiffage pulpaire a été démontrée dans plusieurs études dont celle de Nair et coll. [10]. Les auteurs avaient effectué un coiffage pulpaire au MTA sur 20 dents de sagesse humaines devant être extraites dans un but orthodontique. Ils ont constaté que durant les trois mois qui ont suivi le traitement, il y a eu formation complète d’un pont dentinaire minéralisé en continuité avec la dentine autour du site exposé et dont l’épaisseur augmentait au fur et à mesure. La qualité de cette dentine devenait similaire à celle de la dentine tubulaire, à partir du troisième mois.

Le succès du coiffage pulpaire dépend de plusieurs facteurs essentiels (Pashley et Liewehr [11]): l’étanchéité du produit de coiffage et sa capacité à

91 Endodontie / Endodontics adhérer hermétiquement à la dentine empêchant par la suite toute percolation bactérienne vers le tissu pulpaire; la biocompatibilité du produit utilisé du fait qu’il est placé au contact du tissu pulpaire [1, 11] et la capacité du produit à stimuler la libération par les dentinoblastes de certaines cytokines qui jouent un rôle dans la formation de tissu dur [10].

L’utilisation de dents saines dans un milieu aseptique pour tester le coiffage pulpaire ne reflète pas la réalité clinique. Cliniquement, le coiffage fait souvent suite à une exposition carieuse pouvant entraîner une inflammation du tissu pulpaire. Pour une analyse plus précise, il serait intéressant d’incorporer les notions « inflammation » et « contamination bactérienne » du site dans le protocole expérimental de coiffage pulpaire direct avec le CPM.Tziafas et coll.

[4] ont exposé le site pulpaire sur des molaires et des prémolaires de

chiens à des Streptocoques Viridans pendant 24 heures avant d’effectuer le coiffage direct avec l’hydroxyde de calcium (Ca(OH)2) et la résine de collage. Après quatre semaines, toutes les dents traitées avec Ca(OH)2 ont conservé la vitalité du tissu pulpaire et ont montré un début de formation d’un pont dentinaire au niveau du site exposé.

conclusion Le coiffage pulpaire direct est considéré actuellement comme un traitement définitif et permanent de l’effraction pulpaire. Dans cette étude, le CPM a montré des qualités comparables au MTA en tant qu’agent de coiffage pulpaire, et cela 4 semaines après son application. Il a permis la conservation de l’intégrité du tissu pulpaire et l’induction de la formation d’un pont dentinaire.

Le présent protocole expérimental a montré le bénéfice de l’utilisation du CPM comme produit de coiffage pulpaire. Cependant, des études plus poussées avec des moyens d’investigations plus élaborés seront indispensables pour mieux étudier la nature et la composition du pont formé au contact direct du CPM et le comparer au tissu dentinaire natif.

Un délai d’expérimentation plus long serait nécessaire pour suivre l’évolution de la formation, la densité, l’épaisseur et la continuité de ce pont avec la dentine.

Résumé La technique d’imagerie Cone Beam ou CBCT présente de multiples avantages par rapport aux moyens conventionnels de radiographie et à la tomodensitométrie, tels que la précision, la faible dosimétrie et la rapidité d’acquisition. Pour l’exploration des articulations temporo-mandibulaires (ATM), elle est de nos jours la technique de choix servant à détecter les diverses anomalies des structures osseuses, qu’elles soient d’origine traumatiques, dégénératives, congénitales, arthritiques ou tumorales, avec une grande précision et sans superposition de structures avoisinantes. Trois cas cliniques sont rapportés pour illustrer les avantages et la supériorité du Cone Beam dans le diagnostic des troubles des ATM. Mots-clés : radiographie - articulation temporo-mandibulaire.

Abstract Cone Beam Computed Tomography or CBCT has multiple advantages in comparison with conventional means of radiography and CT scan, such as precision, low radiation and rapidity of aquisition. For temporo-mandibular joints (TMJ) exploration, it is nowadays the best technique used to detect various anomalies of the bony structures, whether of traumatic, degenerative, congenital, arthritic or tumoral origin, with great precision and without superimposition of neighboring anatomical structures.

Three clinical cases are reported to illustrate the advantages and superiority of the Cone Beam in the diagnosis of TMJ disorders.

introduction La plupart des centres de Radiologie Maxillo-Faciale se sont dotés depuis un certain temps d’un outil diagnostique qui constitue une avancée de taille sur la voie de l’imagerie maxillofaciale, et particulièrement dans l’exploration des articulations temporomandibulaires (ATM). Il s’agit de la Tomographie Volumique Numérisée

93 Radiologie / Radiology à Faisceau Conique ou Cone Beam Computerized Tomography (CBCT), ou plus simplement Cone Beam - appellation que nous allons adopter tout au long de cette étude- . Cette technique d’imagerie devient de plus en plus indiquée en pratique maxillo-faciale.

Comparé aux techniques radiographiques conventionnelles comme la panoramique, la transcrânienne et les coupes Scanora, le Cone Beam permet une évaluation morphologique dynamique et surtout tridimensionnelle par le développement de modèles virtuels en 3D, tout en délivrant une quantité considérablement moindre de rayons X. La prise de clichés est plus facile et plus confortable qu’au moyen de la tomodensitométrie (scanner), le coût en est nettement moindre; enfin, la qualité de l’image est de loin meilleure [1]. Contrairement à la radiographie conventionnelle qui donne des images avec superposition de structures osseuses adjacentes rendant l’interprétation difficile et peu précise, la technique Cone Beam permet de visualiser clairement les ATM sans interférence de structures anatomiques avoisinantes [2]. Elle permet en outre de détecter les modifications osseuses mineures (indétectables au moyen des techniques radiographiques conventionnelles) [3]. La technique de reconstruction en 3D, d’intérêt didactique, permet néanmoins aux praticiens une meilleure compréhension spatiale de l’image.

Enfin, le Cone Beam est un moyen précis d’évaluation du volume osseux [3] pour établir, à titre indicatif, la modification volumétrique du condyle résultant de diverses pathologies des ATM. Les données volumétriques obtenues sont utiles pour comparer les côtés droit et gauche de la mandibule et être en mesure d’évaluer l’asymétrie entre les deux condyles dans les divers types de malocclusion. Plus généralement, il rend possible la réalisation de coupes épaisses comme une reconstruction panoramique volumique; la sélection des densités les plus élevées permet d’isoler les structures osseuses et dentaires [4].

intérêt du cone beam dans les diverses pathologies de l’atm À l’issue d’une évaluation clinique, la technique Cone Beam est recommandée en cas de besoin d’évaluation osseuse dynamique (en bouche fermée et en bouche ouverte) en 3D, comparative des deux ATM, avec mesure de l’amplitude du déplacement des condyles mandibulaires et de l’ouverture maximale de la bouche. Plus particulièrement, le Cone Beam est indiqué pour le diagnostic radiologique des pathologies suivantes:

dysfonctionnements temporo-mandibulaires Syndrome algo-dysfonctionnel de l’appareil manducateur (Sadam) Pathologie la plus fréquente causée par un défaut d’adaptation de l’appareil manducateur, dû notamment à des troubles posturaux, un traitement orthodontique ou une chirurgie orthognatique provoquant des altérations de la position du condyle mandibulaire.

L’examen clinique et le bilan occlusal demeurent les moyens les plus utilisés pour diagnostiquer cette pathologie.

Toutefois, l’étude dynamique à l’aide du Cone Beam peut servir à révéler un blocage ou une insuffisance du déplacement condylien mandibulaire [4,5].

ankylose temporo-malaire Causée par un obstacle mécanique squelettique ou attaché au squelette empêchant l’ouverture buccale. Le Cone Beam est notamment indiqué pour le diagnostic de l’hypertrophie des processus coronoïdes, l’hyperostose du malaire bloquant les coronés, un chondrome ou une exostose du processus coronoïde, une myosite ossifiante,... Le Cone Beam constitue un moyen fiable de diagnostic préchirurgical [4].

malformations congénitales de l’atm Elles peuvent aller de la simple asymétrie morphologique aux anomalies majeures des syndromes oto-mandibulaires: le Cone Beam permet de détecter avec précision l’aspect caractéristique de la région temporo-mandibulaire dans les cas de fentes latérales, ainsi que l’hypoplasie du ramus, la dysplasie, l’hypoplasie ou l’aplasie du processus condylien mandibulaire (absence de cavité temporale) [6]. Il se révèle aussi un outil précieux dans le diagnostic de l’hypercondylie, qui provoque une déviation du menton vers le côté sain et une malocclusion [4].

arthropathies manifestations dégénératives arthrosiques Elles résultent de troubles chroniques du triplet fonctionnel ou d’un traumatisme [4]. Le Cone Beam est indiqué pour diagnostiquer ces manifestations [7] en révélant un pincement de l’interligne articulaire entre le condyle temporal et la cavité glénoïde, l’ostéocondensation des surfaces osseuses ou la présence d’ostéophytes (cas clinique n°2), ou même, très rarement, une ostéonécrose [8].

les arthrites inflammatoires L’imagerie par Cone Beam permet de diagnostiquer les arthrites inflammatoires (modifications squelettiques telles que l’irrégularité des surfaces, la déminéralisation osseuse,…) ou infectieuses pouvant notamment causer des ankyloses [4].

lésions traumatiques des atm Les fractures condyliennes résultent le plus souvent d’un choc du menton provoquant un choc entre les condyles et la base du crâne. Quand la violence du choc dépasse le seuil d’absorption de l’énergie, des fractures articulaires

et extra-articulaires [4] ou des ankyloses s’ensuivent. Le Cone Beam vient completer l’examen clinique rigoureux, afin de détecter un déplacement latéral (cas n°1), une bascule interne condylienne (cas n°3) ou le détachement d’un fragment osseux, à l’aide de reconstructions axiales, coronales et sagittales.

tumeurs Les tumeurs sont peu fréquentes au niveau de l’ATM et sont souvent bénignes (ostéomes, exostoses, ostéochondromes [9] …). L’imagerie par Cone Beam permet de faire le bilan d’extension loco-régional d’une tumeur alors que l’IRM est la technique de choix pour mieux définir les limites d’envahissement des parties molles [4].

cas cliniques Les trois cas cliniques présentés ont été explorés par un appareil Cone Beam (NewTom VGI) (Fig. 1), en grand champ de vue pour une vision globale des ATM et des deux maxillaires. La définition moyenne de l’image est de l’ordre de 250 μm.

Les coupes réalisées sont de l’ordre de 0,3 mm d’épaisseur pour les reconstructions axiales, et de 1 mm (coupes jointives) pour les reconstructions sagittales et coronales, ce qui se traduit par une plus grande précision et une absence de chevauchement des structures avoisinantes.

1er cas : lésion traumatique de l’atm Suite à une chute sur le menton, une jeune femme a été transportée au service d’urgence d’un hôpital de la ville qui s’est chargé de soigner la plaie cutanée. Le lendemain, la patiente s’est réveillée avec une douleur intense au niveau de l’articulation du côté gauche et une inocclusion unilatérale droite. Cette douleur était réfractaire aux antalgiques de niveau 1 et aux anti-inflammatoires (Advil 400mg) et ne cessait d’augmenter d’intensité jusqu’à devenir insoutenable, quatre semaines plus tard. Un cliché panoramique pris à la demande de l’occlusodontiste n’avait révélé aucun trait de fracture (Fig. 2).

Par ailleurs, et comme la durée de l’inocclusion s’était prolongée au-delà de deux à trois jours, l’occlusodontiste avait écarté l’hypothèse du réflexe d’éclissage ou myospasme du masséter profond qui est une réaction de rigidité musculaire secondaire à l’inflammation articulaire tendant à protéger l’articulation par une immobilisation réflexe et se traduisant essentiellement par des douleurs musculaires accompagnées d’une limitation de l’ouverture buccale. Comme

la douleur persistait, elle ne pouvait s’expliquer que par la présence d’une fracture. C’est alors que la patiente nous a été référée pour un examen plus poussé à l’aide du Cone Beam. L’examen a été conduit en bouche fermée avec un grand champ de vue, au moyen de coupes sagittales reconstruites perpendiculaires au grand axe du condyle, de coupes coronales parallèles au grand axe, de coupes axiales et de coupes panoramiques. Sur les reconstructions axiales (Fig. 3), un trait de fracture a été relevé au niveau du condyle gauche. Sur les reconstructions panoramiques et sagittales (Fig. 4), ce trait de fracture se prolongeait du milieu de l’échancrure sigmoïde, en passant au-dessous du col du condyle gauche jusqu’au bord postérieur du ramus. Les reconstructions coronales (Fig. 5) montraient, en plus de la fracture, un déplacement. La fracture apparaissait aussi de manière évidente sur les images tridimensionnelles reconstituées (Fig. 6).

2e cas : arthrose Une femme âgée de 50 ans se plaignait de douleurs chroniques condyliennes gauches vieilles de cinq ans avec des épisodes paroxystiques aigus. L’examen buccal montrait une édentation unilatérale postérieure gauche. Le cliché panoramique (Fig. 7) avait révélé une forme anatomique atypique du condyle mandibulaire gauche; toutefois ces informations demeuraient insuffisantes pour pouvoir établir un diagnostic précis. À la demande de l’occlusodontiste, une exploration plus approfondie des structures osseuses au moyen du Cone Beam en bouche fermée avec un grand champ de vue a été réalisée. Les reconstructions sagittales (Fig. 8) montraient un condyle mandibulaire gauche complètement abrasé avec des érosions de la tête condylienne et un ostéophyte antérieur. De même, sur les reconstructions coronales (Fig. 9), on notait un aplatissement du condyle et des érosions de la tête condylienne. Les éléments repérés par le biais de l’imagerie et couplés aux données de l’anamnèse et de l’examen clinique avaient permis de diagnostiquer une arthrose articulaire. Les images tridimensionnelles reconstituées (Fig. 10) étaient encore plus explicites.

3e cas : arthropathie d’origine traumatique Une femme de 75 ans souffrait d’une déviation mandibulaire droite à l’ouverture buccale associée à une douleur à la mastication. Victime d’un accident de la voie publique elle avait été soumise à une chirurgie réparatrice réalisée dans le secteur des prémolaires mandibulaires gauches. Le cliché panoramique avait révélé une morphologie condylienne atypique du côté droit difficilement cernable sur ce cliché (Fig. 11). L’imagerie Cone Beam en bouche fermée a révélé sur la reconstruction axiale une fracture de la tête condylienne droite qui a basculé du côté interne (Fig. 12).

Les reconstructions sagittales ont montré une érosion de la tête condylienne droite et un rétrécissement de l’espace articulaire (Fig. 13). Ces images suggéraient volontiers une arthropathie post-traumatique. Les reconstructions coronales droites montraient la partie condylienne fracturée et confirmaient de ce fait la bascule interne de ce fragment (Fig. 14). Sur les reconstructions 3D de surface on pouvait distinguer clairement le fragment fracturé (Fig. 15).

discussion Les cas décrits mettent en évidence l’importance du Cone Beam pour la détection de pathologies osseuses dégénératives et des fractures traumatiques que les moyens de radiologie conventionnelle n’ont pas réussies ou suffi à diagnostiquer.

Dans le premier cas, le cliché panoramique n’avait pas détecté une fracture.

La persistance de l’inocclusion pour une durée supérieure à 2 ou 3 jours suggérait l’existence d’une fracture, sinon le réflexe d’éclissage aurait rétabli l’occlusion. Le recours aux techniques de l’imagerie par Cone Beam a été rendu incontournable pour confirmer cette hypothèse diagnostique.

Dans le deuxième cas, les érosions et l’ostéophyte n’ont pas non plus été relevés au moyen du cliché panoramique; l’histoire et l’examen cliniques du patient avaient suggéré l’existence d’une arthrose, d’autant que l’édentation unilatérale était l’un des facteurs pouvant provoquer et/ou aggraver l’arthrose. En effet, l’édentation du côté droit pourrait avoir provoqué l’arthrose du côté gauche.

Le cliché panoramique n’a pas non plus montré la fracture et la modification morphologique du condyle dans le troisième cas. Néanmoins, l’historique de la patiente comportant un traumatisme au niveau de la mandibule pouvait évoquer le développement d’une arthropathie.

conclusion La technique du Cone Beam s’est avérée très précise et satisfaisante pour la détection des pathologies dégénératives, des lésions traumatiques et d’autres troubles des ATM. Les techniques tomographiques (scanora, transcrânienne) sont devenues obsolètes en regard des performances considérables du Cone Beam. La radiographie panoramique trouve encore sa place comme examen de première intention pour évaluer l’état global des dents et des maxillaires ainsi que la morphologie des condyles mandibulaires et une éventuelle asymétrie entre les deux côtés.

chairSidE diaGnoStic tESt KitS in pEriodonticS - a rEviEw Sachin malagi*

Abstract Traditionally, periodontal diseases have been diagnosed by radiographic and clinical exams, including the assessment of plaque with a plaque index (PI), gingival inflammation with the bleeding on probing index (BOP), probing pocket depth (PPD) and clinical attachment loss (CAL). Nowadays, clinical chairside tests are in use for more precise molecular diagnostics and treatments. This review describes some diagnostic test kits on the market that can facilitate the clinical exams and the establishment of a diagnosis.

Résumé Traditionnellement, les maladies parodontales sont diagnostiquées par des examens radiologiques et cliniques en utilisant certains indices pour bien déterminer la sévérité et l’importance des atteintes : indice de plaque (IP), indice de saignement (IS), profondeur de poche (PP) et niveau d’attachement (NA). Un inconvénient majeur de ces évaluations est leur incapacité de déterminer les phases actives ou stables de la maladie parodontale. Actuellement, des tests cliniques peuvent être utilisés au cabinet dentaire pour des diagnostics moléculaires plus précis permettant ainsi des diagnostics précoces et des traitements plus prompts. Ce papier est une revue descriptive de quelques tests disponibles sur le marché dont l’usage facilite l’examen clinique et l’élaboration d’un diagnostic.

introduction Periodontitis is a prevalent disease of men characterized by loss of connective tissue attachment and bone around the teeth, in conjunction with the formation of periodontal pockets due to apical migration of the junctional epithelium. Periodontal disease progression is episodic [1] and sitespecific [2].

More than 200 species of microorganisms colonize the oral cavity, but only a few of these are thought to

be pathogens [3]. Among the subgingival bacterial species identified so far, Porphyromonas gingivalis (Pg), Prevotella intermedia (Pi) and Aggregatibacter actinomycetemcomitans (Aa) have been associated with progressive periodontitis [4]. Aa is infrequently found in periodontally healthy individuals [5], whereas Pi has been found in either healthy subjects or patients with gingivitis [6].

Consequently, elevated levels of these putative pathogens may be useful indicators of both active periodontitis and

increased risk of gingival attachment loss. However, knowledge of how their numbers relate to disease progression is still unclear and for longitudinal studies accurate assessment of their numbers in clinical samples is needed.

The goal of periodontal diagnostic procedures is to provide useful information to the clinician regarding the present periodontal disease type, location and severity. These findings serve as a basis for treatment planning and provide essential data during periodontal maintenance and disease

Traditional clinical measurements (probing pocket depth, bleeding on probing, clinical attachment loss, plaque index, radiographs) used for periodontal diagnosis are often of limited usefulness in that they are indicators of previous periodontal disease rather than present disease activity.

The 1990s have seen the emergence of a multitude of diagnostic tests based on physical, chemical, microbiological and immunological methodologies.

The philosophy behind the emergence of such tests is that the earlier active disease is diagnosed, the less invasive, time consuming, and therefore costly the required treatment, and the better the long term prognosis for patients with destructive disease [7].

Furthermore, with the recognition that risk groups and unpredictable disease patterns exist, the benefits of objective testing for initial diagnosis and for the long-term maintenance of periodontal patients become clear. For periodontal diagnosis, the ideal diagnostic test should be [8]: 1. Quantitative.

2. Highly sensitive method capable of analyzing a single periodontal site in health as well as disease.

3. Reproducible.

4. Highly specific.

5. Simple to perform.

6. A rapid, one or two stage procedure.

7. Non-invasive.

8. Versatile in terms of sample handling, storage and transport.

9. Amendable to chairside use.

10. Economical.

11. Dependent upon simple and robust instrumentation.

Several methods have been employed to detect putative periodontopathogens in clinical samples. These include cultural methods, microscopy, immunofluorescent assays, enzyme-linked immunosorbent assays, trypsin-like protease assays, DNA probes [9] and the PCR [10].

Among these tests, chairside periodontal kits provide immediate reports of the microflora associated with the disease compared to cumbersome and

time-consuming traditional laboratory procedures.

microbiological test kits The microbiological tests have the potential to support the diagnosis of various forms of periodontal disease, to serve as indicators of disease initiation and progression and to determine which periodontal sites are at higher risk for active destruction. The bacteriological tests (Microscopy, Culture, Omnigene, Affirm DP and Evalusite) are mainly aimed at spirochetes, Aa, Pg and Pi. Microbial tests can also be used to monitor periodontal therapy directed towards the suppression or eradication of periodontopathogenic organisms.

omnigene (fig. 1) These are DNA probe systems for a number of known periodontopathogen subgingival bacteria. A paper point sample of sub-gingival plaque is placed in the container provided and mailed off to the company for assay.

Probes are available for the detection of A. actinomycetemcomitans, P. gingivalis, P. intermedia, F. nucleatum, C. rectus, T. denticola and E. corrodens.

Reports are provided within very short time periods (few hours to few days).

Evalusite Evalusite is a kit that employs a novel membrane-based enzyme immunoassay for the detection of three putative periodontopathogens: Aa, Pg and Pi. A sub-gingival sample is collected using paper points and added to a sample tube. The eluent is then added to the kit, which employs a sandwich-type ELISA (enzyme-linked immunosorbent

101 Parodontologie / Periodontology assay); a pink spot is displayed if the test organism is present. The main weaknesses of this test kit reside in 1) the assumption that the three detected organisms are causing disease; (2) it is a multistage test; (3) it has a subjective calorimetric end point and (4) there is no permanent record of the results [11].

perioScan® Perioscan is a diagnostic test kit that utilizes the BANA (N-benzoyl-DLarginine-2-naphthylamide)-hydrolysis reaction, developed to detect bacterial trypsin-like proteases in the dental plaque (Fig. 2). A trypsin-like activity has been identified in strains of P. gingivalis, T. denticola, T. forsythia and some Capnocytophagia strains [12, 13]. BANA is an example of a substrateconjugated beta-nepthylamine (p-NA), which is hydrolyzed by this trypsin-like enzyme to release free p-NA. The latter is a chromophore and reacts with a variety of dyes (e.g. Fast-Garnet GBC) to produce colored products.

Subgingival plaque is collected and placed on a BANA-containing strip, which is then folded to contact a second strip containing the “FastBlack” dye reagent. The folded card is placed inside an oven for 15 min at 55°C and any blue-black color that appears is scored positive for the above species. The sensitivity of the method has recently been improved [14]. The main disadvantage of this technique is that it relies upon plaque sampling and assumes that the test organisms identified as being present signify active disease. This is known not to be the case for all patients and sites.

Furthermore, results are qualitative and rely upon the operator’s assessment of the calorimetric end point.

One of the potential difficulties of this test is that it may be positive at clinically healthy sites and might remain so after treatment.

The problem with all microbiological tests is that it is likely that a range of interacting bacteria is capable of causing disease, the combination may

differ between individuals, and tests directed at individual species may be too specific, and may not account for different clonal types.

biochemical test kits Biochemical test kits used in periodontics analyze the gingival crevicular fluid (GCF). Since this fluid is derived from periodontal tissues, evaluating its constituents such as host-derived enzymes, inflammation mediators and extracellular matrix components may provide early signs of alterations.

perio 2000 Various pathogenic microorganisms like P. gingivalis, P. intermedia and T. forsythia produce sulphates, thereby significant levels of volatile sulphide compounds (VSCs) by degradation of serum proteins: cysteine and methionine. Since these VSCs can directly degrade periodontal structures aggravating periodontitis, their evaluation can indicate the subgingival microbial load. Perio 2000 system is designed to display the sulphide level digitally at each site. In brief, the probe tip should be hydrated using sterile wash solution provided by the manufacturer then inserted subgingivally at peak or hold operational mode. After a positive reading, the tip is washed and reinserted in other subgingival site.

prognos-Stik This test kit was released in the year 1993. It detects elevated levels of MMPs in the gingival crevicular fluid such as the elastases. The GCF is collected onto the filter paper strip impregnated with a known amount of buffered elastase substrate labeled with a fluorescent indicator. Elastase on the test strip cleaves the substrate during the reaction time of 4-6 minutes and releases the indicator, visible under fluorescent light. Elastase is released from the lysosomes of polymorphonuclear leucocytes which accumulate at sites of gingival inflammation. The presence of elevated levels of elastase in GCF may thus be indicative of

active disease sites [15]. Although a relationship between elastase levels in GCF and periodontal disease activity has been reported, the position is still far from clear. Further clinical trials are needed before the value of this test kit in clinical practice can be ascertained.

perio-check Periocheck has FDA (Food and Drug Administration) approval in the United States. It is reported to measure neutral protease activity within GCF. The GCF sample strip is placed on a gel containing insoluble dye-labelled collagen fibrils (remazobrilliant blue-collagen substrate powder) and incubated.

In the presence of neutral proteases (which diffuse from the strip into the gel), the insoluble collagen-dye complex is digested to release soluble dyelabelled fragments, which diffuse back into the strip, turning it blue. Again the test is only qualitative and not specific for PMNL collagenase, which is thought to be the dominant collagenase at active sites [16]. Indeed, a high proportion of the enzyme is likely to be bacterial in origin. Furthermore, interproximal sites cannot be sampled, due to the risk of saliva contamination, and this is clearly a major drawback with this method. It is the most rapid chairside test for neutral proteases in GCF like elastases, proteinases and collagenases. The levels of these enzymes in GCF have been noted to increase with the development of gingivitis as well as sites of established periodontitis.

perioGard PerioGard is based on the detection of an enzyme called aspartate aminotransferase (AST). AST is a soluble intracellular cytoplasmic enzyme that is released from within the cell upon its death. Since cell death is an important part of periodontal pathogenesis, AST levels in GCF have great potential as markers of early periodontal tissue destruction. Elevated total AST levels in a 30-second sample have been positively associated with disease-active sites in contrast to inactive sites [17, 18]. This commercial test consists of a

Mises au point | In-depth review tray with two test wells for each tooth, and appropriate reagent for conducting the test. The test involves collection of GCF with the filter paper strip which is then placed in tromethamine hydrochloride buffer. A substrate reaction mixture containing 1-aspartic and α-keto-gluteric acid is added to the sample and allowed to react for ten minutes. In the presence of AST, the Aspartate and α-keto-gluteric acid are catalyzed to oxaloacetate and glutamate. The addition of a dye such as fast red results in a color product, the intensity of which is proportional to the AST activity in the GCF sample [19].

In practice, the PerioGard assay suffers from poor differentiation between colors and is a relatively complex procedure involving multiple steps.

pocket watch The Pocket Watch was developed as a simple method of analyzing AST at the chairside [20]. The principle of this test is that, in the presence of pyridoxal phosphate, AST catalyzes the transfer of an amino group of cysteine sulfuric acid by α- keto- gluteric acid to yield β-sulfinyl pyruvate.

Glutamate β-sulfinyl pyruvate spontaneously and rapidly decomposes and releases inorganic sulfite. The sulfite ion instantaneously reacts with malachite green (MG), simultaneously causing MG to convert from a green dye to its colorless form, thereby allowing the pink–colored rhodamine B dye to show through. The rate of conversion of MG is directly proportional to AST concentration. However, components of the extracellular matrix and its dissolved products are present in GCF of destructive pockets, and they may release sulfide ions. In brief, AST activity determined by Pocket Watch provides not only an index of cell death but of the extent of the destructive pockets.

[21] found an association between the polymorphism in the genes encoding for interleukin-1α and interleukin-1β and increased severity of periodontitis.

Identification of the genetic polymorphism is difficult but now some chairside kits are available for its detection.

pSt® genetic susceptibility test Periodontal susceptibility test (PST®) is the first and only genetic test that analyzes two interleukins (IL-1α and IL-1β) genes for variations. IL-1 genetic susceptibility may not initiate or cause the disease but rather may lead to earlier or more severe disease. The IL-1 genetic test can be used to differentiate certain IL-1 genotypes associated with varying inflammatory responses to identify individuals at risk for severe periodontal disease even before the age of 60. Clinically, PST® is used in [22]: • New periodontal patients to assist in developing treatment plans.

• Patients requiring extensive periodontal and/or implant therapy to determine prognosis, improve patient acceptance and optimize treatment outcomes.

• Smoking patients as an additional incentive for smoking cessation.

• Maintenance patients to set recall intervals and improve compliance.

• Patients with early signs of disease to help determine the need for referral to a specialist.

Genetic test kits

In periodontology, the success of any treatment is dependent upon the accuracy of the initial diagnosis.

At present, the majority of chronic periodontitis cases can be adequately managed using existing diagnostic methodology, although it is clearly more desirable to be able to diagnose “active disease” as it occurs, rather than months later.

ted. The newly commercially available chairside tests for host and bacterial markers of periodontal disease offer prospects which would make the monitoring of specific sites possible.

However, the clinician must ensure that the use of such tests will benefit the patients in terms of both the value of diagnostic data obtained and the cost in time and money.

Validation of novel periodontal diagnostics need to be benchmarked with existing gold standards of the disease, such as alveolar bone levels and clinical attachment levels, in large populations.

EwinG’S Sarcoma of thE mandiblE: a caSE rEport bhandarkar Gowri pandarinath* | Shetty Kushal** Abstract Ewing’s sarcoma of the jaw is a primitive malignant tumor of the bone. It is commonly seen in children and adolescents. Its occurrence in the head and neck region is unusual and generally involves most frequently the mandible rather than the maxilla.

The present case discuss the clinical, diagnostic (radiographic, histopathologic) and therapeutic findings of an ES of the mandible in a 14-year-old boy. Early detection of such lesions is difficult because the signs and the symptoms do not appear until the lesion has progressed considerably. This case elucidates the importance of professional knowledge of the relevant aspects of malignant lesions such as Ewing’s sarcoma.

introduction Ewing’s sarcoma (ES) was originally described by James Ewing in 1921 as arising from undifferentiated osseous mesenchymal cells. Ewing stated that the tumor was not associated with any form of myeloma or osteogenic sarcoma and he thus termed it a “round cell sarcoma”. However, recent studies suggest that Ewing’s tumor might be neuro-ectodermally derived from various degrees of differentiation of the primitive neural tissues. Clinically, this tumor has aggressive behaviour,

Résumé Le sarcome d’Ewing de la mâchoire est une tumeur rare primitive et maligne de l’os. Il est communément observé chez les enfants et les adolescents. Sa survenue au niveau de la région de la tête et du cou est inhabituelle et implique plus souvent la mandibule que le maxillaire. Le cas présenté siège à la mandibule chez un garçon de 14 ans; les éléments cliniques, diagnostiques (radiographie, histopathologie) et thérapeutiques sont développés et discutés. La détection précoce de ces lésions est difficile car les signes et les symptômes n’apparaissent que tardivement, après une progression considérable de la lésion. Ce cas révèle l’importance d’une bonne connaissance des particularités des lésions malignes en général et du sarcome d’Ewing en particulier.

characterized by rapid growth and high probability of micrometastasis.

Approximately 80% of cases occur in the first 2 decades of life [1] with a 2:1 male to female ratio. The majority of the patients affected are between 5 and 20 years whereas it is rarely seen before the age of 5 and after the age of 30 years. It exhibits a marked predilection for Whites and is rarely seen among Blacks. These lesions account for 4-15% of all primary bone tumors [2] and 1% of all malignant tumors in children. 2/3 of all cases appear in the lower skeleton [3] with a predisposition for long bones of the extremities

and the pelvis; the involvement of facial skeleton is very rare (3%) [2], the mandible being the most commonly affected bone [4, 5].

case report A 14-year-old boy visited the Department of Oral Medicine and Radiology at Bangalore Institute of Dental Sciences, Bangalore, with a chief complaint of a 6 months’ history of a painful, rapid, progressively enlarging swelling in the right mandible associated with relapsing fever at night. On clinical examination, the

patient was unable to close the mouth with paresthesia of the lower 1/3rd of the right side of the face (V3). Extraorally, the right side of the face was grotesquely deformed with a diffuse, hard fixed, expansive mass (Fig. 1a).

The tumor was extending superiorly up to 2 cm below the ala- tragus line and inferiorly below the inferior border of the mandible involving the submandibular region. Anteriorly, the tumor was extending up to the midline, crossing it and reaching the opposite side of the mandible up to the left parasymphyseal region. Posteriorly it extended to the right angle of the mandible.

Overlying skin was distended and slightly erythematous. The tumor was hard to firm in consistency.

Intra-oral examination revealed an expansile mass growing mainly against the vestibular plate of the mandible, infiltrating the gingiva and the floor of the mouth. It was extending from the right retromolar area to the left mandibular premolars (Fig. 1b). The tumor was submucosal, and obstructing occlusion of the dental arch with displacement of the tongue to the left.

The overlying mucosa was distended and erythematous. Ulceration was noted in posterior areas. On palpation, the swelling was non-tender and hard to firm in consistency. Bidigital palpation revealed expansion of the buccal cortical plate and perforation of buccal and lingual cortex. Grade II mobility and negative sensibility were

seen in the teeth from right mandibular second molars to left mandibular lateral incisor. No significant palpable lymph nodes were found in the neck.

Chest radiograph revealed no abnormal findings. Mandibular occlusal radiograph showed poorly demarcated destructive lesion with labially displaced anterior teeth (floating teeth appearance) and eroded buccal and lingual cortical plates (Fig. 2). CT scan of the mandible revealed osteolytic lesion eroding the mandibular bone on its right side (Fig. 3).

Given the clinical and radiographic findings, the differential diagnosis included Ewing’s sarcoma, osteogenic sarcoma, rhabdomyosarcoma,

The tumor cells were round with round nuclei, basophilic nucleoli and granular cytoplasm.

Hodgkin’s lymphoma, non-Hodgkin’s lymphoma and metastatic diseases.

Haematological and biochemical investigations were within normal limits except for thrombocytosis and increased alkaline phosphatase levels.

Haematopathologic report of bone marrow revealed no involvement of marrow.

Under local anaesthesia, patient underwent an incisional biopsy. On histopathological microscopic examination, H and E stained sections showed two bits of infiltrating tumor tissue with the tumor cells arranged in diffuse sheets and trabeculae separated by fibroconnective septa. The tumor cells were predominantly round with round nuclei, basophilic nucleoli and granular cytoplasm. Few hyperchromatic cells with scanty cytoplasm were also seen (Fig. 4). PAS stain was negative for glycogen.

Immunohistochemistry was done and the report revealed CK-focal positive; CD20, CD3 and tdt and MPO were negative. Desmin, Chromogranin and Synaptophysin were also negative.

Mic2 showed membrane positivity.

These findings were consistent with Ewing’s sarcoma of the mandible.

Cytogenetics was suggested and their report revealed 46, XY, t(11,22) (q24, q12). Other immunomarkers were all negative. Histopathological, immunohistochemical and cytogenetical findings supported the diagnosis of ES. The patient started a protocol consisting of antineoplastic agents: Vincristin

and Endoxan. After 2 months, the patient underwent surgery with wide resection of the affected area. Viable tumor was present in the resected mandible and all margins were negative. Histopathologic report confirmed the diagnosis of Ewing’s sarcoma.

After the operation, the patient’s chemotherapy protocol was continued for 4 months. The patient was lost for follow-up.

discussion James Ewing first described Ewing’s sarcoma (ES) in 1921, after observing radiosensitivity in a subgroup of bone tumors. Ewing’s sarcoma is the second most common primary malignant tumor of the bone in children and adolescents [2]. The site of predilection is mostly the posterior regions with a 4:1 ratio compared to anterior regions [3]. In the head and neck region, there are non-specific clinical findings for ES; most of the patient’s complaints at time of presentation are due to the mass effect of the tumor, its rapid growth, the swelling of the affected area and the pain sensation. When this tumor arises in the mandible, other signs and symptoms such as loosening of the teeth, otitis and paraesthesia may be observed. Systemic symptoms such as fever, often remittent about 38 °C, weight loss and anemia can be also noticed.

Ewing’s sarcoma is a poorly differentiated neurectodermal tumor with

small, round and blue cells [6]. More than 90% of cases show a characteristic translocation t(11;22) (q24;q12) resulting in the fusion of the EWS and FLI-1 genes [7]. This gene rearrangement causes a fusion product which functions as an oncogenic aberrant transcription factor with structural variability and potentially prognostic impact.

Immmunoreactivity against FLI 1 and CD99 can confirm the diagnosis [6].

The present case exhibited clinical aspects similar to those reported in the literature for cases in the head and neck region: it was located in the mandible, displayed a rapidly progressive growth and was accompanied by pain and fever. This presentation is not specific, but it demands complementary exams that can avoid large errors in the diagnosis.

Radiographically, the lesion is poorly defined, permeative, osteolytic and may be frequently associated with cortical erosion. This aspect is not a pathognomonic feature, as other lesions can have the same image pattern such as osteogenic sarcoma, neuroblastoma, lymphosarcoma, histiocytosis X, rhabdomyosarcoma, pyogenic osteomyelitis and metastatic carcinoma [3]. The presence of “sunray” spicules of periosteal bone and displacement or destruction of unerupted tooth follicles have been described as the commonest radiological features for ES affecting jaw bones. But some authors reported an occasional “sun- ray” appearance [5, 8]. Further, the presence of the laminar periosteal response (known as “onion skin” reaction), pointed out by many authors as a common radiological feature of ES of the long bones, is rarely seen in the jaw lesions [9]. In our case, neither “sun-ray” spicules of periosteal bone nor an “onion skin” reaction of cortical bone were observed. The osteolytic lesion was associated with severe cortical erosion of the mandible.

Histologically, ES is composed of small nucleoli and scanty cytoplasm.

The intra-cytoplasmic glycogen may be demonstrated by PAS stain in 75% of the cases. Biopsy is necessary to

107 Médecine Orale / Oral Medicine make a definitive diagnosis and the tumor must be distinguished from microscopically similar tumors and conditions such as eosinophilic granuloma, malignant lymphoma, metastatic neuroblastoma and rhabdomyosarcoma [10]. Recent advances in electron microscopy, immunohistochemical, cytogenetic and molecular genetic techniques have increased the accuracy of a differential diagnosis of Ewing’s sarcoma. In our case, the use of immunohistochemistry and cytogenetics helped in the diagnosis of this tumor. In general, the tumor cells are positive for Vimentin and CD99 and negative for neural, skeletal, vascular and lymphoid cell markers. Regarding Mic2 antigen, recently published data have confirmed the high sensitivity of the Mic2 gene product (CD99) for all ES family tumors with over 95% of the cases showing positivity for this marker. In fact, the expression of CD99 protein is not conclusive to ES because other round cell tumors, such as Merkel cell carcinoma, small cell osteosarcoma, T-lymphoblastic lymphoma and poorly differentiated synovial sarcoma may express this marker.

It has been reported that combined therapy including surgery, radiothe-

rapy and chemotherapy is the best approach for ES. The treatment should include wide surgical resection and neoadjuvant chemotherapy [6]. The multidisciplinary treatment protocols have dramatically improved the 5-year survival rate of patients with ES from less than 16% to more than 75% of cases. Radiotherapy alone must be undertaken only to treat non-resectable primary tumor or as an neoadjuvant therapy; chemotherapy alone also helps suppressing potential micrometastasis and reducing the size of the tumor prior to surgery. Radiotherapy in doses greater than 4,000 cGy has been effective in short-term control of tumor growth in about 86% of cases [11]. The chemotherapeutic agents most commonly used are Vincristine, Doxorubicine, Cyclophosphamide, Ifosfamide and Actinomycin-D. Metastatic workup of Ewing’s sarcoma of the mandible should include a chest computed tomography scan, bone marrow aspirate and bone scan.

Ewing’s sarcoma is viewed as a systemic disease even when it is clinically localized and current chemotherapy regimens reflect this attitude [1].

Major prognostic factors include the tumor site and its volume as well as

the presence of metastases. Tumors arising in the jaw bones have better prognosis than those located in long bones. Younger children have better event-free survival than older adolescents and young adults [12]. Clinical features such as systemic symptoms (fever, anaemia), high erythrocyte sedimentation rate, thrombocytosis and elevated serum lactate dehydrogenase levels are related to poor prognosis.

Increased serum lactate dehydrogenase levels prior to treatment correlate with metastatic disease and shorter disease-free survival [13].

conclusion The prognosis of ES is poor because haematogenous spread and lung metastasis can occur within few months after diagnosis. The clinical signs and symptoms of Ewing’s sarcoma appear when the tumor is in an advanced stage, which limits treatment options and prognosis. Recently chemotherapy and radiotherapy have dramatically improved long-term survival rates. Surgery is justified only if tumor control and preservation of function are guaranteed.

Abstract The construction of a removable partial denture in distal extension cases is a delicate procedure since the prosthesis is supported by two different tissues, namely teeth and mucosa. The different resiliency of these supporting tissues may lead to the instability of the prosthesis. This paper describes a functional impression technique that aim to equalize the masticatory load between teeth and edentulous areas thus reducing the instability of the partial denture during function and preserving the residual teeth.

Résumé La construction d’une prothèse amovible partielle en cas d’extension distale est une procédure délicate car la prothèse est supportée par deux tissus différents, à savoir les dents et les muqueuses. La résilience de ces différents tissus de soutien peut conduire à l’instabilité de la prothèse. Cet article décrit une technique d’empreinte fonctionnelle qui vise à équilibrer la charge masticatoire entre les dents et les zones édentées, réduisant ainsi l’instabilité de la prothèse partielle au cours de la fonction et la préservation des dents résiduelles.

Different techniques have been proposed for the construction of removable partial dentures in distal extension cases where the management of the difference of resiliency in the supporting tissues is an issue [1-4].

The purpose of these techniques is to decrease the forces on the abutment teeth [5].

McCracken proposed the altered cast technique with the selective tissue placement impression [3]. Christensen, in a less complicated technique, suggested to take the impression of the edentulous arch with an elastomeric material in a custom tray, then to cover this initial layer and the dentate areas with an elastomeric impression material of lighter-viscosity [6, 7]. Hindels introduced his functional technique in order to equalize the load between the teeth and the edentulous ridges [8, 9]. His technique consisted of making a mucostatic impression of the edentulous ridges followed by a pick-up impression with digital compression of the first through holes prepared on the impression stock tray. In the present paper, we propose a simplified modification of the Hindels’ impression technique without jeopardizing its functionality.

1. An individual resin tray is constructed with the self-curing acrylic resin (Formatray; Sybron/Kerr Co, Romulus, Mich) on the edentulous ridges areas of a preliminary cast. Compound occlusal rims (Impression Compound, Kerr (Europe) Co AG CH-4051 Basel) are fixed on the outer surface of the individual tray. Care is taken so that the height of the occlusal rims surpasses the height of the residual teeth in order to ensure a positive and sole contact between the occlusal rims and the impression stock tray later on during the pick-up impression phase (Fig. 1). The tray is selectively relieved and covers the edentulous areas up to the border tissue attachment including the retromolar pads [3, 8, 10, 11].

2. The individual tray is loaded with zinc oxide-eugenol impression paste (Cavex Outline by Cavex Holland BV) and brought to position without any compression while the soft tissues are left in their passive state (Fig. 2).

3. When the material sets, the tray is removed and the impression inspected (Fig. 3). The excess material is trimmed away and teeth are freed from any residual impression paste. The tray is tested in the mouth for stability.

4. A metallic rim-lock perforated stock tray (Coe Stainless Steel Trays no. 264008, GC America Inc., Alsip, IL) covering the whole arch is chosen (Fig. 4). The mucostatic impression of the soft tissue areas, already taken, is inserted in the mouth (Fig. 5).

5. While the metallic tray is being loaded with an alginate impression material (Jeltrate, Dentsply Caulk, Milford, DE), this same material is used to fill the space between the soft tissue impression and the remaining teeth.

6. The loaded metallic tray is inserted into position over the teeth and the acrylic tray. The index fingers are positioned on the tray facing the edentulous areas, and a positive pressure is exerted upon the metallic tray until the alginate impression material sets (Fig. 6). The completed impression is removed (Fig. 7). This maneuver permits to relate the edentulous ridge to the residual teeth in a functional state.

In this technique, the role of the compound occlusal rims is to transmit the positive pressure exerted by the fingers to the edentulous ridge areas and to ensure a functional relationship between teeth and edentulous ridges.

The impression is finally poured with dental stone (Silky-Rock, Whip Mix, Louisville, KY).

discussion Hindels noticed that his technique fulfills three definite requirements needed to ensure proper load distribution in extension saddle partial dentures [8]: 1- The tissue surface of the saddle is a negative reproduction of the anatomic, undistorted surface of the alveolar mucosa.

2- The denture saddle is related to the metal framework in a way similar to the relationship between the supporting teeth and the mucosa when the latter is under a masticatory load.

3- The masticatory load is distributed between the ridge and the abutment teeth during function.

Through a survey of regular periodic examinations of partial denture cases realized according to this technique, less post-insertion adjustments were needed and refitting because of ridge resorption was rare [9].

This modified procedure is easier and faster to realize, compared to the original one. Initially, when an impression is made to establish a relationship between the teeth and the mucosa in a displaced state, a perforated tray has to be provided with two circular openings to exert a pressure on the acrylic tray on the molar regions. In the proposed procedure, the pressure is transmitted throughout the occlusal rims to the full extent of the saddle, with no risk of rotation or displacement of the acrylic tray.

conclusion A modification of Hindels’ impression technique for restoring distal extension cases in removable partial dentures has been described. This variant simplifies the original one while maintaining its objectives and functionality.

Note aux auteurs Le comité rédactionnel de la revue IAJD a décidé de publier tous les articles simultanément en versions française et anglaise. Cette décision prendra cours à dater du prochain numéro de février 2013.

La nouvelle présentation devra faciliter indistinctement à tous nos lecteurs l’accès à tous les articles dans la langue de leur choix.

Dorénavant, Il est demandé aux auteurs de nous soumettre les deux versions sur le site.

Ceux qui n’ont pas la maitrise rédactionnelle dans la 2ème langue pourraient se faire assister de la personne adéquate, de sorte à ce que la syntaxe puisse satisfaire aux exigences de la rédaction scientifique.

La traduction littérale par le biais de certains moteurs de recherche n’est pas appréciable et ne peut en aucune manière exprimer correctement et avec précision les idées rédigées en première intention.

Note to authors The editorial board of the journal IAJD decided to publish all items simultaneously in English and French. This decision will take over dating from the next issue of February 2013.

This new format will facilitate indiscriminately to all our readers the access to all articles in the language of their choice.

From now on, authors are asked to submit both versions on the site.

Those who do not have the mastery of drafting in the 2nd language could be assisted by the right person, so that the syntax can fulfill the requirements of scientific writing.

Literal translation through some search engine (Google...) is not significant and can not in any way correctly and accurately express the ideas drafted in first intention.
