Direct Care: Putting the Doctor-Patient Relationship First

What if going to the doctor felt simpler and more personal again? As healthcare grows more complex and costly, many Thurston County residents and employers are turning to direct care, a model designed to restore time, trust, and connection between patients and physicians. In this Wellness Issue of VOICE Magazine, the Thurston County Chamber explores what direct care is, why it’s gaining momentum, and who is offering it locally.

What Is Direct Care?

Direct care refers to healthcare models where patients pay physicians directly rather than billing insurance. The most wellestablished form is Direct Primary Care (DPC), in which patients pay a fixed membership or direct fee to receive access to primary care services. In addition, Direct Specialty Care (DSC) physicians have emerged to support the broader growth of DPC by offering specialist services on a similar direct-pay basis.

By removing insurance billing, physicians are able to offer longer appointments, transparent pricing, easier access, and more comprehensive care that supports shared decision-making, prevention and wellness.

How Direct Primary Care Works

In a DPC model, patients typically pay a flat monthly fee, often between $50 and $150, which covers most primary care services, including routine and preventive care, chronic disease management, and urgent concerns. Insurance is generally used for emergencies, hospitalizations, or specialty care outside of the DPC clinic. Many DPC practices also offer transparent, wholesale pricing for labs, imaging, and medications to pass along savings to patients.

Without insurance paperwork, physicians spend less time on administration and more time with patients.

Why Direct Care Is Growing

The appeal of direct care is backed by national data:

• Primary care visits average just 18–21 minutes in traditional insurance-based settings, according to the American Medical Association, while many DPC practices routinely offer 30–60 minute visits.

• Physicians spend nearly twice as much time on documentation as with patients, contributing to burnout, per the American Academy of Family Physicians.

• NIHCM notes that insufficient access to primary care can drive more expensive emergency care use across the health system — a key rationale for stronger primary care models.

For patients, the benefits include transparency, access, affordability, and continuity of care. For employers, direct care can offer predictable costs and healthier, more engaged employees.

A Family-Driven Vision for Growth

For Boyce, fitness is a way of life. She credits her mother and business partner, Tracey Pidge, for laying the foundation for her love of movement, health, and community. That influence came full circle when Pidge introduced Boyce to HOTWORX as a client in 2022.

Today, the pair operate two HOTWORX studios and are actively developing seven more across Washington and Alaska, supported by a dedicated team—including general manager Madeline, who has helped earn more than 100 five-star Google reviews.

From the beginning, HOTWORX Lacey has embraced a spirit of appreciation and giving back. “We truly believe in collaboration over competition,” says Boyce.

Pidge has served on numerous boards and currently serves as Interim Executive Director of Saint Francis House, a commitment that continues to shape how the business shows up for the community.

The South Sound YMCA Develops Spirit, Mind, and Body

Health and wellness go hand-in-hand. But success requires more than just a New Year’s resolution which fizzles by Valentine’s Day. The South Sound YMCA was deliberately crafted to acknowledge and welcome everyone. Whatever your background or goals, they’ll work with you each step of the way.

For more than 100 years, the South Sound YMCA has been a vital part of Thurston County. Today, they have three branches which saw approximately 400,000 check-ins in 2025 and are well on their way to 2,000 in the first half of January 2026 alone.

Senior Director Michelle Gipson believes it comes down to the YMCA’s longstanding mission and values statements. These emphasize the organization’s role as an affordable and accessible resource for the positive development of spirit, mind, and body. This happens alongside supporting the community’s critical social needs while fostering care and respect for all people.

Gipson believes it’s their job to bring families closer together and develop health through fitness, sports, fun, and shared interests while maintaining social responsibility, youth development, and dignity for all.

Beyond gym equipment—of which they have a hightech variety—fitness saves lives in many different ways

at the YMCA. For example, says Gipson, “studies reveal that roughly 65% of black children and about 40% of white children cannot swim. Additionally, 87% of nonswimmer youth plan to visit a beach or pool at least once during the summer months, and 34% plan to go swimming at least ten times. Sounds like a recipe for disaster, doesn’t it?”

Since 2020, the South Sound YMCA has had at least 20 lifeguards on staff who worked their way up from in-house swim lesson students. As did both aquatic department managers and many members of their competitive swim team.

They also offer an array of informational classes for all ages, sports camps and summer activities for kids, a new Fitness Recovery Studio with HydroMassage and CryoLounge+ stations, one-on-one personal training, seasonal entertainment, childcare options, and even full-body, medical grade scans you can share with your primary care doctor.

Claudia Suastegui, director of the Shelton branch, says that their “small but mighty” newest location has quickly become a community hub. “Our strength comes from the incredible partnerships we’ve developed; more than 14 organizations offer free, recurring community programs each month all designed to meet the evolving needs of local families.”

New in 2026, Shelton will be launching a Trail & Outdoor Project. Thanks to a T-Mobile Hometown grant, Suastegui says it will “bring new outdoor amenities to the Y grounds, including a forest therapy trail, a youth gathering space, a community garden, and a small orchard. These efforts are strengthened by the collaboration of dedicated volunteers and partners such as Mason County Climate Justice, Mason Conservation District, Hope Plaza, and others.”

If your kids can’t decide which team, activity, or adventure sounds best, check out the Youth Experience Program (or YEP) which provide a new experience and skill training every 30 days. And all three branches work tirelessly with civic, nonprofit, and government groups. Family Education & Support Services and WIC are both housed inside the Shelton YMCA and Olympia staff facilitate showers and foot care programs for unhoused population. To give back, branches also host annual charity drives of socks, blankets, nonperishable food, diapers, toiletries, and holiday gifts.

Whether you need estate planning help or a safe place (with snacks!) for your kids to hang out after school, the YMCA is there. They offer scholarships and personal pricing options so cost is never a barrier.
