It is perhaps no surprise why online shopping is so popular these days, with its convenience and broader selection of products. It does, however, create certain hurdles for vulnerable customers.

Welsh retailer Cantre Mobility is celebrating 30 years of helping people stay mobile and independent this month (p23). Ahead of its fun-filled open day at its Carmarthen showroom, THIIS catches up with the retailer to find out more about the secrets to its success over the years.

Sitting Pretty has launched a new riser recliner which enables independent adjustment of its backrest and footrest to help the user find their perfect position.

Mobility equipment supplier RHealthcare has announced the launch of its latest wheelchair which has been engineered with a larger frame and reinforced components to meet the needs of users who require additional space and support.
The Dash 200 HD Plus wheelchair boasts a 26-inch wide seat and user weight of 225kg. Features include quick-to-engage hub brakes and quick-release wheels for transportation and storage.
The new model is an upgrade of the Dash 200 HD wheelchair which launched last year. Its seat is two inches wider than the largest Dash 200 HD model and the user weight is boosted by 25kg.
Wheelchair accessories provider RehaDesign has launched Wheelchair Skins, retro-reflective stickers which are designed to elevate the appearance of wheelchairs as well as enhance nighttime safety for users.
Developed in collaboration with an international team of wheelchair users, Wheelchair Skins are available in two flexible formats: a long rectangular version for wheelchair frames and a large rectangular version that can be cut to customise canes, laptop cases, mud guards, and more.

A new start-up developing flexible, affordable and available wheelchairaccessible vehicle (WAV) rentals has launched.
Based in the UK, Open Road Access (ORA) states that its mission is to support 15 million trips a year by 2030. ORA previously spent a year incubating a rental service for WAVs, experiencing significant demand from disabled and older people, carers, families, and businesses. Delivering 6,000 trips to over 1,000 customers, ORA’s proposition made headlines last year for offering WAV delivery and collection at major UK airports.
Now ORA, using cutting-edge technology, will become the first platform where consumers and businesses can directly book a WAV that meets their needs.
NEW HOME ADAPTATIONS STANDARD
Foundations, the national body for home improvement agencies in England, has set up the Home Adaptations Installer Network (HAIN) to ensure that essential modifications for older adults and people with disabilities are carried out by reliable and skilled professionals. HAIN is expected to help councils nationwide manage and grow their local builder lists. It will show which builders are active in their area and confirm key checks like insurance, training and experience, helping to ensure they are accessing contractors with DBS checks and TrustMark registration. Foundations hopes to open up the network to the public later this year.
DRIVE DEVILBISS HEALTHCARE REJOINS MOTABILITY SCHEME
Mobility equipment manufacturer Drive DeVilbiss Healthcare has announced that it will officially be rejoining the UK’s Motability Scheme, from 1st July 2025. A spokesperson stated that its operational teams have been working “diligently” behind the scenes over the last six months to enhance its spares and technical processes so that it can ensure a seamless aftersales experience for its Motability customers. The firm also stated that the Royale 4 Sport and Manual Fold+ scooters will be included in the Motability Scheme.
Yorkshire-based specialist furniture manufacturer and retailer HSL Chairs has invested in its Ipswich presence by relocating to a larger showroom that is almost twice the size of its previous showroom.
The newly updated showroom will feature the latest styles from HSL, showcasing a range of riser recliners, adjustable beds, fireside chairs, and complementary sofas. Brands like Stressless, Himolla and Nicoletti will also be displayed at the new location, offering customers a wider selection of chairs and sofas to choose from.
Seating Matters, which is part of the Tierco Healthcare Group, has announced the acquisition of the trade and assets of the bathing and Burnett Remould divisions from Care & Independence, a GBUK Group company. The acquired business will now be rebranded as Tierco by Seating Matters — a new division within the Tierco Healthcare Group dedicated to bathing, patient hygiene and postural support products. Deliveries will be fulfilled from the existing Wigan manufacturing facility, which will be renamed Tierco by Seating Matters.
Mobility solutions provider
Sunrise Medical has introduced a new modern colour palette for its manual wheelchair range to help users express their individuality and style. The re-imagined colour palette applies to adult Quickie and paediatric Zippy manual wheelchairs as well as select Jay backrests and features new, ontrend hues thoughtfully chosen to complement each other and suit styles of all ages. Vibrant shades include midnight blue, purple, green, yellow, orange, red-black, pink and silver metallic.
INVACARE'S 'MOVE FOR MOBILITY' CAMPAIGN
Wheelchair provider Invacare hosted an inclusive sevenmile walk on the all-accessible Keswick to Threlkeld route in Cumbria last month. It was part of a month-long annual fundraising campaign to raise awareness of an initiative which helps build and distribute wheelchairs for people in under-resourced and remote communities around the world.
Motability Operations has unveiled a pioneering passenger wheelchairaccessible electric demonstrator car to encourage car manufacturers to ensure that the transition to electric vehicles (EVs) does not leave behind wheelchair users.
The car, named eVITA, was designed and assembled in Britain in collaboration with design studio Callum using unique insights from disabled people to address the obstacles wheelchair users face with existing EVs.
The EV concept has been developed using inclusive design principles with input from Motability Scheme customers.
The Wheelchair Alliance, an organisation which champions the needs of wheelchair users across England, has appointed a new chief executive officer and chair.
Nick Goldup, a co-founder of the Alliance, takes on the new role of Chief Executive Officer, having previously undertaken the roles of Chief Operating Officer and Chair. With experience as a director within the charity sector, including six years at the MND Association and five years at Whizz-Kidz, Nick
will continue to utilise his skills to help drive forward positive change in wheelchair provision.
Karen Pearce, a fellow founding member and a former project manager of the Wheelchair Alliance, takes over the role of chair from Nick. With over 30 years of NHS experience at clinical and managerial level, alongside seven years as director of care for the MND Association, she will continue to support Nick and the Alliance in lobbying for wheelchair users to further strengthen its voice.

We’re generally all in this business to help people live independently –and to genuinely make a difference to peoples’ daily lives. But this shouldn’t just stop on our doorstep. Even if some of us operate globally, do we really reach everyone that needs our help?
Recently, I volunteered with Yorkshire Aid Convoy, a registered charity that collects, transports, and distributes humanitarian aid throughout Eastern Europe, to go on one of its annual distribution missions. Many of the organisations it works with rely solely on its support to make sure they can continue to provide basic healthcare services to their communities.
I can honestly say the trip was lifechanging. It might sound cliché, but it was one of the most intense, emotional and rewarding experiences of my life. It’s given me perspective, a new sense of purpose, and I was lucky enough to meet people I’ll never forget.

What is a gel battery and why does it make the battery different to an AGM?
In an AGM battery the electrolyte is held within the glass matting, with a gel battery, you got it, it is held in a suspended gelled electrolyte. The suspension of the electrolyte moves slower within the battery, but why is that important?
The Premium gel battery can be used for longer before it needs a recharge. It works better if it hotter or colder than normal and has a higher cycle life, which means it lasts much longer than an AGM.
When it comes to distinguishing between AGM and Gel batteries, it can become somewhat overwhelming. However, the key lies in matching the appropriate battery to the right user. From profiling beds, air-alternating pressure care mattresses and rise and recline chairs to daily living products, specialist footwear and bathing equipment, we also took 100 new towels – something that sounds so simple but was so appreciated. Everyone involved was just incredible.
The residents of the care home were modest and humble. They’d never have asked for help even though they were in desperate need – it was a real reality check. Even though there was a language barrier, I could see the pain they were experiencing each and every day.
After we’d finished, some residents were able to have a bath for the first time in years. Others slept in clean bedding, in warm rooms, with working toilets, heating and space that actually
felt homely. I’ll never forget the look on their faces.
The thing that was most striking is these people would never have asked for help. Even after we’d installed the stairlift, for example, there was genuine resistance to using it – despite the alternative being climbing the stairs with zimmer frames or being carried up in wheelchairs. Why?
I think there was a sense of embarrassment that they needed help and a fear of giving up. This sentiment is the case for a lot of our older customers across the globe – they don’t want to ask for help, even when they desperately need it.
One of the things I hear regularly from our customer services team is customers saying, ‘I just wish I’d got
one sooner’. Once the barriers are down and people have decided that they need a bit of help, they realise just what a difference a mobility aid can make.
It’s so special to be reminded of the resilience and sheer determination of the people we’re trying to help with our products. They have an in-built sense of pride; they’ve worked hard for everything their entire lives and they don’t believe in being given things on a plate.
But it’s our job as an industry to educate people about how mobility products can change their lives for the better and enable independence – something we’re trying to achieve collectively as an industry. Our work here is nowhere near done and my trip to Ukraine has only made me more determined to make a real difference.

This month, Cantre Mobility, a family-run retailer located just off the picturesque coast of West Wales, is looking forward to celebrating 30 years in the industry with a fun-filled open day at its Carmarthen showroom. Members of the public, healthcare professionals and even a Paralympian or two are expected to be in attendance, to help the retailer celebrate this important milestone.
Cantre Mobility was formed in July 1995 by Nest Evans and Huw Thomas. Their mother, Euriana, had multiple sclerosis (MS) and when their father Elwyn went to a local demonstration
day he met with a representative on the J.Carters (now Invacare) stand at an event and started discussing a powered scooter/chair with Mark Hermolle (currently at Kymco UK)
As Nest explains: “There wasn’t a showroom in the area, the nearest being Cardiff or Chester, to go and try the products, which were both a two-hour drive away. After discussing it further, Mark informed Elwyn that they could really do with a mobility dealer in West Wales. The rest as they say, is history!”
Cantre Mobility opened within the existing showroom of the original family business, which was in the garden
machinery industry. This remained its home as it expanded until relocating and expanding further with its new premises in 2015, the current head office, showroom and service centre at Y Bont in Aberaeron (previously anglicised as Aberayron), a picturesque Georgian town in West Wales.
Cantre Mobility repeated its approach with its Carmarthen location, taking up a corner of the existing showroom firstly, before expanding into premises next door, a purpose-made
showroom for mobility in 2012.
Following the sale of the family’s garden machinery businesses in 2024, Cantre Mobility took over the showroom next door at Carmarthen and is now one of the largest showrooms in the UK.
The directors of Cantre Mobility, Nest Evans and Huw Thomas, both have very hands-on roles within the business. Nest has been the driving force behind the mobility business, wanting to provide a knowledgeable and unique service for customers from the day it started trading.
Nest became a specialist in seating and slings, as well as a manual handling trainer and has achieved an NVQ level 4 as an OT assessor. With her mother having MS since she was nine years old, her passion for helping others is at the forefront of the ethical way that Cantre Mobility supports its local community as a business.
Huw has recently taken a larger role at Cantre Mobility following the sale of the family’s existing garden machinery business, which he lead alongside Elwyn for over 50 years. Nest explains: “Huw does anything and everything that needs doing on a daily business within the company, using his experience to help in any which way the growing business requires.”
The other key person at Cantre Mobility is Nest’s son, Sion Evans, who officially joined the company after graduating from university in 2013,
despite spending a lot of his upbringing helping his mother in the family business.
Sion is now a shareholder in the company and manages the day-to-day operation of the business. Sion’s main areas of focus are operations, ensuring the businesses’ systems support the growth of the company, as well as a very hands-on approach with power products, in particular prescriptive products.
Its 22-strong team include specialists, a supportive sales team, highly experienced and trained engineers and an administrative team that ensure everything runs smoothly, every day. Nest says that several members of staff have been working with Cantre Mobility for over 20 years.
Cantre Mobility will be marking its 30th anniversary with an open day at the newly refurbished showroom at Station Approach, Carmarthen on 12 June.
“We are proud to be celebrating 30 years this year,” Nest comments. “We believe our friendly, knowledgeable and supportive sales team, and our excellent after-sales service, has made us stand out, and has fuelled the growth of our company over the past 30 years.”
The retailer will have over 15 suppliers attending and it will be running workshops throughout the day. There will also be a coffee morning to raise money for the Noah’s Ark Children’s Hospital and the local

League of Friends for Glangwili Hospital.
Nest says that Cantre Mobility continually support local and national charities, from donations to supporting beneficiaries with products and support. Its most recent open day raised hundreds of pounds for McMillan Charity locally, a charity close to our hearts as a family.
The award-winning Motability retailer is a member of the Countrywide Care Network (CCN) and it supports charities such as The Royal British Legion, The Masonic Charitable Foundation and SSAFA, the armed forces charity.
Its range of customers from across Wales includes local authorities, nursing and residential homes, architects, hospitals, occupational therapists and physiotherapists.
Cantre Mobility offers servicing on all products sold at both its Aberaeron
and Carmarthen showrooms, and it has many servicing contracts with local authorities and nursing/residential and private customers.
Nest explains: “Our service team are based at the Aberaeron Office and organise installations, servicing, breakdowns and repairs for the team of seven engineers and surveyors, offering 24-hour support to our contract customers.
“We are immensely proud of our work-based apprenticeships which feeds our ever-expanding engineering team and provides opportunities for young people in our area.”
Next explains that Cantre Mobility’s most popular powered wheelchair is the Invacare TDX. “It is our most tried and trusted, and extremely versatile mid-wheel drive prescriptive power chair,” she says. “Our relationship with Invacare spans the full 30 years of trading and we are grateful for their continued support.”
Its best selling and “favourite” new mobility scooter is the Rascal Luxe. Nest adds: “We have been hugely impressed with Rascal’s LiFe lithium battery range. The confidence to give the batteries a lifetime warranty and combined with the lightweight but sturdy and comfortable Luxe, makes this the ultimate combo. We believe they have perfected the lightweight aluminium boot scooter!”
Nest says that Cantre Mobility’s relationship with its suppliers is very important and she values their support on every level. “We focus on working with suppliers that have innovative products, with excellent after-sales service,” she says. “This allows us to provide the ‘gold standard’ level of service we strive for with our customers.
“We are proud to be a main dealer for our area for notable brands such as Stannah Stairlifts, Electric Mobility, Invacare, Sunrise, Primacare and Guldmann.”

of its team dedicated to building, expanding and updating it. Nest comments: “Whilst the main focus of our business is to deal with people face-to-face at our showrooms or at the comfort of their home, we believe our website, and our social media presence gives our customers a shop window to view the products that we offer and get a feel for our business, including our fantastic team and how we operate.”
Embracing e-commerce is vitally important, Nest says, as they recognise that’s how some of their customers prefer to shop for some items. She continues: “We are very proud of our new online hire booking system.
“We do a significant number of hires, from serving visitors to our area, to facilitating hospital discharges in our local community with the equipment they need urgently.
“This allows our customers to book any time of day, and choose the equipment they need at the date they require it, whilst also reducing time and pressure on our team, and speeding up the whole process for us as a business.”
Cantre Mobility’s main focus for expansion for the next 12 months includes developing the Carmarthen showroom into one of the leading UK mobility showrooms, Nest comments.
The plan is also to expand its
installation offering to include throughfloor, home lifts with its partners Stannah, “something we are aiming to bring fully in house,” she says.
Developing the business’s maintenance team is also a priority as it looks to fully refurbish reconditioned mobility equipment, and in particular mobility scooters, at both branches. As Nest explains, this currently only takes place at one head office, so it is looking to offer a full offering at both branches.
Looking at how the mobility sector as a whole Nest believes that the main challenge is losing experienced, knowledgeable and talented individuals to other industries. She says: “Its why we focus a huge amount on education within our business.

Transparency in online pricing is more than just good business practice – it’s a necessity, writes Julia Phipps
Remember the old days of retail?
Customers came into a shop to have a browse, ask questions and try out a product before completing a purchase. How times have changed!
The internet provides many great opportunities for businesses allowing a “shop window” of products and useful information for customers to peruse before making a purchase which is then delivered direct to their door without the need to traipse to the local high street. Ideal for many of our core customer base you would have thought!
However the internet can be daunting place for many of our audience, and the variety of prices on offer frequently causes confusion. So why is this allowed to be the case? Surely all businesses need to follow the same rules? And if not, what is the root cause of this confusion?
In our world of mobility and disability aids, transparency in pricing is more than just good business practice – it’s a necessity for the wellbeing and trust of our customers. Our customers are usually older adults, disabled individuals and those living with long-term conditions, which means that many of the products we sell are essential rather than “nice to have”.
Why then, does pricing of the same item often vary considerably from one business selling on the internet to another? Dig a bit deeper and you will often discover that the answer is a disparity caused by Valued Added Tax – more commonly known as VAT.
The standard rate of VAT increased from 17.5 per cent to 20 per cent
on 4 January 2011 in order for the Government to help cut its deficit in that time (a familiar story to the position we appear to be in today, in 2025!)
The majority of businesses based in the UK are registered for VAT – the threshold now being a turnover of more than £90,000 within a 12-month period.
This means that most of the items sold in the UK are subject to 20 per cent VAT on them with the VATregistered retailer having to pay the 20 per cent VAT element collected from the sale across to HMRC on a regular basis. In the majority of other market sectors the “regular basis” is quarterly –completing quarterly VAT returns.
However the mobility industry is different to most industries in that many of the items sold within our sector can be sold with 'VAT relief' (ie. no VAT included) If the item qualifies (i.e. it was
specifically designed or adapted for a disability) and the person the item is being purchased for qualifies (i.e. they are disabled – have a physical or mental impairment which affects their ability to carry out everyday activities such as blindness – or have a long-term chronic sickness – such as diabetes or are living with a terminal illness). In order to prove qualification the customer is required to complete a simple self-declaration form.
At this point it’s worth noting that the Government clearly states that individuals do not qualify if they’re elderly but not disabled, or if they are temporarily disabled – for example with a broken limb.
The fact that a large proportion of the UK population – 16 million people (according to the House of Commons Research Briefing, August 2023) are “disabled” in some this means that a lot of the mobility products sold both online and in stores are sold without VAT. For this reason many mobility businesses complete and submit monthly VAT returns, rather than quarterly, thereby aiding the cashflow of the business.
It’s easy to educate the customer in store about VAT relief advising whether a customer qualifies or not. However it is not so clear online and, over the years, we have found that many customers don’t even realise that they do qualify for VAT relief – 20 per cent off represents a large and worthwhile saving to most consumers.
For instance, a mobility scooter listed at £1,000 may appear to be a bargain, but if that price is VAT-inclusive and the customer is eligible for VAT relief, the actual cost could be significantly lower – around £833.
Very many businesses, however, particularly those trading on marketplaces, such as Amazon and eBay, fail to include any VAT in their prices and neither do they obtain any self-declaration form for VAT relief from the end customer.
This causes many issues – it unnaturally distorts the market and
confuses customers who see a vast range of different prices for the same item – usually because many prices they are reviewing do not include VAT, making the cheapest price 20 per cent below the rest of the market.
Like most of us, when we are shopping, particularly online where price comparisons are so easy to see, we are drawn to the cheapest option, often highlighted through paid ads on search engines like Google or results served up on Amazon or eBay.
The Digital Markets, Competition and Consumer Act (DMCCA) places a clear obligation on businesses to avoid misleading marketing. This includes ensuring that any pricing reflects the actual amount payable by the intended customer, including clear differentiation between VAT-inclusive and VATexempt/relief pricing.
Yet, despite this, many online businesses fail to provide the necessary transparency, either leaving vulnerable customers at risk of overpaying or, worse still, by not providing the correct information to allow customers to obtain their 20 per cent VAT refund when marketplaces, such as Amazon, do not differentiate between prices including or excluding VAT. At this point it’s worth noting that Amazon states that ALL prices on its marketplace within our sector need to include VAT at the applicable rate – ie. 20 per cent.
For consumers, the stakes are high. Those with disabilities or long-term conditions are often on fixed incomes or managing significant healthcare costs. Unexpected price differences can lead to financial strain or, worse, deter them from purchasing essential

mobility products altogether. For these individuals, clarity isn’t just about making a good deal – it’s about preserving their independence and quality of life.
For businesses within the mobility sector, staying compliant with DMCCA regulations is critical, but it’s not without its challenges. The VAT relief rules themselves are most certainly a grey area – for example, which products are eligible to be sold without VAT and which are not and which customers qualify and which do not.
While the DMCCA sets the framework for fair marketing, the enforcement of these rules is almost non-existent. There is little doubt that HMRC are simply not doing enough to clamp down on rogue sellers who are continually breaking the rules and blatantly advertising prices without any VAT and without obtaining
any customer qualification. It is easy, on most websites, to include both VATinclusive and exclusive prices and allow the customer to purchase with a form of online self-declaration for VAT relief. However this is most certainly not the case on digital marketplaces where only one single price is shown. Those businesses selling without VAT in their prices are not only breaking the law they are also putting other businesses, who follow the VAT rules out of business.
Addressing this issue requires more enforcement and effort:
• Clearer pricing online: Platforms like Google and Amazon should mandate clearer differentiation between VATinclusive and VAT-exempt prices and crack down on those businesses who are breaking the rules.
• Improved customer education: Retailers within this sector need to

If products are typically sold to the public and are subject to VAT, the advertised price must include VAT to comply with transparency rules. Omitting VAT could be considered misleading, particularly
prioritise educating their customers about VAT relief eligibility, perhaps through clearer website messaging or proactive customer support, for example on the phone or through face to face in-store contact.
• Stronger enforcement and penalties: HMRC should be more aggressive in enforcing pricing transparency and penalising misleading practices.
• Simplified VAT relief claims: Streamlining the VAT relief process could reduce the burden on both businesses and consumers, making the system more accessible for those who need it most.
Ultimately, the mobility industry has a responsibility to support those who depend on their products. Clear, honest pricing isn’t just a legal requirement – it’s a matter of respect for the customer’s dignity and financial stability. By addressing the current gaps in transparency, the sector can not only improve trust but also ensure that more people get the lifechanging support they need without unnecessary financial strain.
For trade professionals following the rules this is challenging and without strong enforcement from HMRC and
for consumers who may struggle with complex pricing structures.
Many medical devices or mobility aids may be zero-rated for certain disabled individuals. However, vulnerable consumers may not be aware of VAT exemptions, making it essential for businesses to clearly state whether VAT applies and under what conditions a VAT-free price is available.
If targeting business customers and the majority of the customers are businesses, prices may be shown excluding VAT, but it must be explicitly stated (e.g., “+ VAT”) to avoid confusion. Ensure it is clear that it is a “trade price” or “business price”.
If advertising to both businesses and consumers, at a minimum, the advertised price must include VAT (but also make clear where there any VAT exemptions).

What is comfortable now, may not be so half an hour from now. Comfort is a subjective concept, but what can we do to optimise it for each individual?
Most of us will have experienced sitting down at a meeting or conference where the chair we are sitting on seems OK when we first sit down. However, after a short time we get to fidget because we are no longer feeling comfortable. Most of the readers of this article may well be able to adjust their position to decrease the discomfort, but there are many in wheelchairs, or indeed static seating, who do not have this luxury – so what can we do with the equipment prescription to ensure maximum comfort for the longest time possible?
First of all, what is comfort, or possibly more importantly, discomfort? There are mental and emotional aspects relating to comfort, but in this article we will concentrate on the physical elements.
Dr Barbara Crane, as part of her work for her doctoral dissertation at the University of Pittsburgh, created in 2004 a Tool for Assessing Wheelchair disComfort (TAWC)1. In this the individual is first asked a number of questions, the answers to which give scores for their comfort, but also their discomfort. This General Discomfort Assessment (GDA) brings out scores, separately, for discomfort for example
from: poor positioning; being in one position for too long; aches, stiffness, or soreness; pressure in parts of the body; and/or too hot, cold, or damp.
The second part of the interview to create a Discomfort Intensity Score (DIS) identifies for each part of the body the intensity of the discomfort at that point. In the results of one published study using these scores with mainly a spinal injured population, it was the individuals’ backs which came out with the highest scores, while general discomfort came second, and discomfort at the buttocks came third2 .
Discomfort is a time-related phenomenon (the TAWC looks at the individual’s experience during the previous four hours). While we are seated, there is ‘creep’ in the set up: we settle into our support surface as it reshapes around our buttocks. At the same time, our skin tissues rearrange themselves, distorting their cells, and occluding capillary vessels, with the potential eventually of tissue death if the forces are not redistributed. With time this escalates to being painful, and not just uncomfortable.
The degree to which this damage occurs will depend in part on the material properties of the support surface, and the degree of envelopment that can be achieved to spread localised pressures and shear forces as widely as possible, and in part the health and elasticity of the skin tissues. We have to find the compromise between high levels of envelopment with its benefit of reducing localised pressures, against the restrictions of movement that can result. The best compromise is to redistribute the pressures away from around bony prominences, such as the tail bones and coccyx, to more appropriate load-bearing tissues such as under the thighs.

under the thighs has been shown to be effective at redistributing pressures away from the buttocks, while still allowing the foot supports on the wheelchair to be positioned at a suitable height from then floor (Figure 1).
Alternatively, a custom-fitted system can provide the greatest surface contact, but as our body morphology (or even our seasonal clothing) needs change, it’s ideal if the system can be adjusted to our changing needs. Symmetric Design’s FreeForm system (Figure 2) is ideal for this.
If your seating system does not have good air circulation near the skin, there is risk of discomfort arising from the build up of moisture, or of temperature, or of both. Moist skin is at greater risk of tissue damage from frictional and shear forces. A one degree rise in temperature leads to a 13 per cent increase in metabolic demand – more nutrients and oxygen needed, and more metabolites to get rid of.
The make up of your cushion cover will have a major impact on controlling this source of discomfort. For over 30 years, Varilite cushions (Figure 3) have had a layer of reticulated foam between the cover and the air-foam cushion to allow moisture and temperature control.
Alternative breathable materials are available these days in the form of spun polymer cores, such as in the NEST cushion (Figure 4) which allows the air in the cushion to circulate freely. A further alternative is a medical sheepskin cover (Figure 5) where the wool can absorb up to a third of its weight in water vapour without feeling moist.
For back supports, consider Symmetric Design’s Respira back support (Figure 6), where the air flow through the back support is greater
than normal anyway, but with the optional added air pump leaves the occupant cooler and dryer.
The damage from the absence of regular movement has been documented: impaired dynamic sitting stability is associated with pressure ulcer development3. It is the sensory feedback of the effects of pressure and shear forces on our skin that gives us the feelings of discomfort, and encourages us to move to alleviate these feelings – and it’s this regular movement that allows our tissues to recover. Observed (in one study) over a 70-minute period, ‘normal’ individuals performed spontaneous movements on average every six minutes as viewed from the side, and every nine minutes when viewed from the front4. In contrast, in another study of a group of paraplegics, 21% moved once/hour or less and 55% moved in cycles shorter than 1 hour – and more than half of the study population developed a pressure ulcer5
Some movements are contraindicated, such as sliding down one’s seat, which can induce friction and shear-related skin damage, and result in longer-term skeletal malformations. In our July article in this series, we will be looking in more detail at the effects and causes of sliding, and
management possibilities.
It is important that pelvic positioning belts are positioned in front of the greater trochanters for those with a tendency towards a posterior pelvic tilt (Figure 7). Not only is this a safety matter, to ensure the belt length is sufficiently short to prevent submarining under the belt, but also allows sufficient
forward movement to unload the tail bones, and also permits greater reach.
Freedom of movement, while also redistributing pressure-related forces, not only provides increased functionality, but also improved physiological functions such as breathing, digestion, cardiac output, etc. To this end, it is recommended that wheelchair occupants spend at least an hour each day in a standing position.
We need different postures for different functional day-to-day activities, such as typing, eating, social interactions, relaxing, etc. The choice and placement of support devices on

a chair which permit or assist these postural variations can be important. Going back to the discomfort measures, for comfort and functional reasons, some people will have greater need to move their feet, their hands, or their
heads, than others.
As covered by previous articles in this series, ‘dynamic’ elements permit or support movement of parts of the body – these can be elasticated shoulder supports, arm supports (such as the Bodypoint Dynamic Arm Support –Figure 8), leg supports, or reclinable back supports. Please remember that the pivot point of the flex or extension movement needs to be lined with the related pivot point in the body (for example, see the Stealth EPiC seating system for back support recline –Figure 9).

be better at vibration absorption than pure air or gel cushions. There are new wheel designs in the marketplace which are energy absorbent, some even to the degree that they can assist curb-climbing (Figure 10). Frog Legs have had their shock-absorbing caster mounts around for a number of years –their new Whisper casters additionally offer energy-absorbent tyres (Figure 11).
In conclusion, there is much that can be done to make long-term seating more comfortable!
