Brookhaven Memorial Hospital’s Bellport Primary Care Center opened its doors in April, 2014 to address a critical need in providing quality primary care for local residents. The center and its staff also serve as a source of health education for the hospital’s community partners including civic, community and educational groups, to promote wellness and improved health outcomes. Historically the Bellport community has lacked access to quality health care services and, as a result, residents have had to rely on local emergency rooms to address medical issues or unfortunately, go without care all together. Bellport PCC has been providing these vital health services Monday through Saturday with convenient hours that enable easy access to care for the community. Appointments may be scheduled by calling (631) 2276600 and walk-ins are always welcome. The center’s staff includes Board Certified Physicians, a Nurse Practitioner, a Diabetes Wellness Educator/ Registered Dietician, and Medical Assistants. Support services, including reception, registration, and insurance verification are provided by Patient Access Associates. General medical and preventative services provided include: • Comprehensive physical exams • School Physicals • Vaccinations • Heart disease prevention • Diabetes and wellness education • Diet/exercise counseling • On-site laboratory services • Cancer screenings • Depression screenings and referral to treatment, • Pap smears • Adolescent medicine • Linkage to community resources, including Behavioral Health and Health Homes Plans are also in the works to add a Pediatrician to the staff at Bellport PCC. “We heard from the community that

the area is also in need of pediatric health care.” says Dr. Wehbeh, Brookhaven Hospital’s Vice President and Chief Quality Medical Officer. “Having a pediatrician on staff at the Bellport PCC will help us meet those needs as well.” Continued on page 7
