Health Insurance Federal Form 1095-C • You will receive the federally mandated 1095-C health insurance form prior to the end of the month. • If you were eligible for health insurance, you will receive a form whether or not you elected to participate in the heath insurance plan for 2023. • These forms will be sent to your department for distribution. • If you have any questions, please contact the Human Resources Department at 706-868-3300.
